Abstract: a programme of educational innovation was implemented to improve the attitude toward research methods for psychology students. students used real data to test their own hypotheses using different statistical methods. specifically, they implemented two key methodological principles in research methods: sample size and ordinal claims (frick, 1996). students had to work in teams, using internet data bases for developing the theoretical background of the study, the definition of the objectives, the definition of variables, data analysis, and discussion of results. we assessed the attitudes towards research methods before and after the study. results showed that: 1) the students improved their skills in research methods, and 2) the attitudes towards methodology were more positive.

Sedentary subjects have higher PAI-1 and lipoproteins levels than highly trained athletes

Abstract: The several studies have consistently shown that low levels of plasmatic high-density lipoprotein (HDL) and high levels of low- and very low-density lipoprotein (LDL and VLDL, respectively) are linked with a sedentary lifestyle and are strong predictor to cardiovascular disease [1-5]. In addition, many of these diseases associated to sedentary lifestyle are also characterized by alterations in pro-inflammatory markers in the plasma [6,7]. These inflammatory process include increase in plasma the pro-inflammatory cytokines as tumor necrosis factor alpha (TNF-α), plasminogen activator inhibitor type-1 (PAI-1), C-reactive protein (CRP), interleukin 6 (IL-6) and reduction of anti-inflammatory cytokines as interleukin 10 (IL-10) and adiponectin [8-11].The plasmatic PAI-1 and CRP levels are also strongly related to cardiovascular risk factors, increasing its levels leading of arising the hypertension, high triglyceride levels and likely obesity [6,12-16]. It is now established that reduction in the plasma to adiponectin and increased PAI-1 and CRP levels play a role in the maintenance of an inflammatory state and in the development of cardiovascular disease [4]. However, it is much less known if these pro- and anti-inflammatory markers are related to the lipoproteins and cholesterol levels in plasma. While competitive exercise training programs (characterized by moderate/high intensity and long duration) seem to reduce the LDL and total cholesterol and to increase the HDL concentration, evicting the progression or appearance of inflammatory atherosclerotic process [6], it remains slightly unknown if highly trained athletes have lower pro-inflammatory and higher anti-inflammatory markers than sedentary subjects.Therefore, the objective of this study was to compare plasma levels adiponectin, PAI-1, CRP, and lipoprotein fractions between sedentary subjects and highly trained athletes. Abstract: Endotoxinemia was lower in the highly trained subject group relative to the sedentary subjects (p < 0.002). In addition, we observed a positive correlation between endotoxin and PAI-1 (r = 0.85, p < 0.0001), endotoxin and total cholesterol (r = 0.65; p < 0.01), endotoxin and LDL-c (r = 0.55; p < 0.049) and endotoxin and TG levels (r = 0.90; p < 0.0001). The plasma levels of MCP-1, ICAM/CD54 and VCAM/CD106 did not differ.These results indicate that a lifestyle associated with high-intensity and high-volume exercise induces favorable changes in chronic low-grade inflammation markers and may reduce the risk for diseases such as obesity, diabetes and cardiovascular diseases.A sedentary lifestyle increases the risk of developing cardiovascular disease and diabetes [1,2], as well as many other diseases that are linked to inflammatory markers in the plasma [3]. In previous work, we showed that sedentary subjects present a favorable state that leads to hypertriglyceridemia and hypercholesterolemia together with high levels of plasminogen activator inhibitor type-1 (PAI-1). On the other hand, the performance of highly trained athletes and a large amount of exercise significantly reduced the possibility of risk for cardiovascular diseases [4].In recent years, it has been hypothesized that the microbial ecology in humans could be an important factor in energy homeostasis (i.e., obesity, diabetes, fatty liver) [5-10]. Lipopolysaccharide (LPS), which is also referred to as endotoxin, has been implicated as potentially important in this respect - as it is a potent inducer of inflammation. Under normal circumstances, only small amounts of endotoxin will cross from the intestinal lumen into the systemic circulation, and the absorbed endotoxin will be rapidly removed by monocytes, particularly resident Kupffer cells within the liver. Impacto de la fortificación de alimentos con ácido fólico en los defectos del tubo neural en Costa Rica

