Translation, cultural adaptation and validation of a Spanish version of the Irritable Bowel Syndrome Severity Score Traducción, adaptación cultural y validación al espa ol del cuestionario de gravedad del síndrome de intestino irritable (Irritable Bowel Syndrome Severity Score)

Abstract: Background: the Irritable Bowel Syndrome Severity Score (IBSSS) is a questionnaire only available in English that classifies IBS patients according to the severity of their symptoms and can be used to guide and monitor the treatment. Aims: to adapt and validate into Spanish the English version of the IBSSS questionnaire. Methods: the Spanish version of the questionnaire was obtained through a process of translation-evaluation of the comprehensibility and back translation. In a later phase we evaluated the applicability (n = 15), reproducibility (n = 31) and sensitivity to change (n = 40) of the Spanish version of the questionnaire. Finally we evaluated an alternative version of the ISBSS using a numerical scoring system instead of the original analog visual scale (n = 40). Results: the Spanish version of the IBSSS showed an excellent reproducibility (r = 0.81 for global score) and an adequate sensitivity to change: a decrease of 45 points or more identified worsening of IBS with a 70.6% sensitivity and 87.5% specificity; an increase of 45 points or more identified improvement of IBS with a 85.7% sensitivity and 87.5% specificity. The severity score was practically the same regardless of the scoring system used (r = 0.96). Conclusions: the Spanish version of the IBSSS is a reproducible tool that is able to identify relevant changes over the course of the disease. The use of a numerical scoring system is a valid alternative to the visual scale that improves the applicability of the questionnaire to situations when the written communication is limited or not possible. Introducción: el "Irritable Bowel Syndrome Severity Score" (IBSSS) es un cuestionario disponible solo en inglés, que permite clasificar a los pacientes con SII en función de su gravedad y sirve como guía para orientar y valorar la respuesta al tratamiento. Objetivos: adaptar y validar para su uso en pacientes espa oles la versión inglesa del cuestionario de severidad del SII. Método: la versión espa ola del cuestionario se obtuvo mediante un proceso de traducción-valoración de la comprensibilidad y retrotraducción. En una segunda fase se evaluaron la aplicabilidad (n = 15), reproducibilidad (n = 31) y sensibilidad al cambio (n = 40) de la versión en castellano del cuestionario. Abstract: Introduction: the low prevalence of enterocutaneous fistulas (ECF) in patients with Crohn's disease (CD) justifies the scarcity of studies that analyze the effect of medical treatment and the need for surgery in these patients. The objective of this study was to evaluate the clinical characteristics of patients with CD who have ECF and the response to different treatments. Materials and methods: patients with CD who presented ECF fistula between 1970 and 2009 in three tertiary centers in Spain were analyzed retrospectively for their clinical characteristics, response to treatment as well as the correlation between the response to biological therapies for ECF and perianal disease. The inflammatory activity of the disease was measured using the Harvey-Bradshaw index. The definitions of partial or complete response were based on the treating physician's judgment. Results: twenty six patients were included (18 women); in more than half of cases (67%), anti-TNF treatment improved fistula drainage. Postoperative fistulas responded better to anti-TNF treatment than did spontaneous fistulas (80 versus 57%). One hundred percent and 60% of spontaneous and postoperative fistulas closed after surgery, respectively. Conclusions: although the majority of patients required surgery as the ultimate treatment, anti-TNF drugs improved fistula output in an acceptable percentage of patients who were treated. Therefore, this may be a treatment strategy prior to surgery in a select group of patients. Introducción: la baja prevalencia de las fístulas enterocutáneas (FEC) en los pacientes con enfermedad de Crohn (EC) justifica la escasez de estudios que analicen el efecto del tratamiento médico y la necesidad de cirugía en estos pacientes. El objetivo del presente estudio ha sido evaluar las características clínicas de los pacientes con EC que presentan FEC y la repuesta a los distintos tratamientos. Material y métodos: se analizaron retrospectivamente los pacientes con EC que presentaron FEC desde 1970 a 2009 en tres centros terciarios de Espa a analizando las características clínicas de los pacientes, las respuestas a los tratamientos así como la correlación entre la respuesta a terapias biológicas de la FEC y enfermedad perianal. La actividad inflamatoria de la enfermedad se midió mediante el índice de Harvey-Bradshaw. Las definiciones de respuesta parcial o completa se establecieron a juicio del médico responsable. Abstract: Background: in most cases, inflammatory bowel disease (IBD) debuts at reproductive age. The data available in the literature show infliximab (IFX) to be a safe drug during pregnancy but there is very little evidence about the activity of the disease following drug withdrawal during pregnancy. Aims: determine the drug′s safety in pregnant women in our setting and assess its effect on the foetus, drawing on the experience of several hospitals. Secondly, observe the effect of treatment withdrawal on disease activity during pregnancy. Material and methods: a retrospective study was conducted of women with IBD who had received IFX treatment during pregnancy in five hospitals in Spain. Disease activity was assessed using Crohn′s Disease Activity Index, while UC was assessed using the Truelove-Witts Index in each trimester of pregnancy. Gestational age, weight and diseases in the foetus were determined at birth. Results: the study included 12 women with a mean age of 29 years; 4 had ulcerative colitis and 8 Crohn′s disease, with mean disease duration of 7 years. All but one, who was diagnosed during pregnancy, was receiving IFX treatment at conception. Six patients received uninterrupted treatment throughout the pregnancy, 2 requested voluntary interruption and in 3 cases treatment was interrupted in the third trimester as a precaution. They received a mean IFX dose of 400 mg every 8 weeks. Of the 6 patients who received continuous treatment, in 50% disease was held in remission. The 6 remaining patients suspended treatment for different reasons, presenting disease recurrence in all but one case (83.3%). Eight deliveries were vaginal and 4 by caesarean section. Newborns presented no congenital anomalies, intrauterine growth retardation or low birth weight and there was only one premature delivery. Conclusions: although cases included in the stduy are not significant, in our experience, IFX during pregnancy is a safe treatment for the mother and the foetus. In fact, in our study and in some cases, its withdrawal may lead to a worsening of the disease. However, further control studies are required with larger samples to obtain more representative findings. Abstract: Tina Suárez Rojas es una autora contemporánea de Gran Canaria, Espa a. Su poesía sugiere una variedad de tópicos que se entrelazan con diferentes dicotomías: pasado y presente, tradición y modernidad, y mito y realidad, por ejemplo. El propósito de este artículo es estudiar el poema de Suárez Rojas "Ecce femina" extraído de su libro "Una mujer anda suelta" (1999a, 1999b). En "Ecce femina", Suárez Rojas se inspira en esa mujer singular que no es domesticada y que por ello no sigue las normas. No solo se considerará el feminismo como referencia teórica a los papeles tradicionales de las mujeres en la historia, sino que el término fictocriticismo también se entenderá como una forma feminista que se centra en ficción, crítica y teoría destacando la voz de la mujer en su propio contexto.

