EFECTOS DE INTERACCIóN SUELO-ESTRUCTURA EN EDIFICIOS CON PLANTA BAJA BLANDA

Abstract: Las estructuras con planta baja flexible son muy vulnerables a la acción de sismos. Esto es debido, principalmente, a la falta de rigidez y resistencia en el piso blando. Las Normas Técnicas Complementarias para Dise o por Sismo del RCDF tratan el problema como una condición de irregularidad estructural, limitándose a reducir el factor de comportamiento sísmico que controla las resistencias de dise o. De esta forma se aumenta la capacidad de rigidez y resistencia de toda la estructura, pero no se corrige el contraste que existe entre el piso blando y el resto de los entrepisos. En este trabajo se desarrolló un modelo numérico para estimar la respuesta dinámica de estructuras con planta baja flexible desplantadas sobre suelo blando. El modelo es elástico y tiene en cuenta el alargamiento del periodo estructural debido a la flexibilidad del suelo, así como el incremento en el amortiguamiento debido a la disipación de energía por radiación de ondas en el suelo. Considerando que el amortiguamiento está distribuido a lo largo del edificio, se construye una matriz de amortiguamiento clásico para la estructura sola usando amortiguamiento modal. Para el suelo, en cambio, se considera amortiguamiento elemental haciendo uso de amortiguadores viscosos para los distintos modos de vibrar de la cimentación. Debido a que el sistema acoplado suelo-estructura carece de modos clásicos de vibrar, la respuesta estructural se obtiene con el método de la respuesta compleja en la frecuencia.
DISTRIBUCION DE ELEMENTOS MECáNICOS EN PILAS SUJETAS A FUERZAS EN LA CABEZA Y ANTE LA INCIDENCIA DE ONDAS SíSMICAS

Abstract: El dise o convencional de pilas y pilotes ante cargas laterales considera de manera casi exclusiva la acción de las fuerzas que induce la estructura en su cabeza. El paso de las ondas sísmicas a través de los elementos de cimentación también produce solicitaciones mecánicas debidas a la incompatibilidad de deformaciones entre el suelo y la pila o el pilote. La distribución y magnitud de los elementos mecánicos debidos a ambos fenómenos difiere en gran medida, y depende de la estratigrafía y las condiciones de apoyo de la pila o pilote. En este trabajo se utiliza un método de estratos finitos para estimar los elementos mecánicos en pilas y pilotes ante cargas dinámicas. Se considera la restricción parcial al giro en la punta del elemento de cimentación y la conexión de la cabeza con la superestructura. Estos aspectos tienen influencia en la respuesta lateral de pilas y pilotes, sobre todo para elementos de gran diámetro. Se desarrolla un análisis paramétrico para determinar las diferencias cualitativas entre los elementos mecánicos introducidos por la aplicación de fuerzas en la cabeza de la pila o pilote y aquellos generados por el paso de ondas sísmicas.
Granulocitoaféresis en la enfermedad inflamatoria intestinal: Eficacia a 32 semanas con protocolo de inducción y sesiones de mantenimiento

Abstract: introduction: granulocytapheresis (gcap) eliminates activated granulocytes-monocytes from peripheral blood, thus modifying the circulating pool of leukocytes and reducing intestinal inflammation. objective: tto evaluate the efficacy of gcap in inflammatory bowel disease (ibd) using an induction and maintenance protocol. material and method: a retrospective study including patients with active corticosteroid-dependent or refractory ibd. induction included 5 sessions in ulcerative colitis (uc) and 7 sessions in crohn′s disease (cd); one monthly session was used thereafter until week 32. clinical activity indices and use of corticosteroids were monitored. results: eighteen patients were included (10 with uc, 8 with cd), 10 of them dependent on and 8 refractory to corticosteroids. fourteen of them were refractory and a further 4 were intolerant to immunosuppressants (is). induction was not completed in 2 uc (severe relapses) and 1 cd (side-effects) patients. one uc and 3 cd patients withdrew during maintenance. among patients who completed induction, response or remission was achieved in 87.5% of uc cases (2 and 5 patients) and 71.4% of cd cases (1 and 4 patients), respectively. at week 32 response-remission rates reached 75% in cu (3 and 3 patients) and 42.8% in cd (1 and 2 patients) cases, respectively. corticosteroid withdrawal was possible in 14.2% of cd and in 62.5% of uc patients (25% in remission and 37.5% with response). there were two major side effects (thrombophlebitis and syncope). no colectomies were performed for uc patients who completed gcap induction after a mean follow-up of 97.6 weeks (range: 72-128). conclusions: both uc and cd respond well to gcap induction. at 32 weeks uc patients maintain similar response-remission rates (87.5 vs. 75%), whereas almost one-third of cd patients loose response. granolocytapheresis is an alternative, steroid-sparing treatment modality to induce and maintain remission in uc, while good patient selection and a maintenan
Longevidad y razones para el cambio de restauraciones de amalgama

