Beyond 50. challenges at work for older nurses and allied health workers in rural Australia: a thematic analysis of focus group discussions

Abstract: Nurses and allied health workers aged 50 years and over were invited to attend one of six local workshops held in the Hunter New England region of NSW, Australia. This qualitative action research project used a focus group methodology and thematic content analysis to identify and interpret issues arising from workshop discussions.Eighty older health workers from a range of disciplines attended the workshops. Tasks and aspects of work that have become more difficult for older health workers in hospital settings, include reading labels and administering medications; hearing patients and colleagues; manual handling; particular movements and postures; shift work; delivery of babies; patient exercises and suturing. In community settings, difficulties relate to vehicle use and home visiting. Significant issues across settings include ongoing education, work with computers and general fatigue. Wider personal challenges include coping with change, balancing work-life commitments, dealing with attachments and meeting goals and expectations. Work and age-related factors that exacerbate difficulties include vision and hearing deficits, increasing tiredness, more complex professional roles and a sense of not being valued in the context of greater perceived workload.Older health workers are managing a range of issues, on top of the general challenges of rural practice. Personal health, wellbeing and other realms of life appear to take on increasing importance for older health workers when faced with increasing difficulties at work. Solutions need to address difficulties at personal, workplace and system wide levels.The rural health workforce in Australia is ageing and is older than the urban health workforce [1-4]. In 2005, the average age of nurses in outer regional areas of Australia was 46.0 years, compared to 44.6 years in metropolitan areas [2]. Although younger than the nursing profession, rural allied health workers also tend to be older than their city counterparts [3].
Service network analysis for agricultural mental health

Abstract: A network survey of mental health related links between agricultural support, health and other human services in four drought declared shires in comparable districts in rural New South Wales, Australia. Mental health links covered information exchange, referral recommendations and program development.87 agencies from 111 (78%) completed a survey. 79% indicated that two thirds of their clients needed assistance for mental health related problems. The highest mean number of interagency links concerned information exchange and the frequency of these links between sectors was monthly to three monthly. The effectiveness of agricultural support and health sector links were rated as less effective by the agricultural support sector than by the health sector (p < .05). The most highly linked across all areas of activity were Rural Financial Counsellors, the Department of Primary Industry Drought Support Workers and Community Health Centres. Hence for a mental health service network targeting farming families these are three key agencies across the spectrum of case work to program development. The study limitations in describing service networks relate to the accuracy of network bounding, self report bias and missing data from non participants.Aligning with agricultural agencies is important to build effective mental health service pathways to address the needs of farming populations. Work is required to ensure that these agricultural support agencies have operational and effective links to primary mental health care services. Network analysis provides a baseline to inform this work. Effectiveness of service linkages in primary mental health care: a narrative review part 1

Abstract: A narrative and thematic review of English language papers published between 1998 and 2009. Studies of analytic, descriptive and qualitative designs from Australia, New Zealand, UK, Europe, USA and Canada were included. Data were extracted to examine what service linkages have been used in studies of collaboration in primary mental health care. Findings from the randomised trials were tabulated to show the proportion that demonstrated clinical, service delivery and economic benefits.A review of 119 studies found ten linkage types. Most studies used a combination of linkage types and so the 42 RCTs were grouped into four broad linkage categories for meaningful descriptive analysis of outcomes. Studies that used multiple linkage strategies from the suite of "direct collaborative activities" plus "agreed guidelines" plus "communication systems" showed positive clinical (81%), service (78%) and economic (75%) outcomes. Most evidence of effectiveness came from studies of depression. Long term benefits were attributed to medication concordance and the use of case managers with a professional background who received expert supervision. There were fewer randomised trials related to collaborative care of people with psychosis and there were almost none related to collaboration with the wider human service sectors. Because of the variability of study types we did not exclude on quality or attempt to weight findings according to power or effect size.There is strong evidence to support collaborative primary mental health care for people with depression when linkages involve "direct collaborative activity", plus "agreed guidelines" and "communication systems".The first Australian National Mental Health Policy [1] in 1992 set out to move care from institutions to mainstream health and welfare services. Abstract: A narrative and thematic review of English language papers published between 1998 and 2009. An expert reference group helped formulate strategies for policy makers. Studies of descriptive and qualitative design from Australia, New Zealand, UK, Europe, USA and Canada were included. Data were extracted on factors reported as enablers or barriers to development of service linkages. These were tabulated by theme at clinical and organisational levels and the inter-relationship between themes was explored.A thematic analysis of 30 papers found the most frequently cited group of factors was "partnership formation", specifically role clarity between health care workers. Other factor groups supporting clinical partnership formation were staff support, clinician attributes, clinic physical features and evaluation and feedback. At the organisational level a supportive institutional environment of leadership and change management was important. The expert reference group then proposed strategies for collaboration that would be seen as important, acceptable and feasible. Because of the variability of study types we did not exclude on quality and findings are weighted by the number of studies. Variability in local service contexts limits the generalisation of findings.The findings provide a framework for health planners to develop effective service linkages in primary mental health care. Abstract: In this paper we will define analogs of Gr\"obner bases for $R$-subalgebras and their ideals in a polynomial ring $R[x_1,\ldots,x_n]$ where $R$ is a noetherian integral domain with multiplicative identity and in which we can determine ideal membership and compute syzygies. The main goal is to present and verify algorithms for constructing these Gr\"obner basis counterparts. As an application, we will produce a method for computing generators for the first syzygy module of a subset of an $R$-subalgebra of $R[x_1,\ldots,x_n]$ where each coordinate of each syzygy must be an element of the subalgebra.

