Achados vestibulares em usuários de aparelho de amplifica??o sonora individual

Abstract: purpose: to check vestibular findings in patients with sensoneural hearing loss, hearing aid users. methods: 20 patients (eleven females and nine males) aging from 39 to 85-year-old with bilateral sensorineural hearing loss, from moderate to severe degrees, were attended in a higher education institution evaluated by medical history, otological inspections, complete basic conventional audiological evaluations, acoustic impedance tests and vectoeletronystagmography. results: a) from the 20 evaluated patients, 18 (90%) showed tinnitus complaint, 15 (75%) dizziness complaint and eight (40%) headache complaint; b) there was a prevalence of alterations in the caloric test and in the peripheral vestibular system; c) the results of the vestibular exam showed alterations in 14 patients (70%) being, eight cases (40%) of peripheral vestibular irritative syndromes and six cases (30%) of peripheral vestibular deficiency syndromes; d) we verified significant difference between the vestibular exam results and how long the patient had use the hearing aid; e) from the five patients that did not related vestibular symptoms four (80%) showed alterations in the vestibular exam. conclusion: the importance and sensibility of the functional study regarding the balance system in this type of population must be emphasized because same disorders may occur in the vestibular exam in spite of symptom presence.
Ocorrência de perda auditiva induzida pelo ruído em carpinteiros

Abstract: purpose: to investigate the occurrence of noise induced hearing loss in carpenters, characterizing the hearing loss for age group, time of total exposure to noise and regular use of hearing protectors during the total exposure time. method: retrospective and descriptive study in a population of 80 construction carpenters, attended at a private clinic. 60 workers were analyzed, as data on medical history and record of audiometric testing. results: 49% of the workers shoed normal hearing, being 58% with normal bilaterally hearing thresholds and 35% with audiometric notch in 3 khz, 4 khz and/or 6 khz. 44% showed suggestive audiometric profile of nihl, of these 74% were classified as bilateral nihl and 19% as unilateral nihl. there were significant differences between the nihl group and normal to the age variables (p=0.001), as well as the total time of exposure to occupational noise (p=0,002). conclusion: the greater the age and length of employment as a carpenter were, the greater their hearing loss, mainly due to exposure to loud noise during the working day, and we also noted that measures taken in order to control the use of the auricular protector are insufficient to prevent hearing loss. therefore, we suggest active preventive measures in hearing health in this studied population, in the field of civil construction.

Abstract: purpose: to verify the effectiveness of vestibular rehabilitation exercises in the improvement of tinnitus and dizziness through an evaluation carried out before and after the administration of the dizziness handicap inventory (dhi) and the tinnitus handicap inventory (dhi) questionnaires, both adapted to the brazilian population. methods: six patients (two male and four female), with ages ranging from 43 to 70 years, were evaluated. the patients were submitted to the following procedures: anamnesis, otoscopy, vestibular evaluation through vectoelectronystagmography, and administration of the questionnaires, before and after vestibular rehabilitation, using the cawthorne and cooksey protocol. results: a) regarding the most related complaints, imbalance when walking (83,3%), headache (66,6%) and depression (66,6%), were observed; b) in the vestibular exam all the patients showed alterations in the caloric test, and the most frequent syndrome was the irritative peripheral vestibular (83,3%); c) two cases of irritative peripheral vestibular syndrome, two cases of irritative peripheral unilateral vestibular syndrome, one case of irritative peripheral bilateral vestibular syndrome and one case of peripheral deficient unilateral vestibular syndrome were verified in the vestibular exam; d) in the administration of the dhi, an improvement in the functional and emotional aspects were observed, with the physical aspect remaining unchanged; e) in the administration of the thi, an improvement was observed in all the aspects evaluated. conclusion: the protocol of vestibular rehabilitation used in the study, promoted a decrease in tinnitus and dizziness, hence improving the quality of life of the patients.

