Histopathological Analysis about Autopsies from HIV/AIDS Patients—About Two Decades of Research Comparing Results before and after Antiretroviral Therapy Advent  [PDF]

Abstract:

Objectives: This study considers 489 autopsies of HIV/AIDS patients who died from acute respiratory failure and describes the demographic data, etiology, and histological pulmonary findings of HIV associated diseases, comparing results before and after introduction of antiretroviral therapy. Methods: The following data were obtained: age, sex, and major associated diseases (found at the autopsy). Pulmonary histopathology was categorized as: diffuse alveolar damage; pulmonary edema; alveolar hemorrhage; and acute interstitial pneumonia. Odds ratio of the HIV/AIDS-associated diseases developing a specific histopathological pattern was determined by logistic regression. Results: A total of 355 men were studied. The mean age was 37 years old. Bronchopneumonia presented in 43% and Pneumocystis jiroveci pneumonia in 38% of patients. Pulmonary histopathology showed diffuse alveolar damage in 31% and acute interstitial pneumonia in 23%. The multivariate analysis showed a significant and positive association between diffuse alveolar damage with disseminated tuberculosis, cirrhosis and sepsis; and acute interstitial pneumonia with Pneumocystis jiroveci pneumonia and cytomegalovirosis. After the introduction of antiretroviral therapy we observed an increase in the prevalence of bacterial bronchopneumonia, sepsis and cirrhosis; and a decrease in Pneumocystis jiroveci pneumonia and cytomegalovirosis. Conclusions: Coherent to literature, this study showed a decrease of respiratory failure mortality associated with some opportunistic infections after antiretroviral therapy introduction. But an increased prevalence of sepsis, bronchopneumonia and sepsis was observed too. The most prevalent pulmonary histopathological pattern was diffuse alveolar damage, which suggested a positive association with disseminated tuberculosis, sepsis and cirrhosis.

Abstract: objective: evaluate the frequency of depression in women with endometriosis and actual symptom of pelvic pain to determine the necessity of psychological support. methods: a total of 50 women aged 24 to 48 with endometriosis and referring pelvic pain. the prevalence of depression was evaluated by beck's depression inventory adapted to the brazilian population. results: it was found that 92% of the subjects presented depressive symptoms, being 56% from moderate to high levels. although 66% of the women were using hormonal medication to treat endometriosis, there was no significant correlation between the presence of depression and the use of these medications. conclusion: the results showed a high prevalence of depression symptoms in these patients confirming the necessity of psychological support.

Abstract: OBJETIVO: Avaliar a freqüência de depress o em pacientes com endometriose e queixa atual de dor pélvica para determinar a necessidade ou n o de atendimento psicológico. MéTODOS: Participaram deste estudo 50 mulheres com idade entre 24 e 48 anos, com diagnóstico de endometriose e queixa atual de dor pélvica. Para avaliar a prevalência da depress o, utilizou-se o Inventário de Depress o de Beck, validado em nosso meio. RESULTADOS: Identificou-se a presen a de depress o em 92% das pacientes avaliadas, sendo 56% de intensidade moderada a grave. Apesar de 66% das pacientes relatarem o uso de medica o, n o foi observada correla o significativa entre a presen a de depress o com o uso de horm nios. CONCLUS O: Os resultados demostraram uma alta prevalência de depress o, justificando-se, assim, a necessidade do atendimento psicológico a essas pacientes.
A Risk-Adjusted Retrospective Data Analysis between Younger and Elderly Patients with Acute Coronary Syndromes—Long-Term Prognosis  [PDF]

Abstract: Purpose: To compare the demographic data and outcomes of younger versus elderly patients with acute coronary syndromes. Methods: This was a retrospective data bank analysis study with 966 patients (268 in the younger group (less than 55 years) and 698 in the elderly group (more than 55 years)). Data were obtained about clinical characteristics, angiography, and medication used at hospital and coronary definitive treatment. The primary endpoint was all cause of in-hospital death and combined events. Comparison between groups was made by Anova and Q-square. Multivariative analysis was determined by logistic regression and was considered significative when p < 0.05. Long-term mortality and combined events were studied using Kaplan- Meyer curves with median follow-up of 11.21 months. Results: The median age in the younger group was 48 years versus 69 years in the elderly group. In the younger group 26% was ST-myocardial infarction versus 18% in the elderly group. About 7% of younger patients were submitted to coronary bypass surgery and 42% to percutaneous coronary angioplasty versus 12% and 25% in elderly group, respectively. Significant difference was observed between the younger versus elderly groups in deaths (1.5% × 7.5%, p = 0.004), combined events (14.9% × 26.3%, p = 0.02) and killip III/IV (3.7% × 8.3%, p = 0.04). Long-term mortality was 3.7% × 10.2%, p = 0.01). Conclusions: In patients with acute coronary syndromes age was an important predictor factor of mortality and complications. Significative differences in outcomes were observed between the two groups in-hospital and long-term follow-up.