Abstract: The investigation topicality lies in the fact that many countries continue to realize the socio-economic policy aimed at the elimination of the state economy regulation system. But every time they face the problems that can be solved only by the proper state interference. First of all it concerns public funds management which helps to regulate the appropriate financial flows and ensure the lines of strategic and economic development. The necessity of state power strengthening in these processes is appeared to assure the state function as a market effective funds owner and manager. However there are no mutual approaches to the assessment and control processes.The approaches to the problem of public funds efficiency evaluation are determined and analyzed from the view point of the state financial control in the article.The ways of public funds efficiency evaluation are also cleared up. Two approaches to the use of budget funds, economic agents’ activities and state financial control efficiency evaluation are suggested from the view point of these processes control system. The sum total of these two constituent parts underlies financial efficiency so far as the first one makes the essence of finance, characterizes the quality of their formation and budget funds use. The second constituent part plays a great role in the final goal achievement.The public funds evaluation takes a special place in the system of financial control institutions because their activity influences directly on the evaluation efficiency and the system of state control.
Delivering Key Graduate Attributes via Teams Working in Virtual Space

Abstract: Transferable skills are gaining an increasing emphasis in engineering education. The skills of teamwork, communication, self directed learning and problem solving feature in most of the accrediting agencies criteria. This paper is a case study of a course which uses Problem Based Learning method to deliver key transferable skills to engineering students studying via distance education The students use a range of communication systems including a Learning Management System which offers synchronous and asynchronous communications to work in a team where there is no face to face contact between either the team members or with the supervising academic. The teams solve open ended, contextualised engineering problems. These teams form a learning community which scaffolds individual and team learning goals. Results from a longitudinal study show that students significantly increase their teamwork, communication, problem solving and self directed learning skills. These are key graduate attributes now required by professional accreditation bodies. In addition specific theoretical and technical skills and knowledge are learnt and applied to new problems.

Abstract: This paper examines the meanings attached to sexuality and femininity by Minangkabau teenage girls in schools in West Sumatra, Indonesia. Schools in West Sumatra communicate a hegemonic, normative understanding of womanhood, and a moral consciousness of the female sexual body, to students. Different types of schools – academic, vocational and Islamic senior high schools – have a different ‘curriculum of the body’ (Lesko 1988) and differently discipline bodies and shape sexuality. School girls articulate their understanding of and practise their sexuality in ways that are characteristic of their class, gender and religiosity, mediated by their schools. The schools articulate a religiously-ordained and gendered social order, and impose social control. The different types of school render girls chaste and virtuous to varying degrees. Through everyday practices, this curriculum effects girls’ embodied experience of sexuality. Minangkabau teenage girls have a highly developed sense of their own sexuality, but, far from experiencing a sexual revolution as a result of globalization, most have developed a sexual awareness that is weighted with cultural and religious burdens. Minang female adolescent sexuality is a moral sexuality based on Islam and adat.
Mapping Learning Outcomes and Assignment Tasks for SPIDER Activities

Abstract: Modern engineering programs have to address rapidly changing technical content and have to enable students to develop transferable skills such as critical evaluation, communication skills and lifelong learning. This paper introduces a combined learning and assessment activity that provides students with opportunities to develop and practice their soft skills, but also extends their theoretical knowledge base. Key tasks included self directed inquiry, oral and written communication as well as peer assessment. To facilitate the SPIDER activities (Select, Prepare and Investigate, Discuss, Evaluate, Reflect), a software tool has been implemented in the learning management system Moodle. Evidence shows increased student engagement and better learning outcomes for both transferable as well as technical skills. The study focuses on generalising the relationship between learning outcomes and assignment tasks as well as activities that drive these tasks. Trail results inform the approach. Staff evaluations and their views of assignments and intended learning outcomes also supported this analysis.
