Abstract: this paper presents some experiences and practices in the field of psychosocial rehabilitation and attention directed to mental disorder developed in three different regions of spain. this is a set of tools that make possible consistent changes in mental health care, which are linked into long lines, pillars of the psychiatric reform process both in spain and in brazil: redefining the management of institutional arrangements, the prospect continuity of care, primary care as the gateway and system regulator, the creation of units behind the crisis and attention to emergencies and, especially, investment in rehabilitation programs and community support contemplating alternative housing and work

Abstract: the aim of this paper is to reflect about some elements that are defining the professional culture of the psychologist in brazil, as well as its effects upon the institutional practices in the public health service. the "psychological subject", model of subjectivity among psychologist is one of the defining elements of this professional culture, representation developed from the individualistic values and psi knowledge in our society. the hegemony of this conception of subjectivity has important consequences in the practices accomplished in the public health institutions, such as conflicts in the representations of health and illness between users and professionals; little efficiency in the therapeutic services and high index of desertion of treatment; hierarchies and discrimination among service users. on the other hand, there always appear obstacles to the creation of an "evaluation culture" among professionals and the construction of instruments that could allow the psychologist to continuously evaluate the quality of their services and practices in the public health institutions

Abstract: this is a report on a professional experience in obstetrics and gynecological services at a public maternity in rio de janeiro. the purpose is to elucidate the meanings of prematurity from the point of view of mothers of babies prematurely born and interned on high and medium risk nurseries. data here discussed have obtained through different procedures: participative observation in infirmary and nursery services; half-structured interviews with mothers; weekly discussion groups with users and health team, individual attendance on mothers and babies’ relatives. in general, we can say that, within this specific context, there are frequent hindrances in communication between health team, mothers and relatives, due to different speech styles as well as the different beliefs and values in relation to prematurity. that scene leads to low agreement and little mothers’ adhesion to treatment and medical prescriptions, which has serious implications on babies’ cares. the psychologist, as part of the health team, may play an important role as intercessor and stimulator of more humanized relations inside health services having, this way, an impact on quality of given attendance.

Abstract: the aim of this paper is to reflect on some issues currently under discussion in the field of collective health, and which represent a great challenge for health managers and professionals of the brazilian unified health system. this paper focuses not only on the profile changes that are needed of all health professionals, but rather on the transformation of these professionals into socially committed agents responsible for the renovation of the ethos of the brazilian health system and its clients. a more in-depth discussion of the psychologist?s place in such a context, his practice within public health care, and on the much-desired social commitment is carried out. the analytical tools that will be used are the results of two investigations that were conducted with psychologists that work in the basic health care network of the cities of natal, in the state of rio grande do norte, and teresina, in the state of piauí.

Abstract: in brazil, the practice of psychologists in public health-care institutions is hampered by the professional training that they have received. that training is inadequate in meeting the requirements of primary health care, in satisfying the needs of the patients who use the public health-care system, and in overcoming the social inequalities that exist in brazil. this piece is intended to identify the factors that make it harder to provide adequate psychological care in public health-care institutions, especially in the local basic health units of brazil's public health-care system, the unified health system. these hindrances to providing good care mainly originate in training that points psychologists in the direction of very restricted models of health care. these narrow models prevent the psychologists from responding appropriately to the needs of their patient clientele and of the public institutions. these limited models also make it hard for psychologists to adapt to the dynamic conditions that they will face in the unified health system.
La reforma psiquiátrica y el modelo de atención psicosocial en Brasil: en busca de cuidados continuados e integrados en salud mental

Abstract: Este artículo pretende contribuir al debate sobre los rumbos de la reforma psiquiátrica que actualmente se lleva a cabo en Brasil, presentando un panorama nacional sobre sus avances y desafíos. En este sentido, se pone de manifiesto la coexistencia de dos paradigmas de atención, uno basado en la institución del manicomio y otro enfocado en la atención psicosocial. El paralelismo entre una y otra modalidad genera importantes tensiones en la prestación de los servicios de atención, a la vez que pone en evidencia el carácter vertical del saber psiquiátrico, los atributos hegemónicos de ciertas especialidades y la fragmentación de los procesos de trabajo. Una revisión de tales tensiones y características permitirá identificar qué aspectos en concreto del modelo de reforma brasilero requieren de un fortalecimiento así como de una mayor inversión.

Abstract: Objetiva-se, neste trabalho, desenvolver uma reflex o em torno de algumas quest es que se configuram como desafios importantes para gestores e profissionais ligados ao Sistema único de Saúde - SUS. Diz respeito, também, à discuss o sobre a qualidade e humaniza o das a es de saúde, dentre as quais se inserem aquelas do campo da saúde mental. Ambos os conceitos vêm ocupando um lugar de destaque no processo de reforma da assistência psiquiátrica em curso no Brasil e no discurso dos profissionais do campo, norteando os caminhos por onde deve trilhar a aten o em saúde mental. Trata-se de uma tentativa de abordagem da qualidade e humaniza o de uma perspectiva diversa daquelas que associam esses conceitos à lucratividade, desospitaliza o e assistencialismo, de articulá-los ao princípio da desinstitucionaliza o enquanto nega o da lógica manicomial a que subjazem subjetividades e práticas e de instrumentalizá-las enquanto dispositivos éticos comprometidos com a inven o de novos modos de vida. The objective of this paper is to reflect upon some issues that constitute challenges to the Unified Health System managers and professionals, the quality and the humanization process in the health actions in mental health. Both concepts hold a prominent place in the psychiatric care reform process currently established in Brazil, as well as in the discourse of the professionals in mental health care. It discusses the quality and the humanization concepts differently from the traditional financial gains, nonhospitalization and assistencialist perspective. Instead they are linked to the principle of noninstitucionalization as a negation of the asylum logic that undermines subjective practices and enforces them as ethical mechanisms in the search for innovative life modes.
