Abstract: the committee on ethics of the instituto de investigación de enfermedades raras (ceiier) of the spanish national institute of health carlos iii, presents this article dealing with ethical guidelines regarding the implementation of screening population programmes with special emphasis on genetic screening. after a critical review it has been addressed 24 recommendations concerning 14 topics: evaluation of the opportunity of the programme, including ethical analysis besides scientific evidences and cost/benefits issues; the need to differentiate between research and public health intervention and to built a specific and comprehensive programme; the creation of an interdisciplinary working group which control its implementation and prepare a protocol including justification, development, therapeutic or preventive actions and follow-up activities; the review of the programme by an independent ethical committee; the guarantee of the voluntary, universal and equitable population access, which requires sufficient information on the programme and their specific relevant facts, as incidental detection of heterozygous state in minors in newborn screening and the relevance of non directive genetic counselling specially in prenatal screening offered to pregnant women; considerations regarding future uses of samples for research purposes; total quality and periodic programme evaluation; guarantee of personal data confidentiality and the conflict of interest statement of the members of all the committees involved in the programme.

Abstract: rare diseases are those whose prevalence is below 5 cases per 10,000 inhabitants in the european community. most cases are diagnosed during paediatric age due to their genetic origin, while some others are congenital malformations. nevertheless, a higher prevalence is seen during adulthood as most of the former diseases are very severe and patients die during childhood. at the same time, higher survival rates are related to some chronic rare diseases in adults. the spanish network of research epidemiology for rare diseases (repier) developed the first atlas showing the geographical distribution of rare diseases in spain, assessed the spanish rare disease registries, contributed to the further development of regional plans on rare diseases, as well as to social and health actions, and established the real group of needs to be solved. these have been included in the communication of the european commission on rare diseases as well as in the spanish senate presentation.

Abstract: Enfermedad rara es aquella cuya prevalencia es inferior a 5 casos por cada 10.000 personas en la Comunidad Europea. La mayoría de los casos aparecen en la edad pediátrica, dada la alta frecuencia de enfermedades de origen genético y de anomalías congénitas. No obstante, la prevalencia es mayor en los adultos que en los ni os, debido a la excesiva mortalidad de algunas enfermedades infantiles graves y a la influencia de ciertas enfermedades que aparecen a edades más tardías. La Red Epidemiológica de Investigación en Enfermedades Raras (REpIER) desarrolló el primer atlas de distribución geográfica de las enfermedades raras en Espa a, evaluó la existencia de registros de enfermedades raras existentes, facilitó el ulterior desarrollo de planes autonómicos y acciones sociosanitarias y planteó un marco de necesidades a desarrollar, que más tarde han llegado a ser reconocidos como necesidades a resolver en el marco del Comunicado de la Comisión Europea sobre Enfermedades Raras y en la propia Ponencia del Senado. Abstract: this field note aims to make known the decisions taken by the ethics committee of the instituto de salud carlos iii for toxic oil syndrome regarding the secondary use of research specimens in biological research when informed consent is lacking. this is a common concern in the field of biomedical research. after debating the ethical suitability of the secondary use of these samples, our main conclusion is that researchers conducting prospective studies should expressly solicit written informed consent from participants in the study about i) whether there will or could be any secondary use of the samples and, if so, ii) whether such secondary use would be conditional on the type of research.

Abstract: El objetivo de este artículo es dar a conocer las decisiones tomadas por el Comité de ética del Instituto de Salud Carlos III para el Síndrome del Aceite Tóxico en relación con el desarrollo de proyectos de investigación en los que se podían utilizar muestras recogidas con anterioridad y para cuyo uso no se disponía del consentimiento de los pacientes, sobre una situación muy común en el ámbito de la investigación biomédica. A partir del proceso de debate acerca de la idoneidad ética de la utilización secundaria de estas muestras, se llegó a la conclusión de que en los estudios prospectivos se debe solicitar expresamente y por escrito el consentimiento del participante en el estudio para conservar las muestras y que éstas puedan ser utilizadas en investigaciones futuras, estableciendo con el donante los límites de su utilización.
Design, methods and demographic findings of the DEMINVALL survey: a population-based study of Dementia in Valladolid, Northwestern Spain

