Closed Repair of Stenotic Mitral Valve Comissurotomy (CMVC) or Percutaneous Transvalvular Mitral Valve Comissurotomy (PTMC)

Abstract: The History of closed mitral valve comissurotomy (CMVC) and its indication is discussed in this article. However many of the patients undergo (PTMC) these days as a new procedure and of course more effective and easier way whenever is indicated, yet, (CMCV) should be considered as a simple, and effective surgery when indicated, especialy in third world countries in which economical, social and personal point of views are important."nMy main point in this article is to encourage younger surgeons and colleagues to pay more attention to this kind of cardiac surgery, which at the first glimpse seems difficult and perhaps dangerous to untrained and unskillful hands!
HYDATID CYST OF THE HEART A REPORT OF 5 CASES

Abstract: 5 Cases of hydatid cysts of the heart underwent successful enucleation or removal of the cysts with the aid of cardiopulmonary bypass are being reported. Their location, signs, symptoms, diagnostic procedures and consequences are being discussed. Clinical, radiologic, electrocardiographic and coronary angiographic findings all confirmed the diagnosis. AH the patients had uneventful postoperative course and had been foliowed-up regularly. Cardiac hydatid cyst is seen infrequently, even in the endemic regions. The incidence rate is 0.3 to 2 percent of all hydatid cyst patients, thus it is not a matter of routine cardiac surgery practice.

Abstract: Pulmonary arteriovenous malformations are rare clinical entities thai are associated with right to left shunts, and are often clinically presented by the triad of dyspnea, cyanosis, and digital clubbing. Currently, transcatheter embolization of fistula is gaining increasing popularity as the superior therapeutic option. In certain situations, however, surgery is preferred. Two cases of direct pulmonary artery ■ left atrial fistulas are described who, were treated simply by surgery with no recurrence.
Suprarenal Abdominal Aortic Coarctation Diagnosed During Pregnancy

Abstract: Coarctation of the abdominal aorta is an extremely rare vascular defect inwhich congenital or acquired etiologies have been described. This case concernsa 30-year-old pregnant woman with 15-years history of uncontrolled hypertensionand lower limb claudication presented with worsened hypertension during herfirst pregnancy. Magnetic resonance angiography study of aorta revealed astenosis in abdominal aorta about 12mm from the origin of celiac axisaccompanied by left sided aortic arch and right aberrant subclavian artery. Thiscase highlights the importance of a throughout physical examination in patientspresented with hypertension and it emphasizes considering the coarctation of theabdominal aorta during the diagnostic workup of hypertension, especially inyoung patients. In such cases magnetic resonance angiography of the aorta is auseful tool to reach a definitive diagnosis especially in pregnant women.Also to our knowledge, this patient is the first one found to have aortic archmalformation combined with an abdominal coarctation.
The role of family configuration in early childhood intellectual development in the context of an extended family system in Pakistan

Abstract: Context: The confluence theory of intelligence by Zajonc and Markus emphasizes that individual intellectual difference of children manifests itself in the context of family configuration. Instead of assuming its generalizability, careful scientific work is required before applying the model to South Asian cultures where, predominantly, an extended family type exists. Aims: To assess the role of extended family configuration on the child′s intellectual development in a South Asian setting. Settings and Design: A cross-sectional study was conducted on 4-5-year-old preschool children residing in Karachi, Pakistan. Materials and Methods: Three hundred and forty-two child and mother dyads were assessed through a validated cognitive psychometric tool and through a structured questionnaire. Children who were registered at the main Mother and Child Health Centres (MCH) of the Aga Khan Health Services, Pakistan (AKHSP) Karachi and who were born between July 1st 1993-June 30th 1994 with traceable birth records at the maternity homes, were considered for this study. Statistical Analysis: Multivariate linear regression models were used to identify the individual effect of family configuration on the intellectual scores. Results: Family configuration variables such as number of co-residents ( P < 0.05) and the number of siblings ( P < 0.01) in the house were significantly correlated with the psychometric score. Even after controlling for gender, socio-economic status, birth order and birth intervals, significant differentials were observed in favor of an extended family system on a child′s intellectual development. Conclusion: The findings suggest the positive role of co-residents of an extended family environment on the intellectual development in early childhood.
Congenital Dislocation of Knee: Results of the Treatment in 29 Cases Between 1970 and 2010 Years at Shafa Yahyaian Hospital in Tehran

Abstract: Background: There are not many reports regarding the treatment approaches of congenital dislocation of the knee in the literature. Therefore, the preferred method of treatment of this rare congenital disease is still controversial. Hereby, we report the treatment outcome of 29 patients admitted in Shafa Yahyaian Hospital during 40 years. Methods: In this retrospective study done in Shafa Yahyaian Hospital in Tehran, Iran during 2010, we retrospectively reviewed the medical records of 29 patients treated conservatively or surgically for congenital dislocation of the knee. Patients had been evaluated for an average follow-up of 8.5 years. Results: Overall, 54 knee dislocations had been treated surgically (58%) or conservatively (42%). Range of knee motion, persistent recurvatum, instability, valgus deformity after treatment and limping were the more important factors reported in the two surgically or conservatively treated groups. Knee function was satisfactory in patients with conservative treatment. Despite 80% of instability in the operated knees, patients could ambulate with or without braces and had a range of motion equal to 80 degrees. Knee function was also good after quadricepsplasty with transarticular pins in selected cases. Conclusion: We recommend a conservative approach to the disease, regardless of the patients age at the time of treatment and subsequent surgery in patients with dissatisfactory recovery. Quadricepsplasty with transarticular fixation is recommended as a good option in treating these patients. Posterior capsulorrhaphy for patients with CDK and ligamentous laxity is also recommended.
The Prevalence Of Nosocomial Infections Caused By Enterobacter Cloacae And Antibiotic Resistant Patterns In Samples Isolated From Patients In Two Hospitals In Tehran

