Abstract: objective: patients with ischemic heart failure may benefit from coronary artery bypass grafting. the histopathological variables associated with improvement in ejection fraction 6 months after surgery were assessed. methods: this study comprised 24 patients indicated for coronary artery bypass grafting, ejection fraction < 35%, functional class ii-iv heart failure, and mean age 59 ± 9 years. endomyocardial biopsies were performed during and 6 months after surgery. extension of the fibrosis, number of cells with myocytolysis, and hypertrophy of the muscle fiber were quantified by using a system of image analysis. clinical and functional review was repeated within 6 months. results: a significant improvement in heart failure functional class was observed in 16 patients after 6 months of follow-up (from nyha functional class 2.8±0.7 to 1.7±0.6; p <0.001), but the ejection fraction did not change (25±6 % vs. 26±10%). hypertrophy of the muscle fiber was similar in the specimens biopsied in the pre- and postoperative periods (21±4 vs. 22± 4 μm), but the extension of fibrosis (8±8 vs. 21±15% area) and the number of cells with myocytolysis (9±11 vs. 21±15% cell) significantly increased. however, the composition of a histological score combining those 3 variables indicated a greater increase in the ventricular function of those with a lower degree of preoperative histopathological alterations. conclusion: patients with ischemic cardiomyopathy undergoing coronary artery bypass grafting improved their ventricular function when the preoperative adverse histopathological alterations were of a lower degree.

Abstract: aim: to determine the impact of prognostic factors on survival of patients with metastases from colorectal cancer that underwent liver resection. methods: the records of 28 patients that underwent liver resection for metastases from colorectal cancer between april 1992 and september 2001 were retrospectively analyzed. thirty-eight resections were performed (more than one resection in eight patients and two patients underwent re-resections). the primary tumor was resected in all the patients. a screening protocol for liver metastases including clinical examinations every three months, ultrassonography and cea level until 5 years of follow-up and after every 6 months, was applied. the prognostic factors analyzed regarding the impact on survival were: dukes c stage of primary tumor, size of metastasis >5 cm, a disease-free interval from primary tumor to metastasis < 1 year, cea level > 100 ng/ml, resection margins < 1 cm and extrahepatic disease. the kaplan-meier curves, log rank and cox regression were used for the statistical analysis. results: perioperative morbidity and mortality were 39.3% and 3.6%, respectively. the 5-year survival rate was 35%. the independent prognostic factors were: disease-free interval from primary tumor to metastasis < 1 year and extrahepatic disease. conclusions: the liver resection for metastases from colorectal cancer is a safe procedure with more than 30% 5-year survival. disease-free interval from primary tumor to metastasis < 1 year and extrahepatic disease were independent prognostic factors.

Abstract: with the acquired experience with laparoscopic cholecystectomy, videosurgery has been used more and more for treatment of other diseases. since the first surgery, nearly 15 years ago, the authors evaluate the outcome and the perspectives of this procedure in pancreatic-billiary diseases. they followed the proposition which divides these procedures in routine procedures, in evaluation and in development. in gallbladder diseases the laparoscopic cholecystectomy is considered gold-standard, taking advantage upon open cholecystectomy, excluding bile duct injury and cystic stump leakage. they emphasize the aspects related to acute cholecystitis, cholecystectomy during pregnancy, calcified gallbladder and cancer. the laparoscopic cholecystectomy during pregnancy is considered as a procedure in evaluation. long term effects of pneumoperitonium on foetus are not well known. the approach in calcified gallbladder, because the high incidence of adenocarcinoma, remains questionable. perspectives of videosurgery in complications of laparoscopic cholecystectomy are commented. cystic stump leakage, misplaced clips and partial section or laceration of main biliary tract could be treated by new videosurgery in selected situations. in the treatment of choledocolithiasis, they consider the procedure as one for evaluation, due to the lack of randomized prospective trials. until now, videocholedocolitotomy is reserved to specialized centers. in pancreatic diseases, videosurgery is a procedure in development, except for the non-complicated acute biliary pancreatitis, in which benefits with laparoscopic cholecystectomy has been shown. the same occurs with diversion surgeries for desobstruction of the biliary tree. although performed within the conventional surgery principles there is a lack of comparative studies with other techniques analysing efficacy and efficiency.
Fatores prognósticos na ressec o de metástases hepáticas de cancer colorretal

