Cytotoxicity Tests for Evaluating Medical Devices: An Alert for the Development of Biotechnology Health Products  [PDF]

Abstract: The risks and damages related to the use of products, technologies and services of sanitary interest can be due to defects or manufacturing failures. Certain products already contain a certain degree of risk, which requires strict quality control in their production, distribution and use, as well as in the disposal of their waste in the environment. With continuous development in science and technology, medical devices must undergo intradermal irritation and testing for sensitization, cytotoxicity, and acute systemic toxicity. In health care, biotechnology aims to provide technology-based products or processes related to energy, food, and health, which are capable of stimulating new businesses, expanding exports, integrating the value chain and stimulating new demands for innovative products and processes, taking into account health policies. The present article was prepared by a bibliographical survey of the electronic databases PubMed, Lilacs, and Bireme. Cell culture testing can be successfully employed, as it is reproducible, rapid, sensitive, and financially accessible for performing in vitro toxicity testing. Thus, it has been possible to optimize the development phase of new products by decreasing animal use or even replacing them in certain tests. Some in vitro assays validated by the Organization for Economic Cooperation and Development in the area of health products have already replaced animal testing.
Azufre Elemental como Corrector del pH y la Fertilidad de Alfunos Suelos de la III y IV Región de Chile

Abstract: soils containing calcite are common in northern chile, and this condition decreases the availability of nutrients for plants. the effect of the application of elemental sulphur (s°) was evaluated on soil ph, electrical conductivity and the available micronutrients content in six soils of the iii and iv regions of chile. this was applied in doses of 500 and 1000 mg s° kg-1 and the soils were incubated for periods of 60 and 120 days at 80% of field capacity and 26°c. the experimental design was completely randomized with a factorial design and the treatments included soil, doses of elemental sulphur and times of incubation. the soil characteristics having the greatest influence on acidification by sulphur addition were the caco3, organic matter and sand contents. a highly significant reduction of soil ph occured in those soils with a smaller buffering capacity, as a consequence of lower caco3 and organic matter contents. electrical conductivity increased by the application of elemental sulphur as a result of the rise of soluble salts in the soil. when the ph diminished significantly, the levels of fe, mn and cu micronutrients increased in the soils; mn was the most strongly influenced by the acidification. this information is useful for the application of amendment programs for the calcareous soils of northern chile

Abstract: En los suelos de la zona norte de Chile existen suelos con carbonatos, situación que influye sobre la disponibilidad de nutrientes para los cultivos. En el presente experimento se evaluó en condiciones de laboratorio, el efecto de la aplicación de azufre elemental sobre el pH, conductividad eléctrica y micronutrientes en seis suelos de la III y IV Región, Chile. El azufre se aplicó en dosis de 500 y 1000 mg S° kg-1, y el suelo se incubó por períodos de 60 y 120 días a 80% de su capacidad de campo a 25°C. El experimento se estableció con un dise o de tratamientos completamente al azar con arreglo factorial, donde los factores fueron: suelo, dosis de azufre elemental y tiempo de incubación. Los recipientes en incubación se distribuyeron de acuerdo a un dise o completamente al azar. Las características de los suelos que más influyeron sobre la magnitud del efecto acidificante del azufre elemental fueron los contenidos de CaCO3, materia orgánica y arena. Las reducciones de pH significativas se presentaron en los suelos con una menor capacidad tampón, como consecuencia del menor contenido de CaCO3 y materia orgánica. La conductividad eléctrica se incrementó por la aplicación del azufre elemental, debido al aumento de sales solubles en el suelo. Los niveles de los micronutrientes Fe, Mn y Cu se incrementaron en los suelos cuyos pH disminuyeron significativamente, siendo el Mn el más influenciado por la acidificación. Capsaicina tópica en el tratamiento del dolor neuropático Topical capsaicin for the management of neuropathic pain

