Staphylococcal food poisoning from cream-filled cake in a metropolitan area of South-Eastern Brazil

Abstract: twelve people became ill with vomiting and diarrhoea approximately four hours after eating cake with a cream filling at a birthday party and on the day following. the cake had been prepared by a food handler who had long experience in preparing foods for such functions. staphylococcus aureus that produced enterotoxin a was isolated from the nose, the fingernails, and a healed infection on the neck of the food handler, and from the cake. enterotoxin a was detected in the remaining portion of the cake. the cake, while still warm, had been refrigerated for one hour after it was prepared before it was removed for the party; it was refrigerated after the party. the cake was large (6 kg) and hence it was not adequately cooled in the hour during wich it was refrigerated before the party. the conclusion is that the cake was accidentally contaminated by the food handler and inadequately cooled before it was eaten.

Abstract: an outbreak of staphylococcal food poisoning involving 42 people who had eaten a meal at a restaurant in the municipality of passos, minas gerais, brazil, is reported. thirty-one of the individuals became ill with vomiting, diarrhea and dizziness within 30 minutes after eating the meal. the foods suspected were: chicken pancake, rice, beans, tomato sauce and mashed chick-peas. large numbers (> 2.0x108 cfu/g) of enterotoxigenic staphylococci were present in the chicken pancake. these strains produced enterotoxins a, b and d. swabs from the nasal cavity and throat and from under the fingernails of food handlers were cultured for the detection of enterotoxigenic staphylococci carriers. four out of five of them were healthy carriers of enterotoxin a, b, c and d producing staphylococcus aureus at the sites cultured and one of them was also a nasal carrier of tsst-1 toxin producing s. aureus. these results indicate that the food handlers would have been the source of the food contamination.

Abstract: An outbreak of staphylococcal food poisoning involving 42 people who had eaten a meal at a restaurant in the Municipality of Passos, Minas Gerais, Brazil, is reported. Thirty-one of the individuals became ill with vomiting, diarrhea and dizziness within 30 minutes after eating the meal. The foods suspected were: chicken pancake, rice, beans, tomato sauce and mashed chick-peas. Large numbers (> 2.0x10(8) CFU/g) of enterotoxigenic staphylococci were present in the chicken pancake. These strains produced enterotoxins A, B and D. Swabs from the nasal cavity and throat and from under the fingernails of food handlers were cultured for the detection of enterotoxigenic staphylococci carriers. Four out of five of them were healthy carriers of enterotoxin A, B, C and D producing Staphylococcus aureus at the sites cultured and one of them was also a nasal carrier of TSST-1 toxin producing S. aureus. These results indicate that the food handlers would have been the source of the food contamination.

Abstract: the objective of this article is to analyze continuing education in nursing practice. there is concern nowadays about the changes introduced in the structure of teaching and the application of active methods to train health care professionals. the everyday aspects of nursing practice are examined, as is the way technological-scientific information now is being supplied to practicing nurses in hospitals, so as to enrich nursing care through continuing education. the transformative practices applied by nurse caregivers are described as an element that facilitates the teaching-learning process. there is a wide-ranging review of computer databases. consequently, the choice of texts is based on inclusion and exclusion criteria, giving rise to 50 scientific articles. recent years have seen the introduction of continuing education as a method in the field of health, having become an indispensable feature in practices for training, care, management, the development of public policy and social control. what has emerged is the concept of total care, with know-how and practices having been transformed and included in the real environment of those for whom they are intended. in conclusion, it is the motivation to continually up-date the professional practice of the nursing staff that provides for the application of an interdisciplinary instrument of solubility that makes actions more visible to clients and their families.
Educa o permanente e qualidade da assistência à saúde: aprendizagem significativa no trabalho da enfermagem

Abstract: El objetivo de este artículo es analizar la educación permanente en la práctica de la enfermería. En la actualidad, existe preocupación por los cambios introducidos en la estructura de ense anza y la aplicación de metodologías activas para capacitar profesionales de salud. En el artículo se reflexiona sobre aspectos cotidianos de la enfermería y la manera actual de suministrar o proporcionar información tecnocientífica al enfermero durante la práctica en el hospital, tratando de enriquecer la asistencia de enfermería mediante la educación permanente. Se describen las prácticas trasformadoras aplicadas por el enfermero asistencial como elemento facilitador del proceso ense anza-aprendizaje. Se lleva a cabo una revisión amplia de las bases de datos informáticas. En consecuencia, la selección de los textos se basa en los criterios de inclusión y exclusión, y dio origen a 50 artículos científicos. En a os recientes, la educación permanente se introdujo como un método en el área de la salud, convirtiéndose en un aspecto indispensable en las prácticas de formación, atención, gestión, formulación de políticas públicas y control social. Así se estableció la idea de cuidado total y se trasformaron e integraron los saberes y las prácticas en el ámbito real del público al que va dirigido. Se concluye que la motivación para actualizar permanentemente la práctica profesional del equipo de enfermería prevé la aplicación de un instrumento interdisciplinario de solubilidad que promueva la visibilidad de las acciones por los clientes y sus familias.

