La resistencia a múltiples fármacos: una amenaza para el control de la tuberculosis

Abstract: drug-resistant tuberculosis (tb) was reported soon after the introduction of streptomycin, although it did not receive major attention until recently. it was not considered a major issue in the industrialized world until outbreaks of multidrug-resistant tb (mdr-tb) were reported among hiv infected people. administration of standard short-course chemotherapy (sscc) with first-line drugs under directly observed therapy (dot) is the cornerstone of modern tb control. unfortunately, data available on the treatment outcome of mdr-tb cases under routine programmatic conditions suggest that patients with mdr-tb respond poorly to sscc with first-line drugs. since 1994, the world health organization and the international union against tuberculosis and lung disease (iuatld) have conducted anti-tb drug resistance surveys through a network of subregional laboratories and researchers. drug resistance was present in almost all settings surveyed, and prevalence varied widely across regions. high prevalence of mdr-tb is widespread in the russian federation and areas of the former soviet union (estonia, kazakhstan, latvia, and lithuania) as well as israel, liaoning and henan provinces in china, and ecuador. the global project has surveyed areas representing over one third of notified tb cases. however, enormous gaps still exist in the most crucial areas. the most effective strategy to prevent the emergence of drug resistance is through implementation of the directly observed treatment short (dots) strategy. effective implementation of the dots strategy saves lives through decreased tb transmission, decreased risk of emergence of drug- resistance, and decreased risk for individual tb patients of treatment failure, tb relapse, and death. the world bank recognizes the dots strategy as one of the most cost-effective health interventions, and recommends that effective tb treatment be a part of the essential clinical services package available in primary health care settings. governments a
Evaluación de los indicadores de calidad de la baciloscopía de tuberculosis en los laboratorios provinciales de diagnóstico de Cuba

Abstract: introduction: the quality control of acid fast bacilli (afb) sputum smears has a purpose to improve the efficiency and reliability of the laboratory services, assuring that the information generated by the same, it was an exact and reliable. objective: to evaluate the quality indicators afb smears in the tuberculosis (tb) laboratories of the hygiene and epidemiology provincial centres from cuba. method: it was evaluated the quality of to 2054 slides from january to december of 2006, according to the established in the procedures of the national control program of tb manual. results:, 269 (13.1%) from the evaluated slides presented high codification, 81 (3.9%) were low codification and 1704 (83%) were negative. the rate of error for all false positive was 1,1%; didn?t identify false negative results. the index of general kappa obtained was 0, 9949. the values of sensibility, specificity, predictive positive and negative value were superior to the 99%. conclusions and recommendations: these results suggest the quality of the personnel that carries out the microscopy of afb smears in the provincial laboratories and recommended the continuous training of the technicians for continue improving the quality of the baciloscopy diagnosis in cuba.

Abstract: Se describe la participación del Laboratorio Nacional de Referencia e Investigaciones en Tuberculosis y Micobacterias del Instituto de Medicina Tropical "Pedro Kourí", en la organización del recurso bacteriológico para el diagnóstico y la búsqueda de casos, fundamentalmente en la atención primaria de salud, en apoyo al Programa Nacional de Control de la Tuberculosis. Se hace referencia a las funciones y principales actividades científicas de este Laboratorio Nacional, Centro Colaborador OPS/OMS, como cabecera de los laboratorios de la red de diagnóstico de la tuberculosis en el país. La introducción y aplicación de nuevas tecnologías de avanzada permite contar con valiosas herramientas para dar respuesta a los diferentes retos que plantean la tuberculosis y las micobacteriosis en la actualidad. Con la ejecución de un proyecto del fondo mundial de lucha contra el sida, la tuberculosis y la malaria, se ha fortalecido la red de diagnóstico, lo que debe contribuir, en un futuro cercano, a la eliminación de la tuberculosis como problema de salud en Cuba. The participation of the National Laboratory of Reference and Research on Tuberculosis and Mycobacteria of "Pedro Kouri" Institute of Tropical Medicine in the organization of the bacteriological resources for the diagnosis and screening of cases in the primary health care, in support of the national program for control of tuberculosis, was described in this paper. Reference was made to the functions and main scientific activities of this laboratory, which is a PAHO/WHO collaborating center at the forefront of the tuberculosis diagnosis laboratory network in the country. The introduction and the implementation of state-of-the-art technologies have allowed valuable tools to be used to meet several challenges posed by tuberculosis and mycobacteria nowadays. Thanks to the implementation of a World Fund against AIDS, tuberculosis and malaria project, the diagnosis network has strengthened, which must contribute to the elimination of tuberculosis as a health problem in Cuba in the near future.

