Repercusión del acceso vascular sobre la calidad de vida de los pacientes en tratamiento con hemodiálisis Vascular access impact on quality of life of patients treated with hemodialysis

Abstract: En todo paciente diagnosticado de insuficiencia renal crónia es imprescindible facilitar un tratamiento adecuado y evitar complicaciones derivadas. Solucionada esta premisa la atención se dirige a ayudar a estos pacientes a vivir lo mejor posible con su patología, en definitiva, mejorar su calidad de vida. Si el acceso vascular es clave para un buen tratamiento dialítico, podríamos pensar que sería un factor que pudiera influir en la calidad de vida del enfermo renal. Objetivo: Determinar en qué medida influye el acceso vascular sobre la calidad de vida relacionada con la salud de los pacientes en hemodiálisis. Metodología: Estudio descriptivo transversal en una población de 38 pacientes en programa de hemodiálisis. Para la evaluación del estado de salud percibido se utilizó la escala visual analógica del Euroqol 5D. Se solicitó a los pacientes que indicasen en la misma su estado de salud en el momento actual y sin los inconvenientes e incomodidades que les supone su acceso vascular. Resultados: Aquellos pacientes que se dializaban a través de fístula arteriovenosa interna alcanzaron una puntuación media en la EVA de 58,75±20,31 frente a 60,56±17,39 puntos que alcanzaron los portadores de catéteres venosos centrales, no resultando estadísticamente significativo. Conclusiones: Según nuestro estudio, no existe una influencia significativa del acceso vascular sobre la calidad de vida percibida por el paciente. Los mayores inconvenientes e incomodidades relacionados con el acceso vascular se producen en los portadores de catéteres venosos centrales que llevan más de 1 a o en hemodiálisis. A Nanoconjugate Apaf-1 Inhibitor Protects Mesothelial Cells from Cytokine-Induced Injury

Abstract: Background Inflammation may lead to tissue injury. We have studied the modulation of inflammatory milieu-induced tissue injury, as exemplified by the mesothelium. Peritoneal dialysis is complicated by peritonitis episodes that cause loss of mesothelium. Proinflammatory cytokines are increased in the peritoneal cavity during peritonitis episodes. However there is scarce information on the modulation of cell death by combinations of cytokines and on the therapeutic targets to prevent desmesothelization. Methodology Human mesothelial cells were cultured from effluents of stable peritoneal dialysis patients and from omentum of non-dialysis patients. Mesothelial cell death was studied in mice with S. aureus peritonitis and in mice injected with tumor necrosis factor alpha and interferon gamma. Tumor necrosis factor alpha and interferon gamma alone do not induce apoptosis in cultured mesothelial cells. By contrast, the cytokine combination increased the rate of apoptosis 2 to 3-fold over control. Cell death was associated with the activation of caspases and a pancaspase inhibitor prevented apoptosis. Specific caspase-8 and caspase-3 inhibitors were similarly effective. Co-incubation with both cytokines also impaired mesothelial wound healing in an in vitro model. However, inhibition of caspases did not improve wound healing and even impaired the long-term recovery from injury. By contrast, a polymeric nanoconjugate Apaf-1 inhibitor protected from apoptosis and allowed wound healing and long-term recovery. The Apaf-1 inhibitor also protected mesothelial cells from inflammation-induced injury in vivo in mice. Conclusion Cooperation between tumor necrosis factor alpha and interferon gamma contributes to mesothelial injury and impairs the regenerative capacity of the monolayer. Caspase inhibition attenuates mesothelial cell apoptosis but does not facilitate regeneration. A drug targeting Apaf-1 allows protection from apoptosis as well as regeneration in the course of inflammation-induced tissue injury.

Abstract: the objective of the present article is to illustrate two current perspectives of child development that are inclusive rather than exclusive -michael tomasello (cognitive) and daniel stern (social-subjective)-, in the sense that they integrate the discoveries of the early skills of the baby -carried out as of 1970-80- to their explanation, without giving up the particular theoretical paradigm they come from. they recognize these early skills and the fundamental role of the various factors involved in development -the innate, the environmental (social and cultural)-, different research methods and the limitations of a reductionist view. in addition they postulate that the dialog between the different theoretical perspectives is necessary to achieve a description of the infant as a unit.

Abstract: This paper is concerned with the debate on the secularizacion of parishes in the Philippines during the second half of the 19th century, as analyzed from the perspective of the Archipelago. The essay analyzes both the arguments put forth in the debate and the facts that determined the revival of a problem inherited from the 18th century, the period in which the ultimate roots and the explanation for such disputes have to be sought. Written on the basis of one of the numerous libels published at the time, the essay forms part of the larger project whose aim is to study the history of the parishes in the Archipelago from the second half of the 18th century to the end of the Spanish rule. Este trabajo aborda el debate sobre la secularización de parroquias en Filipinas en la segunda mitad del siglo XIX, desde la perspectiva del propio archipiélago. En él se analizan tanto los argumentos esgrimidos como los hechos concretos que condicionaron el resurgir de un problema heredado del siglo XVIII, contexto en que dichas disputas alcanzan su plena explicación. Elaborado a partir de uno de los muchos alegatos publicados en estos a os, este estudio forma parte de un proyecto más amplio cuyo objeto es estudiar la historia de las parroquias del archipiélago desde la segunda mitad del siglo XVIII y hasta el fin del dominio espa ol.

Abstract: This article studies the history of the Santa Rosa Beatas' House, also called Mother Paula′s, since its foundation in 1750 to its final consolidation in 1789. Along those thirty nine years the centre, born out of the generosity of a Spanish lady, confronted a series of difficulties until it finally obtained royal support and official recognition by the city of Manila. The study is based on unpublished documentation from the Archivo General de Indias ofSeville, and it aims at contributing to the knowledge of the nature and functioning of this type of communities, the history of which is still to be made. En este trabajo se estudia la historia del beaterío de Santa Rosa, o de la madre Paula, desde su fundación en 1750 y hasta su definitiva consolidación, en 1789. A lo largo de estos treinta y nueve a os, el centro, nacido de la generosidad de una espa ola, hizo frente a importantes dificultades hasta conseguir finalmente el apoyo real y el reconocimiento de la ciudad de Manila. El estudio se ha realizado sobre documentación inédita del Archivo General de Indias de Sevilla, y supone una aportación al conocimiento de la naturaleza y funcionamiento de estas comunidades, cuya historia está aún por hacer.