Abstract: El dolor neuropático es uno de los síndromes dolorosos más complejos y de más difícil tratamiento. Se denomina así al síntoma resultante del da o neurológico (periférico, central o ambos) en una parte del sistema de transmisión nerviosa del dolor. Se han utilizado diversos fármacos, pero ninguno ha resultado suficientemente efectivo y los efectos secundarios a menudo impiden su continuación. Puede darse en distintas situaciones, todas ellas más frecuentes en pacientes a osos. Se busca un analgésico potente con el menor número de efectos secundarios posibles. Este artículo es una revisión bibliográfica, mediante la base de datos del Medline, del tratamiento con capsaicina tópica en distintos casos de dolor neuropático: neuralgia postherpética, neuropatía diabética, dolor neuropático asociado al sida, neuralgia del trigémino, síndrome doloroso postmastectomía y dolor complejo regional. La aplicación tópica de capsaicina al 0,075%, ha demostrado ser eficaz en el tratamiento del dolor disestésico, por lo que constituye una alternativa terapéutica en este tipo de dolor. Su mecanismo de acción parece que se basa en la estimulación selectiva de las neuronas de las fibras amielínicas C, provocando la liberación de sustancia P y posiblemente otros neurotransmisores; y finalmente una depleción de sustancia P, con lo que se produciría una alteración de la transmisión del dolor a los sistemas centrales produciéndose un fenómeno de desensibilización. A pesar de que algunos estudios demuestran una mayor efectividad de la capsaicina frente a placebo, son pocos los pacientes que refieren una respuesta importante cuando se utiliza como única terapia del dolor neuropático. En ocasiones es imposible realizar estudios de doble ciego debido a la sensación quemante de la que hemos hablado. Carece de efectos secundarios sistémicos importantes, no obstante en la primera semana de tratamiento puede aparecer prurito o sensación de quemazón en la zona de aplicación, que tiende a desaparecer después de la primera semana. Esta sensación de quemazón constituye, junto con la limitada efectividad clínica, la principal limitación de la aplicación tópica de capsaicina. La principal indicación del uso de la capsaicina tópica es la de coadyuvante de los antidepresivos y anticonvulsivantes en el tratamiento de los diversos cuadros de dolor neuropático, ya que como terapia única parece ser insuficiente. Es una opción a tener en cuenta en los pacientes a osos afectos de dolor neuropático, con lo que disminuiremos la incidencia de efectos secundarios sistémicos y de interacciones medicamentosa

Abstract: differential morphological plant characteristics may influence competition relations between crop and weeds. the aim of this study was to evaluate the influence of plants of four soybean cultivars and two other soybean cultivars, mimicking dicotyledonous weed species, on their relationships during the vegetative growth period. thus, two experiments were carried out: one in pots, to provide data for initial soybean plant growth; and the other in the field, to monitor plant growth characteristics. the following factors and treatments were tested in the field: three competition conditions (absence of competing plants or presence of the soybean cultivars brs 205 or cobb), and four soybean cultivars reagent to competition (ias 5, br-16, cd 205, and fepagro rs-10). soybean cultivars showed varied emergence speed and initial plant growth. the presence of the concurrent plants, regardless of their plant characteristics, affected soybean branching. fepagro rs-10 and br-16 cultivars had a fast growth during the initial development period; cd 205, on the contrary, showed slow growth during this phase. early maturity soybean cultivars cover the soil more rapidly than late maturity cultivars, during the first 45 days of growth.
Evolution of spatio-temporal drought characteristics: validation, projections and effect of adaptation scenarios

Abstract: Drought events develop in both space and time and they are therefore best described through summary joint spatio-temporal characteristics, such as mean duration, mean affected area and total magnitude. This paper addresses the issue of future projections of such characteristics of drought events over France through three main research questions: (1) Are downscaled climate projections able to simulate spatio-temporal characteristics of meteorological and agricultural droughts in France over a present-day period? (2) How such characteristics will evolve over the 21st century? (3) How to use standardized drought indices to represent theoretical adaptation scenarios? These questions are addressed using the Isba land surface model, downscaled climate projections from the ARPEGE General Circulation Model under three emissions scenarios, as well as results from a previously performed 50-yr multilevel and multiscale drought reanalysis over France. Spatio-temporal characteristics of meteorological and agricultural drought events are computed using the Standardized Precipitation Index and the Standardized Soil Wetness Index, respectively, and for time scales of 3 and 12 months. Results first show that the distributions of joint spatio-temporal characteristics of observed events are well simulated by the downscaled hydroclimate projections over a present-day period. All spatio-temporal characteristics of drought events are then found to dramatically increase over the 21st century, with stronger changes for agricultural droughts. Two theoretical adaptation scenarios are eventually built based on hypotheses of adaptation to evolving climate and hydrological normals, either retrospective or prospective. The perceived spatio-temporal characteristics of drought events derived from these theoretical adaptation scenarios show much reduced changes, but they call for more realistic scenarios at both the catchment and national scale in order to accurately assess the combined effect of local-scale adaptation and global-scale mitigation.
Nefrectomía simple laparoscópica NOTES y LESS con el uso de instrumentos laparoscópicos estándar: Experiencia inicial en 3 casos NOTES and LESS laparoscopic simple nephrectomy with the use of standard laparoscopic instruments: Initial experience in 3 cases

