Recomendaciones nutricionales para adultos con sobrepeso corporal

Abstract: health can be supported with consumption of diets differing in a amazing way in its fats, carbohydrates and proteins content, whose net production of atp is of 90%, 75%, and 55%, respectively. change of its ratios in diet has a narrow rank generating poor differences in energy decrease. thus, recommendations to control of body weight must be mainly based more on effect of micronutrients and energy ingestion than that showed in yielding of apt. these recommendations must to consider that the selection of nutrients isn't similar for maintenance or reduction of body weight. body glycogen, genetic load, and life styles may influence on fat oxidation, and fatty tissue dimension, so, fat ingestion and oxidation be proportional. in cuba, obesity increase is generated by cultural patterns, food habits, and by a combined effect of changes in nutrients ingestion and physical activity on glycogen levels and body fat oxidation. biological and social evolution present in our country has leads to more effective mechanisms for searching of not much healthy foods, and not toward decrease of food energy. a future and better availability of desirable foods leads to an excess weight body in more than 50% of adult subjects. these nutrition recommendations must to contribute to prevention of this increase

Abstract: the requirement of food energy in adults is the amount of energy necessary to balance the energy output and to maintain the height, constitution, indicated and desired physical activity level and a good health status. the standing values for the cuban population were set by the institute of nuitrition in 1996. the new values of requirements for the cuban population starting from 2004 and the methodology for their estimation in population groups, based on height and the suitable weight for it in order to attain a body mass index of 21, are given in this paper. the new proposal has been made aimed at covering the nutritional requirements of this population group. attention has been focused on overweight and chronic diseases in the cuban population and the older adult. the values of food energy recommended to the adult population differ only 1 % from the previous ones and they have been structured according to the guidelines established by the fao/who/unu committee of energy experts in 2004.

Abstract: the nutritional recommendations of energy and nutrients, which are the basic working tool of nutrionists and health and economy workers and planners were updated . documents setting the guidelines on the amount of nutrients the population should receive to meet its requirements have been published at the international level since 1943. the nutritional recommendation refers to the amount of a certain nutrient capable of facilitating the normal functioning of the human metabolism in almost the totality of the population.the recommendations have practical ends and a merely population approach.they are regularly updated by international bodies in accordance with the results of the nutritional research. the recommendations made by the u.s. food and nutrition board, 2002, the fao/who/uno expert committees of energy and proteins, and the nutritional recommendations and food guides for the cuban population, 1996, were used as reference documents.

Abstract: El requerimiento de energía alimentaria en adultos es la cantidad de energía necesaria para balancear el gasto energético y mantener el tama o, la composición corporal, el nivel de actividad física indicado y deseado y un buen estado de salud. Los valores vigentes para la población cubana fueron establecidos por el Instituto de Nutrición e Higiene de los Alimentos en el a o 1996. En este trabajo se ofrecen los nuevos valores de requerimientos que rigen para la población cubana a partir del a o 2004 y la metodología para su estimación en grupos poblacionales, basados en la talla corporal y el peso idóneo para esa talla, con el objetivo de alcanzar un índice de masa corporal de 21. La nueva propuesta ha sido establecida con el propósito de cubrir requerimientos nutricionales de este grupo de población. Especial consideración han recibido la prevalencia de sobrepeso corporal y las enfermedades crónicas en la población cubana y en el adulto mayor. Los valores de energía alimentaria recomendados para la población adulta difieren solo en un 1 % de los anteriores y se han estructurado de acuerdo con los lineamientos del Comité de Expertos de Energía de FAO/OMS/UNU del a o 2004. Abstract: La salud puede mantenerse con el consumo de dietas que difieren asombrosamente en su contenido de grasas, carbohidratos y proteínas, cuya producción neta de ATP es 90%, 75% y 55% respectivamente. El cambio de sus proporciones en la dieta tiene un rango estrecho que genera despreciables diferencias en la disipación de energía. Por ello, las recomendaciones para el control del peso corporal deben basarse más en el efecto de los macronutrientes sobre el consumo alimentario y la ingestión de energía, que en el que muestran sobre el rendimiento de ATP. Ellas también deben considerar, que la selección de nutrientes no es similar para mantenimiento o reducción del peso corporal. Glucógeno corporal, carga genética y estilos de vida influencian la oxidación de grasa y la dimensión de tejido adiposo, de forma tal, que la ingestión y la oxidación de grasa resulten proporcionales. El incremento de la obesidad en Cuba esta generado por patrones culturales, hábitos alimentarios y por un efecto combinado de los cambios en la ingestión de nutrientes y en la actividad física sobre los niveles de glucógeno y la oxidación de grasa corporal. La evolución biológica y social en Cuba ha conducido hacia mecanismos más efectivos en la búsqueda de alimentos poco saludables y no hacia la reducción de la energía alimentaría. Una futura mejor disponibilidad de alimentos deseables conducirá a sobrepeso corporal en más del 50% de la población adulta. Abstract: Se realizó una actualización de las recomendaciones nutricionales de energía y nutrientes, los cuales constituyen el instrumento básico de trabajo de nutricionistas, trabajadores y planificadores de salud y economía. Desde el a o 1943 se han publicado al nivel internacional documentos que trazan las pautas de la cantidad de nutrientes que debe ser ingerida por poblaciones para satisfacer sus requerimientos. Se entiende por recomendación nutricional la cantidad de un nutriente determinado capaz de facilitar un normal funcionamiento del metabolismo del ser humano en casi la totalidad de una población. Las recomendaciones tienen fines prácticos y un enfoque meramente poblacional. Estas recomendaciones se actualizan con regularidad por organismos internacionales de acuerdo con los resultados de la investigación nutricional. Para este trabajo se han tomado como documentos de referencia las recomendaciones del Consejo de Alimentación y Nutrición de los EE.UU., 2002, los Comités de Expertos de Energía y Proteínas de FAO/OMS/UNU, 2004 y las Recomendaciones Nutricionales y Guías de Alimentación para la Población Cubana, 1996. Abstract: the literature review on body overweight and obesity disclosed alarming figures worldwide. figures provided by studies conducted in cuba were similar and included adults as well as children. this tendency grows as diabetes and blood hypertension increase, both factors having a joint incidence on morbidity and mortality from chronic diseases. prevention of body overweight and chronic diseases, an issue included in the world health organization strategies, has been consequently structured with a preventive approach throughout life. changes in lifestyles comprising food and regular exercising have determining influence in the world tendencies for these diseases. human genome has slightly varied in its evolution; however, trascendental changes have occured in the lifestyle of the last 2 to 3 generations leading to explosive behaviour of these problemas at the international level. the increased knowledge of gene-nutrient interaction is revolutionizing the management of huge epidemics that will appear in this century. particular examples on genes linked to the metabolism of energy, fat acids, folic acid, appetite control, energy output, tolerance to glucose and fat depot in the human body. the preparation of intervention programs aimed at the control of these disease should take this diversity into account.

