Abstract: the study was carried out to describe the deliveries structural conditions, characteristics of assistance to the deliveries, to establish the percentage of cesarean section, and to verify the association between the presence of the pediatrician and the type of childbirth with socioeconomic variables. a cross-sectional population-based study was carried out including 840 women from 20 to 49 years old resident in s?o leopoldo, rio grande do sul state. among them, 36 (4.3%) had children within 12 months prior the research. the percentage of women who had caesarian delivery was 52.8%. regarding the obstetric delivery, 97.2% had been assisted by a doctor and 72.2% of the women had reported the presence of pediatrician at the time of the labor. in 68.6% of the cases the delivery process was financed by the sus, 20% were by private health care plans and 11.4% were privately paid. the caesarean delivery was less frequent among the women belonging to lower socioeconomic levels (prevalence rate 0.42; ci95% 0.20-0.86). it was verified that 30.4% of the women who had received maternal care by sus did not report the presence of pediatrician in the labor. it was evidenced the association of the type of delivery and some variables related to economic class, making it possible to state that financial compensation intervenes with the type of obstetrical delivery.
Aortic stenosis. Gender influence on left ventricular geometry and function in patients under 70 years of age

Abstract: OBJECTIVE: To verify if adaptive left ventricle (LV) characteristics are also present in individuals under 70 years of age with severe aortic stenosis (AS). METHODS: The study comprised 40 consecutive patients under 70 years of age with AS and no associated coronary artery disease, referred for valve surgery. Out of the 40 patients, 22 were men and 18 women, and the mean age was 49.8±14.3 years. Cardiac symptoms, presence of systemic hypertension (SH), functional class according to the New York Heart Association (NYHA), and valve lesion etiology were considered. LV cavity dimensions, ejection fraction (EF), fractional shortening (FS), mass (MS), and relative diastolic thickness (RDT) were examined by Doppler echocardiography. RESULTS: Fourteen (63.6%) men and 11 (61.6%) women were classified as NYHA class III/IV (p=0.70). There was no difference in the frequency of angina, syncope or dyspnea between genders. The incidence of SH was greater in women than in men (10 versus 2, p=0.0044). Women had a smaller LV end-diastolic diameter index (32.1±6.5 x 36.5±5.3mm/m2, p=0.027), LV end-systolic diameter index (19.9±5.9 x 26.5±6.4mm/m2, p=0.0022) and LV mass index (MS) (211.4±71.1 x 270.9±74.9g/m2, p=0.017) when compared with men. EF (66.2±13.4 x 52.0±14.6%, p=0.0032), FS (37.6±10.7 x 27.9±9.6%, p=0.0046) and RDT (0.58±0.22 x 0.44±0.09, p=0.0095) were significantly greater in women than in men. CONCLUSION: It is the patient gender rather than age that influences left ventricular adaptive response to AS.
Racial inequalities in access to women's health care in southern Brazil

Abstract: the aim of this population-based cross-sectional study was to investigate access by 20 to 60 year-old women - both black and white - to early detection (pap-smear) exams for breast and cervical cancer in two towns - s?o leopoldo and pelotas - in rio grande do sul state, southern brazil. estimates of the association between race/color and access to pap-smear and breast exams were adjusted for income, education, economic class and age. of the 2,030 women interviewed, 16.1% were black and 83.9%, white. black women were significantly less likely to have had a pap-smear and/or breast exam than white women. racial inequalities in access to cancer early detection exams persisted after controlling for age and other socioeconomic factors. racial differentials in access to early detection (pap-smear) exams for breast and cervical cancers might result from racial and socioeconomic inequalities experienced by black women in access to reproductive health care services and programs.

Abstract: Objective: To verify the efficacy of cochlear implantation in a patient with Arnold-Chiari syndrome. Design: Review of a medical chart from a patient with Arnold-Chiari syndrome that underwent to cochlear implant surgery. Case report: A 6 year-old female, with meningomyelocele and congenital hydrocephalus, deaf in the past 3 years, after meningitis. At that time, she used oral language with adequate speech development. Tone audiometry revealed bilateral profound sensorineural hearing loss. The MRI was compatible with the Arnold-Chiari syndrome. Cochlear implantation was performed on the left ear with MED-EL? Implant (model SONATATi100). Currently, the child is 12 year-old, performs speech therapy three times a week, with good hearing and language development. Conclusion: Despite the presence of Arnold-Chiari syndrome (Chiari type II) with neurological disorders and with ventriculoperitoneal shunt, patient had good hearing results after cochlear implant surgery.

