Integralidade, uma diretriz do SUS para a vigilancia sanitária

Abstract: the sanitary surveillance (visa) performs several practices, on different objects and its actions are guided by principles and guidelines of the sus. it was done a critical reflection on the interaction conditions of practice in visa, with a constitutional proposition of the sus: integral care. the analysis was based on the theory of structuration (giddens) that considers mobilization of structural resources as dimensions of social interaction, which would justify the legitimacy exercised since the standards. have been analyzed the following categories: visa and its insertion within the sus; the integral care and the visa; and political impediments. the visa has been organized by national health surveillance agency. nowadays it has as sanitary responsibilities, communication with society and health promotion. the proposal of the literature concerning integral care is based on the assistance issue. the organization of the services in the different federative entities is the sense of integral care most adopted by visa. political impediments focus on the institutional renewal, on the conflicts of interest arena, on the distance between formulated policies and established practices and gaps concerning work management and the insufficiency of financial support.

Abstract: Objective: As isolates of Cryptococcus are frequently kept collection stocks in institutions, sometimes without proper characterization, we sought to determine the genotype profiles, protease and phospholipase activities “in vitro” and the susceptibility testing for azoles and amphotericin B. Methodology: 84 isolates from several regions of Brazil (40 samples from clinical origin and 44 isolates from environmental origin) were maintained at the microorganism’s bank of the Biomedical Science Institute (ICB-USP) of the São Paulo University, in São Paulo, Brazil. This isolates was submitted fungal strains determination, DNA extraction and purification, determination of genotype by URA5 gene RFLP of CGB-positive isolates, protease and phospholipase activity and susceptibility to antifungals. Results: Of six CGB positive isolates tested by RFLP-PCR, only four presented a genomic profile consistent C. gattii species (VGII), while two other were C. neoformans (VNI and VNIII), indicating the existence of canavanine-resistante C. neoformans isolates in the culture collections. The clinical isolates secreted higher levels of phospholipase and environmental isolates but no differences were observed for the protease levels. Almost all isolates were sensible to azoles and amphotericin B. Conclusion: We point out in this research the existence of C. neoformans strains resistant to canavanine and intrinsic characteristic of C. gatti. These results demonstrate the importance to perform a detailed characterization of isolates kept in culture collections.

Abstract: in this study, prevalence estimates of five chronic disease cirrhosis, depression, diabetes, chronic rhenal insufficiency and tuberculosis based on the 1998 pnad health supplement were compared to those obtained by the brazilian global burden of disease project. these estimates were based on systematic literature review as well as on available data set of morbidities. the results show that pnad presented higher number of cases for depression and chronic rhenal insufficiency while the prevalence rates estimated by brazilian global burden of disease project were higher for cirrhosis, diabetes and tuberculosis.

Abstract: introduction: the work-related musculoskeletal disorders (wmsds) are articulated directly to occupational activities and working conditions. objective: the article developed from a qualitative study that analyzed the social representation of physical therapy among workers with work-related musculoskeletal disorders (wmsds), users of the reference center for occupational health (cerest - centro de referência em saúde do trabalhador), through the department of occupational health (dsat - departamento de saúde do trabalhador), juiz de fora (mg). materials and methods: the resource of capturing the information was the semi-structured interview, conducted with 12 workers aged between 29 and 55 years old. results: the information submitted to thematic analysis indicate that physical therapy received echoes over long periods of treatment, and dissatisfaction of the results, through a curative practice, targeted and little resolving, in which prevails the use of equipment at the expense of bodily and holistic maneuvers. conclusion: whereupon the report, we found the need for humanized care, whole and equal with accountability and commitment, in addition to the appreciation of workers and users, promoting better quality of life. it is essential carrying out further studies to enable a better understanding as well as theoretical and practical deepening of the relation between the role of the physiotherapist and the social representation involved.

The objective of this work is to provide an automatic system to count white blood cells in a blood smear. To do so an experiment was assembled, composed by a standard microscope with two step motors coupled to its knobs in order to move the microscope in x and y directions and a web cam which was mounted in the top of the microscope responsible for to acquire images from the smear. The step motors and the web cam are controlled by a microcomputer PC standard via software developed inDelphi. The motors use the parallel port to communicate with the PC and the camera use the USB port. The main idea is to set an initial point into the smear and the automated system will carry over the smear acquiring images (frames with 640 × 480 pixels) and counting the white blood cells encountered. The double histogram threshold technique is implemented to initially exclude the red cells from the image leaving only the white ones. Preliminaries results are obtained and show that the system is quite fast and has a good capacity of selection, even when different kinds of smear are used.

Dispersal of Aedes aegypti and Aedes albopictus (Diptera: Culicidae) in an urban endemic dengue area in the State of Rio de Janeiro, Brazil

Abstract: Experimental releases of female Aedes (Stegomyia) aegypti and Aedes (Stegomyia) albopictus were performed in August and September 1999, in an urban area of Nova Igua u, State of Rio de Janeiro, Brazil, to estimate their flight range in a circular area of 1,600 m where 1,472 ovitraps were set. Releases of 3,055 Ae. aegypti and 2,225 Ae. albopictus females, fed with rubidium (Rb)-marked blood and surgically prevented from subsequent blood-feeding, were separated by 11 days. Rb was detected in ovitrap-collected eggs by atomic emission spectrophotometry. Rb-marked eggs of both species were detected up to 800 m from the release point. Eggs of Ae. albopictus were more numerous and more heterogeneously distributed in the area than those of Ae. aegypti. Eggs positively marked for Rb were found at all borders of the study area, suggesting that egg laying also occurred beyond these limits. Results from this study suggest that females can fly at least 800 m in 6 days and, if infected, potentially spread virus rapidly.
Evolu o da rela o entre peso e altura e peso e idade em crian as de 3 meses a 6 anos assistidas em creche, Sorocaba (SP), Brasil

