Manejo quirúrgico del cáncer de pulmón. Hechos y algunas reflexiones

Abstract: stage i and ii non-small cell lung cancer (nsclc) should be resected, but there is mounting evidence for the use of preoperative induction and postoperative adjuvant therapy in stages ib and ii, as being able to prolong life. some patients in stage iiia should undergo induction therapy, and then have re-staging of the mediastinum by ct/pet or redo mediastinoscopy before considering resection. stages iiib and iv are non-surgical, except very selected cases. reflections are made regarding the control of cigarette smoking, the difficult access of patients from developing countries to the recent costly medical, pharmacological and technical advances; reflections are also made related to some ethical issues regarding medical and surgical treatment of nsclc.

Abstract: in orderto set a diagnosis about the use of hormonal replacement therapy (hrt) in peri and postmenopau-sal women by the obstetricians and gynecologists members of our staff, we did a survey that was answered by 97.6% of them. questions about the future patients desire related to use of hrt or use of natural or alternative medicines were included. there were also questions related to the "risk-benefit" relationship on the use of hrt from an overall point of view as well as from a cardiovascular point of view. the preference of medical doctors (md) on choice of hormone type in hrt use, in respect to the administration, were also asked. among others, questions were asked in order to know md opinions on the relationship with breast cáncer, the changes on acceptability use after the whi study, reasons for the treatment discontinuation, the importance of women's age and the cost of pharmaceuticals producís on hrt use. conclusions: are focused on a need of a solid educational program on use of hrt addressed to gynecologists, as well as to general practitioners, cardiologists, oncologists and others related. educational programs are also quite beneficial to future users as well as women that are currently under treatment. in the absence of conclusive information available at the present moment, we advise that patients considering hrt should seek the coun-sel of medical teams whose specialties include menopause
Pleurodesis en derrame pleural maligno

Abstract: traditionally, pleurodesis has been attempted in patients with confirmed malignant tumors with recurring pleural effusions of more than half the size of the hemithorax, dyspnea that is relieved by evacuation of the fluid, a lung able to reach the chest wall and the expectation of a reasonably long survival period. pleurodesis can be done at the bedside, the emergency room, in a procedure room by medical thoracoscopy under local anesthesia and sedation, or in the operating room by vats under general anesthesia, introducing an sclerosing agent and/or producing pleural abrasion by different means. we propose "first contact closed pleurodesis " for patients with an unequivocal diagnosis of malignancy, a pleural effusion of more than half the size of the hemithorax, even if asymptomatic, and the expectation of a reasonably long survival period, using a vascular or pleural catheter to drain the fluid and introduce the sclerosing substance; depending on the chest x-ray, the catheter can be pulled out, left in situ to repeat the introduction of the sclerosing agent or, if this fails, to drain the fluid as often as necessary with a sterile syringe, ignoring the goal to achieve pleurodesis; the procedure is effective in over 90% of cases and non-expensive, can be done on an outpatient basis and does not require a chest tube nor a pleural drainage system. present day consensus does not support parietal pleurectomy as an elective choice for these patients.
Mal Nutrición por Exceso en Escolares

Abstract: objectives: to implement an educational intervention that contributes to the prevention of malnutrition by excess among adolescents. patients and method: this descriptive study included a sample of 939 individuals attending school at colegio manuel d'alzon, in lota (small town), between 2007-2009. studies included anthropometric evaluations, nutritional intake and training workshops. parents and teachers were included in the workshops. healthy nutrition was incorporated in the curriculum, and a site for distribution of healthy foods was implemented. results: a 3% decrease in prevalence of malnutrition by excess was seen (28% to 25%), most marked among males. this decrease was attributed to reduction in obesity and increase of lower weights. this decrease appears during a period of time when boys show a growth spurt, which occurs earlier among girls. conclusion: collaborative interventions between health and education show positive results in this population.

