Abstract: O cancer gástrico é uma neoplasia frequente e de tratamento cirúrgico. Gastrectomia com linfadenectomia D2 é o procedimento padr o com intuito curativo. Os autores relatam o uso do hook laparot mico como alternativa técnica para linfadenectomia. Gastric cancer is a common malignancy of surgical treatment. D2 lymphadenectomy is the standard procedure with curative intent. The authors report the use of laparotomic hook as an alternative technique for lymphadenectomy.
Interposi??o jejunal após gastrectomia total radical por adenocarcinoma gástrico

Abstract: we performed a retrospective study of the surgical treatment of the gastric adenocarcinoma by a radical total gastrectomy, followed by the reconstruction of the esophagoduodenal path with the interposition of a jejunal pouch. revision of national and foreign papers related to the treatment of the gastric adenocarcinoma by the radical total gastrectomy was done. according to the operability related to the patient and ressecability related to the primary lesion and its evolution. 126 patients were submitted to an interposition of a jejunal loop following radical total gastrectomy. total oncological resection of the stomach and systematic technical fqllow-up of the new gastric reservoir and the esophagoduodenal path. our cases evolued in a satisfactory way, similar to those ones seen in the literature. special emphasis was taken on the technical procedure. more anatomical and much more functional, giving back to the patient a new stomach and gastrointestinal path through the duodenum. the interposition of a jejunal pouch between the terminal esophagus and the second portion of the duodenum acts like a new gastric reservoir. it avoids the esophageal reflux and transports food and fluids to the small intestine through the duodenum. an anatomic and functional path which may give to the patient submitted to a total gastrectomy a better quality of life.

Abstract: Leitura da 1a quest o do capítulo 1 do Comentário ao ‘De Coelo’ dos Jesuítas de Coimbra (1593), “Se o Universo é ou n o perfeito”. Pleiteiase por um interpreta o que deixe falar o texto nos nossos dias, a saber, enquanto interroga o sobre o lugar que o cosmo ocupa no cora o do Homem. N o se podendo pensar o Homem independentemente do “magnus mundus”, o cosmo tem no Homem a condi o da sua legalidade. The article deals with question 1st (chapter 1) of the Coimbra Jesuit Commentary on the Aristotelian ‘De Coelo’ (1593), “Whether the Universe is perfect”. The Author aims at reading anew the Portuguese question, pointing to the place the Universe has in man’s heart by underlining that the cosmos must have in man (“parvus mundus”) its conditions of legality.

Abstract: if an integer solution to the one-dimensional cutting stock problem is required, after solving the linear programming relaxation, one frequently resorts to heuristics based on rounding up and down the continuos solution, or other heuristics similar type. the difficulties arise from the fact that it may not be practically possible to enumerate all the structural variables of the problem, whose number may be in the order of millions, even for instances of moderate size. in this article we present a formulation with a number of variables and constraints that is polinomial with respect to the width of the stock and the number of orders. for some classes of instances, it is possible to enumerate completely all the variables and to obtain an integer optional solution using a branch-and-bound method. for larger instances, we present a procedure that combines column generation and branch-and-bound. we define the subproblem, and the way it is modified during the branch-and-bound phase. computational results are presented for several test problems.

Abstract: the flavone-6-sulfonic acid, 4'-metil-5,7-dihydroxy-flavone-6-sulfonic acid, known as niruriflavone, and the saponin 3-o-[6'-o-4-hydroxybenzoyl]-2-d-galactopyranosyl-ursa-12-en-28-oic acid were isolated, respectively, from wood and leaves of licania arianeae. the structures were established from the nmr and mass spectra data analysis including two-dimensional nmr experiments.
New triterpene isolated from Eschweilera longipes (Lecythidaceae)

Abstract: The phytochemical studies of Eschweilera longipes Miers (Lecythidaceae) have led to the identification of a new triterpene 3beta, 24-dihydroxyfriedelane, the known 1beta, 2beta, 3beta, 19beta-tetrahydroxyurs-12-en-28-oic acid (1beta-hydroxyeucaphic acid) besides the saponin sitosterol 3betaO-betaD-glucopyranoside. The structures were established from the IR, NMR and mass spectra data including 2D NMR experiments of natural substances and of the acetyl derivative of the new triterpene.

Abstract: despite the excellent prognosis, differentiated thyroid carcinoma (dtc) may recur in 20-40%, and prognosis is particularly related to early detection of recurrent disease. therefore, long-term follow-up with sensitive tests is need. serum thyroglobulin (tg) has an established role as a tumor marker of relapse. however, there are technical limitations of tg immunoassays, in special, the interference of anti-tg antibodies and the method sensitivity is dependent on tsh stimulation. detection of circulating malignant cells by amplification of tumor-specific mrna showed initial promising results. however, almost one decade of studies of tg mrna detection in peripheral blood, its real contribution for dtc follow-up had not yet been established. after a critical analysis of published data, it is clear that there are many protocol differences and conflicting results. therefore, it seems that amplification of thyroid-specific mrnas is not superior to sensitive tg assays and illegitimate transcription and alternative splicing of tg are factors that may influence mrna test specificity.
New perspectives on the treatment of differentiated thyroid cancer

Abstract: even though differentiated thyroid carcinoma is a slow growing and usually curable disease, recurrence occurs in 20-40% and cellular dedifferentiation in up to 5% of cases. conventional chemotherapy and radiotherapy have just a modest effect on advanced thyroid cancer. therefore, dedifferentiated thyroid cancer represents a therapeutic dilemma and a critical area of research. targeted therapy, a new generation of anticancer treatment, is planned to interfere with a specific molecular target, typically a protein that is believed to have a critical role in tumor growth or progression. since many of the tumor-initiation events have already been identified in thyroid carcinogenesis, targeted therapy is a promising therapeutic tool for advanced thyroid cancer. several new drugs are currently being tested in in vitro and in vivo studies and some of them are already being used in clinical trials, like small molecule tyrosine kinase inhibitors. in this review, we discuss the bases of targeted therapies, the principal drugs already tested and also options of redifferentiation therapy for thyroid carcinoma.