Abstract: Background: The increasing use of β-lactam antibiotics in clinics for the treatment of different bacterial infections since early 1980s has led to increased rates of resistant bacteria isolated from patients. One of the problems in the treatment of nosocomial infections is related to resistant bacteria such as Enterobacter cloacae due to cross resistance through extended-spectrum beta-lactamase production. The aim of this study was to determine the prevalence of extended-spectrum beta-lactamase producing E. cloacae from different clinical specimens collected from hospitalized patients. Methods: In the present study, 101 E. cloacae confirmed by standard specific microbiologic tests were collected from different specimens in Milad and Motahri hospitals in Tehran, Iran during February 2010 and September 2011. Antibiotic susceptibility tests were conducted according to the process recommended by the Clinical and Laboratory Standards Institute for 13 antibiotics of choice. Extended-spectrum beta-lactamase producing strains were screened for by combined disk method as a phenotypic diagnostic test. Results: From a total of 101 E. cloacae, 33 (33%) were shown to produce extended-spectrum beta-lactamase by phenotypic tests; 5% of the bacteria were resistant to imipenem too. Conclusion: This study clearly showed the high prevalence of resistance to broad-spectrum beta-lactam antibiotics in the isolated E. cloacae among which 5% were multi drug resistant. All the isolated E. cloacae were susceptible to Colistin. These results can be alarming for physicians treating resistant E. cloacae infections, especially extended-spectrum beta-lactamase producing species.
Retinoic Acid Stimulate Differentiation of Hippocampal Stem Cells into Opsin Expressing Cells in vitro

Abstract: The results of several studies have demonstrated that cell differentiation influenced by derivatives of retinoic acid. To determine whether retinoic acid mediate the differentiation of neural stem cells we treated dissociated hippocampal stem cells with different concentrations of all trans or 9-cis retinoic acid and analyzed the effects on cell fate by specific monoclonal antibody for photoreceptors. Addition of exogenous retinoic acid caused a dose dependent specific in the elevation of the cell number that developed as photoreceptors in culture. Also results of immunohistochemical studies using monoclonal antibody demonstrated that the primary effect of retinoic acid was to influence progenitor cells the developed as mature and immature photoreceptors. These results suggest that retinoic acid may play an important effect in the normal development of photoreceptor cells in vitro.
Isolation of rapid growing mycobacteria from soil and water in Iran

Abstract: A total of 350 soil samples were collected from different part of Uremia city and suburbs. We used 3% sodium lauryl sulfate and 1% NaOH for decontamination of soil samples. Of 350 samples, mycobacteria were isolated from 65 (18.3%) specimens. Mycobacterium fortuitum with 18(5.14) strains yielded the highest frequency of isolation. The other isolates were: Mycobacterium peregrinum 11(3.14%), Mycobacterium flvescens 10 (2.85%), Mycobacterium chelonae 6 (1.71%), Mycobacterium mucogenicum 6(1.71%), Mycobacterium thermoresistable 4(1.14%), Mycobacterium abscessus 3 (0.85%), Mycobacterium neoaurum 2(0.57%), Mycobacterium smegmatis 2 (0.57%) and M. fortuitum third biovalant complex 3 (0.85%). The mean pH of soil was 7.89 ± 0.379 (max 8.5, min 7.5). Our data showed an abundant occurrence of mycobacteria in low pH (P value = 0001). We also collected 120 water samples from rivers, brooks and drinking water. Water samples decontaminated were by adding cetyl pyridinium chloride (CPC) to give final concentration of 0.05%. Mycobacteria isolated from 12 water samples. The predominant isolated species were M. fortuitum and Mycobacterium cheloni. The majority isolates were from brooks and surface waters.
Genital Tract Infection in Asymptomatic Infertile Men and Its Effect on Semen Quality

Abstract: Male urogenital tract infection plays an important role in men infertility. Asymptomatic bacteriospermia has been paid attention as a major cause of male infertility. The aim of this study was to microbiological investigation of semen sample of infertile men attending to infertility clinic and evaluation of the effects of bacteriospermia on semen quality. Eighty eight infertile men were evaluated by standard bacterial culture method. Standard semen analysis was performed according to WHO guidelines. Among total cases, 35.22% (31 cases) showed at least one pathogen: 10.22% E.coli, 9.09% Coagulase Negative Staphylococci (Saprophyticcus), 6.81% Group B Streptococci, 5.88% Entrococci, 5.68% Candida sp., 2.27% Gonococci, 2.27% Staphylococcus aureus, 1.13% Klebsiella sp. and 1.13% Providencia sp. There was a significant relation between the bacteriospermia and the rate of no motile and morphologically abnormal sperms (P0.05). It seems that leukocytospermia is a poor marker to predict bacteriospermia.
