Social cost of mental disorders: Disability and work days lost. Results from the Mexican survey of psychiatric epidemiology

Abstract: Introduction When the impact of illness is evaluated by indicators like mortality, mental illness has a less significant impact than other illnesses. As a result, the impact of mental disorders was underestimated until the last two decades of the previous century. This perception began to change as a result of two factors: On the one hand, the study of the Global Burden of Disease reported by Murray and Lopez, and, on the other hand, the definition of mental disorders by the American Psychiatric Association. The common element shared by these two factors is the inclusion of the concept of disability. Disability is the deterioration of the expected functioning of a subject of a particular age and sex in a social context. It is a part of the social cost of illness. Objective To assess the disability burden associated with depression, mania, agoraphobia, social phobia, general anxiety, panic disorder, and post-traumatic stress disorder (PTSD) according to the Mexican Psychiatric Survey and to compare results with the disability produced by some chronic non-psychiatric conditions. Method This survey is based on a stratified, multistage area, probabilistic sample of adults living in urban areas of Mexico. The instrument used was the third version of the Composite International Diagnostic Interview. We report the 12-month prevalence of psychiatric disorders as defined by DSM-IV criteria. We also evaluated non-psychiatric chronic conditions like diabetes, arthritis, hypertension, backache, and other painful illnesses, identified in general as chronic conditions . Indicators of disability were Sheehan s scale and number of work days lost. This is an easy and fast self reporting scale, which can be used both in the clinic or research. The sub-scales can be added or averaged to obtain a total score. The scale of responses is a horizontal line with numerals from 0 to 10 and five verbal descriptions, with the description Not at all corresponding to a value of 0; Mild rangimg from 1 to 3; Moderate from 4 to 6; Severe from 7 to 9; and Very severe corresponding to 10 Results Close relationships and social life were the areas most deeply affected. The disorders found to produce the highest levels of disability were depression, social phobia, and PTSD. The lowest disability levels were observed in chronic conditions. On the total score of Sheehan s scale, disorders with the highest level of disability were PTSD (mean 5.35 ± 0.307) and depression (mean 4.72 ± 0.167). Abstract: objectives: identify whether individualistic or collectivistic values prevail within the nursing profession and whether the profile of prevailing values remains the same for both men and women. check whether there exist gender differences in the degree to which these professionals participate in both individualistic and collectivistic values. material and methods: the scale of measure used is the evat-30. the participants were 186 female and male nurses from an public hospital, located in the south of spain. analyses of mean difference, t tests, and manova analyses were carried out. results: both genders value more individualistic work values than collectivistic values. female nurses show a higher preference for individualistic values than male nurses, there being no gender differences in respect to collectivistic values. conclusions: we suggest the necessity to explore in greater depth the analysis of these values relating them with the efficency of nursing workgroups and to identify the factors that determine the gender differences.

Abstract: Objetivos: Comprobar si en los profesionales de enfermería predominan los valores individualistas o los colectivistas y si el perfil de valores predominante se mantiene tanto en las mujeres como en los hombres. Comprobar si existen diferencias de género en el grado en el que estos profesionales participan de los valores individualistas y colectivistas. Material y Métodos: La escala de medida utilizada es la EVAT-30. Los participantes fueron 186 enfermeros de un hospital público ubicado al Sur de Espa a. Se realizaron análisis de diferencias de media, prueba t y análisis MANOVA. Resultados: Ambos géneros valoran más los valores laborales individualistas que los colectivistas. Las enfermeras prefieren más los valores individualistas que los enfermeros, no existiendo diferencias de género en los valores colectivistas. Conclusiones: Se sugiere la necesidad de profundizar en el análisis de estos valores relacionándolos con la eficacia de los equipos de trabajo en enfermería e identificando los factores que determinan las diferencias de género. Comparación de dos métodos de seguimiento de las fístulas arteriovenosas protésicas incidentes en un área de salud

