Parálisis periódica hiperpotasémica

Abstract: the periodic hyperpotassemia paralysis is a striated muscle channelopathy characterized by recurrent episodes of muscular asthenia that may to be triggered by exercise, cold, not rest after exercise and potassium support. this the case of a female patient aged 13 diagnosed with hyperpotassemia periodic paralysis without family backgrounds of this entity and also without associated myotonia. the seizures of muscular asthenia occurred almost daily and each 2 or 3 days with a variable length from a half hour to 24 to 48 hours. during a episode of muscular asthenia there were blood potassium concentrations of 7,14 mmol/l and the electromyogram showed a myopathic pattern. there was a frequency decrease of episodes of muscular asthenia at 2 months of treatment onset with oral acetazolamide.
Conjunctival-corneal melt in association with carotid artery stenosis

Abstract: Rosalind MK Stewart1, Say Aun Quah1, Dan Q Nguyen2, Stephen B Kaye11Royal Liverpool University Hospital, Liverpool, UK; 2Bristol Eye Hospital, Bristol, UKPurpose: To report a case of severe conjunctival-corneal melt in association with carotid artery stenosis.Methods: Observational case report.Results: A 76-year-old man with a history of bilateral severe carotid artery occlusion and nonarteritic ischemic optic neuropathy developed a spontaneous bulbar conjunctival defect. Despite intensive lubrication, and attempts at surgical closure including an amniotic membrane patch graft, it progressed with subsequent adjacent corneal perforation. Thorough investigations revealed no underlying disease, except markedly delayed episcleral vessel filling on anterior segment fluorescein angiography.Conclusions: Neovascularisation is a known factor in the inhibition of ulceration. In light of the findings in this report, ocular ischemia should be considered as a cause or contributing factor in the differential diagnosis of conjunctival-corneal melt.Keywords: conjunctival melt, corneal melt, ocular ischemia, carotid artery stenosis
A systematic review of inequalities in the use of maternal health care in developing countries: examining the scale of the problem and the importance of context

Abstract: two decades after the safe motherhood campaign?s 1987 launch in india, half a million women continue to die from pregnancy-related causes every year. key health-care interventions can largely prevent these deaths, but their use is limited in developing countries, and is reported to vary between population groups. we reviewed the use of maternal health-care interventions in developing countries to assess the extent, strength and implications of evidence for variations according to women?s place of residence and socioeconomic status. studies with data on use of a skilled health worker at delivery, antenatal care in the first trimester of pregnancy and medical settings for delivery were assessed. we identified 30 eligible studies, 12 of which were of high or moderate quality, from 23 countries. results of these studies showed wide variation in use of maternal health care. methodological factors (e.g. inaccurate identification of population in need or range of potential confounders controlled for) played a part in this variation. differences were also caused by factors related to health-care users (e.g. age, education, medical insurance, clinical risk factors) or to supply of health care (e.g. clinic availability, distance to facility), or by an interaction between such factors (e.g. perceived quality of care). variation was usually framed by contextual issues relating to funding and organization of health care or social and cultural issues. these findings emphasize the need to investigate and assess context-specific causes of varying use of maternal health care, if safe motherhood is to become a reality in developing countries.

The architectural design, being a subject-object interaction process, is claimed to be understood and explained in a comprehensive way . Architectural design as a hermeneutical discourse and text production process, is interrogated relative to divergent philosophical traditions perceiving whole in different perspectives.

