Prevalencia de desnutrición en pacientes oncológicos

Abstract: hyponutrition prevalence among cancer patients is a common problem; malnutrition incidence is estimated in 15 to 40% at cancer diagnosis, and reaches 80% in cases of advanced cancer. the consequences of hyponutrition involve a greater number of postoperative complications, longer hospital stay, higher costs, and an increased mortality. hyponutrition prevalence among cancer patients was studied at the diabetes and nutrition department of marie curie oncology hospital in patients who attended for preoperative nutritional assessment from june 2008 to december 2008. the detection method used was the ulibarri and col. (in conut system). 132 patients were evaluated. 7.2% (n=9) of the assessed patients had lost more than 10% of their usual weight within the previous six months (ci95% 2.2-12.2%); 12.9% (n=16) had lost 5 to 10% (ci95% 6.6-19.2%) of their usual weight, and 79.8% (n=99) had lost 0 to 5% of their usual weight within the previous six months (ci 95% 72.3-87.3%). serum albumin levels were higher than 3.5 g/dl in 97.7% of the assessed patients (ci 95% 94.8-100%); serum proteins were higher than 5 g/dl in 100% of the cases; serum cholesterol was lower than 180 mg/dl in 18.2% of the cases (ci95% 11.2-25.4%); and 63.6% of the sample (ci 95% 55.0-72.2%) had a total lymphocyte count below 1600/mm3. hyponutrition prevalence among evaluated cancer patients was 47.7% (ci95% 38.8-56.6%). hyponutrition appears as an important feature in the studied population and, considering its possible implications, measures should be taken to reverse this situation so that oncologic patients have an optimum nutritional status at the time of surgery.

Abstract: Este artículo evalúa el desarrollo de una intervención comunitaria realizada en una comunidad del pueblo Mapuche (Argentina), y orientada por los lineamientos de la psicología social comunitaria. Basado en el desarrollo de talleres y actividades de distinto tipo con los integrantes de la comunidad, este proceso tuvo por objeto la reafirmación de identidades y el fortalecimiento de los sentimientos de pertenencia comunitaria. El examen de este ejercicio de intervención se encuentra atravesado por una serie de preguntas sobre la posibilidad de promover cambios sociales a través de iniciativas que involucren la reflexión y la autoreflexión, así como sobre el rol del psicólogo en esta clase de dinámicas sociales.

Abstract: La prevalencia de desnutrición en el paciente oncológico es un problema común; se estima que la incidencia de malnutrición oscila entre 15 y 40% al momento del diagnóstico del cáncer, y aumenta hasta 80% en casos de enfermedad avanzada. Las consecuencias de la desnutrición implicarán la aparición de mayor número de complicaciones en el período postquirúrgico, estancias hospitalarias prolongadas, mayor coste económico, y una mortalidad más elevada. Se estudió la proporción de pacientes con desnutrición que concurren al servicio de Nutrición y Diabetes para Valoración del Estado Nutricional pre-quirúrgico en el Hospital Municipal de Oncología Marie Curie, de junio a noviembre de 2008. La detección se realizó por el método propuesto por Ulibarri y col (dentro del sistema CONUT). Fueron evaluados 132 pacientes. Un 7,2% (n=9) de la muestra presentó una pérdida de peso mayor al 10% en los últimos 6 meses (IC95%2,2-12,2%), la pérdida del 5 al 10% del peso corporal se presentó en el 12,9% (n=16) (IC95% 6,6-19,2%), predominando la pérdida de menos del 0 - 5% del peso corporal con un 79,8% (n=99) (IC 95% 72,3-87,3%). El 97,7% de la muestra presentó valores de albúmina mayores a 3,5 g/dl (IC 95% 94,8-100%); todos los evaluados presentaron valores de de proteínas totales mayor a 5 g/l; el 18,2% (IC95% 11,2-25,4%) presentó valores de colesterol menores a 180 mg/dl; y un 63,6% presentó valores inferiores a 1600 linfocitos/mm3 (IC 95% 55,0-72,2%). La prevalencia de desnutrición resultó del 47,7% (IC95% 38,8-56,6%). La desnutrición aparece como un rasgo importante en la población estudiada, y, considerando las implicaciones de dicha situación, deberían tomarse medidas para revertir la situación en pos de lograr que los pacientes oncológicos lleguen a la cirugía bien nutridos. Abstract: the second biomedical revolution puts into question human society as a whole inasmuch as it gives to human being the dominion over three fundamental systems, namely: reproduction, genetics, and the nervous system. current experiment assays go beyond either health protection or health recovery, thus allowing new techniques to be challenged in the sense that they may modify the evolution of species; furthermore they may modify humans as a species. on no account, however, are we suggesting simply that some control program on science be implemented, nor are we willing to sow mutual suspicions likely to generate an endless quarrel on scientific advance. to progress towards an actual problem solving process coupled to a cooperation between scientific culture and human values, it is necessary that research on people must be transformed into researching with people. to which extent, however, medical research is in a position to envision new alliances between scientific tasks and human demands? setting up an expanded scientific community able and willing to present new social actors with a collective appropriation of knowledge is an urgent commitment to be considered thoroughly

Abstract: the desire of knowledge, one of man’s most important goals, there are also mixed wishes of other type, like ambition or competition, which can deviate him from his uninterested objectives. that is why there are many people who throw a suspect on this libido sciendi that has become a libido dominandi where the legal valorization of power and the idolatry of progress are combined. it will be the ethic comisions or comites to play a calming function that can work as a boundary to this cultural anguish. however, these possible deviations must not only be atributed to the desire of knowing in itself, but to the social system, which, troughout the institutions, compels investigators to compete among themselves with projects and publications, and obliges them to enter either into an economic competition or into an international confrontation in order to obtain recognition. as members of a scientific comunity, with its own interests, it is necessary for us to defend the autonomy of this libido cognoscendi in front of those who take social decisions, in the measure that economical resources for investigation depend on them. uninterested investigation is still very important for the cultural wellfare of society, but it is important to be on guard, as man is not only in the centre of investigations, but also of its applications
