Abstract: the presence of an acute appendicitis in an incarcerated inguinal hernia, termed amyand's hernia, is an uncommon and rare condition estimated to be found in approximately 0.13% of adult inguinal hernia repairs. the usual clinical presentation is as a complicated inguinal hernia; this is why the preoperative diagnosis of acute appendicitis requires a high clinical suspicion, even though the diagnosis of amyand's hernia is done mainly during surgery. computed tomography is a good diagnostic method. the treatment is surgical, and consists of an appendectomy with primary repair of the hernia. synthetic mesh should not be used in the repair of contaminated abdominal wall defects, because the prosthetic material can increase the inflammatory response and result in wound infection and a possible appendiceal stump fistula. we report a 75 years old man who presented with a complicated inguinal hernia, in whom the diagnosis of acute appendicitis was made during surgery after opening the hernia sac

In vitro Antistaphylococcal Activity of the Extracts of Several Neglected Plants in Malaysia

Abstract: The present study was carried out to evaluate the antibacterial activity of the aqueous, methanol and chloroform extracts of several plants available in Malaysia, namely Muntingia calabura (L.), Melastoma malabathricum (L.), Bauhinia purpurea (L.), Corchorus capsularis (L.) and Dicranopteris linearis (L.) using the single screening in vitro microtiter plate dilution methods. The extracts, at the dose of 5 μg μL-1, were screened against various strains of Staphylococcus aureus, namely S. aureus 29213α, S. aureus 33591, S. aureus 700699, vancomycin-intermediate S. aureus (VISA) and vancomycin-resistant S. aureus (VRSA). Results: Interestingly, only the methanol extracts of D. linearis exhibited an antibacterial activity against all strains of S. aureus whereas all extracts of M. calabura were effective only against the S. aureus 29213α, S. aureus 33591 and S. aureus 700699. The Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) for D. linearis range between 0.625-1.250 and 1.250-2.500 μg μL-1, respectively whereas for the M. calabura extracts the MIC and MBC range between 1.250-5.000 and 2.500-5.000 μg μL-1, respectively. Although the other plants gave negative results in this study, their potential antibacterial properties should not be disregarded as the present study was carried out using only one low concentration (5 μg μL-1) and that the activity was determined using crude, but not pure, extracts. The present study demonstrated the potential of chloroform extract of D. linearis, which indicate the present of non-polar bioactive compounds, as VRSA antibacterial agents and all extracts of M. calabura as a potential source of antibacterial agents for the treatment of normal S. aureus infection.

Abstract: The use of PRP involves a complex network of molecular events which accelerate tissue regenera-tion due to its richness in growth factors plus a variety of biologic mediators. Several communica-tions tell us about its usefulness on the dental implantology field, plastic surgery, orthopedics and peripheral nerve regeneration after trauma. We have presented our successful experience of its use in pneumology when treating severe hemoptysis, respiratory fistulae, spontaneous pneumothorax and one tracheal rupture. In this article we present two cases of post intubation tracheal rupture successfully treated with the local instillation of PRP on one of them and with the injection of PRP along the lips of the tracheal wound on the other. Tracheal rupture is a rare but life threatening complication of emergency intubation requiring an effective response in order to avoid the risk of patient death due to secondary mediastinitis and sepsis. Up to now there is no general consensus in the treatment of this condition and alternatives in use are not universally accepted. The use of local autologous PRP in our experience has demonstrated a favorable performance in such cases, turning it in a highly promising tool for the treatment of conditions such as this, in which a rapidly effective and minimally invasive handling is required.

Abstract: entrepreneurship and company creation have turned to be one of the principal strategies to define the existing policies focused on socio-economic development; however, one of the commonest barriers of entrepreneurial consolidation and development is based on the actual use of resources by entrepreneurs and new- born company managers to conclude their projects. in the last few years, at world level, different proposals and frameworks have been made to solve problems generated by alternatives of risk capital, which are actually in a developmental stage in colombia. this article shows some of the research project's results made by ean entrepreneurship research group and sabana university entrepreneurial culture group, with the support of colciencias science, technology and innovation administrative department, showing characteristics, needs and perceptions of entrepreneurs when dealing with the financing of entrepreneurial initiatives and projects.
Effective Plasma Frequency for Two- Dimensional Metallic Photonic Crystals

Abstract: Generalized band structure equation for photonic crystals which containing dielectric rods in metals medium was derived by using the plane wave expansion method. From the band structure, we can study band gap of photonic crystals in both E and H polarizations. Since metals are frequency-dependant materials, modification needs to be done on the plane wave expansion equation to calculate the metallic photonic crystals containing dielectric constant rods. To ease the calculation, simple Drude model for metals are used. In this model, the equation is without damping constant. We have plotted the band structure for photonic crystals in metals medium. Then, we studied the ffective plasma frequency of the structure from the band graph in E polarization mode (TM). We found that effective plasma frequency can be tailored as we want. Detailed results are presented with different sizes of radius. Comparison is made for different background materials.

Abstract: objectives: to compare the effectiveness of single (1 panel) vs. double (2 panels) phototherapy in reducing nonhemolytic hyperbilirubinemia in term newborns. methods: term newborns with hyperbilirubinemia were prospectively randomized to receive double or single phototherapy. bilirubin levels were measured at admission and at 12-hour intervals, as well as at a follow-up 48 hours after discharge. results: thirty-seven patients received single and 40 double phototherapy. the mean decrease in bilirubin level in the first 24 hours of treatment was greater in the double phototherapy group (5.1±2.2 mg/dl vs. 4.3±2.1 mg/dl), but without statistical significance (p = 0.18). readmission rates were similar and no adverse effects were found in either group. conclusions: double-surface was not more effective than single-surface phototherapy in the treatment of nonhemolytic hyperbilirubinemia in term newborns. however, our results suggest that double phototherapy may be more effective in those term newborns with higher bilirubin levels at admission.

Abstract: background: an apparent life-threatening event (alte) is an important challenge for the pediatrician, which requires guidelines for finding the etiology and making appropriate clinical decisions. objective: to describe etiologies and clinical features of alte in infants, as determined by a standarized protocol. methods: a prospective study including infants admitted to the emergency room and hospitalized due to an alte between may 2002 and may 2005. they were evaluated with an established protocol that considers the episode description, clinical history, and complementary laboratory studies. results: 109 patients with alte and a mean age of 11 weeks had the following symptoms: apnea (100%), skin color changes (97%), and muscle tone changes (83%). 96% received cpr maneuvers or strong stimulation maneuvers for ending the episode. fourteen percent of the cases were idiopathic, while other etiologies included respiratory infections (46%), gastroesophageal reflux (16%), seizures (6%), and urinary tract infections (6%). 3 patients expired. conclusions: with this protocol, based on tools available in most public tertiary hospitals in chile, we can identify the causes of alte similar to those reported in other studies, howeber with a lesser percentage of idiopathic episodes
