Correlation of Self-Reported Breathlessness with Post Exercise Dyspnea in Obesity  [PDF]

Abstract:
Background: Dyspnea in obesity is common and dyspnea questionnaires are mostly validated for chronic respiratory diseases. The study aims to assess how modified Medical Research Council (mMRC) dyspnea scale correlates with post exercise dyspnea Borg scale in 6-minute walk test of the obese population. Methods: We performed a retrospective observational study on 342 obese patients with 6 MWT tests from February 2008 to November 2014 at a single tertiary hospital. Linear regression analysis was used to assess the relationship between mMRC and Borg dyspnea score. Fractional polynomial regression was used to analyze the relationship of mMRC and Borg scores with BMI. Results: MMRC dyspnea score was a strong predictor of post exercise dyspnea Borg score in the obese population in a univariate regression model (coefficient = 0.764, p ≤ 0.001). When adjusted to age, BMI, 6 MWT distance and PFT parameters in a multivariate regression model, the relationship remained statistically significant (coefficient = 0.587, p ≤ 0.001). The relationship was stronger in obese group with no airflow obstruction. Conclusion: In obese population, pre-exercise mMRC dyspnea score correlates significantly with post exercise dyspnea Borg score regardless of airflow obstruction. This is the first validity study regarding mMRC scale in assessing dyspnea in the obese population.
Convergence Analysis of Meshfree Approximation Schemes

Abstract: This work is concerned with the formulation of a general framework for the analysis of meshfree approximation schemes and with the convergence analysis of the Local Maximum-Entropy (LME) scheme as a particular example. We provide conditions for the convergence in Sobolev spaces of schemes that are n-consistent, in the sense of exactly reproducing polynomials of degree less or equal to n, and whose basis functions are of rapid decay. The convergence of the LME in a locally Sobolev space follows as a direct application of the general theory. The analysis shows that the convergence order is linear in h, a measure of the density of the point set. The analysis also shows how to parameterize the LME scheme for optimal convergence. Because of the convex approximation property of LME, its behavior near the boundary is singular and requires additional analysis. For the particular case of polyhedral domains we show that, away from a small singular part of the boundary, any Sobolev function can be approximated by means of the LME scheme. With the aid of a capacity argument, we further obtain approximation results with truncated LME basis functions in H1 and for spatial dimension d > 2.

Abstract: Background: Genotype G is the least common of all the hepatitis B virus (HBV) genotypes. The existence of the genotype G strain of HBV was first noted in 2000 and little information is available on its global geographical distribution. Previous studies have demonstrated the dominance of genotype D in patients with HBV infections in Turkey.Objectives: To report for the first time in Turkey, the case of a 61 year old male patient who developed the HBV genotype G infection.Case report: According to HBV genotyping using phylogenetic analysis and an INNO-LiPA assay, the patient was infected with genotype G and G+A, respectively.Conclusions: The present clinical study suggests that the transmission of an HBV genotype other than genotype D, namely HBV genotype G, is possible in Turkey. Epidemiological and clinical information on genotype G infection is currently limited, and this is most likely due to its low prevalence throughout the world. Therefore, it may be important to determine the epidemiologic and molecular characteristics of the HBV genotype G as it relates to chronic hepatitis, to enable better understanding of its circulation and progression around the world.

Abstract: Perturbative calculations in field theory at finite temperature involve sums over the Matsubara frequencies. Besides the usual difficulties that appear in perturbative computations, these sums give rise to some new obstacles that are carefully analized here. I present a fast and realible recipe to work out sums over the Matsubara frequencies. As this algorithm leads to deal with very cumbersome algebraic expressions, it has been written for computers by using the symbolic manipulation program Mathematica. It is also shown this algorithm to be self-consistent when it is applied to more than one loop computations.

Abstract: A hierarchy of effective field theories is used to separate the contributions from different momentum scales and to calculate the free energy of QCD at high temperature in powers of the coupling constant up to order $g^5$. The behavior of the perturbative series will also be discussed.

Abstract: Effective-field-theory methods are used to study the high T limit of QCD. These methods unravel the contributions to the free energy of QCD at high temperature from the scales T, gT, and g^2 T. The free energy is explicitly computed to order g^5. Implications for the application of perturbative QCD to the quark-gluon plasma are also discussed.
