Diagnostic accuracy of touch imprint and frozen section analysis in mediastinal lymph node involvement: lung cancer patients

Abstract: Background: It is well known that, at the time of the initial presentation of patients with non-small cell lung cancer, N-status is very important in accurate staging and avoiding inappropriate surgical procedures. Also, if mediastinal lymph node metastasis (N2) of non-small cell lung cancer (stage IIIA) is present, neoadjuvant (preoperative) chemora-diotherapy is indicated; therefore a quick and accurate N-status determination is critical before thoracotomy. We compared the usefulness and accuracy of touch imprint and frozen section analysis for this purpose.Methods: During 2006, fifty eight mediastinal lymph node specimens from 27 patients with lung cancer were excised. After touch imprint preparation, half of each lymph node was sent for frozen section and finally permanent histology.Results: Frozen section and touch imprint had only one and two false negative results, respectively, but neither had false positive reports. The sensitivity, specificity and accuracy of the frozen section evaluation was 95.5%, 100% and 98.2%, respectively, and those of touch imprint were, 90.9%, 100% and 96.4%. The differences in the sensitivity, specificity and accuracy between the two methods were not significant (p>0.05).Conclusion: Frozen section and touch imprint have similarly high accuracies for detecting mediastinal lymph node metastasis in lung cancer. However, since the touch imprint method is more rapid and simple, it maybe method of choice.
Double-lumen endotracheal tube positioning: Bag compliance Vs. fiberoptic bronchoscopy

Abstract: Background: Lung separation is the basis of thoracic anesthesia, which is performed by different instruments. Checking probable malpositioning of tracheal tube needs fiberoptic bronchoscopy. The aim of this study was to compare respirator suggested compliance with fiberoptic findings in detecting major tracheal tube malpositioning.Methods: A total of 256 patients undergoing thoracic surgery with double-lumen tracheal tube insertion in Imam Khomeini Hospital, Tehran, Iran, during 2010-11 were divided into three groups (n=86). We used left-sided double-lumen tube (DLT) for left or right-sided surgeries (groups 1 and 2), and right-sided DLT for left-sided surgeries (group 3). The position of the tubes was evaluated and compared using bag compliance versus fiberoptic bronchoscopy.Results: The mean age of the study population was 44.7±13.4 (16-73) years, while 155 (59.9%) were male. The sensitivity, specificity, positive and negative predictive values, and the accuracy of bag compliance test for left-sided DLT in supine position were 40% (95% CI: 20-60%), 99% (95% CI: 96-99%), 84% (95% CI: 54-94%) 92% (95% CI: 88-95%) and 92% (95% CI: 87-95%), respectively. The above-mentioned variables for lateral decubitus position respectively were 27%, 98%, 76%, 89%, and 88%. Malpositioning was more prevalent in right-sided DLTs (P=0.02).Conclusion: Based on the results of this study, and the high specificity, positive predictive value, and accuracy of bag compliance test, its use is encouraged as an alternative to fiberoptic bronchoscopy for checking DLT position, specially, in emergent surgeries or when fiberoptic bronchoscopy is unreachable due to lack of expertise or personnel.

"n"n Methods: Two groups of 16 patients in a randomized clinical trial compared. In one group bronchial stump closed with stapler and the other stump closed with hand sewn method. Bronchial closing time, Operative time, time of airleak, time of chest tube, time of discharge and complication recorded and compared. "n"n Results: In the stapler group time of bronchial closing was significantly shorter. Operative time was not different. time of air leak was not statistically different. No patients with long airleak, and the number of patients without airleak was greater. In the stapler group, time of having chest tube was statistically shorter and time of discharge in stapler group was shorter than hand sewn group. "n"n Conclusions: Bronchial closing with stapler in pulmonary resection is a safe method and in addition to rapidity, time of chest tube and time of discharge was shorter. Although Time of air leak was not statistically different but patients in stapler groups had less Days with air leak and long air leak was zero in this group.
Coincidental pulmonary leiomyoma and arterio- venous malformation: a case report

Abstract: Background: Lung leiomyoma and pulmonary arterio-venous malformation (PAVM) are both rare diseases. Occurrence of them in a patient is rare too. Because of their limited signs and symptoms and so serious complications, accurate diagnosis of both diseases is important. Case report: We present a 73 -year old female with PAVM combine with a coincidental solid mass in left upper lobe. Because of fistula size and combination with a solid mass, the lesion is treated by left upper lobectomy. Results: As most of lung diseases will be present without any signs and symptoms, the presence of asymptomatic findings in the lung such as nodule on chest x- ray should be assessed carefully
The Investigation of Antibacterial Effects of Salvia Sahendica Extracts on Some Pathogenic Bacteria