Abstract: OBJETIVO: verificar os achados vestibulares em pacientes com perda auditiva neurossenssorial usuários de aparelho de amplifica o sonora individual. MéTODOS: vinte pacientes, 11 do sexo feminino e nove do sexo masculino, com idades entre 39 e 85 anos, com perda auditiva neurossenssorial bilateral de grau moderado e severo foram atendidos em uma Institui o de Ensino Superior e submetidos a uma anamnese, inspe o otológica, avalia o audiológica, imitanciometria e ao exame vestibular por meio da vectoeletronistagmografia. RESULTADOS: a) dos 20 pacientes avaliados, 18 (90%) apresentaram queixa de zumbido, 15 (75%) queixa de tontura e oito (40%) queixa de cefaléia; b) houve predomínio de altera o na prova calórica e no sistema vestibular periférico; c) o resultado do exame vestibular esteve alterado em 14 pacientes (70%), sendo, oito casos (40%) de síndrome vestibular periférica irritativa e seis casos (30%) de síndrome vestibular periférica deficitária; d) verificou-se diferen a significativa entre o resultado do exame vestibular e o tempo de uso do aparelho de amplifica o sonora individual; e) dos cinco pacientes que n o referiram nenhum sintoma vestibular, quatro (80%) apresentaram altera o no exame. CONCLUS O: ressalta-se a sensibilidade e importancia do estudo funcional do sistema do equilíbrio neste tipo de popula o, uma vez que podem ocorrer altera es na avalia o labiríntica independente da presen a de sintomas. Abstract: OBJETIVO: investigar a ocorrência de perda auditiva induzida pelo ruído (PAIR) em carpinteiros, caracterizando a perda auditiva por faixa etária, tempo de exposi o total ao ruído e uso regular de protetores auditivos durante o tempo total de exposi o. MéTODO: estudo retrospectivo, descritivo, em uma popula o de 80 carpinteiros da constru o civil, atendidos em uma clínica particular. Foram analisados 60 trabalhadores, conforme dados obtidos na anamnese e ficha do exame audiométrico. RESULTADOS: 49% dos trabalhadores apresentaram audi o normal, sendo 58% com limiares auditivos normais bilateralmente e 35% com entalhe audiométrico em 3 kHz, 4 kHz e/ou 6 kHz. 44% apresentaram perfil audiométrico sugestivo de PAIR, destes 74% foram classificados como PAIR bilateral e 19% como PAIR unilateral. Houve diferen a estatística significante entre os grupo PAIR e Normal em rela o às variáveis idade (p=0,001), assim como o tempo total de exposi o ao ruído ocupacional (p=0,002). CONCLUS O: quanto maior a idade e o tempo de profiss o como carpinteiro, maior é a sua altera o auditiva, principalmente, devido à exposi o ao ruído elevado durante a jornada de trabalho, sendo também constatado que as medidas de controle pelo uso do protetor s o insuficientes para prevenir perdas auditivas. Portanto, sugerem-se medidas preventivas em saúde auditiva ativamente nessa popula o estudada, no ramo da constru o civil. Abstract: objective: to verify the efficiency of dichotic listening in senior citizens with peripheral auditory loss. methods: 30 patients were evaluated, all over 60 years and suffering from neurosensory auditory loss, during the period from march to november 2007, in a private institution in the city of curitiba, state of paraná. the following procedures were carried out: anamnesis, otorhinolaryngologic, audiologic and imitanciometric evaluations and an evaluation of auditory processing by the application of the staggered spondaic word test - (ssw). results: a) 70% of the patients presented alterations in at least one of the ears in competitive left or right conditions; b) the degree of alteration was from light to severe and the right ear presented a statistically significant advantage in relation to the left; c) the tendencies of answers obtained were: type a - 70%, order effect - 36,6%, auditory effect - 13,3% and inversions 6,6%; d) there was a significant difference in the tendency for ea (auditory effect) answers, comparing the american standard to the findings of the present study. conclusion: the efficiency of dichotic listening in senior citizens can be jeopardized due to peripheral auditory loss, and as such, loss of acoustic information has been observed, which reduces the probability of understanding speech. therefore, the evaluation of dichotic listening is an important tool in the improvement of the selection and adaptation of individual sound amplification aids so that their use is more meaningful.

Abstract: purpose: to check the effectiveness of vestibular rehabilitation exercises (rv) by means of an evaluation of a pre and post application of the dizziness handicap inventory (dhi) questionnaire (brazilian version). methods: eight patients were evaluated (three female and five male), in the age group varying from 48 to 71, referred from the paraná association of parkinson to the otoneurological laboratory of tuiuti university of paraná. the patients were divided in two groups and submitted to the following procedures: anamnesis, otorhinolaryngological evaluation, vestibular evaluation through vectoelectronystagmography (veng) and an application of dhi questionnaire (brazilian version) before and after rv, using cawthorne and cooksey (group a) and herdman (group b) protocols. results: a) regarding the otoneurological complaints referred to in the anamnesis, the prevalence of dizziness (100.0%), trembling (100.0%) and deviation during walking (75.0%) were observed; b) in the vestibular exam, all patients (100.0%) presented alterations, with the largest incidence being the deficient peripheral vestibular syndromes (62.5%); c) there was significant improvement of the functional (p = 0,020470) and emotional (p = 0,013631) aspects after accomplishing the rv exercises using the cawthorne and cooksey protocol and of the emotional aspect (p=0,007316) using herdman protocol. conclusion: comparing the two protocols used, a significant improvement of group a patients, submitted to the protocol of cawthorne and cooksey (p=0.0231), was confirmed.
Zumbido e ansiedade: uma revis o da literatura Tinnitus and anxiety: a literature review