Abstract: Este estudo investigou as rela es de gênero em crian as de quatro a cinco anos de idade que freqüentam grupos de educa o infantil na rede pública de ensino de Florianópolis. O objetivo central constituiu-se em perceber como as crian as v o construindo as identidades e os papéis de gênero nas rela es que estabelecem com seus coetaneos e os adultos mais próximos nos espa os educacionais. Embasamo-nos em Scott (1995) e Louro (1997) para as defini es sobre rela es de gênero e tomamos a idéia de identidade de gênero de Grossi 2 SAY O. D. T. A constru o de identidades e papéis de gênero na infancia... (1998) e Stoller (1993). Partimos do ponto de vista de que as intera es estabelecidas pelas crian as fazem parte do rol de experiências objetivas e subjetivas que v o criando as suas personalidades, bem como os papéis sociais de gênero. A investiga o, de cunho etnográfico, pautada em Geertz (1989) e Fonseca (1999), evidenciou que mesmo entre as crian as de pouca idade os papéis de gênero est o muito próximos daqueles vividos pelos adultos que convivem no mesmo contexto cultural, e as identidades de gênero s o experimentadas em diversos momentos de intera o entre as crian as. This study investigated gender relations in children from 4 to 5 years old who go to infant education groups in the public schools of Florianopolis. The main aim was to understand how children build indentities and gender roles in the relations they establish with their peers and with adults who are closer to them in these educational spaces. We base our studies in Scott (1995) and Louro (1997) on the definitions of gender relations and we borrow the idea of gender identity from Grossi (1998) and Stoller (1993) . We start from the idea that the interactions established by children are part of a number of objective and subjective experiences that create their identities, as well as their social gender roles. This investigation, of the ethnographic type, based on Geertz (1989) and Fonseca (2000) showed that even among very young children gender roles are very close to the roles lived by adults who live in the same cultural contexts and that gender identities are experienced at different stages of interaction between these children. Abstract: Uma das quest es centrais para a constru o de uma Pedagogia da Infancia é aquela que diz respeito aos lugares do corpo e suas experiências. O presente texto procura problematizar alguns pontos de tens o da rela o entre corpo e infancia, argumentando em favor de uma amplia o dos espa os de protagonismo do primeiro, no que se refere ao respeito às múltiplas express es da segunda. Para tanto, apresenta, em um primeiro momento, uma série de quest es que dizem respeito à tradi o ocidental que separa o corpo de uma dimens o que lhe seria alheia (“mente”, por exemplo), que por sua vez se materializa em diferentes disciplinas do conhecimento, em especial, uma certa ortopedia pedagógica. Logo a seguir o texto recorre a uma narrativa literária infantil para, ao mimetizá-la, recontar uma rela o de ensino por meio dos corpos: o da crian a como aluna, o da professora que constrange a corporalidade de ambas, o de um c o. Argumenta, ao final, pelo rompimento com este modelo, ainda presente, mas que exige sua supera o nos marcos de uma outra rela o, a de educar e cuidar, de professores e professoras com as crian as.One of the most important questions about the construction of a Childhood Pedagogy is related to the parts of the body and its experiences. This text tries to problematize some tension points of the relation between body and childhood, arguing for the extension of the importance of the body in reference to the respect of the multiple expressions of childhood. So, firstly the article lists a series of questions related to the western tradition that separates the body from a dimension that would differ from it (“mind”, for example), which materializes in different subjects of knowledge, specially a kind of pedagogic orthopedics. Then, the text brings a literary narration for children in order to, while producing its mimesis, retell a relation of teaching using bodies: the one of the child as a student, the one of the teacher that constrains the corporality of both of them and the one of a dog. Finally, it argues for the abandonment of this model, which is still present, but that needs to be suppressed by another relation, one of educating and taking care, of teachers and children.

Abstract: We give a new characterization of $\mathsf{NL}$ as the class of languages whose members have certificates that can be verified with small error in polynomial time by finite state machines that use a constant number of random bits, as opposed to its conventional description in terms of deterministic logarithmic-space verifiers. It turns out that allowing two-way interaction with the prover does not change the class of verifiable languages, and that no polynomially bounded amount of randomness is useful for constant-memory computers when used as language recognizers, or public-coin verifiers. A corollary of our main result is that the class of outcome problems corresponding to O(log n)-space bounded games of incomplete information where the universal player is allowed a constant number of moves equals NL.