Abstract: OBJETIVO: Determinar o impacto de fatores prognósticos na sobrevida de pacientes com metástases hepáticas ressecadas e originadas de cancer colorretal. CASUíSTICA E MéTODOS: Foram analisados os prontuários de 28 pacientes submetidos a ressec o hepática de metástases de cancer colorretal de abril de 1992 a setembro de 2001. Foram realizadas 38 ressec es (8 pacientes com mais de uma ressec o no mesmo tempo cirúrgico e 2 pacientes submetidos a re-ressec es). Todos haviam sido submetidos previamente a ressec o do tumor primário. Utilizou-se protocolo de rastreamento de metástases hepáticas que incluiu revis es clínicas trimestrais, ecografia abdominal e dosagem de CEA até se completarem 5 anos de seguimento e após, semestralmente. Os fatores prognósticos estudados foram: estágio do tumor primário, tamanho das metástases > 5cm, intervalo entre ressec o do tumor primário e surgimento da metástase < 1 ano, CEA >100 ng/mL, margens cirúrgicas < 1cm e doen a metastática extra-hepática. O estudo foi retrospectivo e a análise estatística foi feita pela curva de Kaplan-Meier, log-rank e regress o de Cox. RESULTADOS: A morbidade foi 39,3% e a mortalidade operatória foi 3,6%. A sobrevida em 5 anos foi de 35%. Os fatores prognósticos independentes adversos foram: intervalo < 1 ano entre ressec o do tumor primário e surgimento da metástase, e doen a metastática extra-hepática. CONCLUS ES: A ressec o hepática de metástases de cancer colorretal é um procedimento seguro com sobrevida em 5 anos acima dos 30%. Foram fatores prognósticos independentes adversos: doen a metastática extra-hepática e intervalo < 1 ano entre ressec o do tumor primário e surgimento da metástase.

Abstract: purpose: to evaluate the effects of alcohol and caffeine in a pancreatic carcinogenesis mouse model induced by 7,12-dimethylbenzantracene (dmba), according to the panin classification system. methods: 120 male, mus musculus, cf-1 mice were divided into four groups. animals received either water or caffeine or alcohol or alcohol + caffeine in their drinking water. in all animals, 1 mg of dmba was implanted into the head of the pancreas. after 30 days, euthanasia was performed; excised pancreata were then fixed in formalin, stained with hematoxylin-eosin and categorized as follows: normal ducts, reactive hyperplasia, panin-1a, panin-1b, panin-2, panin-3 or adenocarcinoma. results: panin lesions were verified in all groups. adenocarcinoma was detected in 15% of animals in the caffeine group, 16.6% in the water group, 23.8% in the alcohol + caffeine group and 52.9% in the alcohol group (p<0.05). conclusions: the experimental pancreatic carcinogenesis mouse model using dmba effectively induces panin lesions and pancreatic adenocarcinoma. this study verified the association between alcohol use and pancreatic adenocarcinoma; caffeine did not present the same effect.
Cognitive-Behavioral Treatment for Depression in Adolescents

Abstract: The goal of this article is to briefly review and summarize the rationale and research support for cognitive-behavioral therapy (CBT) as a treatment for depressed adolescents. A primary focus of the paper is on our group CBT treatment for adolescent depression, entitled the Adolescent Coping With Depression Course. In addition, initial findings from a large, recently-completed study contrasting individual CBT to fluoxetine for depressed adolescents (Treatment of Adolescents with Depression Study) are presented. Although the research support for CBT as a treatment for depressed adolescents is generally encouraging, we need to better understand which depressed adolescents benefit from CBT, how and when to incorporate medication and family-based interventions into CBT treatment, how to treat depressed adolescents with comorbid psychiatric conditions, and how CBT interventions fare with non-European-American depressed adolescents.