Abstract: Objetivo: Implementar una intervención educativa que contribuya a prevenir la malnutrición por exceso en adolescentes. Pacientes y Método: Estudio descriptivo observacional que incluyó una muestra de 939 personas pertenecientes a la comunidad educativa del Colegio Manuel D'Alzon de la Comuna de Lota, período 2007-2009. Se incluyo estudiantes a quienes se les realizó evaluaciones antropométricas, de ingesta alimentaria, y talleres educativos, se incluyó igualmente a docentes y apoderados con quienes se efectuaron talleres educativos. Se logró la incorporación de temas de alimentación saludable en las actividades curriculares, y la implementación de un kiosco saludable (tipo b). Resultados: Se observó una disminución en la prevalencia de malnutrición por exceso en 3 puntos porcentuales de un 28% inicial a un 25% final, siendo en el género masculino en donde se hace más evidente esta reducción. Atribuyéndose esto al aumento del estado de bajo peso y reducción de la obesidad, ello se sustentaría en el peak que alcanza el estirón puberal en los varones en este período de vida, lo que en las ni as ocurre a lo menos dos a tres a os antes. Conclusiones: Estas intervenciones colaborativas entre el sector salud y educación logran resultados positivos en disminuir la mal nutrición por exceso en la población escolar. Abstract: Serology is the foundation of any brucellosis control and eradication program worldwide, thus it is important to define accuracy diagnostics assays and cut-off of those assays, due to variations from country to country and even among specific areas in the country. The variation of cutoff values depended on: prevalence of disease, vaccination status, animal management, and control and eradication programs. Therefore, a cut-off for the diagnosis of bovine brucellosis through fluorescence polarization assay (FPA) in Carchi—Ecuador was determined. The survey has been carried out in Carchi province of Ecuador, who is considered a province of high prevalence of brucellosis and the vaccination status is unknown due to the lack of registers. Sera samples (n = 200) were obtained from individual cows from randomly selected herds. Blood sera were tested through Fluoresce Polarization Assay (FPA) and competitive enzyme-linked inmunosorbent assay (cELISA) as confirmatory test, and then receiver operating characteristic (ROC) analysis was done. The sensitivity and specificity values of FPA were 88.7% and 92.50% respectively using a cut-off of 89.90 mP. Abstract: Corn is one of the main grasses used to produce human or animal food. One of the main problems with the use of corn is the presence of mycotoxins, where aflatoxin B1 is one of the most harmful for human and animal health. Chemical methods such as the addition of antifungals and sequestrants are used to control this contaminant in food; however, these methods can leave dangerous residues. The aim of this research was to evaluate the effectiveness of irradiation with gamma rays on corn for the control of aflatoxin B1. For this purpose, three treatments were compared: treatment 1, application of gamma radiation, treatment 2, application of antifungal and treatment 3, combination of gamma radiation and application of antifungal. Corn without exposure to any control of aflatoxin was used as a witness group. Irradiation doses of 2, 6 and 10 kGy were tested, and the dose of 6 kGy was selected as the best since it lowered aflatoxin B1 more effectively. The corn analyzed in this study was stored during 45 days at 23°C, and every 15 days. We determined the concentration of aflatoxin B1, presence or absence of pathogenic microorganisms and insects, and we evaluated the physicochemical characteristics of the grain. Inferring mixed-culture growth from total biomass data in a wavelet approach

Abstract: It is shown that the presence of mixed-culture growth in batch fermentation processes can be very accurately inferred from total biomass data by means of the wavelet analysis for singularity detection. This is accomplished by considering simple phenomenological models for the mixed growth and the more complicated case of mixed growth on a mixture of substrates. The main quantity provided by the wavelet analysis is the Holder exponent of the singularity that we determine for our illustrative examples. The numerical results point to the possibility that Holder exponents can be used to characterize the nature of the mixed-culture growth in batch fermentation processes with potential industrial applications. Moreover, the analysis of the same data affected by the common additive Gaussian noise still lead to the wavelet detection of the singularities although the Holder exponent is no longer a useful parameter
