Prevention of Pre-Eclampsia and Eclampsia. A Systematic Review  [PDF]

Abstract:
Every pregnant woman needs continuous, timely and supportive care throughout during pregnancy for safe motherhood. The objective of this study was to analyze and evaluate the available medications and techniques for the prevention and treatment of pre-eclampsia and eclampsia. The standard methodology of systematic review without meta-analysis was followed and only RCTs and systematic reviews were included in the review. Three electronic data sources (PubMed/Medline, CINAHL, and Cochrane) were searched for studies, published between 1986 and 2016 on the prevention and control of pre-eclampsia and eclampsia. 47 studies were finally included in the review, of which 18 were systematic reviews and 29 were RCTs. Technologies and techniques used in the included studies for the prevention and control of pre-eclampsia and eclampsia are Magnesium Sulphate, Aspirin, Antioxidant (Vitamin C, E and Lycopene), Calcium supplementation, Chinese Herbal Medicine, physical activities, Nitric Oxide, Marine Food Oils, Low Salt Diet, Garlic, Plasma Volume Expansion, Low-dose Dopamine, Progesterone, Smoking, and Diuretics. Magnesium sulfate appears to be the most effective treatment which reduces the risk of eclampsia by more than 50%. However, its best dose and route are still controversial and need further research. The knowledge and experience of nurses in properly using the protocols and evidence-based interventions are necessary for the wellbeing of pregnant women.

Abstract: a study of maternal mortality over a period of 5 years (1975 to 1979) was carried out on the basis of data supplied by maternity and general hospitals in florianópolis (sc) and the santa catarina state health department. it was found 44 deaths (which 65.9% were not resident in florianópolis) and 43,380 liveborn (lb) and by using these data specific maternal mortality rates for type of death, chronological age, parity, place of residence and type of delivery were determined. the results showed that the maternal mortality rate was high, with an average rate of 10.14/10,000 lb for the period, with no tendency to reduction. the direct specific obstetric death rate showed fairly constant values for the period with an average of 7.61/10,000 lb. the age group which presented the highest age specific death rate was that of women between 35 and 39. the death rates were higher for women who lived in other places as compared with the capital. the highest death rate related to the type of delivery was found for women with cesarean sections, with a average rate 9.29/10.000 lb for the period. the average rate for normal delivery was 3.90/10,000 lb for the period.

Abstract: Este artigo aborda o tema: Indicadores de altas habilidades entre os reclusos do Centro de Atendimento Sócio-educativos do Município de Santo ngelo-RS. Tendo como finalidade identificar indicativos de altas habilidades entre os menores infratores, foram utilizados como instrumentos de coleta de dados Questionários. Instrumento de avalia o de habilidades dos alunos; Instrumento de indicadores de interesse; Instrumento de indicadores de alunos com habilidades; e Entrevista semi-estruturada. Os resultados indicam que eles mesmos, reclusos por haverem praticado atos considerados anti-sociais, n o reconhecem as altas habilidades como tal, n o as vislumbravam como algo que possa contribuir para sua melhor inser o em um meio social. As pessoas que com eles trabalham percebem estas habilidades, mas n o as reconheciam como algo a ser trabalhado como instrumento sócio-educativo para sua inclus o social.
The Contribution of Technological Approaches in Reducing Maternal Mortality: An Integrative Review  [PDF]

Abstract: Aim: To analyze the role of available health technologies and techniques in reducing the number of women dying during pregnancy and childbirth. Background: Health condition of women during pregnancy and childbirth is very sensitive. During this period, proper and timely care plays an important role in reducing maternal mortality. Design: Whitte more and Knal’s framework for integrative reviews was followed in the review. PUBMED/MEDLINE, CINHAL and COCHRANE were searched for published studies between 2008 and 2015. Studies included were systematic reviews, randomized control trials, prospective cohort study, literature review and descriptive studies. Methods: 28 articles were selected to be included in the review. An integrative review approach was followed to analyze data and draw conclusions. Screening of titles and abstracts along with data extraction was completed by two authors independently. Study quality is not reported because of the methodological difficulties. Data synthesis consists of writing descriptive summaries and thematic analysis of the key findings in the included articles. Results: The included studies were based on health technologies and techniques including different devices and tools, medication and drugs, massage techniques, immersion in water, intervention radiology, simulations and training for the health professional. Conclusion: There are numerous health technologies and techniques which are relatively simple to develop, effective to use and safe to apply with no adverse effects. These include simulations and training programs, educational and awareness programs, decision support tools and information systems. Besides these, there are technologies and techniques which show promising results with no or very little adverse effects. These include Active management of the third stage of labour, use of magnesium sulphate, uterine balloon tamponade, wearable cardiac defibrillator and non-pneumatic anti-shock garment. The government, hospitals and care providers should make policies to develop and implement such type of technologies and techniques.

