Abstract: a centraliza??o político-administrativa é bastante grande no estado brasileiro, heran?a das condi??es históricas e das políticas implementadas recentemente no país. acompanhando o processo de transi??o para democracia nos anos 80, os debates apontaram a centraliza??o como o maior obstáculo ao desenvolvimento econ?mico e à implanta??o de políticas públicas baseadas no direito de cidadania. no setor saúde, o processo de centraliza??o a partir da década de 20 também foi notório, tanto na saúde pública quanto na assistência médica previdenciária. as dificuldades para a descentraliza??o s?o inúmeras no ambito geral do estado e também no setor saúde, incluindo aí as rea??es do próprio funcionalismo com respeito às propostas e projetos de descentraliza??o e unifica??o das administra??es públicas. em fun??o disso desejamos conhecer, em um primeiro momento, a composi??o desta categoria social, seu desenvolvimento na sociedade brasileira e as rela??es que vem estabelecendo no interior do aparelho de estado. assim, este artigo se prop?e a apresentar algumas tendências do crescimento do funcionalismo público no país e do segmento inserido na área de saúde na década de 80. trabalhamos com dados do ministério da saúde integrantes do sistema de informa??es organizado por roberto passos nogueira para os diversos estados, entre os quais destacamos o rio de janeiro, paraíba, rio grande do sul e s?o paulo.

Abstract: studies on nursing assistants are rare, even on nursing assistants in family health teams. the article is based on a study that intended to determine the profile of nursing assistants in the family health program in the metropolitan area of s?o paulo, to identify their level of satisfaction with work, to seek suggestions for improving the family health program, and to assess their expectations as to their professional future. in 2004, the department of primary health care and family health program of the municipal health secretariat of s?o paulo sent out a questionnaire to the members of the city's teams. the analysis of the answers of 901 nursing assistants received support from unesco. the major results presented in this article are the predominance of women, young individuals and individuals only with secondary school education in the teams; most of whom had been through basic training for the family health program; and only a small number of whom had completed technical courses. nurses are satisfied with work relations and work processes, but dissatisfied with work conditions. the most mentioned reasons for having chosen the profession were solidarity and the care for other people. on the other hand, the reasons for having entered the family health program were the fact that it was an alternative in the job market and because of society's appreciation of the field. most of them intend to continue training, later completing undergraduation in nursing and specializing in public health or family health.
Perfil dos trabalhadores da aten??o básica em saúde no município de S?o Paulo: regi?o norte e central da cidade

Abstract: research conducted by s?o paulo's observatory of human resources in health in 2009. it investigated 87 unidades básicas de saúde (ubs - primary care units) located in the north and central regions of the city of s?o paulo. objective: to survey the profile of the ubs workers. the ubs were organized under different models: health programs, family health and mixed. methodology: information about the universe of 4,968 workers was surveyed in the smssp (municipal health department of s?o paulo) and interviews were conducted with a sample of 621 workers through the etac system (computer-assisted telephone interview). results: 80.7% of the interviewees were women, but only 56.4% among the physicians; 50.5% were aged 21-40 years and 49.1% were aged 41-60 years; 61.4% were born in the city of s?o paulo; a great deal of them presented a higher level of schooling than what was required to perform the functions; in the superior level, 13.9% were physicians; in the middle level, 21.3% were nursing assistants; and in the operational level, 20.7% were acs (community health agents); 40.1% had been working at the ubs for more than five years, 12.9% for more than 10 years and 6.3% for more than 20 years; 64.7% worked only at the ubs, 46.7% had had a previous experience in the hospital area; 92.3% of those who worked at the health programs ubs were statutory civil servants, and 77% of those of the family health ubs were hired through brazil's labor laws, by the partner institutions of the smssp. conclusions: predominance of female workers; process of consolidation and renewal of the quota of workers; the majority was born in the city of s?o paulo and had a level of schooling that was higher than what was required for the function; predominance of doctors, nursing assistants and community health agents; outsourced contracts of employment.

Abstract: Neste artigo, apontam- se inicialmente algumas quest es fundamentais para o entendimento das rela es entre ciências sociais e saúde desde o inicio do século XX. Posteriormente, discute-se a presen a das ciências sociais na saúde pública e a constitui o do campo da saúde coletiva, com a cria o da Associa o Brasileira de Pós- Gradua o em Saúde Coletiva - ABRASCO, ao final dos anos 70. Aborda-se a defini o de três eixos de pesquisa do novo campo - Política de Saúde e Organiza o de Servi os de Saúde, Condi es de Vida e Saúde, Recursos Humanos na Saúde - e dois temas emergentes de pesquisa: trabalho e saúde, violência e saúde. Nos últimos tópicos, dedicado aos desafios para a produ o do conhecimento na interface entre ciências sociais, saúde pública e saúde coletiva, a autora apresenta suas considera es sobre alguns pontos abordados por Amélia Cohn na Mesa Redonda promovida pela Revista Saúde e Sociedade em 20 de mar o de 2012 e os novos desafios, eixos de pesquisa e temas emergentes para a saúde coletiva e para as ciências sociais nesse campo, no contexto dos anos 2000. In this article, we firstly point out some fundamental questions to understand the relationship between Social Sciences and Health from the beginning of the 20th century.The presence of Social Sciences in Public Health is discussed, as well as the constitution of the field of Collective Health, with the creation of the Associa o Brasileira de Pós Gradua o em Saúde Coletiva- ABRASCO ("Brazilian Association of Post Graduation in Collective Health"), in the late 70's; the definition of three research lines in the new area: Health Policies and Health Service Organization, Life and Health Conditions, Human Resources in Health; and two emerging research themes: Work and Health, and Violence and Health. In the last topics, dedicated to the challenges in knowledge production at the interface of Social Sciences, Public Health and Collective Health, the author presents her considerations on some points discussed by Amélia Cohn by occasion of a Round Table organized by Revista Saúde e Sociedade ("Health and Society Magazine") on March 20th, 2012, and the new challenges, research lines and emerging themes for collective health and for social sciences in this area, in the context of the years 2000.
Universalization of the access to the Unified Health System in Brazil: challenges