Abstract: Se realizó una revisión del sobrepeso corporal y la obesidad, los cuales muestran cifras alarmantes al nivel internacional. Las cifras de estudios en Cuba son coincidentes e incluyen a adultos y ni os. Esta tendencia crece paralela al incremento de diabetes e hipertensión arterial, factores que en conjunto inciden sobre la morbilidad y mortalidad por enfermedades crónicas. La prevención del sobrepeso corporal y las enfermedades crónicas, incluida en las estrategias de la Organización Mundial de la Salud, han sido estructuradas consecuentemente con un enfoque preventivo a lo largo de toda la vida. Las modificaciones de los estilos de vida que comprenden a la alimentación y a la realización de actividad física regular tienen una influencia determinante en la tendencia mundial informada para estas afecciones. El genoma humano ha variado poco en la evolución, sin embargo, los cambios trascendentales ocurridos en el estilo de vida de las últimas 2-3 generaciones han generado el comportamiento explosivo en la prevalencia de estas afecciones al nivel mundial. El creciente conocimiento de la interacción gene-nutriente está revolucionando el manejo de las grandes epidemias que se avecinan para este siglo. Se ofrecen aquí ejemplos concretos sobre genes vinculados con el metabolismo de la energía, los ácidos grasos, el ácido fólico, el control del apetito, el gasto energético, la tolerancia a la glucosa y la deposición grasa en el organismo humano. La estructuración de programas de intervención dirigidos al control de estas afecciones debe contemplar esta diversidad. Abstract: the values of haemoglobin and of anti-helicobacter pylori igg antibodies were measured in 133 boys and 136 girls at school age from havana city in 2004. the helicobacter pylori infection is related to numerous affections of childhood. the reports about the existance of a relation to iron-deficiency anemia at school age and about the recovery from this affection after the erradication of the bacterium are usual. anemia is the main health problem in the cuban population connected with food and nutrition among shool age children. the prevalence of anemia and helicobacter pylori infection in school age children was 27 % and 70 %, respectively. the level of infection of the anemic children, without differences of sex, was 79 %, compared with 68 % of infection in non anemic children (p=0.462). the haemoglobin levels did not show assocation with helicobacter pylori infection in all the children. the associations between iron-deficiency anemia and helicobacter pylori were found in girls (p= 0.014), but not in boys (p= 0.940), maybe because girls are more vulnerable to iron-deficiency anemia. helicobacter pylori is capable of affecting iron absorption in the stomach and of exacerbating thus its deficiency in children at school age, among whom the iron supply is marginal.

Abstract: the seroprevalence of igg antibodies against helicobacter pylori detected by immunoassay was determined in a case-control study that included 59 patients with diabetes mellitus and 19 healthy individuals from havana city that were incorporated as a control group. patients were divided into 3 groups according to recent recruitment, treatment only with diet, or with diet plus hypoglycemic agents. the seroprevalence of anti- helicobacter pylori antibodies in the groups of diabetic patients was 90, 94 and 77 %, respectively, in relation to the value of 58 % of the control group. helicobacter pylori infection was markedly associated with diabetes mellitus in recently recruited patients with diabetes mellitus, or in those treated with diet only (p= 0.03, p= 0.01).