Abstract: Introducción: La cirugía endoscópica transluminal a través de orificios naturales (NOTES) y la cirugía laparoendoscópica a través de sitio único (LESS) son tecnologías emergentes, que permiten realizar procedimientos quirúrgicos minimizando el uso de incisiones abdominales. Sin embargo, existen limitaciones respecto al equipamiento disponible para simular la cirugía tradicional. Nuestro objetivo es presentar nuestra experiencia con la técnica de nefrectomía laparoscópica NOTES y LESS con el uso de instrumentos laparoscópicos estándar. Materiales y Métodos: Dos pacientes de sexo femenino de 23 y 26 a os, ambas con diagnóstico de infecciones urinarias recurrentes y atrofia renal secundaria. Se les realizó una nefrectomía simple con asistencia de NOTES, utilizando un puerto de trabajo transvaginal para la cámara y dos puertos adicionales de 10 (umbilical) y 3 mm en el abdomen. Un tercer paciente de sexo masculino de 15 a os de edad, fue operado a través de la técnica LESS por vía transumbilical con el uso de cistoscopio fl exible e instrumentos laparoscópicos estándar. Resultados: La media del tiempo operatorio fue de 110 min (40-200), la pérdida sanguínea promedio fue de 133 cc (0-200). No se reportan complicaciones, siendo todos los pacientes dados de alta antes de 36 horas. Conclusión: La nefrectomía laparoscópica con asistencia de NOTES y LESS es técnicamente posible con el uso de instrumentos laparoscópicos estándar. El desarrollo y acceso a trócares y pinzas especiales, permitirá realizar una técnica pura sin el uso de incisiones abdominales. Introduction: Natural orifice transluminal endoscopic surgery (NOTES) and Laparoendoscopic Single Site Surgery (LESS) are emerging technologies, which allows to performed surgical procedures avoiding any surgical scars. However there are some problems due to the lack of equipment available for these procedures. The aim of these study is to present our initial experience with the transvaginal nephrectomy NOTES and LESS using standard laparoscopic instruments. Material and Methods: Two female patients (23 and 26 years old) with diagnosis of recurrent urinary tract infection and renal atrophy. A laparoscopic simple nephrectomy with transvaginal NOTES assistance was performed, using one access port for the camera and two abdominal work ports of 10 and 3mm. In a third patient (15 years old) a transumbilical LESS nephrectomy was preformed with the use of standard laparoscopic instruments. Abstract: neuropathic pain is one of the most complex painful syndromes and one of the most difficult to treat. it is a symptom resulting from neurological damage (peripheral, central or both) affecting the pain nervous transmission system. several drugs have been used, but none of them has resulted effective enough and their side effects frequently deter the continuation of the treatment. it can show up in different situations, all of which are more frequent in aged patients. a powerful analgesic with as little number ofside effects as possible is needed. this paper is a bibliographic review performed through the medline data base of the treatment with topical capsaicin in different cases of neuropathic pain: postherpetic neuralgia, diabetic neuropathy, neuropathic pain associated to aids, trigeminal neuralgia, post-mastectomy painful syndromes and regional complex pain. the topical administration of capsaicin 0.075%, has shown to be effective for the management of dysesthesic pain and hence it is a therapeutic alternative for this type of pain. its mechanism of action appears to be based on the selective stimulation of the neurons of amyelinic fibers c, causing the release of substance p and perhaps of other neurotransmitters; and finally, a depletion of substance p, which would disturb pain transmission to central systems, causing a desensitization phenomenon. while some studies have shown a greater effectiveness of capsaicin versus placebo, few patients have reported a significant response when it is used as monotherapy for neuropathic pain. sometimes double-blind studies cannot be conducted due to the burning sensation referred to above. its lacks significant systemic side effects, but during the first week of therapy an itching or burning sensation can appear in the application area that tends to disappear after the first week. this burning sensation is, together with the limited clinical effectiveness, the major limitation of the topical administration of capsaicin. th

Abstract: we expose our experience on the development and management of and acute pain service (aps), on a regional hospital, that provides an integral care of acute pain, with a low cost model, sustainable by the public health care system. though, there is a common tendency to identify acute pain with postoperative pain, it's essential to remark that this supposition contains a big mistake: to reduce the scope of the acute pain unit, to the treatment of the pain consecutive to a surgical procedure. at hospital, coexists other acute pain models, with the same or more significance, in qualitative and quantitative terms, than postoperative pain, and so, must receive the specialized care of an acute pain service; good examples are: obstetric pain, vascular ischemic pain, pain at intensive care units (adult and paediatric), emergency room's pain and any other acute pain independent of it's origin. our aps, integrated on the department of anaesthesia from "puerta del mar" university hospital, on cádiz, has been designed with the purpose to offer guidelines and analgesic protocols, in order to achieve a whole coverage for any type of acute pain. this huge medical project, exploits the special privileged characteristics of a reference hospital for more than 1.250.000 persons, but nevertheless, still includes on a single functional unit, all surgical services and has not suffered the segregation of the maternal, paediatric and orthopaedic units, as any other great hospital on our country.
