Abstract: this study had the aim to verify the necessities of training and improvement of the 4th regional coordination in health of rio grande do sul state family health teams. the data collection was carried out through the application of questionnaires with objective questions, composed by two parts: field and competence nucleus. for data analyses, normality, chi-square and g tests were used as statistical procedures for easy and difficult attributions indicated by the professionals. professionals have shown less difficulty with generic attributions, related with basic methods and techniques of each health area. however, they have revealed a lot of difficulties with attributions relating to the search of new partners in health inside communities, stimulation of the people participation in the discussion of health rights, and completion of the sia/siab forms, and others. even after 12 years of the foundation of the family health program (psf), several difficulties are still observed concerning the way to work with this new health strategy in brazil. proposals are made for the complete understanding and improvement of the psf strategy.

Abstract: the present study aimed to assess mortality rates and related demographic factors among indigenous peoples in the state of mato grosso do sul, central-west brazil, compared to the state's general population. mortality rates were estimated based on data obtained from the health care database for indigenous peoples and monthly patient care records as well as demographic data from the brazilian unified national health system (sus) and mortality data from the sus mortality database. compared to the overall population, among indigenous peoples there were proportionally more individuals under 15 years of age and fewer elderly, besides higher mortality rates at early ages and from infectious and parasitic diseases. indigenous men showed significantly higher mortality rates from external causes and respiratory and infectious diseases, while among women the mortality rates from external causes and infectious diseases were higher. suicide rates among young indigenous individuals were also particularly alarming. indigenous people's health conditions are worse than those of the general population in mato grosso do sul.

Abstract: this study analyzes the work by a family health program (fhp) team, identifying the potential for developing receptiveness by the team and fostering links between the health professionals and clients. this was a case study with a qualitative approach. the data were collected through free observation, a semi-structured interview, and documents. a dialectic approach was used to analyze the material. a relationship was observed between the organization of the work process and the achievement of receptiveness and links to clients. there are gaps in receptiveness to health services clients, especially in the openness to demand, accountability for the population's health problems, and encouragement for client autonomy. production of links is related to the development of clinical activities. according to fhp team members, health services clients are viewed alternately as subjects and objects. the principles and guidelines of the unified national health system (sus) do not represent a project as conceived by the health professionals. the study concludes by proposing greater nursing staff involvement in clinical activities and receptiveness to users of the program.

Abstract: this article presents an integrative review about studies that address the relationships between health professionals and users in health care practices. it aimed to identify aspects that were researched on the daily life of the services concerning such relationships. data were collected from the lilacs and pubmed databases based on these descriptors: user embracement; professionalfamily relations; professionalpatient relations; humanization of the care; and the bonding word associated to the single health system descriptor. two hundred and ninety studies, published from 1990 to 2010, were selected. through thematic analyses, five meaning cores were created: the relevance of the confidence in the professionaluser relationship; feelings and senses in the health care practice; the importance of communications in health care services; ways to organize health care practices and (de)colonialism. it was found that relationships established in health care practices have a transformative dimension. however, the challenge to humanize health care services remains. nursing stands out in the production of knowledge on such theme.

Abstract: brazil's national health care system (sistema único de saúde) advocates integrality as a guideline for its health care practices. despite advances in public health, one notes difficulties in exercising integrality in the daily practice of health services in brazil. this research aimed to better understand care practices in a family health strategy health care team and their relationship with integrality. it is a qualitative and descriptive study. data was collected using the research strategies of participant observation, interviews, and documental investigation and then submitted to thematic analysis. the following meanings of integrality were found: as a perception of the users' different dimensions and complexities; as an articulation of the ways of care; as an identification and meeting of health needs; as effort from the health care team to produce health; as care to specific groups. data analysis affirms the need to construct new practices geared to reorienting the care model in order to make it possible to exercise integrality.

Abstract: El Sistema único de Salud recomienda la integralidad como línea orientadora de las prácticas de cuidado en salud. A pesar de los avances en la salud colectiva, hay dificultades para ejercerla en el cotidiano de los servicios de salud. El objetivo fue el de conocer las prácticas de cuidado de un equipo de Estrategia de Salud Familiar y su relación con la integralidad. Se trata de una investigación descriptiva y cualitativa. La recolección de los datos se hizo a través de la observación participante, de entrevistas y consulta a documentos, los cuales se analizaron según el análisis temático. Se encontraron los siguientes sentidos de la integralidad: como percepción de las diferentes dimensiones y de la complejidad de los usuarios; como articulación de las formas de cuidar; como identificación y atención a las necesidades en materia de salud; como esfuerzo del equipo para producir salud; como cuidado a grupos específicos. A partir del análisis de los datos, se hace necesario construir nuevas prácticas dirigidas a la reorientación del modelo asistencial con el fin de hacer posible el ejercicio de la integralidad.
Risk factors for atherosclerosis in students of a private university in S o Paulo - Brazil

Abstract: OBJECTIVE: To characterize the risk profile for atherosclerosis (AS) in adolescents and young adults of a private university in S o Paulo. METHODS: Clinical, nutritional, and laboratory parameters were evaluated in 209 students of both genders aged 17 to 25 years. In addition to determination of the lipid profile, the association of its abnormal values with other risk factors for AS was also investigated. RESULTS: Increased levels of total cholesterol, LDL-C and triglycerides (TG) were observed in 9.1%, 7.6% and 16.3% of the students, respectively, and decreased levels of HDL-C in 8.6% of them. Prevalence of the remaining risk factors analyzed was elevated: sedentary life style (78.9%); high intake of total fat (77.5%); high cholesterol intake (35.9%); smoking, hypertension (15.8%) and obesity (7.2%). There was an association between elevated LDL-C and TG levels and sedentary life style and body mass index. CONCLUSION: The high prevalence of risk factors for AS in young individuals draws attention to the need for adopting preventive plans.
Rede de aten o em saúde mental na perspectiva dos coordenadores de servi os de saúde Mental health care network from the view of health service coordinators