Abstract: most public debate surrounding the hypothetical adverse effects of vaccines is an extension of the polemics, controversies, and discussions that have arisen within the medical field itself. examples include the adverse effects of the mercury contained in a number of vaccines, such as the one against measles-mumps-rubella (mmr), which can cause inflammatory bowel disease and autism; the link between the hepatitis b vaccine and multiple sclerosis; the polio vaccine, the sv40 virus, and cancer; reverse transcriptase in chicken embryo cells and the risk of infection by retrovirus; and vaccines as a cause of asthma and insulin-dependent diabetes. in some cases, the public health has clearly been harmed-in england, for instance, when alarming reports circulated about the risks of the dtp vaccine in the 1970s or, more recently, involving the mmr vaccine, likewise in england, and the vaccine against hepatitis b, in france. groups of activists against vaccines have also spread alarming stories through the media, especially the internet. the ones who gain from this situation are the people and lawyers who sue for millions in damages, whether or not their allegations have any scientific basis. this has brought further problems for the public at large, especially in countries like the united states, with the cost of vaccines rising to cover the cost of lawsuits and with a number of companies halting their vaccine production, thus contributing to crises of vaccine availability. brazil's national immunization program has been successful in dealing with the public aspects of adverse events by training the health professionals working in this sector, not just in technical aspects but also in social communication.

Abstract: this article presents the vision of managers and health professionals about the care of elderly victims of violence in rio de janeiro, showing the specificities of this attention in pre-hospital, hospital and rehabilitation areas, including mental health services. based on a qualitative methodology, 20 semi-structured interviews with managers and health professionals were produced. the analysis considered the perceptions about violence against elderly, and the specificities of attention provided by sus services to elderly victims. the state, the family and the social support organizations emerged as structuring categories of the network of care for elderly victims of violence. all attention areas presented insufficient care services or specific programs for the elderly victims of accidents and violence. also there was a precarious network of social support for care of dependent elderly. with few exceptions elderly are treated as anyone else in public health services, without the priority set by the statute of the elderly. the family is called to participate in the care for the elderly in all areas of care. abandonment, neglect, physical and psychological abuse against seniors cases were reported.

Abstract: A grande maioria das polêmicas públicas relativas a hipotéticos malefícios provocados pelas vacinas é extens o de controvérsias e discuss es que surgem no próprio meio médico. Exemplos s o os malefícios do mercúrio contido em várias vacinas, como a tríplice viral (MMR), provocando doen a inflamatória intestinal e autismo; a vacina contra hepatite B e esclerose múltipla; o vírus SV40 em vacinas contra poliomielite e cancer; a transcriptase reversa em células de embri o de pinto e riscos de infec o por retrovírus; vacinas como causa de asma, diabetes insulino-dependente. Em algumas situa es houve nítido prejuízo para a saúde da popula o, como aconteceu na Inglaterra após noticiários alarmantes sobre os riscos da vacina tríplice DTP na década de 1970 e, mais recentemente, com a vacina tríplice viral, contra sarampo, caxumba e rubéola, também na Inglaterra, e a vacina contra hepatite B, na Fran a. Além disso, grupos ativistas contrários à vacina veiculam notícias alarmantes através dos meios de comunica o, especialmente pela Internet. Beneficiam-se dessa situa o pessoas e advogados que, com ou sem fundamento científico para as suas alega es, buscam indeniza es milionárias, o que tem causado malefícios para a popula o, principalmente em países como os Estados Unidos, pelo aumento no custo das vacinas, para fazer frente aos processos judiciais, e pela desistência de produ o por várias empresas, contribuindo para crises de disponibilidade das vacinas. O Programa Nacional de Imuniza es do Brasil tem tido êxito em lidar com os aspectos públicos dos eventos adversos, através da capacita o dos profissionais de saúde que atuam no setor, tanto nos aspectos técnicos como na comunica o social.
