Abstract: extraction of the permanent canine surprises many traditional orthodontists who believe that negative functional and unaesthetic results may arise from the procedure. although esthetics is the major concern of layperson people, the occlusal aims of the orthodontic treatment are esthetics, occlusal function and periodontal health, with or without canines. the current paper describes the possibility of extracting the canine instead of the premolar in an orthodontic treatment with teeth extraction. in such a situation, which is not easily accepted by orthodontists and patients, the canine is eliminated but not its cusp function in lateral mandibular movements, thus maintaining the gnathologic functional concepts.

Abstract: species of the genus aspergillus, mainly a. flavus strains, occuring in the araraquarense region (state of s?o paulo, brazil), were isolated and their ability to produce aflatoxin was studied. the isolation was conducted following routine methods in microbiology. the cultures were obtained from samples of peanuts collected in the 'rainy season" and in the "dry season". after the isolation the cultures were classified as follows: 102 a. flavus; 4 a. oryzae var. effusus; 2 a. oryzae; 2 a. parasiticus and 1 a. ochraceus. during the work, 4 cultures were lost, remaining 107 cultures. the cultures were then tested in their ability to produce aflatoxin, which was extracted from the mycelium and from the culture medium. the metabolites were separated by thin layer chromatography using the solvent benzene-ethyl acetate-ethanol and quantified under long wave u. v. light. from 107 cultures of aspergillus only 33 (31%) produced aflatoxin; seven of those produced it only in the culture medium, while two, only in the mycelium. the production of aflatoxin was much higher in the mycelium than in the culture medium; some of them produced very high quantities, up to 400 p.p.m. of b1 and 300 p.p.m. of g1. from the 33 cultures producing aflatoxin, 32 belonged to the a. flavus species and only one to the a. oryzae. previous studies when high yields of aflatoxin g1 as related to b1 were reported in that region, were confirmed in the present work.

Abstract: as a first step in a program of evaluation of ground beef microbiological quality in the city of piracicaba, sp, forty samples of ground beef were taken at two kinds of retail markets (meat shop and supermarket), related to an old and a new system of meat merchandising. the samples were taken weekly in the morning and in the afternoon, during a two month period. total bacterial counts were determined by a procedure recommended by the american public health association and plates were incubated at 21oc (72 hours) and at 32oc (48 hours). the results can be summarized as follows = 1) total counts were generally high, with several samples in the range 107 to 109 cells/gram; average counts were 6.9x 107 cells/gram (21oc) and 2.5xl07 cell/gram (32oc). 2) incubation at 21oc resulted in total counts generally higher than those found at 32oc, although there was no statistically significant difference between the two incubation temperatures. 3) total counts were found to be higher in samples from the small shop (old system) than in samples from the supermarket (new system); the difference was related to higher counts in the morning samples taken at the former retail market.

Abstract: in brazilian supermarkets, ground beef may sometimes be prepared using meat that has been previously frozen and stored to be consumed during periods of the year when there is a decrease in the amount of fresh meat available. the results of this study suggest that a modified resazurin test, previously described by the authors for the evaluation of ground beef microbiological quality, can be used in different periods of the year, even if frozen meat is involved.

Abstract: the authors present a series of seven case reports of localized pigmented villonodular synovitis (lpvns) treated by arthroscopy resection between june of 1994 and october of 2001. at the baseline evaluation, symptoms ranged from diffuse pain to localized swelling of the knee, with or without mechanical blockage of the joint. magnetic resonance helped in localizing the lesion and in the follow up evaluation. diagnosis was confirmed through anatomicopathological examination. by the end point evaluation, none of the patients presented recurrence of the pre-operative symptoms referred before. magnetic resonance imaging also showed absence of symptoms. despite of the small number of patients, typical of this affection, the authors believe that arthroscopic excision of the lpvns is an effective method, with low morbidity, providing remission of the symptoms and a low recurrence potential.

Abstract: total bacterial counts (32°c and 5°c) and most probable numbers (mpn) of coliform organisms and e. coli were determined for samples of three retail bovine meat cuts and cutting and boning equipment three supermarkets. in the oldest of these, total counts were higher on meat cuts prepared in the cutting room than on meat boned in the packing plant. total counts were generally high on the equipment (higher for incubation at 32°c), with the wooden surfaces (tables, cutting boards) showing significaitly higher numbers than the metallic ones (eletric saw, knives). no differences could be found between counts on "clean" equipment and counts on equipment being-used. coliform organisms were generally present; when meat samples were tested for this group of bacteria and for e. coli, it was found that 96% of the samples were positive for both.