Abstract: O presente artigo objetiva fazer uma análise dos procedimentos de discursiviza o do conto Fernando o verdadeiro e Fernando o falso. Para tanto foi dividido em duas partes: a primeira discorre, sinteticamente, sobre a sintaxe do discurso sob a proposta da semiótica greimasiana; a segunda prop e a análise de duas vers es do conto popular referido, cuja narrativa está centrada no fazer de um rapaz de origem humilde que deseja romper com a realidade em que vive, sonhando casar-se com uma mulher que lhe prazer e riqueza. Para realizar este desejo, o rapaz vive uma série de obstáculos por ele superados O conto aborda a cultura de um povo que é mostrada através da análise do discurso enunciado. Depreende-se a partir do estudo realizado, a identidade humana caracterizada por valores, a exemplo do desejo de conquistas que possibilitam maturidade. O herói desse conto é revestido de qualidades excepcionais, que refletem as ideologias apreciadas pelo povo, mas que, no fundo, foram cria es das estruturas de domina o, objetivando a constru o de modelos ideais, a serem seguidos para facilitar o seu trabalho. Esse tipo de narrativa rememoriza uma cultura, presentificando-se pelo imaginário, saberes de uma tradi o, muitas vezes, em conflito com a modernidade.

Abstract: the radiographic examination is one of the most applied diagnostic method in chronic valvular disease, evaluating the cardiac silhouette and/or lung abnormalities. the radiographic cardiac image can be measured by the “vertebral heart size” system, proposed by buchanan and bücheler (1995). seventy dogs who presented murmur on auscultation, symptomatic or not, were studied, and the radiographic examination of them were evaluated by the “vhs” method. clinical and radiographic findings were compared, indicating that the worse the severity of symptoms the higher the probability of finding cardiomegaly in radiographies. besides, the higher the murmur intensity the more often the radiographic signs of cardiomegaly were observed.

Abstract: O exame radiográfico é um dos métodos mais utilizados para o diagnóstico da doen a valvar cr nica, seja para avalia o da silhueta cardíaca seja para avalia o de campos pulmonares, que podem apresentar anormalidades decorrentes da insuficiência cardíaca. Um dos métodos de avalia o da silhueta cardíaca é o sistema “vertebral heart size” , proposto por BUCHANAN e BüCHELER (1995). Setenta c es portadores de sopro à ausculta o, sintomáticos ou n o, foram caracterizados quanto à severidade dos sinais clínicos e à intensidade do sopro presente. As radiografias de todos os animais foram analisadas pelo método de mensura o “VHS” . Os dados clínicos e radiográficos foram comparados entre si, verificando-se que, quanto mais severos os sinais clínicos, maior a probabilidade de se encontrarem altera es radiográficas indicativas de cardiomegalia. Observou-se, ainda, que, quanto maior a intensidade do sopro, mais freqüentes e severas s o as altera es radiográficas.

Abstract: objective: to identify mycobacterial species in the sputum of patients suspected of having pulmonary tuberculosis and to determine the impact that the acquisition of this knowledge has on the therapeutic approach. methods: we evaluated 106 patients suspected of having pulmonary tuberculosis and referred to the pulmonology department of a public hospital in the city of teresina, brazil. morning sputum specimens were evaluated for the presence of mycobacteria by sputum smear microscopy and culture. we used pcr and restriction enzyme analysis of the hsp65 gene (pra-hsp65) to identify the strains of mycobacteria isolated in culture. results: a total of 206 sputum samples were analyzed. patient ages ranged from 15 to 87 years, and 67% were male. there was cough in 100% of the cases. the predominant radiographic pattern was moderate disease, observed in 70%. smear positivity was 76%, and isolation in culture occurred in 91% of the cultures. traditional tests identified nontuberculous mycobacteria (ntm) in 9% of the isolates. the pra-hsp65 method confirmed these data, showing seven band patterns that were able to identify the isolated species of ntm: mycobacterium kansasii; m. abscessus 1; m. abscessus 2; m. smegmatis; m. flavescens 1; m. gordonae 5; and m. gordonae 7. all of the patients with ntm were over 60 years of age, and bronchiectasis was seen in 88% of the x-rays. there were two cases of reinfection, initially attributed to m. abscessus and m. kansasii. conclusions: in immunocompetent patients, ntm can infect the lungs. it is important to identify the specific ntm in order to establish the correct diagnosis and choose the most appropriate therapeutic regimen. the pra-hsp65 method is useful in identifying ntm species and can be implemented in molecular biology laboratories that do not specialize in the identification of mycobacteria.
