Abstract: knowledge about foot care among diabetes mellitus (dm) patients contributes to avoid future complications. a quasi-experimental study undertaken at health centers in san luís potosí, mexico, aimed at describing the sociodemographic and clinical profile of dm patients and their foot care habits before and after an educative intervention, based on participatory and traditional communication. the study was developed in two groups. the experimental group participated in the foot care education program using participatory communication, while the control group used the traditional communication method. for data collection, interviews were held at three different times: before the start, at the end of the program and six months after the end of the intervention. most subjects were female, with a mean age of 52 years, had not finished primary education and, besides dm, suffered from arterial hypertension. as regards foot care habits, statistical tests showed a significant difference (p <0.001) between the first and second measure, which may be due to the effect of the course and the better results achieved through the education program that used participatory communication. thus, it was concluded that the effect of the educative intervention based on participatory communication entailed positive changes in the diabetic patients' foot care, and also favored learning and the choice of care conducts.

Abstract:

Objectives: Personal beliefs about illnesses have received increasing interest because these cognitions help to explain and predict preventive and therapeutic coping efforts, adjustment to a disease and health outcomes. We sought to explore and compare non-specialised illness representations of hypertension among adults never suffering from hypertension who had and had not lived with hypertensive patients. Design: Hypertension representations were explored in a community-based, convenient sample of normotensive Spanish adults of both genders from different educational backgrounds and with different family experience with this illness. Method: An adapted Illness Perception Questionnaire-R was used to assess such perceptions among healthy people in nine dimensions: Identity, Time-line, Consequences, Personal Control, Treatment Control, Illness Coherence, Evolution, Emotional Representations and Causes. Results: The participants’ beliefs mixed accurate and folk knowledge, while gender, age and education level had little impact, family experience (having or not having a relative with hypertension) strongly determined the content of hypertension representations. Participants with family experience held significantly stronger beliefs of controllability of the disease, both by patients and treatments, considered the disease as less stable and reported a lower emotional impact when thinking on suffering from hypertension. Family experience was the only significant predictor of illness cognitions. Conclusions: This study allowed us to know the perceptions of hypertension among non-patients and healthy relatives

Abstract: systemic inflammatory response (sir) constitutes generalized, non- specific response to tissue injury of whatever etiology, and is a rapid, highly amplified, controlled humeral and cellular response. cardiopulmonary bypass (cpb) is necessary in many cardiac surgery as in adults as children. also we know the undesirable effects of sir. the pediatric surgical team to treat of management very well if exist the threat of undesirable outcome after cpb. there are several key components of the inflammatory response to cardiac surgery involve the complement, immune and endothelial systems. cytokines may exert either proinflammatory or antiinflammatory effects. cytokines are essential for immunologic and physiologic homeostasis, are normally subject to thight homeostatic control, and are produced in response to a variety of physiologic and pathologic stimuli. an uncontrolled inflammatory response appears to play a significant role in the morbidity or mortality observed in patients undergoing cpb. the inflammatory response contributes to the pathogenesis of acute pulmonary, cardiovascular, neurologic, splanchnic, hematologic, and immune system dysfunction following cardiac surgery. the development of strategies to control the inflammatory response following cardiac surgery is currently the focus of considerable research efforts. diverse techniques, including maintenance of hemodynamic stability, minimization of exposure to cpb circuitry, and pharmacologic and immunomodulatory agents have been studied. also hemofiltration, leukodepletion, the use of serine protease inhibitors and corticosteroids. molecular biology is revolutionizing medicine and the ability to assess the impact of genetic variability on disease characterization and perioperative outcome. recent evidence suggests that the degree and severity of surgical- induced inflammation may be significantly influenced by genotype.