Abstract: Introduction: Twenty-five years after the promulgation of the 1988 Federal Constitution in Brazil, whichdefined a “Sistema único de Saúde” (‘SUS’, or “Unified Health System” - UHS) for the country, many advanceshave occurred, but many challenges still remain. Objective: To present the theoretical and practical bases of the“Aten o Primária à Saúde” (‘APS’, or “Primary Health Care” - PHC), in the Brazilian and Sao Paulo Stateexperiences; to discuss the service “access” category, entrances to UHS and challenges for the universalizationof the access based on PHC. Methodology: Results analysis of the researches that have investigated the differentBrazilian and Sao Paulo State situations. Results: Different organization models coexist in PHC: the “A esProgramáticas” (or “Programmatic Actions” - 1970) and the “Saúde da Família” (or “Family Health” - 1996).Results that are slightly more positive appear in the “Family Health” strategy: health indicators, staff, completenessof approach, and quality of assistance and access. Difficulties: different degrees in the deployment of the programand staff in different municipalities; home visits only by CHA (community health agent); constant human resources (HR) shifts; difficulties in the follow-up to other levels of assistance; inequality among municipalities;fragmentation and different development levels of the local systems and health regions; under-financing of the system, especially at out-patient level and diagnosis support. Conclusion: Strategic importance of Human Resource matters: relation between services and education institutions, amount of professionals and workers,distribution, labor market, formation, profile, expectations and resistance.

Abstract: this article presents some features of the intellectual production developed into the disciplines of social and human sciences ending health, during the 90's and the years of 2000 and 2001 in s?o paulo. we initiated by identifying these works in the libraries of universidade de s?o paulo, universidade estadual de campinas, pontifícia universidade católica de s?o paulo and escola de administra??o de empresas of funda??o getúlio vargas de s?o paulo. the 258 mastering dissertations and doctoring thesis were found and classified according to: academic institution, year of conclusion, degree of postgraduation, disciplines, thematic approaching and advisers. those teachers that oriented most of these works were interviewed with the purpose of knowing their experiences and thinking about the inclusion of health as an investigation matter in the programs of post graduation among the disciplines of sociology, anthropology, politics, geography, history, administration, law, economics, education, social psychology and social service.

Abstract: Este artigo apresenta algumas características da produ o intelectual desenvolvida em disciplinas de pós-gradua o das ciências sociais e humanas tendo como objeto a saúde, durante os anos 90 e nos anos de 2000 e 2001 em S o Paulo. Partimos da identifica o desses trabalhos nas bibliotecas da Universidade de S o Paulo, Universidade Estadual de Campinas, Pontifícia Universidade Católica de S o Paulo e Escola de Administra o de Empresas da Funda o Getúlio Vargas de S o Paulo. As 258 disserta es de mestrado e teses de doutorado encontradas foram classificadas de acordo com: institui o de ensino, ano de conclus o, nível de pós-gradua o, disciplinas, temáticas abordadas e orientadores. Os professores que orientaram o maior número desses trabalhos foram entrevistados com o intuito de conhecermos suas experiências e reflex es sobre a inser o da saúde como objeto de investiga o nos programas de pós-gradua o das disciplinas de antropologia, sociologia, política, geografia, história, administra o, direito, economia, educa o, psicologia social e servi o social.
Integralidade e Aten??o Primária à Saúde: avalia??o na perspectiva dos usuários de unidades de saúde do município de S?o Paulo

Abstract: in the city of s?o paulo, the primary health care (phc) outpatient clinics function based on the models of the family health program (fhp) and health program. both models involve integrality issues. the aim of this study is to assess the integrality achievement in phc outpatient clinics located in the city of s?o paulo, based on users' opinion. the categories were: access to first contact, continuity, coordination, services catalogue, community orientation and family orientation. the two models of services organization at the phc outpatient clinics in the city were compared using the portuguese translated version of the primary care assessment tool (pcatool). the adapted pcatool was administered to a sample of users of the two phc models. the main results indicated good performance in the categories of first contact, services catalogue and coordination. family orientation, community orientation and accessibility received the worst evaluation. significant differences between the two models involve better medical records, better community-oriented activities, lower number of referrals to specialized care and more adequate working hours in the fhp model. the integrality issues that were best evaluated were those closely related to individual health care, while the issues concerned with services' organization and culture received the worst scores.
Políticas sociais: desigualdade, universalidade e focaliza??o na saúde no Brasil

Abstract: this article aims to contribute to the debate concerning social policies in health and the elaboration of viable strategies for organizing health services, extending them to areas that are not currently covered and consequently, transforming present conditions of population's health. having these goals in mind, it proposes a reflection on the significance of current policies and their repercussions with respect to the conceptions of universality and focalization. the relevance of health concepts and practices, established over the last few years, towards establishing a decrease in the problems of inequity and poverty is also discussed. this analysis is based on a reflection concerning the experience of implementing the family health program in the central area of the city of sao paulo, its repercussions as well as the necessary adjustments made. this, in turn led to broader queries with respect to the characteristics and qualities that should constitute such a program according to the specificities of each region of the country. the paper concludes by affirming that the principle of universality has contributed towards an increase in access to health services, but does not create the conditions for establishing equity, for which a reformulation and adaptation of current health programs and policies are required.