Abstract: the aim of this study was to determine in vivo the medium survival rate and failure reasons of amalgam restorations. two clinicians (cohen′s kappa=0,75) studied clinically and by xray 56 patients (18-80 years) (mean value 28±17) (with 213 restorations classes i=135, ii=78). patients and dentist were consulted the date in which restorations were made. the failures of the restorations were evaluated by the following six parameters; 1: marginal adaptation (ma), 2: anatomy (a), 3: roughness (r), 4: contacts (c), 5: sensitivity (s) and 6: secondary caries (sc), according to ryge modified criteria. the median longevity of restorations until the study date was 5.8 years. the failure reasons expressed by parameter in quantity and (percents of all restorations) were: sc: n=13 (35.1%), ma: n=10 (27.0%), a: n=8 (21.6%), r: n=5 (13.5%), s: n=1 (2,7%), c: n=0 (0,0%). the main failure reasons were secondary caries, marginal adaptation and anatomy. the less frequency of failure was observed in the parameters contacts and post operative sensitivity. early and greater failures were observed in class ii than class i.
Planificación y guía docente en el marco de la convergencia europea de educación superior

Abstract: present day societies are characterized by the globalization of their economies and, the fostering of a " knowledge-based society". higher education plays a part in these phenomena, and, in europe, is undergoing an important reform seeking to homogenize the system of university degrees. these attempts come together in the european space of higher education which will mean a new paradigm centred on "learning" and upheld by a system of common credits, teaching staff planning, innovations in methodology, the professional competences and the handling of information in the constantly changing knowledge in today?s world. in this framework staff planning, and the production of teaching guides is of the utmost importance. the aim of which being an awareness on the part of the student of the benefits to be had when taking each course, what activities should be carried out, guidance on chronology, on assessment. regarding the subject under discussion, we have evolved some reflections on the lines of each of these topics, putting forward a model illustrated with examples on how to make planning and teaching guides useful instruments for the student. we conclude stressing the importance of a teaching plan for convergence and noting that despite the difficulties, changes are possible.

Abstract: the survey of the health care quality through the analysis of claims is an accredited tool in the biography consulted. although users′ assessments concerning nurses are usually positive, complaints are nevertheless increasing through different ways. our objectives are to study both, the user's perception of our deficiencies and the professionals′ attitude with their patients.we propose an observable evaluation through the analysis of documents (claims presented by radiology service users from january 2000 to september 2003 and reported by the nursing department, as well as the supervisor′s reports to the management of the hospital). most of the claims denounce: delay in the appointments, lengthy periods of waiting, incorrect manners, insufficient information, bad professional praxis, lost of explorations, lack of material, etc.we agree on the revised surveys that the claims are valid as indicators of a deficient assistance and in their systematic study to detect and to correct organisational, structural defects, etc. the changes we have embarked on will not be useful if no other changes in the relationship professional/patient take place, which starts by "knowing what they say to us".

