Repercusión del acceso vascular sobre la calidad de vida de los pacientes en tratamiento con hemodiálisis

Abstract: in all patients diagnosed with chronic renal failure is essential to facilitate an adequate treatment and to prevent complications. when this premise is resolved, the attention is directed to assist these patients to live as well as possible with their disease, improving their quality of life ultimately. objective: to determine the extent vascular access influences the health related quality of life related of hemodialysis patients. methodology: cross-sectional study in a population of 38 hemodialysis patients. euroqol 5d visual analogue scale was used to evaluate their perceived health status. patients were asked about their health status at present time, and without the inconvenience and discomfort which gives them their vascular access. results: visual analogue scale mean score of patient with an arteriovenous fistula was 58.75 ± 20.31points vs60.56 ± 17.39 points of patients with central venous catheter (no statistically significant). conclusions: according our study, there is no significant influence of the vascular access on the patient's perceived quality of life. the major inconveniences and discomfort associated with vascular access occur in patients with a central venous catheter, who are treated with hemodialysis more than 1 year.

Abstract: Con el objetivo de encontrar nuevas aplicaciones y/o formas de procesamiento para el aceite cíclico ligero (ACL) que es un producto de la desintegración catalítica que usualmente era empleado como componente de la carga a hidrotratamiento para la producción de diesel, se obtuvo la distribución química a lo largo de la curva de destilación. Se presenta también la distribución química y la caracterización completa de seis muestras diferentes de aceite cíclico ligero provenientes de refinerías mexicanas. Para lograr este objetivo se desarrolló un nuevo método de análisis cuantitativo utilizando cromatografía líquida de alta eficiencia (CLAE).
Nursing procedure in the temporary transvenous pacemaker implantation

Abstract: The temporary transvenous pacemaker (MPTT) implantation is a technique that consists in stimulating artificially the heart when the natural pacemaker is unable to maintain the rhythm and the appropiate frequency. This type of electric stimulation is carried out by means of an electrocateter that was implanted in the endocardium through a central vein connected to an external generator, requiring equipment and personal with specific abilities. It is commonly carried out in the intensive care units when a continuous temporary stimulation is suitable and in most cases it is carried out in situations of extreme medical urgency in critical and unstable patients. The nursing professionals play an important role in the whole procedure, becoming essential that they possess knowledge not only about technique and handling of the device but also on the performance in all its phases. This research work intends to be a practical reference that gathers those essential aspects for the development of this assistance practice including, in a detailed way, the nursing performance in the preparation, the implantation and the pursuit of the patients who a temporary transvenous pacemaker was placed in.

Abstract: Three quarters of ankle injuries are diagnosed as sprains. For the most part sprains are caused by a forced inversion movement with involvement of the lateral collateral ligament (LCL). One of the recommended guidelines is immobilization by taping. The aim of this article is to unify the recommendations for nursing, on taping in the treatment of ankle sprains. The methodology used was a literature review, analyzing the information found in books and journals in hospital libraries and nursing databases on the Internet. The main results are a set of guidelines for the most accurate and therapeutic taping.