Abstract: Background This article describes the rationale and design of a population-based survey of dementia in Valladolid (northwestern Spain). The main aim of the study was to assess the epidemiology of dementia and its subtypes. Prevalence of anosognosia in dementia patients, nutritional status, diet characteristics, and determinants of non-diagnosed dementia in the community were studied. The main sociodemographic, educational, and general health status characteristics of the study population are described. Methods Cross-over and cohort, population-based study. A two-phase door-to-door study was performed. Both urban and rural environments were included. In phase 1 (February 2009 – February 2010) 28 trained physicians examined a population of 2,989 subjects (age: ≥ 65 years). The seven-minute screen neurocognitive battery was used. In phase 2 (May 2009 – May 2010) 4 neurologists, 1 geriatrician, and 3 neuropsychologists confirmed the diagnosis of dementia and subtype in patients screened positive by a structured neurological evaluation. Specific instruments to assess anosognosia, the nutritional status and diet characteristics were used. Of the initial sample, 2,170 subjects were evaluated (57% female, mean age 76.5 ± 7.8, 5.2% institutionalized), whose characteristics are described. 227 persons were excluded for various reasons. Among those eligible were 592 non-responders. The attrition bias of non-responders was lower in rural areas. 241 screened positive (11.1%). Discussion The survey will explore some clinical, social and health related life-style variables of dementia. The population size and the diversification of social and educational backgrounds will contribute to a better knowledge of dementia in our environment.

Abstract: the aim of this work is to observe the emergence and development of metapragmatic skills related to the regulation of information. a transversal study was carried out, made up of 40 participants, aged between 22 months and 10 years. the mother tongue and that used for acquisition of knowledge were catalan and spanish. an observational methodology was used, recording interactive situations between the child and the adult. a category system was developed, divided into two groups: regulation and adjustments of communication and regulation based on the interlocutor as the centre of interest. the results show us that the regulation and adjustments of communication category group appear at a very early age and is maintained active practically throughout all the ages observed. initially the categories aimed at the obtaining of information predominate, while the child also facilitates information in order to maintain adequate interaction. regarding the group of regulation categories based on the interlocutor as the centre of interest, this appears at a later stage, when the child possesses a greater degree of competence, except the category reference to the speech or language of others, which has been detected at an earlier age.

Abstract: El objetivo de este trabajo es observar la emergencia y desarrollo de las habilidades metalingüísticas relacionadas con la regulación de la información mediante un estudio transversal. Intervinieron 40 participantes de edades comprendidas entre los 22 meses y los 10 a os. La lengua materna y la usada en la adquisición de conocimientos eran el catalán y el castellano. Se utilizó una metodología observacional en un contexto comunicativo natural y espontáneo entre el ni o y el adulto. Se elaboró un sistema de categorías (Navarro y Hernández, 2004; Navarro, 2007; Navarro y San Martín, 2007) dividido en dos grupos: Regulación y ajustes de la comunicación y Regulación basada en el interlocutor como centro de interés. Los resultados nos indican que el grupo categorial Regulación y ajustes de la comunicación aparece a una edad muy temprana y se mantiene activo prácticamente a lo largo de todas las edades observadas. Inicialmente predominan aquellas categorías encaminadas a la obtención de la información, a la vez que el ni o también la facilita para poder mantener una interacción de forma adecuada. Respecto al grupo de categorías Regulación basada en el interlocutor como centro de interés aparece tardíamente, cuando el ni o posee un mayor grado de competencia, excepto la categoría Referencia al habla o al idioma de los demás que se ha detectado a una edad más temprana. Abstract: After a long introduction on the subject, the paper deals with the problem related with the use of statistical sources when looking at international migrations. Secondly, it addresses the interaction between migration patterns and demographic dynamics in Western Europe. Next, the article looks into the labor markets in current western societies and the role of immigrants therein. Finally, the main features of immigration policies in the EU countries are discussed. The rising importance of immigration in Spain is clearly underlined. Tras una extensa introducción para enmarcar el fenómeno descrito, el artículo aborda el problema de las fuentes estadísticas para el estudio de los procesos migratorios internacionales, la incidencia de la inmigración actual en la dinámica demográfica europea, los aspectos más sobresalientes de los nuevos mercados de trabajo y las líneas previsibles de la política migratoria de la Unión Europea a corto y medio plazo. El artículo refleja la importancia relativa de la inmigración espa ola en lo que va de siglo XXI, cuando se compara con la de otros países occidentales.