Abstract: objective: evaluate the clinical and functional behavior of the ventricular and atrioventricular stimulation modes in the elective replacement of pulse generator in patients with chagasic cardiopathy and atrioventricular block. methods: twenty-seven patients under ventricular and atrioventricular stimulation were comparatively evaluated at the beginning of the study, and alternately in ventricular and atrioventricular modes in two 90-day phases, with regard to: the clinical behavior evaluated according to quality of life and functional class, and the functional behavior evaluated by transthoracic echocardiography and the six-minute walk test. the statistical analysis was performed with patients at baseline, and under ventricular and atrioventricular modes, using the chi-square test and the repeated measures analysis of variance, and taking into consideration a 0.05 level of significance. results: the mean quality-of-life scores were: functional capacity (vvi 71.3+/-18.2 , ddd 69.3+/-20.4); overall health status (vvi 68.1+/-21.8, ddd 69.4+/-19.4) and vitality (vvi 64.8+/-24.6 , ddd 67.6+/-25.5); on echocardiography: lvef (vvi 52.5+/-12.8 , ddd 51.8+/-14.9), lvdd (vvi 53.0+/-7.7 , ddd 42.4+/-7.8), la (vvi 38.6+/-5.4 ddd 38.5+/-5.1), and in the six-minute walk test: distance walked (vvi 463.4+/-84.7, ddd 462.6+/-63.4). there were four cases of complications, three of them associated with the change in stimulation mode. conclusion: this study showed no differences between the two stimulation modes in the clinical behavior assessed by quality of life and functional class, and in the functional behavior, evaluated according to the ecochardiographic findings and the six-minute walk test.

Abstract: objective: to study the prevalence and natural evolution of arrhythmic events and conduction disturbances in myotonic dystrophy; to correlate the genetic defect with cardiovascular findings; to assess cardiac mortality, frequency, and predictive factors of sudden death; to correlate the severity of the neuromuscular and cardiac involvement; and to define the role of the electrophysiological study (eps), in myotonic dystrophy. methods: periodic clinical assessment and the following tests were performed in 83 consecutive patients with a mean follow-up of 42±30.63 months: complementary examinations, genetic tests, electrocardiography, echocardiography, and holter; electrophysiological study was performed in 59 cases. results: atrial tachyarrhythmia was observed in 10 (12%) patients, nsvt in 14 (17%), first-degree avb in 24 (29%), lbbb in 19 (23%), and rbbb in 13 (16%). symptoms, an increase in the pr interval, qrs enlargement, lvef < 60%, and age were predictive factors of death. nine patients died (4 sudden deaths; 2 due to heart failure; 3 due to other causes). electrophysiological study: h-v interval >70 ms in 34% and > 100 ms in 11% (postprocainamide). conclusion: the prevalence of arrhythmic events and conduction disturbances ranged from 50% to 80% after 6 years, and did not correlate with the genetic defect. atrial flutter was the most common sustained arrhythmia. cardiac involvement increased as the neuromuscular disease became aggravated, but progression of the cardiac involvement was more rapid than that of the neuromuscular disease. overall mortality was low (11%) and sudden death occurred in half of the cases. the eps identified a group at risk for pacemaker implantation.

Abstract: background: echocardiography is a useful method for screening and assessing response to cardiac resynchronization therapy (crt). 3d echocardiography has already established its role in the evaluation of ventricular volumes and ejection fraction (lvef) with excellent correlation of results when compared with mri. objective: to compare the evaluation of ventricular volumes (lvdv, lvsv), lvef, and lv mass before and after crt by 2d echocardiography and three-dimensional echocardiography. methods: we evaluated 24 patients with hf fciiiouiv (nyha), sinus rhythm qrs > 150 ms, during an optimized therapy for hf undergoing crt. we conducted electrocardiogram (ecg), clinical evaluation, 2d and 3d echocardiography before, three and six months after crt. the comparison between the techniques was performed using pearson's correlation (r). results: at baseline, the correlation between methods was 0.96 for evaluation of lvdv, 0.95 for evaluation of lvsv, 0.87 for lvef and 0.72 for lv mass. after three months of crt, the correlation between methods for analysis of lvdv was 0.96, 0.95 for lvsv, 0.95 for lvef, and 0.77 for lv mass. after six months of crt, the correlation between 2d and 3d echocardiography for analysis of lvdv was 0.98, 0.91 for lvsv, 0.96 for lvef, and 0.85 for lv mass. conclusion: this study reported was a reduction of lvdv, lvsv, besides improvement in lvef after crt. there was an excellent correlation between the 2d and 3d echocardiography for evaluation of ventricular volumes and lvef, and a good correlation between methods for evaluation of left ventricular mass before and after crt.

Abstract: cardiac involvement is present in more than half of the patients with systemic lupus erythematosus (sle). however, studies on the prevalence of arrhythmias in this disease and laboratorial correlations predictive of their development do not exist. it seems possible that the classic second mortality peak is related to arrhythmias, mainly due to the sudden nature of those deaths. autoimmune process, atherosclerotic complications, and even adverse effects secondary to the treatment of this disorder (chloroquine cardiotoxicity) seem to be the main pathophysiological mechanisms of those disturbances. the direct participation of autoantibodies, such as anti-ro/ssa and anti-rnp, is still controversial. all types of av blocks (avb), intraventricular conduction disturbances, and sick sinus syndrome have already been described in this disease. tachycardias identified more often include sinus tachycardia, atrial fibrillation, and atrial ectopies. long qt syndrome and the presence of late potentials in signal-averaged ecg have also been described in sle patients and they can be associated with increased mortality rates. cardiac toxicity secondary to chloroquine could be responsible for several types of arrhythmias. however, few cases of fascicular block evolving to complete av block have been described. since these adverse effects are rarely reported, the beneficial anti-inflammatory and immune properties support the use of antimalarials in this disease. a complete cardiologic evaluation should include the conduction system and must be carried out in all sle patients to identify arrhythmias, therefore preventing symptoms and also sudden cardiac death.