Abstract: El análisis de bioimpedancia es una herramienta útil para determinar el estado nutricional y de hidratación del enfermo en hemodiálisis. Los dos sistemas más utilizados son la bioimpedancia de monofrecuencia vectorial y la bioimpedancia multifrecuencia espectroscópica que utilizan diferentes criterios para clasificar el estado de hidratación del enfermo dializado. El sistema de bioimpedancia de monofrecuencia vectorial utiliza una escala ordinal de 7 puntos que va desde el valor 3 (mayor hidratación) hasta el valor -3 (mayor deshidratación) definiendo la hiperhidratación cuando el vector está en el eje de hidratación por debajo de la elipse de tolerancia del 75% (valores 3 y 2 de la escala ordinal). El sistema de bioimpedancia multifrecuencia espectroscópica emplea dos criterios para definir el estado de hiperhidratación prediálisis: OH superior a 2,5 litros o mayor del 15% del volumen de agua extracelular. El objetivo del presente trabajo fue analizar el grado de concordancia entre ambos sistemas de bioimpedancia para definir el estado de hiperhidratación en enfermos tratados con hemodiálisis. Se llevó a cabo un estudio transversal en 54 enfermos tratados con hemodiálisis a los que se les realizó un análisis de bioimpedancia con monitores de monofrecuencia vectorial y multifrecuencia espectroscópica inmediatamente antes de una sesión de hemodiálisis. El grado de equivalencia en la definición de hiperhidratación entre ambos monitores fue mejor cuando se utilizó, en el monitor de multifrecuencia, el criterio de OH>15% del agua extracelular (índice kappa 0.81, concordancia excelente) que cuando se utilizó el criterio de OH>2,5 litros (índice kappa 0.71, concordancia aceptable). Conclusiones: Hay una buena relación entre la escala vectorial de hidratación y el valor OH (exceso de hidratación). Los criterios utilizados por los monitores de monofrecuencia vectorial y multifrecuencia espectroscópica para definir los estados de hiperhidratación prediálisis tienen una equivalencia aceptable y sus resultados pueden ser intercambiables. Bioimpedance analysis is a useful tool for determining the nutritional and hydration status of haemodialysis patients. The two most commonly used systems are bioimpedance vector single frequency analysis and bioimpedance spectroscopic multiple frequency analysis, which use different criteria to classify the hydration status of the dialysed patient. Abstract: nowadays it is indisputable that kidney transplant is the preferred replacement therapy for patients with chronic kidney disease. in our hospital we carry out basically two types of kidney transplants, the classic brain dead donor and the asystolic donor. the aim of our study was to evaluate whether there were differences in the workload of nursing staff depending on the type of transplant. this is a transversal study of 73 patients who received a kidney transplant in 2010, 18 of which were from a brain dead donor and 55 from an asystolic donor. the kidney transplants from asystolic donors showed a higher incident of primary graft failure (15% v. 1.4%) and a greater delay in kidney function (18 days v. 0 days). because of this, our care task was increased in this type of transplant as a greater number of kidney biopsies were required which involve care in preparation and supervision of complications, the need for more haemodialysis session and the administration of a more doses of thymoglobulin. no significant differences were identified in the incidence of infections between the two types of transplant, or in the average hospital stay although the latter could be influenced by the size of the sample. despite the complications mentioned above, kidney function in the medium term in both types of transplant was similar, which encourages the use of asystolic donors.

Abstract: qualitative research is dynamic and flexible and is, particularly, oriented into research process. nature of phenomena which are studied from qualitative research needs that flexibility and dynamism so that research process will respond to the research object needs. flexibility requires reflexivity strategies, constant critical attitude with all those methodological decisions we adopt. this is necessary in order to assure methodological rigour and congruence. this article tries to clarify flexibility and reflexivity terms in the context of a particular research.

Abstract: La metodología de la investigación cualitativa se caracteriza por ser dinámica y flexible y por estar muy orientada al proceso. La naturaleza de los fenómenos a los que nos acercamos desde la investigación cualitativa requiere de esta flexibilidad y dinamismo para que el proceso de investigación se vaya ajustando a las propias necesidades de la realidad estudiada. Esta flexibilidad, sin embargo, requiere de una reflexividad o actitud crítica constante con aquellas decisiones metodológicas que se van adoptando para salvaguardar la rigurosidad y congruencia metodológica, así como la credibilidad de los resultados. El presente artículo hace un recorrido sobre los conceptos de flexibilidad y reflexividad ilustrándose en el caso particular de un proceso de investigación. Nuevas dianas terapéuticas para el carcinoma de células renales metastásico: Resultados del tratamiento con bevacizumab en monoterapia en un único centro hospitalario

Abstract: aim: along the past decade a number of new agents have been developed showing promising results in the treatment of metastatic renal cell carcinoma. we describe our experience with the use of bevacizumab in monotherapy in the urology and medical oncology departments of our institution. material and methods: a consecutive series of patients treated with bevacizumab 10 mg/kg every 14 days between january 2006 and january 2008 has been assessed. data concerning the type of surgery, histological subtype, tumor stage, motzer?s risk group, metastatic sites number of bevacizumab courses given, time to progression and overall survival were collected. results: 25 patients (16 male, 9 female) were analyzed. median age was 59 years. nineteen of them (75%) had prior surgery: radical nephrectomy 14, cytoreductive 4 y tumorectomy 1). clear cell carcinoma was the most frequent subtype (22 patients). according to motzer?s prognostic index 14 had good prognosis, 8 intermediate and 3 poor prognosis. median time to progression was 19 weeks. overall median survival since metastatic disease was diagnosed 8,7 months. conclusions: bevacizumab offers, as single agent, modest activity in the metastatic renal cell carcinoma. the recently labeled use in combination with interferon as well as the newer targeted agents will improve the results of the treatment of this disease in the next future.
