Brachial Plexus Blockade in Elbow, Arm or Hand Surgeries

Abstract: This study was performed to compare the transarterial (T) and paraarterial (P) approaches for brachial plexus block, in terms of success rate, onset time and duration of analgesia and complications. Hundred patients scheduled for elbow, arm or hand surgery at Tabriz Shohada hospital from October 2005 to December 2006, randomly allocated into two groups (n = 50 per group), based on the approach chosen to block the brachial plexus. For local anesthesia each patient received 22.5 mL of 2% lidocaine with 17.5 mL distilled water (in total volume of 40 mL and total dose of 450 mg) and 1/200000 epinephrine with a standard 23 gauge needle. All patients were sedated with 1 μg kg-1 of fentanyl and 0.02-0.05 mg kg-1 of midazolam. There was no statistical difference between the groups in duration of analgesia but the onset of anesthesia was significantly quicker in paraarterial technique (3.5 vs. 13.4 min, p<0.001). Success rate was 86% in group T and 98% in group P (p = 0.03). Two percent of patients in group P and 6% in group T had total failure of the block and 8% of the group T required supplementary drug. Paraarterial method for axillary block is preferable due to quicker onset of blockade and higher success rate.

Multicultural education and its politics

Abstract: This article is an account of how multicultural education is experienced within the US society and its national school curriculum. The has had an opportunity to partly witness this situation, when he was selected as one of the South African Educators who participated in the June-July 1999 South African Transformation Institute (SETI) at the Indiana University in US. Multicultural education, as a transformative concept or movement as others view it, is very vital to prepare members of the society and learners in the schools to develop intercultural skills. On the other hand, it is also not an appreciated concept in the minds of those who do not identify with transformative concepts/movements like it. In this article the , by reflecting on the US experience, attempted to communicate this message through the literature survey, and by reporting about the workshop activities that he took part in regarding the concept of multicultural education. Major P.J. Pretorius and the decimation of the Addo elephant herd in 1919-1920: important reassessments

Abstract: Between June 1919 and August 1920, the largest population of elephants in South Africa at the time was reduced from about 130 to 16 individuals by one man. Major P. J. Pretorius. Conflict between farmers and the elephants over dwindling water resources, coupled with the threat that the elephants posed to the future agricultural development of the region, precipitated the Provincial Administration's extermination order. Major Pretorius' figure of "120-odd" elephants killed during the year is reasonably accurate and the fate of the animal products is traced. Most of the skins were processed, by Pretorius himself, to make whips. A few specimens can be traced to local and overseas museums. Because records of the sex and age of animals killed by Major Pretorius have either been lost or were never detailed, reconstruction of the Addo elephant herd before the decimation, is difficult. Finally, details of the alleged public debate are discussed. It is concluded that it was probably a handful of individuals that convinced the Provincial Administration to spare 16 animals. The Rev J.R.L. Kingon as well as Major Pretorius himself are two key figures in the debate. There is little evidence to confirm the view that a public outcry, in the modem sense of the word, stopped the killing. Six photographs are included as an appendix. They show Major Pretorius at work in the Addo Bush.
Importance Of Frame Selection In Video Quality Assessment

Abstract: Video sequences contains multiple frames therefore their quality is estimated by determining individual quality metric of each frame then apply the temporal masking affect. However, the integration of each frame’s quality metric into one score is very important because each video frame has different spatial features hence have different quality metric. There are several methods available to combine the metric into one score like averaging, linear weighting, worst frames averaging etc. Taking the average of each frame’s score is not very useful as humans give more attention to the worst values (most distorted frame) while rating their values. In this paper we evaluated the performance of different integration methods and a different approach is proposed which includes the average of worst selected frames which is discussed in later sections. The work is tested on LIVE video database which consists of 40 video sequences. They have provided the mean opinion scores for each video with the database. The correlation coefficient of 88.21% is achieved when tested with the best model designed.

Abstract: Background and purpose: In Iran, wheat is one of the main cereals for human consumption. Wheat grains containing zearalenone can cause genital problems and reproductive disorders in domestic animals. In human, episodes of breast enlargement in young boys and sexual precocity have been reported. This toxin may therefore have a role in hormonal balance and mammary cancer in regions with high zearalenone ingestion. In this study the incidence of zearalenone in wheat samples from Mazandaran warehouses were investigated.Material and methods: A total of 118 wheat samples (1 kg each) were randomly collected from all 12 warehouses of Mazandaran province. Competitive microtitre plate enzyme immunoassay for the quantitative determination of zearalenone was used according the manufacturer instructions (Riedel-de Haen Sigma-Aldrich Laborchemikalien GmbH). Results: Zearalenone was found in 81% of samples and 64.4% of samples contained >200 μg/kg of this toxin. The minimum levels of zearalenone were 29 μg/kg. All of the warehouses were positive for the presence of zearalenone. 50-100% of samples in each of the warehouses had >200 μg/kg contamination of zearalenoneConclusion: Our results showed a high percentage and levels of contamination of wheat samples with zearalenone. With regard to proposed standards given by the Standard Administration of Iran, under which the content of zearalenone should not exceed 200 ppb in food products, 64.4% of samples meet these criteria.
Methodology of evaluation of value created in the productive processes

Abstract: Purpose: Of this paper was to present the methodology of analysis of the productive processes with applicationof value analysis and multi-criterion-analysis which allow to evaluate the technology and organization of theproductive processes.Design/methodology/approach: Presented in the paper methodology of evaluation of the productive processesis based on analysis of activities in the productive processes and their characteristics with reference to createdvalue in the productive chain.Findings: The paper presents elaborated by the author methodology and computer application for applicationin the chains of the productive processes.Research limitations/implications: Presented methodology allows to evaluate effectivity of the productivechains connecting aspects of the management and economics of the process.Practical implications: Presented methodology was used in analysis of the productive chains and theireffectivity, and also as a benchmarking instrument.Originality/value: The paper presents originally elaborated computer application for the value added analysisin the productive chains.

Abstract: Purpose: The purpose of the study was to present a view of problems of quality assurance/management system in university education in the field of education and research.Design/methodology/approach: In the paper there was presented an approach to quality management system in the education and associated requirements and purposes.Findings: The paper presents problems of implementation and functioning of quality assurance/management system based on legal requirements obligatory in Poland, either on standard ISO 9001:2000 and guide IWA 2:2007.Research limitations/implications: The article presents problems of implementations and functioning of quality assurance/management systems in the education.Practical implications: Presented in the paper opinion should acquaint with advantages of introduction in the universities the quality assurance/management system based on legal requirements obligatory in Poland, either standard ISO 9001:2000 and guide IWA 2:2007in the field of education and research.Originality/value: The paper is based on personal experience of the authors in introduction of the quality assurance/management system in educational units with usage of standard ISO 9001:2000 and on legal requirements obligatory in Poland.
