BAFF: Una citoquina reguladora de linfocitos B implicada en autoinmunidad y cáncer linfoide BAFF: A regulatory cytokine of B lymphocytes involved in autoimmunity and lymphoid cancer

Abstract: BAFF (B cell activating factor belonging to the TNF family) is a cytokine implicated in the survival and maturation of peripheral B lymphocytes and T and B cell activation. BAFF binds to three different receptors: TACI, BCMA and BAFF-R, whose expression is restricted to B and T lymphocytes. BAFF and BAFF-R-deficient mice show a dramatic loss of peripheral B lymphocytes and a severely reduced immune response. In contrast, an enhanced BAFF expression leads to B cell hyperplasia and autoimmunity in mice. In vivo, administration of soluble decoy receptors for BAFF effectively decreases disease progression in various autoimmune mouse models. These evidences render BAFF as a potentially new therapeutic target. Elevated BAFF levels have been detected in the serum of patients with autoimmune diseases, such as Systemic Lupus Erythematosus, rheumatoid arthitis, Sj gren's syndrome, lymphoid cancers and HIV infection. In addition to BAFF receptors, malignant B cells abnormally express BAFF, which attenuates apoptosis through both autocrine and paracrine pathways. The data suggest that an increase in the expression of BAFF induces an enhanced B and T cell activation and the survival of pathologically active B cells. In this article, we review and discuss the participation of BAFF and its receptors in the immune response and its involvement in immunodeficiency, autoimmunity, infections and lymphoid cancers as well as the currently investigated therapies using BAFF antagonists in the treatment of these diseases

Abstract:


Metabolic Syndrome (MS) has reached a pandemic proportion and has an impact on the incidence and severity of cardiovascular pathologies. This study was conducted to evaluate the effect of different processed food commonly consumed in Maroua (Far North Region, Cameroon) on the Glycemic Index (GI). To ameliorate the follow up of MS and population’s nutrition education, a survey was conducted among 200 families. The ten most consumed meals were selected after a food survey among 200 families. The meals were mostly made of a vegetable soup associated to a high carbohydrate staple. Vegetables and cereals were processed according to the food survey results. Then, a food tolerance test was performed on 30 male volunteers living in the same region. Among them, 12 were selected with an average age of 28.25 and a mean BMI of 19.63. They were studied on separate occasions in the morning after an overnight fast. After fasting, blood glucose was taken before and (15, 30, 45, 60, 90 and 120 minutes) after eating a test meal compared to sucrose and glucose solution (33.33%) used as reference. Glycemic response curves were used to calculate the gi of each meal. Results show that GI of the tested meals varied significantly (p < 0.05) between 20 and 79. Red millet porridge with kelenkelen-beans (Cochorus oletorius-Vigna unguiculata) sauce and red millet porridge with kelenkelen-peanut (Cochorus oletorius-Arachis hypogea) sauce exhibited the lowest GI. The highest GI was observed in corn porridge associated with fresh okra (Hibuscus esculentus) or with tasba (Cassia tora)-okra-beans sauce.

Abstract: invasive fungal infections are an increasingly common problem in patients with cancer and other vulnerable groups. we report a case of hepatosplenic hialohifomycosis probably by aspergillus sp. in a patient with acute leukemia, and prolonged neutropenia, treated with corticosteroids and broad spectrum antibiotics. a review concerning diagnosis, clinical course and treatment of this condition is presented. Multi-criteria assessment of the Representative Elementary Watershed approach on the Donga catchment (Benin) using a downward approach of model complexity

Abstract: This study is part of the AMMA - African Multidisciplinary Monsoon Analysis- project and aims at a better understanding and modelling of the Donga catchment (580 km2, Benin) behaviour in order to determine its spatially distributed water balance. For this purpose, we applied the REW concept proposed by Reggiani et al. (1998, 1999), which allows the description of the main local processes at the sub-watershed scale. Such distributed hydrological models, which represent hydrological processes at various scales, should be evaluated not only on the discharge at the outlet but also on each of the represented processes and in several points of the catchment. This multi-criteria approach is required in order to assess the global behaviour of hydrological models. We applied such multi-criteria strategy to the Donga catchment (586 km2), in Benin. The work was supported by an observation set up, undertaken since 1998 consisting in a network of 20 rain gauges, an automatic meteorological station, 6 discharge stations and 18 wells. The main goal of this study was to assess the model's ability to reproduce the discharge at the outlet, the water table dynamics in several points of the catchment and the vadose zone dynamics at the sub-catchment scale. We tested two spatial discretisations of increasing resolution. To test the internal structure of the model, we looked at its ability to represent also the discharge at intermediate stations. After adjustment of soil parameters, the model is shown to accurately represent discharge down to a drainage area of 100 km2, whereas poorer simulation is achieved on smaller catchments. We introduced the spatial variability of rainfall by distributing the daily rainfall over the REW and obtained a very low sensitivity of the model response to this variability. Simulation of groundwater levels was poor and our results, in conjunction with new data available at the local scale, suggest that the representation of the processes in the unsaturated zone should first be improved, in order to better simulate soil water dynamics and represent perched water tables which were not included in this first modelling study. Abstract: in this article we present a synthesis of paulo freire’s proposal regarding curricular theory, pointing out the structural and functional aspects of said theory. the proposal is theoretically based on the idea that both the educator and the educated have the right and responsibility to contribute to the curricular content. there is a need for the creation of a program based on dialogue with a humanistic vision and a scientific character. the principle that guides freire’s proposal is to work on different processes such as conscientization, developing curiosity and critical thought in students within the school. this would mean supporting the development of the students’ ability to decide on their own what, when and how they should learn; depending on their reality, experiences, needs and specific interests, assuming their own convictions without having to wait for others to interpret the facts from a passive position. to discover this mediating reality, freire proposes the search for significant themes in the present situation with the knowledge and interpretation of problems and their relationship with others, with dialogue as the principal component of this thematic research. starting from cooperative research undertaken by local communities (of teachers, and other participants in the educational process) a critical, participative and redimensioned curriculum theory is created.
Estenosis aórtica asintomática y muerte súbita

