Abstract: O artigo tem como objetivo discutir a sala de aula como um cronotopo específico onde as rela es s o tecidas a partir de um tempo/espa o constituidor e constituinte dos sujeitos. Com essa concep o, discutimos as atividades de leitura e de escrita, na sala de aula de língua materna, tendo como referência as postula es de M. Bakhtin e o círculo sobre os gêneros discursivos.
Pancreatite aguda grave: resultados do tratamento cirúrgico em 68 pacientes

Abstract: background: to evaluate the results of surgical treatment of acute necrotizing pancreatitis, as well as its necrotizing alcoholic form. methods: patients were analyzed according to a prospective protocol. surgery was performed on 68 patients due to necrotizing acute pancreatitis and acutized chronic pancreatitis from years 1982 to 2000. patients were grouped into three categories: 1) biliary acute pancreatitis; 2) acute pancreatitis of unknown etiology; 3) necrotizing alcoholic pancreatitis. results: patients had the following clinical indications for laparotomy: 1) uncertain diagnosis (32.3%); 2) infected necrosis (60.3%); 3) necrosectomy (7.4%). the surgical findings were: infected necrosis (70.6%); biliary complicatios (20.6%) and large necrosis 1(7.4%). global hospital mortality was 38.2%. individual categories showed a mortality of 33.3% (biliary acute pancreatitis), 45.0% (undetermined acute pancreatitis) and 37.0% (necrotizing alcoholic pancreatitis). reoperations were indicated once or more times in 25 patients, and among these, the mortality was 40%. abscesses were the main cause of death (52.0%), and all the other complications that resulted in death invariably evolved to infection. conclusions: the indications of early surgical treatment for gallbladder stones, as well as when the diagnosis is not clear, should be reduced to very specific circumstances. reoperations are common in these patients and infection was the main immediate cause of death.

Abstract: background: to evaluate the indications, methods and results of the surgical treatment of 74 patients operated on for chronic pancreatitis complications. method: consecutive patients' charts were studied prospectively, during the years of 1971 to 2000. surgery was indicated for pain, billiary tract stricture, pseudocysts, pancreatic ascitis and pancreaticopleural fistulas. pancreaticojejunal anastomosis, billiary tract derivations and resections were performed. follow-up was achieved through ambulatorial appointments or by mail and telephone calls. results: of 270 patients listed with chronic pancreatitis, 74 (27.4%) were operated on. 97.7% of them were males. patients varied in age from 15 to 63 years old, with an average of 39.4 years for alchoolists and 33.1years for those with pancreatitis of other etiologies. the disease was caused by alchool in 68 patients (90.5%). the other causes were: hyperparathyroidism (1), hereditary pancratitis (1), retroperitoneal fibrosis(1) and in three patients the etiology was unknown. indications for surgery were: 1. pain in 44.6% of the patients; 2. billiary tract stricture (28.4%); 3.pseudocysts (12.2%); 4. pancreatic ascitis and pancreaticopleural fistulas (10.8%). derivations were performed in 50 patients (67.5%), and resections in 24 (32.5%). complications ocurred in eight patients with five hospital deaths related to abscesses, suture leaking and surgical bleeding. conclusions: there was less morbidity and mortality for derivations and than after resections.

Abstract: functional health literacy (fhl) is the degree to which individuals have the capacity to obtain, process and understand basic health information and services needed to make appropriate health decisions. this theme concerns researchers/health professionals/public policy makers. its interdisciplinary, relational and interactional nature indicates that fhl is influenced by health/educational systems, by the media, the family, and by work, community and policy-making spaces. fhl concepts are analyzed in light of the interaction user-professional-health care system, guided by the following questions: what is fhl? to what extent does fhl interfere in the communication among users, professionals and the health care system? what are its consequences to public health? measures to increase fhl should improve individual skills and humanized health services, enhancing their written and oral communications, and meeting users' needs/abilities.
Alcoholism and Strongyloides stercoralis: Daily Ethanol Ingestion Has a Positive Correlation with the Frequency of Strongyloides Larvae in the Stools