Abstract: El estudio de la calidad asistencial mediante el análisis de reclamaciones es una herramienta acreditada en la bibliografía consultada. Aunque las valoraciones de los usuarios respecto de los enfermeros suelen ser positivas, sin embargo, crecen las manifestaciones de quejas a través de distintos medios. Nuestros objetivos son estudiar la percepción del usuario acerca de nuestras deficiencias y de la actitud de los profesionales en su relación con el paciente.Planteamos una evaluación observacional mediante el análisis de documentos (reclamaciones informadas por enfermería presentadas por usuarios del servicio de radiología desde enero de 2000 hasta septiembre de 2003, así como los informes del supervisor a la dirección). La mayoría denuncian demoras en las citas, esperas prolongadas, trato incorrecto, información insuficiente, mala praxis profesional, pérdida de exploraciones, falta de material, etc.Coincidimos con los estudios revisados en la validez de las reclamaciones como indicador de las deficiencias en la atención y en su estudio sistemático para detectar y corregir defectos organizativos, estructurales, etc. Los cambios emprendidos servirán de poco si no se operan cambios en la relación profesional/paciente que empiezan por "conocer lo que nos dicen". Planificación y guía docente en el marco de la convergencia europea de educación superior Planning and educational guide in the higher education european convergence framework

Abstract: Las sociedades actuales se caracterizan por la globalización de las economías, y la potenciación de la " sociedad del conocimiento". La educación superior participa de estos fenómenos y, en Europa, experimenta una importante reforma que pretende homogeneizar el sistema universitario. Estos intentos se sustancian en el Espacio Europeo de Educación Superior, que supone un nuevo paradigma centrado en el "aprendizaje" y sustentado en un sistema de créditos común, la planificación de la docencia, las innovaciones metodológicas, las competencias profesionales y el manejo de la información y el conocimiento. En ese marco cobra relevancia la planificación de la docencia y la elaboración de Guías Docentes con la finalidad de que el alumno sepa cuáles son las ganancias que obtendrá al cursar cada asignatura, las actividades que va a desarrollar, orientaciones sobre la cronología o sobre la evaluación. En este trabajo desarrollamos algunas reflexiones respecto de cada uno de estos apartados, proponiendo un modelo de cómo hacer de la planificación y la guía docente instrumentos útiles para el alumno. Concluimos resaltando la importancia de la planificación de la docencia en la convergencia y se alando que los cambios que ésta supone, pese a las dificultades, son posibles. Abstract: objective: the development of a tool that provides the systematic, registration and care continuity in the nuclear medicine services. methodology: this research was developed during 2004 as a result of the creation of the nuclear medicine service in the torrecárdenas hospital, in almeria, spain. a nursing problem review was undertaken from the data that captured in the bibliography review and clinical practice. it was carried through consent techniques, a language standarization and its codification following the nanda, noc and nic taxonomies. starting from an initial valuation for focus basic needs, it described nursing diagnoses, complications and the associated interventions to the nursing diagnose and the collaboration problems. finally, the evaluation and the care continuity report were established. results: tables with data for the initial evaluation were compiled of the main nursing diagnoses (deficient knowledge, fear, breast-feeding interruption, and injury risk) related to the hoped results, expressed according to noc taxonomy (knowledge: procedure, fear self-control; breast-feeding: weaning and risk control) and the nursing intervention (teaching: procedure, nervous tension decrease, breast-feeding suppression, environmental management: security, etc.); as well as the intervention associated to the collaboration problems. conclusion: this care map provides the work using a nursing process. it also helps to register in a easy way the interventions made to patients with nuclear medicine procedure.

Abstract: the european convergence process is a major change affecting most of european states, by introducing new elements in order to harmonize its policies. in terms of education, there are significant changes in educational systems, especially those concerning university studies. in this frame, new educational methodologies in nursing studies have started to develop through pilot experiences in different european universities. in this article, one of these particular experiences, developed by the university of almería (spain) is described, especially in terms of objectivity and considering exams as an exclusive assessment methodology. after an exhaustive review of this topic, and due to the unspecificity of reliable tools for assessing knowledge, as well as attitudes and skills, we propose an ethical-critical review of the assessment concept, from a practical rationality that goes far beyond positivist and utilitarian aspects.