Abstract: purpose: to evaluate short-term efficacy of awareness programs (ap) in reducing coronary heart disease risk factors (chdrf). methods: high risk hypercholesterolemic patients were divided in 2 groups during 16 weeks. group a (n=417, 54.3±10.0 years, 55% males) received verbal and written orientation on chdrf control, and group b (n=180, 54.4±10.9 years, 45% males) received only verbal orientation. all participants received pravastatin 10mg q.d. for 12 weeks. the evolution of body weight, arterial pressure, lipid profile, castelli's i and ii indexes (tc/hdl and ldl/hdl), and framingham scores were evaluated. results: at baseline, a had a lower hdl-c (40.0±11.0 vs 43.0±11.0mg/dl, p=0.013) and a higher index i (8.2±3.0 vs 7.6±2.3, p=0.008) than b. after 16 weeks, a had greater change than b in tc (-28.0 vs -25.0, p<0.05), ldl-c (-29.0 vs -27.6, p<0.05), hdl-c levels (+13.7 vs +10.8, p<0.05) and in the castelli's index (-39.0 vs -33.0; p<0.05). in both groups pravastatin use potentialized the effects of diet on the lipid profile. conclusion: the ap seemed to be more effective than verbal orientation alone in chdrf reduction at short-term.

Abstract: OBJETIVO: Avaliar a eficácia de programas de conscientiza o (PC) sobre o controle de fatores de risco (FR) para doen a cardiovascular (DCV). MéTODOS: Pacientes hipercolesterolêmicos de alto risco para DCV foram divididos em 2 grupos durante 16 semanas. O grupo A (n=417, 54,3±10,0 anos, 55% homens) recebeu orienta o verbal e escrita sobre controle de FR, e o grupo B (n=180, 54,4±10,9 anos, 45% homens) apenas orienta o verbal. Todos os participantes receberam 10mg/dia de pravastatina por 12 semanas. Avaliaram-se o peso, press o arterial, o colesterol total (CT) e fra es, triglicérides, índices I e II de Castelli (CT/HDL-C e LDL-C/HDL-C) e escores de Framingham. RESULTADOS: No basal, A diferiu de B no HDL-C (40,0±11,0 vs 43,0±11,0mg/dl, p=0,013) e no índice I (8,2±3,0 vs 7,6±2,3, p=0,008). Após 16 semanas as varia es % foram maiores em A do que B no CT (-28,0 vs -25,0, p<0,05), LDL-C (-29,0 vs -27,6, p<0,05), HDL-C (+13,7 vs. + 10,8, p<0,05) e índice I (-39,0 vs -33,0; p<0,05). A pravastatina potencializou os efeitos da dieta sobre os lípides. CONCLUS O: O PC parece ser mais eficaz a curto prazo, em reduzir os FR para DCV do que a orienta o apenas verbal.
Fine litter accumulation in Central Amazonian Tropical Rainforest canopy

Abstract: fine litter dynamics within the canopy differ from litter dynamics on the forest floor for reasons such as differences in microclimate, substrate, disturbance level, stratum influence and decomposition rates. this study is the first attempt to quantify the fine litter accumulated in the canopy of central amazonian forests. we compared the canopy litter accumulation to fine litter-layer on forest floor and to other forests and also investigated which were the mostly accumulated litter omponents. we found that central amazonian rainforest intercepts greater fine litter in the canopy (294 g.m-2) compared to other forest formations with higher winds speed as in a costa rican cloud forest (170 g.m-2). the mean canopy fine litter accumulated at the end of the dry season was less than a half of that on soil surface (833 g.m-2) and the fine wood component dominates the canopy samplings (174 g.m-2) while leafy component predominate on soil surface litter (353 g.m-2).

Abstract: this trial was conducted to study the effects of replacing corn with increasing levels of cassava starch by-products on carcass and meat characteritics for fedlot young bulls. percentages of substitution were 0, 25, 50, 75, and 100, in dry matter basis. forty crossbred bulls (? and ? bos taurus x bos índicus), averaging 21 months old and 363 kg of initial body weight and 570 kg of final body weight were assigned to a block design with eight replicates by treatment. no effect of treatment on carcass traits was detected. the averages were as follows: carcass yield, 54.79%; conformation, 12.32 (good +); carcass length, 132.5 cm; cushion thickness, 28.12 cm; ribeye area, 79.25 cm2; porcentages of bone, 15.51%, of muscle, 64.12%, and of fat, 20.85%; muscle + fat/bone relation , 5.52; edible portion percentage, 85.07%; fat thickness, 4.02 mm; and marbling, 3.9 (light -). no effect of treatment on chemical composition of meat was detected, with averages of 1.01; 19.16 and 2.24%, respectively, for ash, protein, and fat. there was significant difference for humidity, with higher percentage for 50% replacement diet (74.67%) and lower for 25% replacement diet (72.89%). the other treatments had intermediate values. no treatment effects were observed and mean values for the studied traits indicate that the obtained carcasses fulfill the expected requirements of market.