Abstract: Background: The salvia plant is one of the Lamiacea family members, that is being cultiurated all over the world. In Iran almost all spices of this plant could found, that are used for medical, food industry and parfumers production. Salvia sahendica genus is the member of this family, that its antibacterial effects on Klebsiella pneumonia, Staphylococcus aureus and Pseudomonas aeruginosa have been investigated. Methods: In this study are used standard strains of Pseudomonas aeruginosa (ATCC 27852) Staphylococcus aureus, (ATCC 25923) and, Klebsiella pneumonia (ATCC 3583). Salvia was provided from Tabriz mall and was powdered. Then this powder was suspended with ratio 1:10 with Metanol, Aceton, Chloroform, Hexan, Ethyl acetate and water. After 24h extractions isolated and concentrated with distillated in vacuum system. Which of bacteria were cultured to over night in Mohler Hinton agar medium, then compared with 0.5 macfarland. In continue extraction were tested by used of well assay method and then the diameter of zone measured. Experiments repeated tree times and the average of data obtained. In each of tests we had a control from special solvent. Results: Regarding to the results, all of salvia sahendica extractions showed antibacterial activity on Staphylococcus aureus, Pseudomonas aeruginosa and. That regarding Klebsiella pneumonia, Ethanol, Chloroform, Ethyl acetate, Hexan, and regarding Staphylococcus aureus Metanol, Aceton, Chloroform, Hexan, Ethyl acetate and Pseudomonas aeruginosa only Chloroform this plant has antibacterial effects. Chloroform extraction of salvia shows the most antibacterial activity (with the biggest diameter of antibacterial zone). Conclusion: Regarding the carried out study it could be noted that for inhibition and destroying Staphylococcus aureus and Klebsiella pneumonia, plants effects extraction could be used.
The Trend of Changes in Surface Wind in the Indian Ocean, in the Period from 1981 to 2015, Using Reanalysis Data, NCEP/NCAR  [PDF]

Abstract: Knowing the pattern of surface winds on the seas and oceans and how it changes over time is of great importance. In this research, the monthly surface wind fields on the Indian Ocean have been studied and analyzed for a 35-year period (1981-2015), using NCEP/NCAR data reanalysis. The results show that transition from cold to warm pattern happens in May and that the summer monsoon pattern begins in June and continues until August. The wind speed pattern tends to the winter monsoon from November on. The maximum average wind speed in June is 13 m/s and its minimum is 2 m/s in October. Direction of prevailing winds is the southwest in the summer. The highest wind speed happens in the latitude of 10 - 15 degrees. Analysis of the wind distribution shows that the wind speed of 2 - 5 m/s happens in about 60% of the cases. There is probability of blowing 0.5 - 4 m/s wind for all months; but this probability is higher in the autumn (October and November) than that in the summer (July and August). Probability of the monthly over 5 m/s winds shows a definitely opposite distribution; that is, wind speed in July and August is higher than that in October. A long-term survey on the speed of surface water wind and sea surface temperature shows an opposite changing trend in wind speed and sea surface temperature during a 55-year statistical period. Wind speed reduced, while the sea surface temperature was increasing. The wind speed gradient in the upper levels of atmosphere graph has been increasing; this phenomenon confirms the effects of global warming and ocean warming on the monsoon system patterns in the Indian Ocean. The Role of Tracheal Stenting As an Alternative Treatment in Cicatricial and Non-Operable Tracheal Stenosis

