Analysis of the environments of seven Mediterranean tropical-like storms using an axisymmetric, nonhydrostatic, cloud resolving model

Abstract: Tropical-like storms on the Mediterranean Sea are occasionally observed on satellite images, often with a clear eye surrounded by an axysimmetric cloud structure. These storms sometimes attain hurricane intensity and can severely affect coastal lands. A deep, cut-off, cold-core low is usually observed at mid-upper tropospheric levels in association with the development of these tropical-like systems. In this study we attempt to apply some tools previously used in studies of tropical hurricanes to characterise the environments in which seven known Mediterranean events developed. In particular, an axisymmetric, nonhydrostatic, cloud resolving model is applied to simulate the tropical-like storm genesis and evolution. Results are compared to surface observations when landfall occurred and with satellite microwave derived wind speed measurements over the sea. Finally, sensitivities of the numerical simulations to different factors (e.g. sea surface temperature, vertical humidity profile and size of the initial precursor of the storm) are examined.
FRACTIONAL EXCRETION OF UREA IN SEVERELY DEHYDRATED ELDERLY WITH DEMENTIA

Abstract: Fractional excretion of urea (FEurea) is a useful marker to evaluate renal perfusion in people with normal renal function. The value of this marker in a well hydrated person is around 50%, in a low renal perfusion state is lower than 35%, and in an over-hydrated state is higher than 65%.A recent study reported that well-hydrated healthy old and very old people had FEurea higher than young people under the same conditions. That faced us with the fact that there is no information about FEurea values during abnormal renal perfusion states in the elderly.We present two dehydrated elderly patients with dementia that provided us with an opportunity to evaluate the FEurea.Conclusion: We observed that only during severe dehydration an old patient with dementia reached a low FEurea (20%) as a young person. When these patients were dehydrated (hypernatremic) but not hypotensive they showed FEurea higher than a young patient. It seems that new FEurea values are needed in this age group to interpret altered renal perfusion states.
EXCRECIóN FRACCIONAL DE UREA EN ANCIANOS DEMENTES SEVERAMENTE DESHIDRATADOS

Abstract: La excreción fraccional de urea (EFurea) es un marcador útil en la evaluación del estado de perfusión renal en personas con ri ón normofuncionante. El valor de este marcador en un paciente bien hidratado es de alrededor del 50%, en una persona con hipoperfusión renal es menor del 35% y en una persona sobrehidratada es mayor del 65%. Estos valores de referencia provienen de estudios en personas jóvenes.Un estudio reciente demostró que en personas ancianas (edad entre 65-79 a os) y muy ancianas (edad mayor a 79 a os) sanas y en buen estado de hidratación, la EFurea fue mayor que la observada en jóvenes sanos normohidratados. Esto nos hizo notar que no existen hasta la fecha valores normales conocidos de EFurea en pacientes gerontes cursando estados de alteración de la perfusión renal.Presentamos los siguientes casos clínicos por ser una buena oportunidad para apreciar el valor de EFurea alcanzado por personas ancianas dementizadas bajo un estado de severa hipoperfusión renal, a raíz de un cuadro de deshidratación con severa hipernatremia.Conclusión: De los dos casos analizados sólo uno presentó en sus inicios además de severa deshidratación hipotensión arterial, y fue sólo en ese contexto que logró una EFurea baja (EFurea 20%) como la que hubiera logrado una persona joven. Fuera del estado de hipotensión y aun con signos de deshidratación (hipernatremia) ambos pacientes ancianos y dementizados mostraron valores de EFurea más elevados que los que hubiera logrado un paciente joven. Pareciera que nuevos valores de referencia de EFurea son necesarios para la interpretación de los estados de alteración de la perfusión renal en esta población.
SINDROME INTERMEDIO: UN PATRóN TíPICO DE INSUFICIENCIA RENAL AGUDA EN EL ANCIANO

