Is There Any Correlation between Insulin Resistance and Nitrate Plasma Concentration in White Coat Hypertensive Patients?

Abstract: We evaluated a relationship between nitric oxide plasma correlation and insulin resistance in white coat hypertensive patients. Patients were screened for white coat hypertension using an ambulatory blood pressure monitoring. The homeostasis model assessment insulin resistance index (HOMA-IR) was used to assess insulin resistance, and plasma nitrate concentrations were determined by chemiluminescence. The HOMA-IR was significantly higher in hypertensive (3.84 ± 0.62) when compared to white coat (2.11 ± 0.36) and normotensive (1.40 ± 0.21). We found no correlation between HOMA-IR and plasma nitrate levels in all three groups. We suggest that the more important issue for these patients is to focus on the metabolic abnormalities where lifestyle interventions such as weight control and exercise have proven effective.

Abstract: the objective of this systematic review was to retrieve topics of relevance to the implications of leptin in modulating blood pressure and in the genesis, prevention and treatment of hypertension associated with obesity. considering the hormone as a common link between the two pathological conditions, were identified actions to reduce the leptinemia, proposed and discussed in the literature. it was found that nursing lacks production of knowledge to subsidize the assistance to hypertensive obesity that present alterations in serum leptin. were described signs and multifactorials symptoms dynamically interrelated and paradoxical in the manifestation of the clinical case studied, with effective management interventions in caring for those clients.

Abstract: objective: assess the prevalence of white coat hypertension in the municipality of dumont, in s?o paulo state, and map study participants' demographics, and physiologic and metabolic changes. methods: one hundred and nine (109) users of the municipal health service were selected and then divided into three groups: normotension, essential hypertension, and white coat hypertension after blood pressure was measured through oscilometry and blood pressure monitoring exam at outpatient unit. variables found between the groups were compared based on interview, data measurements, and laboratory exams. anova and tukey tests were used for statistical data. results were expressed as means ± standard error of means. p<0.05 was considered statistically significant. results: the prevalence of white coat hypertension was 34.1%. females were predominant: mean age 45.3 years, increased body mass index, waist/hip ratio, plasma glucose level and creatinine when compared to hypertensives and/or normotensives. no correlation was found between white coat hypertension and demographic variables. conclusion: the differences found between the groups and clinical and biochemical variations lead to the conclusion that white coat hypertension is a condition that should be investigated in normotensive and hypertensive individuals distinctively.

Abstract: the integration between basic science and research in the field of nursing is an emerging subject and expanding in many countries. however, in brazil, researchers have shown little interest in exploring this line of research, in spite of the numerous benefits it may bring to nursing care practice. in this article, by means of a bibliographic review, we will focus on the importance of basic research and take into consideration our own experiences and expectations as nursing researchers, emphasizing the possibility of integrating fundamental and experimental science with nursing research and practice.

Abstract: Estudio exploratorio y descriptivo con objeto de determinar qué factores están relacionados con la estandarización de los catéteres urinarios, en una ciudad del interior del estado de S o Paulo, Brasil. El estudio fue realizado a través de entrevistas, mediante aprobación del Comité de ética en Investigación de la Escuela de Enfermería de Ribeir o Preto. De las nueve instituciones hospitalarias entrevistadas, 100% posee un procedimiento de cateterización vesical estandarizado. Las instituciones presentan diferencias en el procedimiento propuesto, lo que interfiere de manera positiva y negativa en su estandarización y pone en duda si el estandarizado es el que está siendo realizado. Se concluye que existe la necesidad de reevaluar la estandarización propuesta, con discusiones interdisciplinarias, verificar si el propuesto está siendo realizado y buscar estrategias de entrenamiento y ense anza pasibles de aplicación.

Abstract: Este estudo teve como objetivo identificar e comparar a prevalência da síndrome metabólica entre hipertensos e normotensos. Os aspectos adotados para a defini o de síndrome metabólica foram os definidos pelo National Cholesterol Education Program/Adults Treatment Painel (NCEP-ATP III). Os dados referentes aos perfis clínicos e bioquímicos foram processados no programa SPSS para cálculo de frequências absolutas e porcentagens. Utilizou-se o teste t de Student para compara es das médias, sendo as diferen as consideradas estatisticamente significantes para p<0,05. A amostra foi composta por 93 participantes normotensos e 168 hipertensos. Identificou-se a presen a de síndrome metabólica em 60,7% dos hipertensos e 18,3% dos normotensos. Os portadores de hipertens o arterial apresentam diferen a significante nos valores de press o arterial (p<0,001), circunferência abdominal (p<0,001), glicemia (p<0,05) e triglicérides plasmáticos (p<0,05). A frequência dos riscos cardiometabólicos associados à síndrome metabólica é maior na presen a de doen a hipertensiva. Los aspectos adoptados para la definición de síndrome metabólico fueron los definidos por el National Cholesterol Education Progam/Adults Treatment Painel (NCEP-ATP III). Datos referentes a perfiles clínicos y bioquímicos fueron procesados con software SPSS para cálculo de frecuencias absolutas y porcentajes. Se utilizó la prueba T de Student para comparaciones de los promedios, considerándose las diferencias como estadísticamente significativas para p<0,05. Muestra compuesta por 93 participantes normotensos y 168 hipertensos. Se identificó presencia de síndrome metabólico en 60,7% de los hipertensos y en 18,3% de los normotensos. Los hipertensos presentaron diferencia significativa para valores de presión arterial (p<0,001), circunferencia abdominal (p<0,001), glucemia (p<0,05) y triglicéridos plasmáticos (p<0,05). La frecuencia de riesgos cardio-metabólicos asociados al síndrome metabólico se acentúa en presencia de enfermedad hipertensiva. Abstract: objective: to identify blood pressure values and estimate the frequency of risk factors for hypertension among middle school students. methods: this was a descriptive, cross sectional study, conducted in schools in the southeastern region of brazil. the sample consisted of 184 adolescents enrolled in the second semester of middle school, in 2009. in addition to the measurement of clinical variables, instruments were applied to identify risk factors associated with hypertension. results: an alteration in blood pressure was a parameter detected in 22.3% of the sample. among the risk factors investigated, family history of cardiovascular disease and alcohol consumption were the most prevalent. conclusion: there is need to enhance the measures of primary prevention and early detection of arterial hypertension among adolescents, with special attention to the assessment of family history and adoption of risk habits.

Abstract: among the challenges faced by brazilian nursing, the situation of indefiniteness about the nursing role in the hospital context demands a vigorous determination by nursing leaders. hospital institutions aim at specific objectives, without satisfactorily considering the needs, values, potential and diversities of different professionals. objective: carry out a comparative data analysis and extract points of congruence, delimitations and possibilities that allow us to point out directions with a view to more efficient actions. the methodology covered a secondary analysis of five classical studies about nurses' functions, developed in brazilian literature throughout four decades. this analysis resulted in the discussion and proposal of flexible and humanizing presuppositions to guide the role of nurses in hospital care practice.