Abstract: O zumbido, som percebido pelo indivíduo sem que haja estímulo externo, resulta da intera o dinamica de centros do sistema nervoso central, incluindo vias auditivas e n o auditivas. O resultado desta intera o, especialmente sistema límbico e sistema nervoso aut nomo, seria responsável pelo desencadeamento de associa es emocionais negativas e rea es de inc modo em pacientes com zumbido. A ansiedade, condi o fisiológica inerente ao ser humano, quando exacerbada gera um transtorno de humor, comprometendo o pensamento, o comportamento e a atividade psicológica. Se um estímulo interno ou externo for interpretado como perigoso ou amea ador, desencadeará uma rea o emocional caracterizada como estado de ansiedade. Este artigo tem como objetivo realizar uma revis o da literatura sobre a rela o do zumbido e da ansiedade. Em se tratando do zumbido, a forma como o som é percebido pode ser alterado pela ansiedade, agu ando assim a sensibilidade para detectar sons que parecem uma amea a em potencial, pois para muitos o zumbido é sin nimo de enfermidade grave. A etiologia do desenvolvimento da depress o e ansiedade pode estar relacionada ao zumbido. Muitos adquirem esse sintoma por problemas físicos e, consequentemente, desenvolvem a depress o e a ansiedade. Outros com graus diferentes de angústias adquirem o zumbido devido ao comprometimento emocional. Dessa forma, existe um vínculo entre o zumbido e problemas emocionais, mas nem sempre é fácil identificar o precursor. Estudos demonstram que pacientes acometidos pelo zumbido apresentam maior tendência ao suicídio, depress o e ansiedade. Além disso, referem o efeito aditivo da ansiedade e da depress o na qualidade de vida e no zumbido dos indivíduos. Tinnitus is a sound perceived without external stimulus, and results from the dynamic interaction of the centers in the central nervous system, including non-auditory and auditory ways. The result of this interaction, especially with the limbic system and autonomic nervous system, would be responsible for triggering negative emotional associations and distress in patients with tinnitus. Anxiety, a physiological condition inherent to human beings, when exacerbated, generates a mood disorder, affecting thinking, behavior and physiological activity. If an inner or outer stimulus is interpreted as dangerous or threatening, it may trigger an emotional reaction characterized as a state of anxiety. Abstract: purpose: to compare the results of the test referred to as voice detection lower thresholds, using a previously-recorded list of trisyllabic words and in live voice in individuals with flat configuration audiogram. methods: 60 subjects (120 ears) took part in the study, each with an audiometric exam within normal patterns and tonal audiometry at a flat configuration, without auditory incidents, in the age group going from 20 to 30. the test was given with a live voice and with recorded material, in an acoustically-managed booth, by means of an ac40 audiometer and with the aid of a linked cd player. results: voice detection lower thresholds were found to be 10 db better than the tritonal average in 35 ears (57% of the sample) when accomplished by live voice and in 10 ears (17%) when accomplished by recorded material; and were found to be 5 db worse than the tritonal average in one ear (3%) when accomplished by live voice and in 23 ears (38%) by recorded material. conclusion: a) when using recorded material for the establishment of ldv in flat configuration curves, one should consider intensity levels equal to the tritonal average or 5 db lower as standards of normality and b) a difference was revealed between the results obtained with recorded material and those from live voices, demonstrating the importance of using recorded tests for determining lower auditory thresholds in logoaudiometry.

Abstract: to describe a possible case of unilateral keratoconus in a patient with continual eye rubbing due to an ipsilateral nasolacrimal obstruction in a 17-year-old woman complaint of progressing low visual acuity in the left eye. presented with constant epiphora in the left eye throughout her life as a result of ipsilateral nasolacrimal obstruction, which required frequent wiping of this eye. based on ocular examination, the patient was diagnosed as having unilateral keratoconus in the left eye. as in other studies, keratoconus is thought to be associated with persistent eye rubbing. this patient represents a case of unilateral keratoconus, where there is a confirmed history of habitual eye rubbing due to ipsilateral nasolacrimal obstruction, thereby suggesting a possible causal relationship between eye rubbing and keratoconus.