Abstract: sensory evaluation of nutritionally improved beans (phaseolus vulgaris l.) in two cuban communities. the objective of this study was to evaluate the sensory characteristics of nutritionally improved common bean (line 38) and a local varie ty (bat 304) among bean producers and consumers in holguín, cuba. an exploratory questionnaire revealed that small-seeded black beans were the most consumed, and that rice and bean casserole were the main recipes. three sensory tests were applied to 80 individuals from march to june 2008: triangle discrimination, acceptability, and preference. fifty participants correctly discriminated between the two bean varieties (p<0.000001). there was no statistical difference (p=0.9, on a scale of 1 to 4) in the number of participants who classified broth consistency, the taste of the casserole, and the texture of the beans. however, when words were used to describe and score these characteristics, there was a difference between the bean varieties in consistency and texture, but not in broth taste. seven participants indicated no preference for the casserole prepared with either bean. of the remaining 73 that expressed a preference, there was no statistical difference (p=0.13) in the proportion who preferred any bean type. in conclusion, there are sensory differences between the nutritionally improved and locally consumed beans, but preference was similar, which suggests good consumer acceptance of nutritionally improved beans.
Pattern of Regional Cortical Thinning Associated with Cognitive Deterioration in Parkinson’s Disease

Abstract: Background Dementia is a frequent and devastating complication in Parkinson’s disease (PD). There is an intensive search for biomarkers that may predict the progression from normal cognition (PD-NC) to dementia (PDD) in PD. Mild cognitive impairment in PD (PD-MCI) seems to represent a transitional state between PD-NC and PDD. Few studies have explored the structural changes that differentiate PD-NC from PD-MCI and PDD patients. Objectives and Methods We aimed to analyze changes in cortical thickness on 3.0T Magnetic Resonance Imaging (MRI) across stages of cognitive decline in a prospective sample of PD-NC (n = 26), PD-MCI (n = 26) and PDD (n = 20) patients, compared to a group of healthy subjects (HC) (n = 18). Cortical thickness measurements were made using the automatic software Freesurfer. Results In a sample of 72 PD patients, a pattern of linear and progressive cortical thinning was observed between cognitive groups in cortical areas functionally specialized in declarative memory (entorhinal cortex, anterior temporal pole), semantic knowledge (parahippocampus, fusiform gyrus), and visuoperceptive integration (banks of the superior temporal sulcus, lingual gyrus, cuneus and precuneus). Positive correlation was observed between confrontation naming and thinning in the fusiform gyrus, parahippocampal gyrus and anterior temporal pole; clock copy with thinning of the precuneus, parahippocampal and lingual gyrus; and delayed memory with thinning of the bilateral anteromedial temporal cortex. Conclusions The pattern of regional decreased cortical thickness that relates to cognitive deterioration is present in PD-MCI patients, involving areas that play a central role in the storage of prior experiences, integration of external perceptions, and semantic processing.