Abstract: vascular access (va) is an important challenge, on which the patient's quality of life depends. object: to find out whether a computerize va monitoring programme reduces the rate of thrombosis of incident prosthetic avf (ip-avf). material and methods: observational prospective study of ip-avf monitoring, between 2007 and 2009, in area 8 of the cam. nursing recorded on an access database: dynamic venous pressure, negative arterial pressure and pump flow in the first hour of dialysis. we compared the results with a historical control group (hcg) of ip-avf from 2004-2006, when data were only recorded on the nursing chart. ip-avf thrombosis after 2006 was not valued. results: diabetes, sex and age were not related to thrombosis in any group. ip-avfs at risk in 2007-2009 were: 49 v. 45 of the hcg. we compared the results for the period 2007-2009 compared to the hcg: annual thrombosis rate: 0.37(18/49) v. 0.67(30/45), (p<0.003). median first episode of thrombosis 217 v.167 days. annual rate of fistulographies due to dysfunction 0.98(48/49) v. 0.51(23/45), (p<0.000). positive predictive value: 94% v. 87%. sensitivity: 82% v. 44%. diagnosed stenoses 45 v. 20. annual repair rate for dysfunction: 0.80(39/49) v. 0.31(14/45), (p<0.000). annual repair rate before first thrombosis: 0.41(7/17) v. 0.19(4/21), (p<0.001). annual repair rate for thrombosis: 0.43(21/49) v. 0.69(31/45), (0.01). through the computerized recording of va parameters, nurses contribute to diagnosing pre-thrombosis events in ip-avfs, increasing the number of preventive repairs for dysfunction and reducing the rate of thrombosis.
Program to encourage the process of attachment in children with developmental disorders pervasive through children’s massage

Abstract: During the first year of life, children develop a double-faced emotional tie with their parents or caregivers that provide them the essential confidence to approach psychological and social competences. This special link is called attachment.Attachment may be modified by different situations like the absence of an appropriate person to make tie, the absence of an identification through physical care are assured or the impossibility to assure the child psychical and emotional care (due to depression, psychosis, interest failing or any other wide range of subjects). So, during the first year of life, children may suffer from several kinds of disorders.According to DSM-IV-CR, Pervasive Developmental Disorder is characterized by difficulties in social relations, imaginative activities and verbal and nonverbal communication as well as reduced and repetitive interests.This work aims to decreasing anxiety and improving attachment between children with PDD and their mothers through the infantile massage. Establishing a program that involves 5 sessions of 90 minutes each, with theoretical and practical information about infantile massage, crying, birth, life changes, different roles, feelings of guilt, etcetera. Thanks to it, children with PDD get a quantitative and qualitative improvement of attachment that gets them closer to the external world, reduces their anxiety for changes and allows them a better adaptation to life.
Correlación clínico-histológica de la nefritis lúpica

Abstract: a total of 643 renal biopsies of patients admitted to the service of nephrology of university pediatric hospital in centro habana from january 1988 to december 1998 were examined, of which 29 corresponding to lupus nephritis were selected, accounting for 4.51% of the total. at the moment of the diagnosis, 25 patients were over six years-old (86.2%) with predominance of females with 20 patients for 68.9%. 75.86% of cases had renal disorders, 7 patients with nephrotic syndrome, 11 with hematuria, 3 blood hypertension and one with nephritic-nephrotic syndrome and chronic renal failure. other frequent clinical manifestations were arthritis in 48.27% of cases and facial erythema (37.93%). according to a histological classification, 12 patients showed lupus nephritis class ii, 8 had class i and 8 classes iv. only one case had class iii. the renal tissue examination on immunofluoresce microscope revealed immunoglobulin deposits and hemolytic complement fractions characteristic of the disease. in the correlation of clinical manifestations and histologic classification, it was found that in nephritis class i the renal disorders were nephrotic syndrome and hematuria; in class ii, nephrotic syndrome , hematuria and blood hypertension; in class iii, hematuria and in class iv, nephrotic syndrome, hematuria, blood hypertension, nephritic-nephrotic syndrome and chronic renal failure.