Abstract: Introduction: Tracheal stenosis is normally caused by trauma, infection, benign and malignant tumors, prolonged intubation or tracheostomy. The best treatment for tracheal stenosis is resection and anastomosis of trachea. Yet the major surgical complication of tracheal surgery is postoperative stenosis. The goal of this paper is to study the result of tracheal stenting as a replacement therapy for patients suffering from tracheal stenosis who are not good candidates for surgery. Materials and Methods: This study presents the results of stenting in patients with: Inoperable tumoral stenosis,Non-tumoral stenosis being complicated due to prior surgeries,Inability to undergo a major surgery.The study was performed between September 2002 and July 2011 and poly flex stents were used by means of rigid bronchoscopy. Results: A total of 25 patients received stents during this study. Among them 15 patients suffered from benign and 10 suffered from malignant tracheal stenosis. The patients were followed up for at most 12 months after the stenting operation. The mean age of the patients was 35 years. The most common cause of stenosis was prolonged intubation (75%). The most common indication for stenting was the history of multiple tracheal operations. The most common complication of stenting and cause of stent removal was formation of granulation tissue. 30% of patients with benign tracheal stenosis were cured and about 10% improved until they could stand a major operation. Ten patients in benign group and 2 patients in malignant group (20%) needed T-Tube insertion after stent removal but other patientcure by stenting. Conclusions: In benign cases stenting is associated with recurrence of symptoms which requires other therapeutic techniqus, so the stenting may not be named as a final solution in benign cases. However, this technique is the only method with approved efficacy for malignant cases with indication.
Investigating Apoptotic Effects of Methanolic Extract of Dorema glabrum Seed on WEHI-164 Cells

Abstract: We aimed to investigate the apoptotic effects of the methanolic extract of Dorema glabrum seed on WEHI-164, cancerous cells in comparison with L929, normal cells and compared them with the cytotoxic effects of Taxol. So, MTT test and DNA fragmentation assay were performed on cultured and treated cells. Also electrophoresis which was followed by immunoblotting was done to survey the production of Caspase-3 and Bcl2 proteins, and to inquire into their relative genes expression, RT-PCR was used. According to our findings, the methanolic extract of Dorema glabrum seed can alter cells morphology as they shrink and take a spherical shape and lose their attachment too. So, the plant extract inhibits cell growth albeit in a time- and dose-dependent manner and results in degradation of chromosomal DNA. Induction of apoptosis by the plant extract was proved by the reduction of pro-Caspase-3 and Bcl2 proteins and increase in Caspase-3 gene expression and decrease in that of bcl2 too. Our data well established the antiproliferative effect of methanolic extract of Dorema glabrum seed and clearly showed that the plant extract can induce apoptosis and not necrosis in vitro. These results demonstrated that Dorema glabrum seed might be a novel and attractive therapeutic candidate for tumor treatment. 1. Introduction Normal cells grow and divide in an ordered fashion, in accordance with the cell cycle. Defective apoptosis (programmed cell death) which results in enhanced growth describes most cancer cells [1]. Several proteins control the timing of the events in the cell cycle, which is tightly regulated to ensure that cells divide only when necessary. The loss of this regulation is the hallmark of cancer [1, 2]. Initially, somatic cell fusion and nuclear transplantation studies, together with the selective use of growth factors and inhibitors of macromolecular biosynthesis, established the fundamental parameters of cell cycle regulation [3, 4]. Our understanding of the complexities of apoptosis and the mechanisms evolved by tumor cells to resist engagement of cell death has focused research effort into the development of strategies designed to selectively induce apoptosis in cancer cells [5–7]. Abstract:

Propagation characteristics of low latitude whistler duct characteristics have been investigated based on day-time measurements at Jammu. The morphogical characteristics of low latitude whistlers are discussed and compared with characteristics of middle and high latitude whistlers. The Max. electron density (Nm) at the height of the ionosphere obtained from whistler dispersion comes out to be higher than that of the background which is in accordance with the characteristics of whistler duct. The equivalent width is found to be close to the satellite observations and the characteristics of whistler duct in low latitude ionosphere are similar to those in middle and high latitude ionosphere. The width of ducts estimated from the diffuseness of the whistler track observed during magnetic storm is found to lie in the range of 50 - 200 Km.

Abstract: Immobilized molecules using biomaterials and nanobiotechnology is a very interesting topic that touching almost all aspects of our life. It uses the sciences of biology, chemistry, physics, materials engineering and computer science to develop instruments and products that are at the cutting edge of some of today’s most promising scientific frontiers. In this review article, the author based on his experience in this arena has tried to focus on some of the supports for im-mobilization; the most important molecules to be immobilized such as DNA, cells, enzymes, metals, polysaccharides, etc and their applications in medicine, food, drug, water treatment, energy and even in aerospace. He specified a special section on what is new in the arena of supports and technologies used in enzyme immobilization and finally a recommendation by the author for future work with a special attention to up-to-date references.