Abstract: objective: evaluate pain and depression scores before and after multi-professional group intervention in women with endometriosis as compared to those who did not participate in this activity. methods: a total of 128 women attended at the endometriosis outpatient facility caism/unicamp, was assessed and distributed equally into two groups: women who received group intervention and those who did not. all women completed the beck depression inventory and the analogical visual scale. results: there was a significant decrease in pain and depression scores at the end of group intervention. women who had not participated in the intervention had higher initial scores of pain when compared to those who had. on the other hand, in the group without intervention,depression scores were lower. a positive correlation was identified between pain and depression in both groups. conclusion: group intervention was efficient in diminishing pain and depression in women with endometriosis and may be included in the conventional treatment for patients affected by this disease.
Sensory analysis of hydrolysed meat preparations

Abstract: the use of hydrolysed meat in diets contributes to the improvement of protein, vitamin and mineral supply. this work aims at checking the acceptance pattern in meat hydrolysates. four preparations have been developed with three types of hydrolysates in domestic-like conditions. acceptance was verified by means of sensory analysis using the nine-point hedonic scale. sensory tests have been carried out in three sessions (according to the kind of hydrolysates). in the evaluation file, information on age groups has been included. the statistical analysis has been made by anova and tukey test. the best accepted preparation have been the turkey and chicken hydrolysed balls. hydrolysates can be used in many different kinds of preparations, but it is necessary to know both the age group it will be used to and its sensory and chemical-physical features to ensure the taste and the original appearance of the final product.

Abstract: Apresenta um relato de experiência da atua o do bibliotecário no Núcleo de Inteligência Competitiva (NIC) da empresa Knowtec em Florianópolis (SC). Aborda o conceito de inteligência competitiva (IC), descreve suas etapas e aponta os atores envolvidos. Destaca o trabalho do bibliotecário no processo de IC a partir da descri o das atividades de coordena o de pesquisa, monitoramento de mídias e pesquisa de informa es. Infere-se que onde existe a necessidade de coletar, organizar, tratar e disseminar informa o, também há a necessidade do trabalho de um bibliotecário. Portanto, fica claro que é importante investir na forma o de profissionais multifacetados, aptos a trabalhar em diferentes segmentos, já que a informa o está presente em todas as organiza es e os sistemas de inteligência competitiva, em particular, exigem cada vez mais profissionais com inúmeras habilidades pessoais e profissionais.

Abstract:

The importance-performance analysis method (IPA) is used in market research in order to measure the level of customer satisfaction. The aim of this study was to evaluate and highlight the use of IPA as a management tool to measure quality of dental services. We suggest that this method can be easily implemented in a dental educational setting, as a performance outcome measure that includes patient input. The study was conducted in a dental service through a valid questionnaire, SERVQUAL. This instrument explores levels of service quality perceived by patients. It was conducted in four companies in Brazil in 2011 (derived from different industry segments; two from textiles, one from hospital care and the other from the manufacturing industry) in two States (S?o Paulo and Minas Gerais). These companies were covered by the same dental managed care organization which contracted with independent dentists to provide care. The study was divided into two stages: the first, in which beneficiaries answered the questionnaire prior to receiving dental treatment, and the second when the same beneficiaries answered after completing their dental treatment. Data obtained from SERVQUAL generated graphs

that were used to characterize the IPA matrix using several dimensions of care. The Assurance Dimension had the highest average in both expectation and perception. The Reliability dimension showed the value of the most negative GAP among the dimensions, and the best value occurred in the Responsiveness dimension. The IPA tool may be effective in Dental Medicine since it highlights the key points to be improved in the delivery of dental services in a clinical setting.