Abstract: Background Significantly higher prevalence of Strongyloides stercoralis has been reported in chronic alcoholic patients. The aim of this investigation was to report the prevalence of Strongyloides larvae in stools of chronic alcoholic patients with known daily ethanol intake. Methods From January 2001 through December 2003 the results of fecal examinations and the daily ethanol intake were retrieved from the records of 263 chronic alcoholic and from 590 non-alcoholic male patients that sought health care at the outpatients unit of the University Hospital C A Moraes. Alcoholic patients were separated into four groups, with 150g intervals between the groups according to the daily ethanol intake. Results (a) The frequency of Strongyloides was significantly higher in alcoholic patients than in control group (overall prevalence in alcoholic 20.5% versus 4.4% in control group; p = 0.001). Even in the group with a daily intake of ethanol equal to or less than 150g the prevalence was higher than in control group, although non significant (9.5%, versus 4.4% in control group; p = 0,071); (b) the prevalence of Strongyloides in alcoholic patients rises with the increase of ethanol intake (Pearson's Correlation Coefficient = 0.956; p = 0.022), even in patients without liver cirrhosis (Pearson's Correlation Coefficient = 0.927; p = 0.037). Conclusion These results confirm and reinforce the hypothesis that chronic alcoholism is associated with Strongyloides infection, which is in direct relationship with the severity of alcoholism, independently of the presence of liver cirrhosis.

Abstract: research objectives were to identify psychosocial factors that influence in the etiology of depression, to apprehend and to compare the social representations of depression elaborated by teenagers in gymnasium, in public and private schools from two northeastern brazil capital cities, being them natal/rn and teresina/pi. 505 subjects took part in this research, being 269 from teresina and 236 from natal. it was used as instrument the children depression inventory and words free association test (wfat). the sample from teresina/pi presented a symptomatology index higher than natal/rn's. about genres differences, it was verified that the masculine genre subjects present depressive signs more related to obedience and conduct problems, as the girls would develop more subjective signs. analysis of the data taken from wfat found out that the social representations from the analyzed subjects, in general, show unfitness from depressive subject in the psycho-affective and psychosocial inter-relationships.

Abstract: Objective: To evaluate the suitability of educational videos produced in Brazil that addressed aspects related to prevention and/or management of infant diarrhea according to content; language; graphic illustrations; stimulation for learning/motivation and cultural fit. Method: Documentary study, which evaluated six educational videos from an adapted and validated tool, the Suitability Assessment of Materials. Three nurses evaluated the videos. Data were processed using the Statistical Package for the Social Sciences, version 17.0, and presented in tables and graphics. Results: It was found that 4 (66.6%) of the educational videos were classified as appropriate, ranging from 60% to 63.3% of full approval, and 2 (33.3%) were assessed as higher, reaching up to 83.3% to 90% approval. Considering the socioeconomic and cultural contexts of the videos to the audience, they assigned notes ranging from 6 to 9 in a rating ranging from 0, the minimum score, to 10, the highest score. Conclusion: The educational videos were considered adequate, with positive results about the content on the prevention and management of infant diarrhea, being appropriate for the target audience. As a result, greater credibility is ratified as the use thereof, since these resources can contribute to health promotion and prevention of diarrhea in children.

Abstract: these guidelines are an up-to-date and comprehensive tool to aid health professionals in treating smokers, recommending measures and strategies for managing each case based on clinical evidence. written in a simplified and objective manner, the text is divided into two principal sections: evaluation and treatment. the sections both present comments on and levels of evidence represented by the references cited, as well as some proposals for the reduction of damage and for intervening in specific and still poorly explored situations, such as relapse, passive smoking, physician smoking, and tobacco use in specific environments.

Abstract: isolated and painless infraspinatus atrophy and weakness are described in two top-level volleyball players. emg revealed isolated denervation of the infraspinatus muscle. one athlete continued playing and his clinical features have not changed. the other recovered her muscle bulk and strength after stopping playing. these findings were attributed to intense activity of the shoulder joint, without any direct trauma. on clinical grounds, we did not consider these cases as true examples of entrapment neuropathy. pathogenesis was related to traction of the distal branch of the suprascapular nerve during the act of reception of the ball (?manchete?).

Abstract: the multiple-use of planted forests by the forest products industrial sector has been increased gradually. today, eucalyptus wood is used for nobler applications. however, some limitations, such as the presence of growth stresses, are responsible for great losses in the sector. growth stresses generate cracks in the tops of logs and boards followed by warping. in this context, the objectives of the present work were to evaluate the growth stresses in individual corymbia citriodora and eucalyptus urophylla with different diameter and height classes, by the longitudinal residual strain (lrs) nondestructive method, around the trees circumference. the average longitudinal residual strain, associated to the corymbia citriodora and eucalyptus urophylla growth stresses, were 0.106 and 0.092 mm, respectively. for corymbia citriodora, the diameter at breast height (dbh) had a negative significant correlation with the lrs, while the height did not show a significant correlation. for eucalyptus urophylla, a negative significant correlation between lrs and both variables, dbh and height, was observed. indeed, growth stresses were lower for greater dbh and heights.
