Escala de Locus de controle da dor: adapta??o e confiabilidade para idosos

Abstract: contextualiza??o: a dor é considerada uma experiência multidimensional e muito prevalente em idosos. o locus de controle da dor tem se tornado fundamental para entender como percep??es, expectativas e cren?as individuais se relacionam a comportamentos, atitudes, enfrentamento e aderência dos idosos frente às condi??es de saúde e propostas de tratamento. estudos focados na adapta??o e confiabilidade de instrumentos s?o necessários para os profissionais de saúde. objetivos: realizar a adapta??o transcultural, para o brasil, do instrumento pain locus of control scale (forma c da multidimensional health locus of control ) e avaliar sua confiabilidade intra e interexaminadores em uma amostra de 68 idosos comunitários, com dor cr?nica n?o oncológica. métodos: a adapta??o transcultural da escala foi feita conforme metodologia padronizada por beaton et al. (2000)*. para análise estatística, foram usados os coeficientes de correla??o de pearson (ccp) e de correla??o intraclasse (cci) (p<0.05). resultados: a média de idade dos idosos foi de 69,6±5,5 anos, predominando mulheres, de baixa renda e escolaridade. o tempo médio de evolu??o da dor foi de 10,2 anos, e o principal diagnóstico clínico foi a osteoartrite. a confiabilidade da escala mostrou-se adequada com correla??o de regular a muito forte (ccp=0,60 a 0,93) e de moderada a quase perfeita (cci =0,60 a 0,93), principalmente nas subescalas de controle ao acaso e de profissionais médicos e de saúde. conclus?o: após as adapta??es e as adequa??es da escala para aplica??o em idosos, verificou-se sua aplicabilidade e confiabilidade adequadas na amostra estudada.
Factorial analysis of the Multidimensional Health Locus of control scale: form C for elderly

Abstract: background: pain is a multidimensional experience. locus of control is an important factor related to chronic pain experience and treatment. there is a gap in the literature when discussing issues related to pain evaluation in elderly. objectives: to analyze the factorial structure, intra and inter-rater reliability of the brazilian version of the pain locus of control scale - form c for elderly with chronic pain living in the community. methods: one hundred and eighty one elderly individuals (71.5±6.8 years of age) answered a clinic and social-demographic questionnaire and the ploc-c scale. a factorial analysis with varimax rotation of the ploc-c scale was performed. the scalewas applied twice by two observers to evaluate the intra and inter-rater reliability analyzed using pearson's correlation coefficients. results: the factorial analysis of the 18 item ploc-c scale revealed six factors. four items (1 and 6 from the chance locus of control subscale; 2 and 4 from the internal locus of control subscale) migrated toward unpredictable factors in the original factorial structure. analysis with the removal of the four items demonstrated a better factorial structure and higher levels of internal consistency (α=0.836 and 0.669) and reliability (intra-examiner: r=0.65 and 0.93; inter-examiner: r=0.82 and 0.92) when compared to the complete subscales. the variance explained was of 48.7% for the 18 items and 62.4% for the 14 items scale. conclusion: the results demonstrate a better applicability of the reduced scale on the sample. the reduced version may contribute to greater knowledge and consequently better chronic pain management in the elderly.