Abstract: Acute renal failure is a frequent entity in the elderly. This is due on one hand to the structural and physiological changes of the aged kidney, and on the other hand to the exposure of this population to polypharmacy and their reduced capability to metabolize drugs. In the present report we present a case of a seventy year-old woman who developed acute renal failure secondary to severe dehydration with a clinical and laboratory pattern of intermediate syndrome: laboratory results compatible with parenchymal renal insufficiency (elevated urinary sodium, plasma urea and creatinine), but with a positive response to hydration. The main characteristics of the aged kidney that predispose to the development of an intermediate syndrome are: the vascular dysautonomy and reduced capability of sodium and water reabsorption. The intermediate syndrome is a typical pattern of pre-renal insufficiency in the elderly. RESUMEN: La insuficiencia renal aguda es frecuente en el anciano. Esto se debe por un lado a los cambios estructurales y funcionales propios del ri ón senil, y por otro a la gran exposición que esta población tiene a la polifarmacia, y su reducida capacidad para metabolizar los medicamentos. En este reporte presentamos el caso de una mujer de 70 a os que desarrolló una insuficiencia renal aguda secundaria a severa deshidratación, mostrando un patrón clínico y de laboratorio propio de un sindrome intermedio: laboratorios compatibles con una insuficiencia renal parenquimatosa (sodio urinario, uremia y creatininemia elevadas), pero con una respuesta favorable a la hidratación. Las principales características del ri ón senil que predisponen al desarrollo del sindrome intermedio: son la disautonomía vascular y la reducida capacidad en la recuperación de sodio y agua El sindrome intermedio es un patrón típico de insuficiencia prerrenal en el anciano.

Abstract: SUMMARYSecondary hyperparathyroidism is one of the main deragements caused by chronic renal failure, and parathyroid hormone is considered one of the toxins of the uremic syndrome. Prolonged fever due to primary hyperparathyroidism have already been described in the literature but not yet as induced by secondary hyperparathyroidism. In this case report a patient suffering from an erythema nodosum and prolonged fever associated to secondary hyperparathyroidism that disappeared through subtotal parathyroidectomy is presented.RESUMENEl hiperparatiroidismo secundario es uno de los principales disturbios causados por la insuficiencia renal crónica, y la paratohormona es considerada una de las toxinas del sindrome urémico. El sindrome febril prolongado secundario a hiperparatiroidismo primario ya ha sido descripto en la literatura, aunque no lo ha sido aun el inducido por hiperparatiroidismo secundario. Abstract: El hiperparatiroidismo secundario es uno de los principales disturbios causados por la insuficiencia renal crónica, y la paratohormona es considerada una de las toxinas del sindrome urémico. El sindrome febril prolongado secundario a hiperparatiroidismo primario ya ha sido descripto en la literatura, aunque no lo ha sido aun el inducido por hiperparatiroidismo secundario. En el presente reporte se presenta un caso de eritema nodoso y sindrome febril prolongado asociado a hiperparatiroidismo secundario y que resolvió luego de efectuada una paratiroidectomía subtotal.

Abstract: Using information from all published reports and data collected during several parasitological surveys betweenApril 2003 and September 2009, a checklist of the parasites of fishes from Guandu River, southeastern of Brazil wasgenerated. A total of 85 parasite species, 54 named species (1 Acanthocephala, 1 Cestoda, 2 Crustacea, 13 Digenea, 11Nematoda, 23 Monogenea and 3 Myxozoa) and 31 undetermined species (3 Acanthocephala, 2 Cestoda, 1 Crustacea, 8Digenea, 8 Nematoda, 4 Hirudinea, 3 Monogenea and 2 Myxozoa) in 21 fish host species from Guandu River, were listed inthe current study, including 36 new locality records and 36 new host records. Also, a host-parasite list is included herein.
Kinetic electrostatic structures in current-carrying pair plasmas

Abstract: The existence and properties of phase-space structures in current-carrying pair plasmas is studied by means of the finite amplitude expressions of the pseudo-potential method. Emphasis is given to double layers, solitary structures and periodic waves. The results can be applied to electron-positron plasmas as well as to plasmas containing heavier charged particles with the same mass and opposite charges, such as fullerene ions and dust particles. They can also help to understand numerical simulations that showed the spontaneous formation of phase-space holes in linearly stable regimes.

Abstract: as curious it may look like, the educative matter under gomecism has not received the same historian?s attention from one or another trend of thought; the study of the economical and political matters have been more exhaustive than the cultural aspects, in general, and particularly the evolution of our scholar system. as gomecism we understand the system initiated with the restoring revolution under cipriano castro′s lead, whose most noticeable features are: the presidentialism, the elimination of leaders, the centralism, the andean presence in the national politics and the domain of a positivist liberal elite, placed backwards to the majority′s interests. gómez continued the political begun by castro. referring to education, three figures left their track in that period since 1899 through 1935 and beyond. these were: the historian josé gil fortoul, the doctor felipe guevara rojas and the jurist rubén gonzález. each one of them worked under very particular political circumstances which, in part, explain their performance. to that historical circumstance we should add the particular education of each one of them, their political orientations, and their sense of citizen responsibility. the essential is that, even though gomecism did not characterize by promoting the national education beyond elementary school and university for exclusive groups, it is possible to find in its interior critics, innovations, reflections and even clashes on aspects such as the state intervention and the laicism, just to point out a few. this is what we are proposed to present.