Abstract: Introducción: el desarrollo de nuevas vacunas antituberculosas requiere de la caracterización de la respuesta de inmunidad celular, inducida por el nuevo candidato vacunal frente a los antígenos principales de Mycobacterium tuberculosis. Objetivo: determinar el potencial inmunogénico de ′Mycobacterium habana′ TMC-5135, cuando se usa como vacuna subcutánea en ratones Balb/c. Métodos: en este estudio se inocularon subcutáneamente ratones Balb/c con la cepa viva ′Mycobacterium habana′ TMC-5135 y se determinó la producción in vitro de IFN gamma en cultivos celulares de pulmón, bazo y ganglios inguinales estimulados con antígenos solubles totales y el antígeno 85b. Como grupo control se vacunaron ratones con BCG subcepa Phipps. Resultados: particularmente en los ganglios linfáticos inguinales, ambos antígenos indujeron mayor producción de IFN gamma en los ratones vacunados con ′Mycobacterium habana′que con BCG. Conclusiones: los resultados justifican la realización de nuevas investigaciones usando ′Mycobacterium habana′ TMC-5135 como candidato vacunal para prevenir la tuberculosis. Introduction: development of new antituberculosis vaccines requires the characterization of the cell-mediated immune responses induced by mycobacterial antigens. Objective: to determine the immunogenic potential of ′Mycobacterium habana′ TMC-5135 when using subcutaneous vaccine in Balb/c mice. Methods: in this study, Balb/c mice were inoculated subcutaneously with live ′Mycobacterium habana′ TMC-5135. The production of IFN gamma in cell suspensions obtained from the lungs, the spleen and the lymph nodes after stimulation with mycobacterial antigens Ag85b or culture filtrate antigens (CFA) was recorded. Results: the production of IFN gamma after stimulation with CFA and Ag85b was higher in mice vaccinated with ′M. habana′ than in animals immunized with BCG. Conclusions: these results encourage new research on ′M. habana′ as vaccinal candidate against tuberculosis.
Identificación de cepas de Mycobacterium tuberculosis utilizando tiras de papel para la técnica de niacina

Abstract: Se identificaron cepas de Mycobacterium tuberculosis con la utilización de tiras de papel para la técnica de niacina, elaboradas en el Laboratorio Nacional de Referencia de Tuberculosis y Micobacterias del IPK. Se compararon los resultados obtenidos con la técnica de niacina convencional y se obtuvo el 100 % de coincidencia, se reportan las ventajas, sensibilidad y estabilidad de las tiras.
Evaluación de la prueba de la pirazinamidasa para la determinación de la susceptibilidad de Mycobacterium tuberculosis a la pirazinamida