Abstract: BACKGROUND: Pain is a multidimensional experience. Locus of control is an important factor related to chronic pain experience and treatment. There is a gap in the literature when discussing issues related to pain evaluation in elderly. OBJECTIVES: To analyze the factorial structure, intra and inter-rater reliability of the Brazilian version of the Pain Locus of Control Scale - Form C for elderly with chronic pain living in the community. METHODS: One hundred and eighty one elderly individuals (71.5±6.8 years of age) answered a clinic and social-demographic questionnaire and the PLOC-C scale. A factorial analysis with varimax rotation of the PLOC-C scale was performed. Abstract: background: frailty syndrome in elderly people is characterized by a reduction of energy reserves and also by a decreased of resistance to stressors, resulting in an increase of vulnerability. objective: the aim of this study was to verify the effect of a muscle-strengthening program with load in pre-frail elder women with regards to the functional capacity, knee extensor muscle strength and their correlation. methods: thrity-two pre-frail community-dwelling women participated in this study. potential participants with cognitive impairment (meem), lower extremities orthopedic surgery, fractures, inability to walk unaided, neurological diseases, acute inflammatory disease, tumor growth, regular physical activity and current use of immunomodulators were excluded. all partcipants were evaluated by a blinded assessor using: timed up and go (tug), 10-meter walk test (10mwt) and knee extensor muscle strength (byodex system 3 pro? isokinetic dynamometer at angular speeds of 60 and 1800/s). the intervention consisted of strengthening exercises of the lower extremities at 70% of 1rm, three times/ week for ten weeks. the statistical analysis was performed using the anova and spearman tests results: after the intervention, it was observed statistical significance on the work at 1800/s (f=12.71, p=0.02), on the power at 1800/s (f=15.40, p=0.02) and on the functional capacity (tug, f=9.54, p=0.01; tc10, f=3.80, p=0.01). there was a good negative and statistically significant correlation between the tug and work at 600/s, such as the tug and work at 1800/s (r=-0.65, p=0.01; r=-0.72, p=0.01). conclusion: the intervention improved the muscular power and the functional capacity. the increase of the power correlated with function, which is an important variable of the quality of life in the pre-frail elders. article registered in the isrct register under number isrctn62824599.
Adapta o transcultural e análise da confiabilidade do Southampton Assessment of Mobility para avaliar a mobilidade de idosos brasileiros com demência

Abstract: O objetivo deste trabalho foi adaptar transculturalmente o instrumento Southampton Assessment of Mobility e testar sua confiabilidade intra e interexaminadores para idosos brasileiros da comunidade, com demência, classificados quanto à gravidade pelo Clinical Dementia Rating. O instrumento adaptado foi aplicado em uma amostra de 107 idosos (76,26 anos ? 7,59; 27,1% homens, 72,9% mulheres) com diagnóstico clínico de demência dado pelo servi o de geriatria do Centro de Referência em Aten o ao Idoso Professor Caio Benjamin Dias, do Estado de Minas Gerais, Hospital das Clínicas, Universidade Federal de Minas Gerais, Minas Gerais, Brasil. Dentre os avaliados, 39 (76,85 anos ? 7,75; 23,1% homens, 76,9% mulheres) foram aleatorizados para avalia o da confiabilidade. A ferramenta estatística foi o teste kappa. Os resultados mostraram que a confiabilidade intra e interexaminadores foram, respectivamente: demência leve 0,89-0,86; moderada 0,79-0,85 e grave 0,53-0,49. O instrumento adaptado demonstrou ser aplicável à popula o alvo e demonstrou ter confiabilidade "quase perfeita" para demência leve e moderada. Para a demência grave os índices de confiabilidade foram "moderados".

Abstract: objective: to carry out a systematic review of the literature on physical therapy interventions and their effect on frail community-dwelling elders. methods: systematic review of studies published until june 2008 in the databases medline, embase, pedro, scielo, lilacs and cochrane library. we excluded studies with samples composed of institutionalized, hospitalized and non-frail participants, studies not aimed at treating frailty, and studies that were not specifically related to physical therapy. results: in accordance with the exclusion criteria, out of the 152 medline articles, only 15 were considered for analysis, out of the 71 pedro articles only one was considered as the other ten had already been selected in medline, and out of the 461 embase articles only two that had not been selected in others databases were included in this study. a total of seven different types of interventions were verified: 1) muscle strengthening; 2) exercises for muscle strengthening, balance, coordination, flexibility, reaction time and aerobic training; 3) functional training; 4) physical therapy; 5) at-home physical therapy; 6) environment adaptation and prescription of assistive device; 7) water exercise. the results of some studies were contradictory even with similar interventions. the analyzed studies had different definitions for fragility, which made it difficult to compare the results. conclusion: there is little evidence of the effect of physical therapy intervention on frail community-dwelling elders; thus, it is not possible to reach a consensus or conclusion on the effectiveness of the therapeutic regimens proposed for this complex syndrome.
Fatores clínicos, funcionais e inflamatórios associados à fadiga muscular e à fadiga autopercebida em idosas da comunidade