Abstract: objective: evaluate the impact of the fortification of food with folic acid on prevalence trends for neural tube defects (ntd) and the infant mortality rate (imr) associated with this disorder in costa rica. methods: the surveillance data from the congenital disease registry center and the central american population center were analyzed. the neural tube defects considered were anencephaly, spina bifida, and encephalocele. the trends from 1987-2009, as well as the differences in prevalence and mortality rates prior to and up to 12 years after food fortification with folic acid, were examined (95% confidence interval [ci]). the contribution of fortification to the decrease in the overall imr was determined. results: during 1987-1997, prior to the period of food fortification with folic acid, ntd prevalence was 12/10 000 births (95% ci: 11.1-12.8), whereas in 2009 prevalence was 5.1/10 000 births (3.3-6.5). the imr associated with ntd was 0.64/1 000 births (46-0.82) in 1997 and 0.19/1 000 births (0.09-9.3) in 2009. there were significant decreases in the imr associated with ntd and the prevalence of ntd: 71%, and 58%, respectively (p < 0.05). the overall imr decreased from 14.2/1 000 births in 1997 to 8.84/1 000 births in 2009 (p < 0.05). the decrease in the imr associated with ntd contributed to an 8.8% decrease in the overall imr from 1997 to 2009. conclusions: food fortification with folic acid caused a decrease in ntd at birth and the imr associated with this malformation during the 1997-2009 period. it also led to a decrease in the overall imr. there is a temporal relationship between the introduction of fortification policies and the decrease in prevalence and mortality associated with ntd. this intervention should be promoted in latin american and caribbean countries where it has not yet been implemented.
Fracturas Maxilofaciales en Individuos Chilenos

Abstract: the oral and maxillofacial trauma represent a 6% to 8% approximately of the body fractures. these injuries are of complex handling and demand an accurate and precise diagnosis. the hernán henríquez aravena regional hospital, of temuco city, does not have oral and maxillofacial surgeons in the immediate urgency staff, so that first approach to handle those patients is made by other clinicians. after one year, a total of 34,931 consultations were made in the urgency service at the hernán henríquez aravena hospital. of them, a representative sample of 742 subjects was chosen from which general information and the kind of injuries was obtained. statistical association with the chi - square test was obtained. six percent of the consultations in the hernán henríquez aravena regional hospital were associated to maxillofacial fractures. 64.4% of the sample corresponded to male subjects. the relation man to woman was 3.1:1, displaying a greater difference in patients aged over 15 years. of the total of sample, the group with more consultations was those aged below 20 years and the 31,1% of the consultations were caused by aggressions, whereas 24,4% were associated to falls. the main zone of fracture was related to the orbit nose etmoidal complex with 55,6% and dental fractures with a 22.2%. statistically significance association was found between age, sex, aggression, mandible and dental fractures (p < 0,05) the data reflect the high prevalence of maxillofacial fractures in temuco, chile, similar to other parts of the world. the group of young people is the one of main consultation where the aggressions play a fundamental roll. with the present descriptive study we think that the presence of oral and maxillofacial surgeons in the urgency staff of the hernán henríquez aravena regional hospital, temuco, chile is justified totally

Abstract: pneumonia is an inflammatory disease, generally of infectious character that affects the units of gaseous exchange, and produces various degrees of alteration in the diffusion and alveolar ventilation that may set in risk the patient's life. the term of community acquired pneumonia (cap) is given to the infection of the pulmonary parenchyma that occurs in children who have not been hospitalized in the last week or that appears 48 hours after hospitalization, accompanied or not by symptoms and/or respiratory signs of less than 15 days of evolution. all patients with some type of serious immunodeficiency or patients with chronic pathology, as well as those patients with pulmonary tuberculosis and neonatal pneumonia should be excluded from this group.

Abstract: objective: to study the fetal mortality during 1998 to 2002, to know its incidence, and determine the maternal characteristics, obstetrical conditions and related factors. method: descriptive, retrospective, epidemiological and analytical study of 304 cases. setting: department of obstetrics and gynecology. "dr. adolfo prince lara" hospital, puerto cabello, carabobo state. results: perinatal mortality for the studied period was 41.66 per one thousand births. there was a predominancy of patients of 24 years or less (56.9 %); in family history the most frequent were hypertension (37.24 %) and diabetes (30.43 %). admission diagnostic was fetal deaths 27.96 % (85/ 304), threatens of premature delivery 22.37 % (68/304) and toxemia 18.75 % (57/304). there was no prenatal care in 56.43 % (57/101); were multigravida 60.85 %, with gestational age ≤ 36 weeks 79.09 %, and ended in normal delivery 76.64 %. male fetuses predominated (55.93 %), with weight ≤ 2 500 g (78.88 %) and size ≤ 49 cm (76.78 %). the more prevalent direct factor of death in 169 cases was placental insufficiency (45.6 %), subsequent to hypertensive pathology, followed by placental hemorrhage (32.54 %). conclusion: the component of the fetal perinatal mortality is high and should be establish a strategy for attention of hypertensive pathologies, diabetes, prematurity and placental hemorrhages, and recommend the early prenatal care.
Fracturas Maxilofaciales en Individuos Chilenos Maxillofacial Fractures in Chilean Subjects

