Paciente obstétrica de alto riesgo: dónde debe realizarse su cuidado periparto? Where should peripartum care take place?

Abstract: Este artículo presenta los patrones que determinaron el ingreso de las gestantes críticamente enfermas a la Unidad de Cuidados Intensivos (UCI) o de Alta Dependencia Obstétrica (UAD), y las causas de complicaciones y muerte más frecuentes, al igual que los factores que afectan el pronóstico perinatal. Se plantea la implementación de las Unidades de Alta Dependencia Obstétrica para el manejo de las gestantes críticamente enfermas, y el entrenamiento del personal involucrado en el manejo obstétrico de estas pacientes. This article outlines the patterns determining critically ill pregnant women’s admittance to an intensive care or high-dependency unit, the most frequently occurring causes of complications and death and the factors affecting perinatal prognosis. High-dependency units would then seem to have become necessary for treating critically ill pregnant women. Suitable training of the personnel involved in these patients’ obstetric care has also become a priority.

Abstract: Introducción: la calidad de vida relacionada con la salud (CVRS) permite evaluar el estado de salud de las personas y dise ar, implementar y evaluar programas de salud. Objetivo: determinar la percepción de la CVRS en usuarios de un programa de actividad física (AF). Metodología: estudio transversal en 177 sujetos pertenecientes a un programa de actividad física. Se aplicó el cuestionario SF-36v1.2 para evaluar la CVRS; se evaluó el consumo máximo de oxígeno (VO2máx) por medio la prueba de 2.000 metros y se determinó la prevalencia de algunos factores de riesgo cardiovascular a partir de las historias clínicas. Resultados: el puntaje promedio de CVRS más alto fue en el funcionamiento físico (FF) 90,5 (DE: 10,8) y más bajo en dolor corporal (DC) 78,1 (DE: 22,4) y vitalidad (VT) 78,1 (DE: 15,5). En las personas fumadoras, hipertensas, diabéticas, obesas y con niveles bajos de actividad fisica la mayoría de los puntajes de CVRS fueron más bajos (diferencias clínica y estadísticamente significativas). El VO2máx mostró correlación con los dominios funcionamiento físico (FF) (rho = 0,371; p = 0,0001), desempe o físico (DF) (rho = 0,177; p = 0,018) y dolor corporal (DC) (rho = 0,207; p = 0,006). Conclusión: las personas estudiadas tienen una percepción buena de la CVRS; sin embargo, en quienes reportaron tabaquismo, hipertensión arterial, diabetes mellitus, obesidad, dislipidemias y baja potencia aeróbica se presentó una menor percepción de la CVRS; las personas con obesidad mostraron mejores puntajes en los dominios del componente mental y aquellos con bajo nivel de AF mostraron puntajes bajos en dicho componente.

Abstract: introduction: health related quality of life (hrql) allows to assess people health status, and to design, implement and evaluate health programs. objective: to determine the perception of hrql in users of a physical activity (pa) program. methodology: cross-sectional study in 177 subjects belonging to a physical activity program. sf-36v1.2 questionnaire was used to assess hrql; maximal oxygen consumption (vo2max) was evaluated by the 2.000 meters test and prevalence of some cardiovascular risk factors was determined from medical records. results: the higher average score for hrql was in physical function (pf) 90,5 (sd: 10.8), compared with the lowest scores of body pain (bp) 78,1 (sd: 22,4) and vitality (vt) 78,1 (sd: 15,5). in smokers, hypertensive, diabetics, obese and people with low levels of physical activity most hrql scores were lower (clinically and statistically significant differences). vo2max correlated with pf subscales (rho = 0.371; p = 0.0001), physical performance (rho = 0.177; p = 0.018) and bp (rho = 0.207; p = 0.006). conclusion: the studied individuals have a good perception of hrql; nevertheless, those who reported smoking, hypertension, diabetes, obesity, dyslipidemia and low aerobic power, had a reduced perception of hrql; obese people showed improved scores in mental component domains and those with low levels of pa showed low scores in that component.

Abstract: objective: determining the hematocrite (hct) and hemoglobin (hgb) values that could approach the reference values of sportsman assessed at the "laboratorio clínico del instituto departamental de deportes de antioquia, (indeportes)", to obtain comparison parameters for a more precise interpretation of these measurements. design: cross sectional prospective study. participants: the simple measurements of 432 apparently healthy sportsmen (234 men and 198 women), ages between 15 and 49 years that met all the inclusion criteria, in whom 656 hct and hgb were measured during 2003, 2004 and 2005. methodology: hct and hgb analysis were performed taking into account gender, age groups in each gender (15-17 years, 18-24 years and from 25 years up), and taking also into account the sports group in each gender (1-group of competitive art de art, 2-group, combat, 3-group, strength, 4-group ball games and 5 group, endurance). the reference intervals were at 95% for hct and hgb in each one of the 5 sports groups in both genders, using the bootstrap methodology. results: the differences in the hct and hgb values in both genders found have already been reported in other studies. no significant statistical differences in hct. were found among the 5 sports groups in both genders, or for hgb. in males, there were only significant borderline differences in hmgb (p=0.054) in the combat vs. the endurance group in females. there were no statistically significant differences in the three age groups for hct or for hgb in both genders. conclusions: the reference intervals for hct and hgb in the 5 sports groups studied in each gender will help optimizing the methodology used for the interpretations of these measurements in the control and follow up of this sportsmen population. the reference intervals obtained for hct and hgb in male sportsman, indicate that those that do not use any doping substance may exceed the critical limits established for these blood tests by some international and nati

Abstract: un examen panorámico de la obra literaria de juan rulfo (1917-1986) sugiere la presencia apenas esporádica de alusiones a lo indígena, hecho que se agrava por el confeso escepticismo del autor frente a preocupaciones antropológicas. sin embargo, una revisión detenida de la misma deja ver que, de un modo latente, el escritor jalisciense plasmó una auténtica cosmovisión indígena, e incluso a través de procedimientos propios del indigenismo latinoamericano representado por autores como mauricio magdaleno y jorge icaza.

Tratamiento endovascular de la insuficiencia de los ejes safenos mediante laser diodo 980 NM

Abstract: background: great saphenous vein reflux is the main cause of superficial vein insufficiency. endoluminal ablation of this vein is a minimally invasive treatment for varices. aim: to evalúate the results of great saphenous vein endoablation using diode 980 nm laser. material and methods: a prospective study in 146 lower extremities of 90 patients aged 25 to 91 years (69 females), subjected to great saphenous vein endoablation with diode 980 nm laser, between 2005 and 2007. immediate permeability of the treated saphenous vein was evaluated with dúplex ultrasound. complications related to the procedure were also recorded. results: great saphenous vein occlusion after endoablation with diode 980 nm laser was 99% at the end of first week and 97% at 3 months. recorded complications were induration in relation to the treated great saphenous vein in 5.4%, erythema in 5.4%, pain in 5.4% and ecchymosis in 9.4%. all these complications resolved. all patients reported and improvement of symptoms and were satisfied with the procedure. conclusions: this endovascular method for the treatment of great saphenous vein reflux has a low incidence of complications, fast recovery and return to normal activities