Abstract: dyspnea, angor and syncope are the most characteristic symptoms in stenosis aortic valve disease. sudden death, as part of natural history of symptomatic stenosis aortic valve, is well know. on the other hand, sudden death in asymptomatic stenosis aortic valve is rarer. different guidelines recommend a conservative management of these patients. we present here the case of a 58 year old woman, previously healthy, who arrived at hospital because of sudden dyspnea at rest. the patient was diagnosed of pulmonary edema and died two hours later. necropsy showed a stenosis aortica valve with a valve area of less than 0.8 cm2. we make a short review in medical literature about the incidence of sudden death in asymptomatic stenosis aortic valve, the risk groups and their management.

Abstract: Se realizó un estudio con el objetivo de investigar si la sensibilización a estos ácaros ( Dermatophagoides pteronyssinus [ Dp ], Dermatophagoides siboney [ Ds ] y Blomia tropicalis [ Bt ] ) constituye un problema de salud en nuestra población infantil, pues el estudio de los problemas alérgicos ha identificado a los ácaros domésticos como los principales agentes causales de asma y otras afecciones. Se seleccionaron 32 ni os alérgicos de nuestro consultorio que se clasificaron según su grado de exposición al polvo doméstico. A todos se les realizó la prueba por punción cutánea en el brazo derecho, y se utilizaron 3 extractos alergénicos estandarizados Valergen ò ( Blomia tropicalis, Dermatophagoides siboney y Dermatophagoides pteronyssinus ) de producción nacional (BIOCEN). La prevalencia de la sensibilización fue mayor de forma significativa (p<0,05) para todos los ácaros en los pacientes con mayor exposición al polvo doméstico con respecto a los de menor exposición. Para los ácaros del género Dermatophagoides se obtuvo una sensibilización mayor. El extracto de Dp provocó un tama o medio del habón mayor que los otros productos. A study was conducted aimed at investigating if the sensitization to these dust mites (Dermatophagoides pteronyssinus [Dp], Dermatophagoides siboney [Ds] and Blomia tropicalis [Bt] ) is a health problem in our infantile population, since the study of the allergic problems has identified the house dust mites as the main agents causing asthma and other affections. 32 allergic children of our office were selected and classified according to their degree of exposure to house dust. All of them underwent the skin test in the right arm. 3 standardized allergenic extracts Valergenò (Blomia tropicalis, Dermatophagoides siboney y Dermatophagoides pteronyssinus) of national production (BIOCEN) were used. The prevalence of sensitization was significantly higher (p<0.05) for all the dust mites in patients with a greater exposure to house dust compared with those with lower exposure to it. A higher sensitization was attained for the dust mites of the genus Dermatophagoides. The extract of Dp produced a wheal with a mean size greater than the one caused by other products.

Abstract: La morbilidad de las enfermedades alérgicas mediadas por el anticuerpo IgE, se ha incrementado en los últimos 40 a os, y además, se han identificado a los ácaros domésticos como sus principales agentes causales. En Cuba, los ácaros más frecuentemente son: Dermatophagoides pteronyssinus (D. pteronyssinus), Dermatophagoides siboney (D. siboney) y Blomia tropicalis (B. tropicalis). En este estudio investigamos si la sensibilización a estos ácaros constituye un problema de salud en nuestra población infantil, y su relación con la exposición al polvo doméstico. Se seleccionaron 103 ni os alérgicos del pueblo de Guatao, que se clasificaron según su grado de exposición al polvo doméstico. A todos se les realizó la prueba por punción cutánea por duplicado en ambos antebrazos y se utilizaron los extractos alergénicos estandarizados VALERGEN-BT ò (Blomia tropicalis), VALERGEN-DS ò (Dermatophagoides siboney) y VALERGEN-DP ò (Dermatophagoides pteronyssinus), todos de producción nacional en el Centro Nacional de Biopreparados. La sensibilización fue mayor de forma significativa (p<0,01) para todos los ácaros en los pacientes con mayor exposición al polvo doméstico, con respecto a los de menor exposición. Además, registramos una sensibilización mayor a los ácaros del género Dermatophagoides, especialmente para el D. pteronyssinus. El extracto D. pteronyssinus provocó un tama o medio del habón mayor que los otros productos en los pacientes positivos de ambos grupos (p<0,04), por lo que concluimos que la mayor exposición de los pacientes al polvo doméstico está relacionada con una mayor sensibilización a los ácaros. Morbidity of allergic diseases mediated by the IgE antibody has increased in the last 40 years and the house mites have been identified as their main causal agents. In Cuba, the most common mites are Dermatophagoides pteronyssinus (D. pteronyssinus), Dermatophagoides siboney (D. siboney) and Blomia tropicalis (B. tropicalis). In this study, it was investigated if the sensitization to these mites is a health problem in our infantile population and its connection with the exposure to house dust. 103 allergic children from the Guatao town were selected. All of them underwent the skin puncture test in both forearms and the VALERGEN-BT ò (Blomia tropicalis), VALERGEN-DS ò (Dermatophagoides siboney) and VALERGEN-DP ò (Dermatophagoides pteronyssinus) standardized allergenic extracts, which were produced in our country by the National Centre of Biopreparations, were used.
