Evaluación de un programa de atención sanitaria con enfermera tutora en cirugía de traumatologia en un hospital de Granada, 2004

Abstract: background: the current trend in patient care is towards a more humanized and higher-quality healthcare. the objective of the present study was to evaluate the results of implementing a programme of healthcare with a reference nurse who welcomes the patient at hospital admission, visits regularly during hospital stay, and resolves doubts and problems. methods: quasi-experimental study (149 cases and 454 non-tutored controls) in patients admitted for scheduled trauma surgery at the virgen de las nieves university hospital, granada. sociodemographic and clinical data were gathered at admission. at 15 days after discharge, patients were administered with an interviewer-administered zung score and anad hoc questionnaire on satisfaction with different healthcare aspects during hospital stay and understanding of information received. bivariate analyses and linear and multivariate logistic regression analyses were performed with a study of confunding variables. results: for the outcome variable anxiety level non-tutored patiens had a b coeficient of 2.64 (p<0,01) in the multivariate linear regression analisys controlling for the other variables in the final model: sex, presence of informal carer, professional activitiy, days of hospital stay and understanding of health information. for the outcome variable inadequate understanding information non-tutored patients showed an odds ratio of 3.48 in the multivariate analysis controlling for educational level and presence of informal carer. satisfaction with he friendliness of health care personnel and with the hospital setting did not significantly diference, although the percentage of dissatisfied patiens was higher in the non-tutored group: 15% vs 11% (p= 0.34) and 18% vs 12 % (p= 0.11) respectively. conclusions: the implemented continued care programme proved effective for these patients, increasing their understanding of information received and reducing anxiety levels.
El estudio CRUSADE, modelo de evaluación de la calidad en la intervención coronaria percutánea

Abstract: improvement of care quality does not end with the publication of clinical trials that show clinical evidence of effectiveness or with its support by the different international therapeutic guides. this quality improvement requires evaluation in the real population. this can be done by analysis of clinical registries, that would evaluate adequate compliance of the clinical guides and their effectiveness in the real population. the crusade study is a study that evaluates use, prognosis and factors of prediction, of invasive strategy by early percutaneous coronary intervention (pci) (first 48 hours of the ischemic event) in high-risk patients with non-st segment elevation acute coronary syndrome (nste-acs). of the 17926 patients studied, 8037 (44.8%) underwent cardiac catheterism in the first 48 hours of the ischemic event. intrahospital mortality of the invasive strategy was significantly less than medical treatment (2.5% versus 3.7%). the patients who underwent an early invasive strategy were a selected population, as the more solid independent prediction factors were associated to early invasive treatment: cardiology care, earlier age, absence of renal failure, absence of heart failure both previously or on arrival to the hospital and lower heart rate. finally, it could be concluded that, in spite of the decrease of mortality achieved with the early invasive strategy, this would not done in most of the patients, being reserved for subgroups with lower comorbidity and for those seen by the cardiologists.

