Bone Transport in Tibial Gap Non-Union—A Series of 25 Cases  [PDF]

Abstract:

Gap non-union of the tibia is frequently associated with deformity, infection and shortening. Ilizarov’s method of bone transport was used in the management of twenty-five cases of gap non-union of the tibia. The mean bone gap was 6.53 cm (range 4 to 12 cm). Union was achieved in all cases within a mean period of 11.12 months. The mean time taken for union, per centimeter of bone gap was found to be 1.7 months/cm. Pin tract infection was the commonest complication, seen in 9 cases (36%). The other complications encountered were neuropraxia (n = 3) deviation of the transported bone segment (n = 7), buckling of skin at the advancing side of bone (n = 4), traumatic corticotomy (n = 3), incomplete corticotomy (n = 1), equinus deformity (n = 4), knee stiffness (n

Ilizarov′s method for treatment of nonunion of diaphyseal fractures of the humerus

Abstract: Background: Nonunion in diaphyseal fractures of the humerus can be treated by various modalities like plating and bone grafting, exchange nailing, fibular strut grafting and Ilizarov′s method of ring fixation. To achieve union in infected nonunion in which multiple surgeries have already been done is further challenging. We conducted a prospective study wherein the outcome of the treatment of nonunion of diaphyseal fractures of the humerus by Ilizarov′s method was analyzed. Materials and Methods: Nineteen patients with diaphyseal nonunion of the humerus were treated by Ilizarov′s external fixator. These included nonunion after plating (n=11), intramedullary nailing (n=1) or conservative methods (n=7). In post-surgical infected nonunion (n=6), the implants were removed, debridement done, bone fragments were docked followed by application of ring fixator and compression. In aseptic nonunion (n=13), distraction for three weeks followed by compression was the protocol. Early shoulder and elbow physiotherapy was instituted. The apparatus was removed after clinical and radiological union and the results were assessed for bone healing and functional status. Results: Fracture union was achieved in all the 19 cases. Pin site infection was seen in 2 cases (10.52%). The bone healing results were excellent in eighteen cases (94.73%) and good in one case (5.26%).The functional results were found to be excellent in fourteen cases (73.68%), good in four (21.05%) and fair in one case (5.26%). Conclusion: Ilizarov′s method is an excellent option for treatment of septic and aseptic non union of diaphyseal fractures of the humerus as it addresses all the problems associated with non union of the humerus like infection, deformity and joint stiffness.
Evaluation of the Cementation Index as a Predictor of Failure in Coonrad-Morrey Total Elbow Arthroplasty

Abstract: Background. The aim of this study is to objectively evaluate the quality of cementation by a novel method called the cementation index and assess its utility as a predictor of failure. Materials and Methods. Fifty elbows with primary Coonrad-Morrey total elbow replacement were included. The quality of cementing was assessed by novel methods, the vertical and horizontal cementation index, which were statistically evaluated as predictors of failure. The mean period of followup was years (range: from 5.08 to 10.25 years). Results. The mean vertical cementation index of the humerus (vCIH) was 1.22 ± 0.28 and that of the ulna (vCIU) was . Radiolucent zones were noted in two cases in the humerus with a horizontal cementation index of 0.21 and 0.14, respectively. Both of the cementation indices were not found to be statistically significant predictors of failure ( ). The five-year survival rate was 94%. Discussion and Conclusion. The cementation index, being a ratio, reduces the confounding effect of taking radiographs in different positions of the limb with different magnification in followup radiographs. It is an easy and objective method of assessment of cementation, the results of which need to be validated by a larger study. 1. Introduction Total elbow replacement is used in the reconstruction of elbows afflicted by rheumatoid arthritis, osteoarthritis, and trauma. There has been a significant improvement in outcome with changes in the implant design, from the initial linked and fully constrained prostheses to the present-day unlinked and linked, semiconstrained implants. Linked prostheses, like the Coonrad-Morrey implant, have been reported to have better long-term functional outcomes than unlinked implants, like the Souter-Strathclyde prosthesis [1, 2]. Aseptic loosening is one of the most common causes of failure of the prosthesis in the long term [3, 4]. Inadequate cementing has been established to contribute to this complication [5, 6]. The aim of this study is to objectively evaluate cementation by a novel method called the cementation index and assess its utility as a predictor of failure. 2. Materials and Methods Fifty elbows (forty-seven patients), wherein primary total elbow replacement was performed, were included in the study. The Coonrad-Morrey type III prosthesis was used in all cases. The mean age of the patients was sixty-seven years (range from thirty-two to eighty-eight years). The male: female ratio was 20?:?30 elbows. The procedure was performed on the dominant limb in thirty-five elbows and on the right-hand side in thirty-one elbows.

Abstract: New, simple and cost effective UV-spectrophotometric method was developed for the estimation of Duloxetine hydrochloride in bulk formulations. Duloxetine hydrochloride was estimated at 290 nm in 20% Acetonitrie. Linearity range was found to be 10–50 μg ml–1 (regression equation: 0.017 + 0.016; r2 = 0.999). The apparent molar absorptivity was found to be 5.922×10-3 mol 1 cm 1 in 20% Acetonitrile. These methods were tested and validated for various parameters according to ICH guidelines and USP. The quantitation limits were found to be 0.2405 μg ml–1 and 0.7289 μg ml–1 in 20% Acetonitrile respectively. The results demonstrated that the procedure is accurate, precise and reproducible (relative standard deviation < 2%), while being simple, cheap and less time consuming and can be suitably applied for the estimation of Duloxetine hydrochloride in different dosage forms and dissolution studies.

Abstract:

ADPKD is an inherited systemic disorder that predominantly affects the kidney, but may affect other organs including liver, pancreas, brain, and arterial blood vessel. APKPD occurs worldwide affecting about 1 in 500 - 1000 people. Hypertension is the most common manifestation of ADPKD and the major contributor to renal disease progression. A definite diagnosis of ADPKD relies on image testing. Renal ultrasound is commonly used because of its cost effectiveness.

Abstract: This paper presents a new current mode (CM) single-input and multi-output (SIMO)-type biquad using two multiple output OTAs and one current follower as an active device and having two grounded capacitors. This SIMO type circuit realizes all the five filter functions as low pass, band pass, high pass, band reject and all pass filter transfer functions simultaneously. This circuit has the unity gain transfer function for all the five types of filters. The circuit enjoys electronic tunability of angular frequency and bandwidth. The 0.18 μm TSMC technology process parameters have been utilized to test and verify the performance characteristics of the circuit using PSPICE. The sensitivity analysis, transient response and calculations of total harmonic distortion have also been shown.

Abstract: In this study, the factor analysis techniques is applied to water quality data sets obtained from the Sanganer Tehsil, Jaipur District, Rajasthan (India). The data obtained were standardized and subjected to principal components analysis (PCA) extraction to simplifying its interpretation and to define the parameters responsible for the main variability in water quality for Sanganer Tehsil in Jaipur District. The PCA analysis resulted in two factors explaining more than 94.5% of the total variation in water quality data set. The first factor indicates the variation in water quality is due to anthropogenic sources and second factor shows variation in water quality due to organic sources that are taking place in the system. Finally the results of PCA reflect a good look on the water quality monitoring and interpretation of the surface water.