Abstract: Se revisaron un total de 643 biopsias renales a pacientes ingresados en el Servicio de Nefrología del Hospital Pediátrico Universitario "Centro Habana" en el período comprendido entre enero de 1988 y diciembre de 1998; de éstas se seleccionaron 29 correspondientes a nefritis lúpica, para el 4,51 %. En el momento del diagnóstico, 25 pacientes tenían más de 6 a os de edad (86,2 %), con predominio del sexo femenino, 20 pacientes, para el 68,9 %. En el 75,86 % de los casos estuvieron presentes las alteraciones renales, 7 pacientes con síndrome nefrótico, 11 con ematuria, 3 con hipertensión arterial y 1 con síndrome nefrítico-nefrótico e insuficiencia renal crónica. Otras manifestaciones clínicas frecuentes fueron la artritis en el 48,27 % y el eritema facial (37,93 %). Según la clasificación histológica, 12 pacientes mostraron nefritis lúpica clase II; 8, clase I e igual cantidad clase IV y 1 caso con clase III. En el estudio del tejido renal al microscopio de inmunofluorescencia se hallaron los depósitos de inmunoglobulinas y fracciones del complemento hemolítico característico de la enfermedad. En la correlación entre las manifestaciones clínicas y la clasificación histológica, se encontró en la nefritis clase I, que las alteraciones renales que se presentaron fueron síndrome nefrótico y hematuria; en la clase II, síndrome nefrótico, hematuria e hipertensión arterial; en la clase III, hematuria en la clase IV, síndrome nefrótico, hematuria, hipertensión arterial, síndrome nefrítico-nefrótico e insuficiencia renal crónica. Abstract: background: the current trend in patient care is towards a more humanized and higher-quality healthcare. the objective of the present study was to evaluate the results of implementing a programme of healthcare with a reference nurse who welcomes the patient at hospital admission, visits regularly during hospital stay, and resolves doubts and problems. methods: quasi-experimental study (149 cases and 454 non-tutored controls) in patients admitted for scheduled trauma surgery at the virgen de las nieves university hospital, granada. sociodemographic and clinical data were gathered at admission. at 15 days after discharge, patients were administered with an interviewer-administered zung score and anad hoc questionnaire on satisfaction with different healthcare aspects during hospital stay and understanding of information received. bivariate analyses and linear and multivariate logistic regression analyses were performed with a study of confunding variables. results: for the outcome variable anxiety level non-tutored patiens had a b coeficient of 2.64 (p<0,01) in the multivariate linear regression analisys controlling for the other variables in the final model: sex, presence of informal carer, professional activitiy, days of hospital stay and understanding of health information. for the outcome variable inadequate understanding information non-tutored patients showed an odds ratio of 3.48 in the multivariate analysis controlling for educational level and presence of informal carer. satisfaction with he friendliness of health care personnel and with the hospital setting did not significantly diference, although the percentage of dissatisfied patiens was higher in the non-tutored group: 15% vs 11% (p= 0.34) and 18% vs 12 % (p= 0.11) respectively. conclusions: the implemented continued care programme proved effective for these patients, increasing their understanding of information received and reducing anxiety levels.
Utilidad para establecer diagnóstico y severidad de los síntomas y signos más frecuentes en la paciente preeclámptica

Abstract: objective: our aim was to determine that signs and symptoms are tools in establishing diagnosis and severity of preeclampsia. materials and methods: our study design was prolective, comparative, cross-sectional for evaluation of diagnosis. our sample included 408 patients. the study employed classification criteria of the american college of obstetricians and gynecologists. one blinded family physician interrogatedandexaminedeach patient. the sample included patients with recent diagnosis and without treatment. patients with hellp syndrome, eclampsia, and those in intensive care were excluded. clinical signs evaluated included headache, phosphenes, acuphenes, tinnitus, vomiting, epigastric pain, right hypochondrium pain, ecchymosis, hematomas, and hyperreactive reflexes. results: a total of 192 patients without preeclampsia, 63 with mild, and 153 with severe preeclampsia were included. clinical manifestations were absent in 60,21 and 8% respectively of patients in each group. presence of three or more signs or symptoms had sensitivity of 60% (cl95% 53-67), specificity of 84% (cl95% 79-89), and positive likelihood ratio of 3.8 and negative, 0.48. most usefulness data for diagnosis of preeclampsia are hyperreactive reflexes, phosphenes, acuphenes, right hypochondrium pain, and epigastric pain. conclusions: the symptoms and signs taken alone are tools for evaluation of severity but not for detection of preeclampsia. there is necessary to develop new way for it's diagnosis during prenatal care.

Abstract: prevention and health promotion activities are dealt with in the general population by the programme of preventive activities and health promotion (papps). given their high joint morbility and immunological alteration, patients in haemodialysis (hd) should be highly compliant. the aim of our work was to use a transversal observational study on 72 patients in outpatient hd to see the fulfilment of some activities of the adult sub-programme of the papps. results. recommendations on lifestyle: smoking, 19.4% smoked an average 14.78 cigarettes/day. alcohol, 41.7% were consumers of an average 106.08+/-87.1 g/week in men and 79.6+/-91 g/week in women. cancer prevention programme: breast cancer, average overall fulfilment was 54.4%; 42.8% completely, 35.7% incompletely and 21.4% had no mammography. cervical cancer, average overall fulfilment was 59%; 56.2% fulfilling 100%, 43.7% incompletely and 18.75% of patients had had no kind of cervico-vaginal cytology. infectious disease prevention programme: 19.4% were vaccinated against pneumococo; 22.2% against tetanus-diphtheria; 73.6% against influenza and 97.7% against hepatitis b. activities to prevent alternations in the vision of the elderly: average fulfilment was 30.34%; 13.6% completely; 52.3% incompletely and 34.1 % had no ophthalmologic revision. compliance with the recommendations of the papps in hd is low. nursing should take an active role in increasing these activities.