Abstract: La mejora de la calidad asistencial no finaliza con la publicación de ensayos clínicos que demuestran evidencia clínica de efectividad, ni por su respaldo por las distintas guías terapéuticas internacionales. Esta mejora de la calidad requiere una evaluación en la población real, ello se puede realizar mediante el análisis de registros clínicos, que valorarían el adecuado cumplimiento de las guías clínicas y su efectividad en la población real. El estudio CRUSADE es un estudio que evalúa la utilización, el pronóstico y los factores de predicción de la estrategia invasiva mediante la intervención coronaria percutánea (ICP) precoz (primeras 48 horas del evento isquémico), en pacientes con síndrome coronario agudo sin elevación del segmento ST (SCASEST) de alto riesgo. De los 17.926 pacientes estudiados, a 8.037 (44,8%) se les realiza cateterismo cardíaco en las primeras 48 horas del evento isquémico. La mortalidad intrahospitalaria de la estrategia invasiva fue significativamente menor que el tratamiento médico (2,5% frente a 3,7%). Los pacientes a los que se les realizó una estrategia invasiva precoz representaban una población seleccionada, al ser los factores independientes de predicción más sólidos, asociados al tratamiento invasivo precoz: la asistencia cardiológica, la edad más joven, la ausencia de insuficiencia renal, ausencia de insuficiencia cardíaca previa, o a la llegada al hospital y una frecuencia cardíaca más baja. Pudiéndose concluir finalmente, que a pesar de la disminución de la mortalidad conseguida con la estrategia invasiva precoz, ésta no se realiza en la mayoría de los pacientes, quedando reservada para subgrupos de menor comorbilidad, y para aquellos atendidos por los cardiólogos. Abstract: To clarify the species status of sturgeon from rivers of the Iberian Peninsula, eight molecular markers (4 nuclear and 4 mitochondrial) have been analysed in different specimens from historical museum samples and prehistoric samples from archaeological sites. These analyses indicate that one of these specimens (UGP captured in the Guadalquivir River in the 19th century) is A. sturio, based on all the eight molecular markers, four of them used from the first time in this study. In previous analyses based on 5 genetic markers, our group assigned two specimens captured in this river in the 1970-80s (EBD8173 and EBD8401) to the species A. naccarii, suggesting the presence of this species in the Iberian Peninsula. In this work, this conclusion is drawn after successfully obtaining a mitochondrial marker in a very old scute from a prehistoric site (Acinipo, about 1500 BC, from the Guadalquivir River basin). On the other hand, in the specimen EBD8174 captured in the Guadalquivir in 1975, we have obtained two new mitochondrial markers confirming that it can be considered A. sturio for all the mitochondrial markers, but nuclear ones identify it as A. naccarii. Finally, two very old samples (Nerja E-VI and Nerja N/62-63) were not successfully characterized by any molecular markers. Some aspects and consequences of our results are discussed, such as the origin of the “mosaic” specimen EBD8174 and, above all, the native status of A. naccarii in historic and prehistoric times in the southern Iberian Peninsula.

Abstract: objective. describe the frequency of high degree atrioventricular block (hdavb) in patients with unstable angina (ua), analyze the variables associated with their appearance and evaluate whether hdavb is independently associated with increased mortality or increased length of icu stay. design. retrospective descriptive study of patients with ua included in the ariam registry. setting. icus from 129 hospitals in spain. patients. from june 1996 to december 2003 a total of 14,096 patients were included in the ariam registry with a diagnosis of ua. main variables of interest. variables associated with the development of hdavb, variables associated with the mortality of patients with ua, variables associated with the length of icu stay of patients with ua. results. hdavb frequency was 1%. development of hdavb was independently associated with the killip classification and the presence of sustained ventricular tachycardia or ventricular fibrillation. crude mortality of patients was significantly increased when hdavb was present (9% versus 1%, p < 0,001). when adjusted for other variables, hdavb was not associated with increased mortality. development of hdavb in patients with ua was independently associated with an increase in the length of icu stay (adjusted odds ratio 1.89: 95% confidence interval: 1.33-5.69). conclusions. patients with ua complicated with hdavb represent a high-risk population with an increased icu stay.

Abstract: RESUMEN El texto revisa las relaciones entre tecnología, habitar y contexto sociocultural a lo largo del ciclo de lo moderno, indagando en la situación actual de los mismos. Si lo que caracteriza el pensamiento contemporáneo es la comprensión y la revisión del periodo de la modernidad, debemos extender ese procedimiento a la arquitectura si pretendemos integrar las herramientas y las soluciones habitacionales y constructivas de nuestra tradición disciplinar en el presente. Trata de realizar un repaso genealógico que quiere recordar la utilidad y el sentido que términos como caba a, máquina o caravana, pueden tener para la comprensión de la vivienda prefabricada contemporánea. También se procura el acercamiento a dos casos concretos, la casa de los Eames y el pabellón solar de los Smithson, para explorar la superación de algunas de los problemas básicos detectados en la temática de estudio. Por último, y a modo de conclusiones, se abordan algunas líneas de dilatación en los modos de hacer y pensamiento contemporáneos. SUMMARY The paper reviews the relationship between technology, living and socio-cultural context, during the modern cycle, delving into their current states. If understanding and review of the period of modernity characterize contemporary thought, we must extend this procedure to architecture if we expect to integrate the tools and housing and construction solutions of our disciplinary tradition into the present. A genealogical review is made to recall the utility and meaning that terms such as cabin, machine or caravan may have for understanding the contemporary prefabricated home. Also, two specific cases are approached, the Eames House and the Smithson's Solar Pavilion, in an effort to explore how some of the basic problems identified in the subject matter are overcome. Finally, some aspects of contemporary methods of production and thought are expanded upon as a conclusion.