Abstract: As condi es nutricionais da popula o brasileira melhoraram nas últimas 3 décadas. Mesmo assim, é pouco conhecido o papel da assistência oferecida por institui es públicas ou filantrópicas que atendem crian as pré-escolares de baixa renda e de maior risco nutricional. Visando avaliar o impacto nutricional do programa municipal de assistência alimentar em creches do município de Sorocaba, Estado de S o Paulo, Brasil, realizou-se um estudo quase-experimental. Durante 1 ano, em quatro observa es trimestrais, acompanhou-se a evolu o no peso e altura de 444 crian as de 3 meses a 6 anos completos: 164 assistidas em uma creche (grupo de interven o) e 280 n o assistidas (grupo de n o interven o), residentes na área adstrita à creche porém n o freqüentes à mesma, e identificadas em censo específico. Depois de colhidos termos de assentimento e executado um pré-teste, profissionais e alunos universitários entrevistaram as m es das crian as participantes no estudo, segundo técnicas e condi es padronizadas. A antropometria foi realizada por dupla de investigadores, um deles sempre fixo na equipe, com equipamentos aferidos pela agência técnica regional. Avaliou-se a concordancia entre a primeira observa o e as observa es subseqüentes, com estratifica o da popula o em três grupos de escores z de peso para idade e peso por altura (tendo como referência as curvas do National Center for Health Statistics) verificados ao início do estudo e em três faixas etárias (menores de 24 meses, de 24 a 36 e maiores de 36 meses). Os grupos de interven o e n o interven o foram semelhantes quanto às condi es sócio-demográficas, exceto trabalho materno (as m es no grupo de interven o trabalhavam fora com maior freqüência); foram significativamente distintos quanto à idade (mais velhas no grupo de interven o), condi es nutricionais (piores no grupo de interven o) e prevalência de interna o hospitalar anterior (maior no grupo de interven o). Já nos primeiros 3 meses, 32% das crian as no grupo de interven o migraram para estrato de status nutricional superior, enquanto no grupo de n o interven o apenas 13% apresentaram melhora. A diferen a favorecendo o grupo de interven o aumentou com a dura o da interven o e foi maior entre crian as mais velhas, permitindo concluir que a assistência na creche melhorou a nutri o das crian as atendidas.
Perfis de saúde dos idosos no Brasil: análise da Pesquisa Nacional por Amostra de Domicílios de 2003 utilizando o método grade of membership

Abstract: the aim of this study was to identify functional disability and health profiles of the elderly in brazil, as well as their prevalence rates, based on the national household sample survey for 2003. the sample size was 33,786 elderly individuals. grade of membership was used to define the profiles: "healthy elderly" (profile 1), whose pure types had a lower probability of disability and chronic illness; "elderly with mild functional disability" (profile 2), whose pure types had mainly hypertension and lower back problems and were independent in activities of daily living, although with high difficulty in mobility; and "elderly with severe disability" (profile 3), with higher probability of chronic illness, high difficulty with activities of daily living, and high dependency in terms of mobility. in conclusion, the profiles indicate that a consistent approach to functional disability is essential for promoting the health of the elderly.

Abstract: this article aims to review the main concepts and measures of functional disability currently used in studies focusing on this subject. according to this review, functional disability can be defined as a difficulty or the need for help for performing basic or more complex daily activities necessary for keeping an independent life in the community. as refers to measurement criteria, the studies do not follow a clear-cut standard, varying substantially in relation to scales, dimensions and classifications besides depending directly or indirectly on the kind of available information. a correct method for rating functional disability does not exist at yet. for this reason it is essential that the researchers clearly indicate the concept of functional disability used in the studies and detail, which measures or tools were applied.
Fatores associados à incapacidade funcional dos idosos no Brasil: análise multinível

Abstract: objective: to analyze the influence of demographic, socioeconomic, health, and contextual factors of the brazilian federation units on the functional disability of the elderly. methods: cross-sectional study based on data from the 2003 pnad (brazilian national household survey), of ibge (brazilian institute of geography and statistics) and ipea (institute of applied economic research). the sample was composed of 33,515 individuals aged 60 years and older. the dependent variable was functional disability, measured by difficulty in climbing slopes or stairs. the independent variables were divided into two levels: individual (demographic, socioeconomic and health-related characteristics) and contextual (gini index and gross domestic product per capita by brazilian state in 2000). a multinomial and multilevel logistic regression model was utilized in order to estimate the effect of the independent variables on the functional disability of the elderly. results: functional disability was associated with demographic, socioeconomic and health factors. at the individual level, sex, level of schooling, income, occupation, self-perception of health and chronic diseases were the factors that were most strongly related to functional disability. at the contextual level, income inequality proved to exert an important influence. conclusions: self-perception of health is the factor that is most strongly related to the functional disability of the elderly in brazil, followed by chronic diseases. sex, occupation, level of schooling and income are also highly associated with it. actions that approach the main factors associated with functional disability can contribute significantly to the well-being and quality of life of the elderly.
