Abstract: this is a qualitative study that is part of an evaluation research of a medicine course with the use of active teaching-learning methodologies based on the triangulation of methods. the aim is to evaluate the results related to the extended general practice concept. the sources of information used in the study include 17 semi-structured interviews with ex-prisoners and a situation that simulated the medical practice, of which seven ex-prisoners and a simulated patient participated. the analysis of the information and the production of the data were based on the method of interpretation of senses, according to the referential hermeneutic-dialectic system. the results point to aspects that justify the extended general practice, evidenced in two themes: the doctor-patient relationship and the patient approach. in conclusion, it is observed that the evaluated medical course brings together the education of the general, humanist, critical and reflexive doctor that may intervene in the different levels of health attention as well as in the individual and collective approach. it is also concluded that there are limits in operating an extended general practice in diverse health situations.

Abstract: The cramoll 1,4 is a well-studied lectin. However, few studies about its biodistribution have been done before. In this study, we radiolabeled the cramol 1,4 with Tc-99m and analyzed the biodistribution. The results showed that the cramol has an abnormal uptake by the bowel with reflections on its clearance mechanism.

Abstract: Germline mutations in BRCA1, BRCA2 and TP53 genes have been identified as one of the most important disease-causing issues in young breast cancer patients worldwide. The specific defective biological processes that trigger germline mutation-associated and -negative tumors remain unclear. To delineate an initial portrait of Brazilian early-onset breast cancer, we performed an investigation combining both germline and tumor analysis. Germline screening of the BRCA1, BRCA2, CHEK2 (c.1100delC) and TP53 genes was performed in 54 unrelated patients <35 y; their tumors were investigated with respect to transcriptional and genomic profiles as well as hormonal receptors and HER2 expression/amplification. Germline mutations were detected in 12 out of 54 patients (22%) [7 in BRCA1 (13%), 4 in BRCA2 (7%) and one in TP53 (2%) gene]. A cancer familial history was present in 31.4% of the unrelated patients, from them 43.7% were carriers for germline mutation (37.5% in BRCA1 and in 6.2% in the BRCA2 genes). Fifty percent of the unrelated patients with hormone receptor-negative tumors carried BRCA1 mutations, percentage increasing to 83% in cases with familial history of cancer. Over-representation of DNA damage-, cellular and cell cycle-related processes was detected in the up-regulated genes of BRCA1/2-associated tumors, whereas cell and embryo development-related processes were over-represented in the up-regulated genes of BRCA1/2-negative tumors, suggesting distinct mechanisms driving the tumorigenesis. An initial portrait of the early-onset breast cancer patients in Brazil was generated pointing out that hormone receptor-negative tumors and positive familial history are two major risk factors for detection of a BRCA1 germline mutation. Additionally, the data revealed molecular factors that potentially trigger the tumor development in young patients.

Abstract: the importance of atmospheric lead in environmental global contamination is receiving increasing scientific attention. the main exposure monitoring approach is biological, but the environmental one has a key role, since the environment is the major source of exposure. the study aimed to identify the contamination sources and potential risk factors of the exposure in a 64 subject group formed by 0-16 years-old children from an economically deprived community in rio de janeiro. lead concentrations in soil, water, dust and air were determined and neurological and carcinogenic risk factors for ingestion and inhalation were calculated. blood samples were collected and used in the analysis of pb-b, ala-d% and ala-d genotyping. the observed neurological risk factor was 549 times higher than reference dose for dust and 554 times higher in the case of ingestion. the carcinogenic risk factor for ingestion was about 4 times. mean pb-b was 5.6μg/dl and 40% of the children presented pb-b levels above the 6μg/dl cutoff value. mean ala-d% was 40.3% and a correlation between pb-b and ala-d% was observed. alad1-2 genotype was identified in 10% of the children. the results will allow the understanding of our reality, supporting public health and environment organizations to carry out control actions and all-encompassing environmental surveillance.

Abstract: this paper presents the behavior of two warm fronts which affected the weather in rio grande do sul state. satellite images and various meteorological fields were analyzed for both situations. in the first one the system moved from argentina and gradually reached the various regions of the state, from the west to the east. during the passage of this system, occurrence of precipitation was registered; in some stations of the shore, cumulonimbus clouds were observed. during about 24 hours the rio grande do sul state was under the influence of that system. in the second case, the warm front that reached the same state was not associated with a clearly defined vortex. in the second situation, a warm front arrived at state mentioned earlier, while the vortex to which the front was associated, was still over the pacific ocean. for this case, vertical sections of potential temperature were constructed and it was observed a slope of the isentropic surfaces, which is related to the presence of vertical motions. the mesoscale analysis showed that there was a region with higher pressures in the northeast region of rio grande do sul and lower pressures in the west/northwest. also in this second situation cumulonimbus clouds were observed in various stations of the state, including some in the west part of the state. it was also registered the occurrence of fog during the two situations.

Abstract: Based on the centrality of the firms in the modern world, the discourse adopted by them can be an important route of penetration of market logic on solidarity field. Whereas the Critical Discourse Analysis aims to explore the relationship between the use of language and ideology, we appealed to this methodological support to deconstruct some organizationaldiscourse in order to verify the role of business in the naturalization process of expansion ofmarket logic to other social fields precisely in the field of cooperative actions. In this article, the intention was to analyze the textual dimension, deconstructing texts and seeking to reveal the ideational meanings contained in the speeches analyzed in order to understand the beliefs produced in the text. It is part of the corpus of the study the institutional discourse available in the sites of three companies. The analysis undertaken allow us to say that cooperative actions proposed by the companies are inextricably linked to the acquisition of products. Relations between people articulated as consumer relations show the consumer society of Bauman (2008), and accountability of the company by defining and implementing actions in solidarity refers to the company world as Solé (2004) discusses. Language as an instrument of power, is a medium capable of naturalizing ideas within society (Wodak, 2004).

Abstract: american cutaneous leishmaniasis is endemic in widespread areas of latin america. the causative agents include l. (v.) braziliensis, l. (l.) mexicana, l. (v.) panamensis, and related species. the spectrum of disease includes single, localized, cutaneous ulcers, diffuse cutaneous leishmaniasis, and mucosal disease.the main reservoirs for l. (v.) braziliensis and other leishmania (vianna) spp. are small forest rodents. the vectors are ground-dwelling or arboreal lutzomyia sandflies, which are abundant in the forest. disease is most common in persons working at the edge of the forest and among rural settlers. the incubation period of cutaneous leishmaniasis varies from two weeks to several months. a wide variety of skin manifestations ranging from small, dry, crusted lesions to large, deep, mutilating ulcers may be seen. ulcerative lesions are usually shallow and circular with well-defined, raised borders and a bed of granulation tissue. in l. (v.) braziliensis infection, regional lymphadenopathy often precedes the development of cutaneous lesions by one to 12 weeks. a definite diagnosis depends on the identification of amastigotes in tissue or promastigotes in culture. antileishmanial antibodies are present in the serum of some patients with cutaneous leishmaniasis as detected by elisa, immunofluorescent assays, direct agglutination tests or other assays, but the titers are usually low. the leishmanin skin test result usually becomes positive during the course of the disease. for treatment two pentavalent antimony-containing drugs are used: stibogluconate sodium, and meglumine antimoniate (glucantime). amphotericin b deoxycholate is an alternative for persons who fail to respond to pentavalent antimony. immunoprophylaxis and immunotherapy are promising new approaches to prevention and treatment.