Abstract: Trata-se de um estudo descritivo exploratório com abordagem qualitativa, que teve como objetivo compreender como coordenadores de servi os de saúde, envolvidos no cuidado dos sujeitos em sofrimento psíquico, percebem a organiza o da rede de aten o em saúde mental de Santa Maria, Rio Grande do Sul, Brasil. Os dados foram coletados por meio de entrevista semiestruturada e de vinhetas nos meses de abril e maio de 2008. Participaram da pesquisa dez coordenadores de servi os de saúde. Os dados foram trabalhados mediante análise temática e, posteriormente, agrupados em três núcleos de sentido: o entendimento dos fluxos assistenciais; o funcionamento da rede; e as dificuldades e potencialidades vislumbradas. Evidenciou-se uma rede em processo de constru o, destacando-se a existência de movimentos que visam à melhoria da articula o entre os servi os e consequente qualifica o da assistência para os usuários da rede de cuidado em saúde mental. Prop e-se um estreitamento das rela es entre os servi os, intensificando a articula o entre a aten o primária, os servi os substitutivos e a aten o hospitalar. Destaca-se o papel da gest o nesse processo, considerando a necessidade de envolvimento de diferentes setores para a efetiva o de um servi o de aten o à saúde mental mais qualificado. This is an exploratory descriptive study based on a qualitative approach which purpose was to understand how health service coordinators involved in providing care to individuals in psychological distress perceive the organization of the mental health care network in Santa Maria, state of Rio Grande do Sul, southern Brazil. Data were collected through semi-structured interviews and vignettes in the months of April and May 2008. Ten health service coordinators participated in the study. The data were processed through thematic analyses and, subsequently, grouped into three clusters of meaning: The understanding of care flows, network operation, and of the difficulties and potential envisioned. The results showed a network under construction, highlighting the existence of movements aimed at improving coordination between the services and the resulting quality of care provided to the users of the mental health care network. The authors propose a reinforcement of the relations between the services, heightening the relationship between primary care, substitutive services, and hospital care. Emphasis is put on the role played by management in the process, considering the need for the involvement of different sectors in order to attain a more qualified mental health care service.

Abstract: RESUMO: O objetivo deste estudo foi analisar as tendências históricas nas produ es científicas da enfermagem relacionadas ao Sistema único de Saúde, no período de 1990 a 2006. A pesquisa bibliográfica foi realizada através dos resumos das disserta es e teses encontradas nos Catálogos do Centro de Estudos e Pesquisa em Enfermagem, selecionando-se 31 resumos que continham Sistema único de Saúde como descritor. Os temas foram agrupados em cinco categorias: Atua o do Profissional Enfermeiro; Controle Social; Forma o Profissional em Saúde; Organiza o dos Servi os de Saúde e Direito à Saúde. Concluiu-se que a tendência na constru o do conhecimento de enfermagem vincula-se aos aspectos técnicos e operacionais dos enfermeiros, em detrimento das abordagens relacionadas às quest es sociais e políticas do sistema de saúde, demonstrando um distanciamento entre o movimento para consolida o do SUS e as produ es acadêmicas da enfermagem.
Saúde mental na estratégia saúde da família: revis o da literatura brasileira Mental health in the family health strategy: a review of Brazilian literature

Abstract: A Estratégia Saúde da Família orienta a organiza o da Aten o Primária à Saúde no Brasil e inclui importantes congruências programáticas com a Reforma Psiquiátrica. Os princípios de territorializa o e acompanhamento longitudinal devem favorecer a constru o de a es inovadoras de promo o, preven o e reabilita o em saúde mental. O objetivo desta revis o foi analisar os principais temas da literatura científica brasileira sobre saúde mental na Estratégia Saúde da Família. Foram lidos os títulos de 267 artigos, publicados entre 1999 e 2009, e 38 artigos foram selecionados, segundo critérios específicos, para análise temática. A literatura abordou os temas da demanda em saúde mental, das percep es e práticas dos profissionais e da inser o do psicólogo na Aten o Primária. Os estudos revisados apontaram problemas como vis es estereotipadas sobre os transtornos mentais, predominancia da lógica manicomial, ausência de registros, fluxos, estratégias, apoio qualificado às famílias e de integra o em rede. A meta-análise qualitativa indicou quest es que podem fomentar o debate sobre o tópico e a realiza o de outras pesquisas, bem como a reflex o sobre a atua o profissional na interface entre Saúde Mental e Saúde da Família. The Family Health Strategy establishes the principles of the Brazilian Primary Health Care and shares important goals with the Psychiatric Reform. The principles of territory-centered care and longitudinal care should enhance innovative actions of mental health promotion, prevention and rehabilitation. The aim of this review was to analyze the main themes approached by the Brazilian scientific literature concerning mental health in the Family Health Strategy. We read the titles of 267 articles published between 1999 and 2009. We followed specific criteria to select 38 articles for thematic analysis. The main themes were the demands in mental health, the perceptions and practices of health personnel and the role of the psychologist in Primary Care. The publications identified several problems: stereotypical views about mental disorders, the dominance of the hospitalization rationale, and the absence of clinical reports, strategies, qualified support to families and integrated health actions. The qualitative meta-analysis indicated questions that may strengthen the debate on the topic, the reflection on further research and on professional practice in the interface between Mental Health and Family Health.