Abstract: Las fracturas orales y maxilofaciales representan un 6% a 8% de las fracturas óseas corporales. Estas lesiones son de complejo manejo y exigen un diagnóstico precoz, certero y preciso. El Hospital Regional Hernán Henríquez Aravena, de la ciudad de Temuco, no posee cirujanos orales y maxilofaciales en los equipos de urgencia inmediata, de forma que los ingresos son realizados por otros profesionales. En un a o de seguimiento se realizaron 34.931 consultas medico-legales en el Servicio de Urgencia del Hospital Regional Hernán Henríquez Aravena. De esas consultas se escogió una muestra representativa de 742 sujetos de quienes se obtuvo información general y de las lesiones en particular. Se obtuvo también asociación estadística mediante la prueba Chi - Cuadrado. Seis porciento de las consultas en el Hospital Regional Hernán Henríquez Aravena fueron asociadas a fracturas maxilofaciales, donde el grupo con más consultas fueron el de los menores de 20 a os. Del total muestral, el 64.4% correspondió a sujetos del sexo masculino, manteniendo una relación hombre:mujer de 3.1:1; sin embargo, esta relación aumento su diferencia en sujetos mayores de 15 a os. El 31.1% de las consultas fueron causadas por agresiones, mientras que 24.4% fueron asociadas a las caídas. La principal zona de fractura fue el complejo orbito-naso-etmoidal con un 55.6% y las fracturas dentoalveolares con un 22.2%. Existió asociación estadísticamente significativa entre edad, sexo, agresión, fracturas mandibulares y fracturas dentoalveolares (p < 0.05). Los datos reflejan la alta prevalencia de fracturas maxilofaciales en la comuna de Temuco, Chile, siendo similar a lo que ocurre en otras partes del mundo. El grupo de los jóvenes es el de principal consulta donde las agresiones juegan un papel fundamental. Con el presente estudio descriptivo creemos que se justifica plenamente la presencia de cirujanos orales y maxilofaciales en los equipos de urgencia del Hospital Regional Hernán Henríquez Aravena, Temuco, Chile. Abstract: La miasis humana es el parasitismo de órganos y tejidos producido por especies de larvas del orden Díptera. El diagnóstico se realiza con base en hallazgos clínicos y se confirma con estudios entomológicos. Se presenta el caso de una ni a de siete a os de edad que fue llevada por su padre al servicio de urgencias por presentar fiebre asociada a una lesión abscedada en el cuero cabelludo, con salida espontánea de larvas. Como hallazgo en el examen físico se reportó pediculosis grave. La paciente fue hospitalizada y tratada con clindamicina, gentamicina e ivermectina, y mostró mejoría de sus condiciones clínicas. Se dio alta médica al cuarto día de estancia hospitalaria. Se recolectaron larvas en estadio dos de Cochliomyia hominivorax (Diptera: Calliphoridae) directamente del área lesionada, observándose la asociación miasis-pediculosis; por lo tanto, se alerta a los trabajadores del área de la salud del riesgo potencial que representa la pediculosis para el desarrollo de la miasis. Human myiasis is the parasitism of human tissues by fly larvae. Diagnoses are based on clinical pattern of tissue damage and presence of insect stages. Herein, a case myiasis is described in a seven-year-old female child. She presented with fever associated with abscessed scalp lesions containing exposed larvae. Severe pediculosis was also observed. The patient was hospitalized and treated with clindamycin, gentamicin (for bacterial secondary infections) and ivermectin (treatment for lice) after which the patient showed clinical improvement and was discharged four days later. Since human myiasis can be caused by a number of different species, larvae were collected from the patient and identified as those of Cochliomyia hominivorax (Diptera: Calliphoridae). Because other cases of coinfestation of flies and lice are on record, health workers are to be alerted about the possible pediculosis-myasis risk.