Abstract: Se utilizó la prueba de la pirazinamidasa para la determinación de la susceptibilidad frente a la pirazinamida en 55 cepas de Mycobacterium tuberculosis recibidas en el Laboratorio Nacional de Referencia e Investigaciones en Tuberculosis y Micobacteria del Instituto de Medicina Tropical "Pedro Kourí" durante el primer semestre de 1997. Al comparar los resultados obtenidos con el método de las proporciones se obtuvo una correlación del 83,6 % de sensibilidad, con 16,4 % de falsos positivos. Se demostró que la prueba de la pirazinamidasa no debe sustituir el método de las proporciones. The pyrazinamidase test was used to determine susceptibility to pyrazinamide in 55 strains of Mycobacterium tuberculosis received at the National Laboratory of Reference and Research of Tuberculosis and Mycobacteria of the "Pedro Kourí" Institute of Tropical Medicine during the first semester of 1997. On comparing the results obtained with the method of proportions, it was attained a correlation of 83.6 % of sensivity with 16.4 % of false positives. It was proved that the pyrazinamidase test should not replece the method of proportions.
Evaluación de los indicadores de calidad de la baciloscopía de tuberculosis en los laboratorios provinciales de diagnóstico de Cuba

Abstract: El control de calidad de la baciloscopía (BK), es un sistema dise ado para mejorar la habilidad, eficiencia y el uso de la microscopía, como opción de diagnóstico y monitoreo, asegurando que la información generada por el mismo, sea exacta, fiable y reproducible. El objetivo del estudio fue evaluar los indicadores de calidad de la BK en los laboratorios de diagnóstico de tuberculosis de los Centros Provinciales de Higiene y Epidemiología de Cuba. Se realizó el control de calidad a 2054 láminas en el periodo enero - diciembre del 2006, según lo establecido en el Manual de Procedimientos del Programa Nacional de Control de Tuberculosis. De las láminas evaluadas, 269 (13.1%) presentaron codificaciones altas, 81 (3,9%) codificaciones bajas y 1704 (83%) fueron negativas. La tasa de error para todos falsos positivos fue de 1,1%; no se identificaron resultados falsos negativos. El índice de kappa general obtenido fue de 0,9949. La sensibilidad, especificidad, valor predictivo positivo y negativo presentaron valores superiores al 99%. Estos resultados sugieren la calidad del personal que realiza la BK de tuberculosis en los laboratorios provinciales y recomendamos no descuidar las continuas supervisiones y mantener un programa de entrenamiento constante de los técnicos para continuar mejorando la calidad del diagnóstico baciloscópico en Cuba. Abstract: several sputum and blood culture simples from a cuban hiv/aids patient were analyzed to discard the presence of alcohol acid resistant bacillus. after 2-3 weeks the culture revealed in both kinds of samples some colonies from non-pigmented mycobacterium strain with slow growth and belonging to iii runyon group. this strain was classified as mycobacterium avium by conventional methods established for mycobacterium identification. to diagnostic confirmative method was used the analysis of fraction mycolic acid by bi-dimensional thin layer chromatography. the main objective of this study was to report the first case of systemic mycobacteriosis in a cuban hiv/aids patient.
Control de calidad de la baciloscopia de esputo BAAR en laboratorios provinciales en Cuba

Abstract: introduction: the laboratory is a critical component in the diagnosis, treatment, prevention and control of tuberculosis. objective: to analyze the behavior of quality control of bacilloscopy in provincial reference laboratories of cuba, by the re-checkup of blind plates. methods: the quality control was carried out by re-checkup method of plates at 5 424 plates of baar sputum received in the national reference laboratory of tuberculosis and mycobacteria of the "pedro kourí" institute, originating in the laboratories of tuberculosis of provincial centers of hygiene, epidemiology and microbiology of cuba from 2007 to 2009. results: fifty four reading errors were identified; from them 20 false-positive, 13 false-negatives and 21 coding error. the sensitivity, specificity and concordance showed values of 84.7, 99.6 and 99.4 %, respectively. the concordance between the nrltb pki and the provincial tuberculosis laboratories assessed was of 99.4 % (kappa rate 0.8105) and the discordance was only of 0.6 %. conclusions: results suggest an appropriate staff quality of the provincial laboratories staff to carry out the quality control of baar sputum bk. it is recommended to continue with visits to network laboratories to detect the deficiencies and to implement appropriate corrective measures to go on with the diagnosis quality of bk and so to eliminate the tb as a health problem in cuba.
