Fístula arteriovenosa postraumática. A propósito de un caso: dos flagelos del subdesarrollo. A propósito de un caso atendido en Timor Leste

Abstract: the traumatisms that affect the extremities represent 80% of all the vascular traumatisms approximately. the inadequate handling of these it contributes to fateful consequences as the loss of the life or of the function of the extremity. the vascular lesions for firearm constitute one of the main causes with the appearance of arteriovenous fistula. we present the case of a 36 year old masculine patient with antecedents of having hurt 1 year ago by firearm in the root of the left thigh goes to present increase of volume of the extremity and difficulty to the march with sensation of fatigue and gravity. to the physical exam increase of volume of the limb more marked affection was verified in the root of the thigh, in the area related with the bullet impact thrill is felt and blow holosistólico is auscultated. the echo doppler and the tomography with contrast use evidenced the presence of a arteriovenous fistula. with this diagnosis was carried out surgical exploration and repair by means of veins bond for exclusion and interference of ilio-femoral arterial implant of politetrafluoroethylene (ptfe). the postoperative evolution was satisfactory with regression of all the clinical signs.

Abstract: a case of a patient of the nephrology service is presented that is integrated to the hemodialysis program since five years ago by presenting an irreversible chronic renal failure, not able for transplant, with multiple conventional vascular accesses carried out (direct and prosthetic) that have failed by progressive and generalized decay of the vascular system (arterial and venous), due to the effect of the associated pain to the renal disease and the triggering ones by the therapeutic intervention. because of that was surgically intervened and a vascular access at the level of the right inguinal region was performed with the objective to connect the venous system with the arterial one by means of an artificial prosthesis of polytetrafluoroethylene that was implanted in the form of a loop and in superficial planes to facilitate the access to the same one. the use of prosthetic implants of polytetrafluoroethylene in the surgery of vascular access for hemodialysis is a first election alternative when the conventional methods fail.

Abstract: Los tumores del cuerpo carotídeo, paragangliomas o quemodectomas son neoplasias poco frecuentes que se originan a partir de los tejidos del paraganglio, son generalmente benignos, de crecimiento lento y curso indoloro, presentan un gran componente vascular en su estructura. Pueden manifestar un comportamiento locorregional agresivo a pesar que histológicamente no muestren signos de malignidad. Se observan frecuentemente en pacientes que habitan en regiones de elevada altitud. Pueden ser esporádicos o familiares y se asocian en ocasiones a paragangliomas en otros sitios. Presentamos un caso clínico tratado en nuestro servicio en el que se evidencia la agresividad local en su comportamiento. Los traumatismos vasculares representan aproximadamente el 80 % de todos los traumatismos que afectan a las extremidades. El manejo inadecuado de estos ocasiona consecuencias funestas como la pérdida de la vida o de la función de la extremidad. Las lesiones vasculares por arma de fuego constituyen una de las principales causas de la aparición de fístulas arteriovenosas. Se presenta el caso de un paciente de sexo masculino de 36 a os de edad, con antecedentes de herida por arma de fuego en la raíz del muslo izquierdo, este paciente acudió a la consulta del servicio de Angiología, por presentar aumento de volumen en la extremidad y se le dificultaba la marcha, además sentía sensación de cansancio. Al realizar el examen físico se constató aumento de volumen del miembro afectado más pronunciado en la raíz del muslo, en la zona relacionada con el impacto de bala se palpó frémito y se auscultó soplo holosistólico. La ecocardiografía Doppler y la tomografía con uso de contraste evidenciaron la presencia de una fístula arteriovenosa. Con este diagnóstico se realizó una exploración quirúrgica y la reparación mediante ligadura venosa por exclusión e interposición de injerto arterial ilio-femoral de politetrafluoroetileno. La evolución postoperatoria fue satisfactoria con regresión de todos los signos clínicos.

Abstract: a literature review was made about rehabilitation techniques in general and of vascular diseases in special, rehabilitation in patients presenting with edema of a high limb after mastectomy from breast cancer, helps in improvement of its quality of life. here are the physiopathologic bases for rehabilitation, exercises program, and the use of a compressive glove for high limbs. our interest is to give advice to primary care physicians to increase knowledges and seek solutions for handicapped patients with vascular conditions. we propose activities and orientations for family physician and the rehabilitation methodology for patients with postmastectomy lymphedema.

Abstract: several arguments have been presented against content externalism. i deal here with one of these arguments, which -like some others- comes from boghossian. it is related to our inferential rationality (to use his terms). according to his reasoning, the apriority of our logical abilities is inconsistent with certain externalist assumptions. this conclusion is not justified. the problem posed by boghossian constitutes an important challenge for any theory of content, not just for externalism. furthermore, when we examine what internalists may propose to solve the problem, we see that externalists have at their disposal a more promising repertoire of possible replies than internalists. in that sense, insofar as boghossian's scenario is relevant to the debate externalism/internalism, it can be seen -against boghossian's original intention- as providing additional evidence for content externalism.

Abstract: i approach in this article one of the problems which purportedly would show the incompatibility of externalism and authoritative self-knowledge. the problem is based on some alleged consequences of externalism regarding the existence of substances or objects external to the thinker: if externalism is a priori, then a subject can know a priori that water exists by knowing a priori (by introspection) his thought about water. the two main compatibilist responses i work out exploit the role played by experience in a priori knowledge. taking into account the experience required to entertain the relevant thoughts makes less puzzling that a subject could acquire a priori knowledge about water's existence, or about a weaker related condition which is the real commitment of externalism.

Abstract: a bibliographic review of professional competence was made and concepts of competence, evaluation and conditions to create evaluative tools were analyzed. the different levels of professional performance were described according to miller′s model. reference was made to the newest evaluation test, the objective structured clinical examination, which is one of the most complete methodologies at present, although it is made clear that the evaluation is carried out under a created and simulated situation. it is concluded that the bases for making the evaluative tools that will measure the competence or incompetence of our medical sciences professionals should be structured.