Abstract: background: fatigue is a common and nonspecific symptom associated with chronic health problems in the elderly. the modifications and adaptations of the aging process associated with complex and multidimensional nature of fatigue favors the interaction of multiple factors in the genesis of this phenomenon. objectives: to investigate the association of clinical, functional and inflammatory factors with muscle fatigue and self perceived fatigue in elderly women. methods: participated in the study one hundred and thirty five community elderly women, all sedentary, with a mean age of 71.2±4.57. a structured questionnaire and functional testing were used to evaluate clinical and functional characteristics. plasma concentrations of inflammatory mediators (il-6 and stnfr1) were measured by elisa method. muscle fatigue was measured by isokinetic dynamometer and self-perceived fatigue was measured by a visual analog scale. statistical analysis was performed by multiple linear regression and spearman correlation coefficient with statistical significance of 5%. results: the regression models showed that the variables age, body mass index, physical activity level, functional capacity and peak torque were associated with muscle fatigue (r2=0.216, p<0.01). self-perceived fatigue was associated with number of comorbidities, depression, physical activity level, functional capacity, peak torque and perceived health (r2=0.227, p<0.01). conclusion: this study showed a psychophysical interaction of the fatigue, by identifying the main factors associated with muscle fatigue and self-perceived fatigue in elderly women. these findings demonstrate the importance of evaluation and treatment of modifiable factors in both muscle fatigue and self-perceived fatigue, seeking a better physical and functional performance of elders.
The effects of a muscle resistance program on the functional capacity, knee extensor muscle strength and plasma levels of IL-6 and TNF-α in pre-frail elderly women: a randomized crossover clinical trial - a study protocol

Abstract: The study design is a randomized crossover clinical trial evaluating 26 elderly women (regardless of their race and/or social condition) who are community residents, older than 65, and classified as pre-frail according to the criteria previously described by Fried et al. (2004). All subjects will be assessed using the Timed up and go and 10-Meter Walk Test functional tests. The plasma levels of IL-6 and TNF-α will be assessed by ELISA (enzyme-linked immunosorbent assay) with high sensitivity kits (Quantikine?HS, R&D Systems Minneapolis, MN, U.S.). Knee extensor muscle strength will be assessed using the Byodex System 3 Pro? isokinetic dynamometer at angular speeds of 60 and 180°/s. The intervention will consist of strengthening exercises of the lower extremities at 50 to 70% of 1RM (maximal resistance) three times per week for ten weeks. The volunteers will be randomized into two groups: group E, the intervention group, and group C, the control group that did not initiate any new activities during the initial study period (ten weeks). After the initial period, group C will begin the intervention and group E will maintain everyday activities without exercising. Abstract: background: the decrease in muscle strength in older adults has an impact on functionality. muscle strengthening programs have been proposed, however there is some controversy surrounding the effects of stretching prior to strengthening exercises on muscle strength gain. objective: to verify the impact of static stretching on the gain in knee-extensor strength of community-dwelling elderly women after a training program. methods: this was a quasi-experimental study that included a ten-week knee-extensor strengthening program and a final assessment of 12 community-dwelling elderly women divided into two groups: group se - strengthening exercises with prior static stretching in all sessions, mean age 73.8 (±5.36) years; and group e - same strengthening exercises as se without prior stretching, mean age 72.14 (±5.43) years. to measure knee-extensor strength gains, we used the isokinetic dynamometer biodex system 3 pro, in concentric mode at angular velocities of 60o and 180o/s, and for statistical analysis we used the difference between pre- and post-intervention means of work adjusted by body weight. results: the data showed normality in the shapiro-wilk test (p>0.266). when comparing the differences between the pre- and post-intervention means using the t test for independent samples, there was no significant difference in any of the limbs at the velocities evaluated (p>0.383). conclusion: previous static stretching did not interfere in muscle strength gain following a ten-week muscle strengthening program in the population studied. article registered in the isrctn register under number isrctn62824599.

Abstract: the objective was to perform a cross-cultural adaptation of the southampton assessment of mobility and test its intra- and inter-examiner reliability for brazilian elderly living in the community and diagnosed with dementia, with severity classified according to the clinical dementia rating. the instrument was applied to 107 elderly (76.26 years ± 7.59; 27.1% males, 72.9% females) diagnosed with dementia by the geriatric clinic at the university hospital of the federal university in minas gerais. from the initial group, a randomized sample of 39 elderly (76.85 years ± 7.75; 23.1% males, 76.9% females) was selected for the reliability tests. the statistical tool was the kappa test. the respective reliability indices were: mild dementia - 0.89-0.86; moderate - 0.79-0.85; and severe - 0.53-0.49. after cross-cultural adaptation and reliability tests, the instrument proved adequate for the target population, with "near-perfect" reliability for mild and moderate dementia. for severe dementia, the reliability was moderate.
