Abstract: the growing incidence of congenital syphilis in rio de janeiro city led the city health department to organize the congenital syphilis elimination campaigns in 1999 and 2000, in order to call attention to the matter and to qualify health care personnel in the management of the disease. the aim of this paper was to evaluate the effectiveness of the campaigns, following the pregnant women identified with syphilis through the well-established health information systems (sinasc - live births, sim - mortality and sinan - national disease surveillance data system). it became evident that a significant reduction on perinatal morbidity and mortality was achieved on the group of children born to mothers exposed to the campaigns, pointing out that the antenatal routine practice wasn't able to deal with the matter. the low number of congenital syphilis cases found on the surveillance system denotes a huge under-reporting of the disease.

Abstract: available nutritional information in brazil, generated by non-periodic population surveys, has scarcely contributed to the development of policies aimed at improving the nutritional status of the brazilian population, although it has helped establish the magnitude and distribution of this problem. we suggest that data available in the various health information systems be used to feed, monitor, and evaluate nutritional policies, for updating and local follow-up purposes. deficiencies in the quality of data available in these health information systems can be minimized through validation studies, thus improving the information generated by the system as a whole and consequently the quality of health care.

Abstract: As informa es nutricionais existentes no País, oriundas de inquéritos populacionais sem periodicidade definida, embora contribuindo na defini o da magnitude e distribui o do problema, têm se mostrado de pouca utilidade para subsidiar a formula o de políticas que redundem na melhoria do estado nutricional da popula o brasileira. Sugere-se considerar os dados disponíveis nos diversos sistemas rotineiros de informa o da área de saúde para alimentar, monitorar e avaliar as políticas nutricionais, possibilitando atualidade e acompanhamento local. As deficiências na qualidade dos dados nesses sistemas da área da saúde podem ser minimizadas através de estudos de valida o, gerando uma melhoria das informa es do sistema como um todo, com reflexos na qualidade da assistência à saúde.

Abstract: O aumento da incidência da sífilis congênita no Município do Rio de Janeiro levou a Secretaria Municipal de Saúde a realizar as campanhas para a sua elimina o nos anos de 1999 e 2000, com o objetivo de dar visibilidade à doen a e capacitar os profissionais de saúde no manejo do agravo. Neste estudo realizou-se uma avalia o da efetividade das campanhas, servindo-se do seguimento das mulheres grávidas identificadas com sífilis durante as campanhas, pelos sistemas de informa o em saúde (SINASC, SIM e SINAN) utilizando-se da compara o deste grupo a um grupo controle de gestantes n o envolvidas na campanha. Foi possível evidenciar uma redu o significativa da morbidade e mortalidade perinatais nos filhos das mulheres atendidas pelas campanhas, demonstrando que as a es da rotina de pré-natal nas unidades municipais n o estavam conseguindo resolver o problema. Foi baixo o número de casos de sífilis congênita encontrado no SINAN em rela o aos casos reconhecidos por intermédio das campanhas, demonstrando uma subnotifica o importante.

Abstract: background: postpartum weight retention is a trigger for developing obesity in women. it is often associated with weight gain during pregnancy, parity, age, marital status, food intake, physical activity and lactation. however, the associations found have proven to be inconclusive or contradictory. objectives: the purpose of this review was to study risk factors associated with postpartum weight gain and retention. methods: a systematic review of the literature between january 1993 and june 2003 was performed through medline and lilacs databases. results: twenty-eight studies were found: 23 cohort studies, four cross-sectional studies and one case-control study. most studies were performed in the us and the postpartum follow-up time was mainly between six weeks and 24 months. twenty five percent of the studies had loss to follow-up above 30%, 61% included adolescents and 75% used self-reported pregravid body weights. the most reliable results came from seven studies, whose strengths were: use of measured pregravid body weight and comparison groups, exclusion of adolescents, loss to follow-up rates below 30% and adequate control for confounders. conclusion: gestational weight gain, black women and parity are positively associated with postpartum weight retention. the effect of lactation on weight retention is still controversial and there is lack of evidence on physical activity and food intake. improved longitudinal studies are needed to clarify theses issues.
Alojamento conjunto e parto cesáreo em maternidades do Estado do Rio de Janeiro, Brasil

Abstract: objective: to disclose the existence of rooming-in (ri) in public and government contracted private hospitals that offer obstetric beds, in the state of rio de janeiro, brazil, and to examine whether there is any association between ri and another quality care indicator which influences breastfeeding, namely the rate of cesarean section operations performed in these hospitals. method: a survey was made of the existence of ri through a questionnaire sent to the municipal health offices, the information collected being confirmed by telephone with each maternity hospital. the c-section rate data was obtained from the rio de janeiro state health office and divided into 2 groups: "below 40%" and "40% and above". the prevalence ratio was applied to the measurement of the association between the variables. results: a rooming-in rate of 65.2% was found for the state as a whole, with regional variations: a better situation in the capital (84.8%), an intermediate one in the interior (69.9 %), and a worse one in the metropolitan belt (44.2%). the public maternity hospitals revealed a higher rate (89.7%) than that of the government contracted private hospitals (53.3%). a direct relation between the practice of ri and low c-section rates was found in the hospitals. however, this association did not present the same weight in all regions of the state. the lowest proportion of hospitals adopting ri was verified in the metropolitan belt, whereas the highest relative number of hospitals with high c-section rates was observed in the interior. conclusion: it is concluded that to reverse the observed status, government authorities must fulfil their gerencial role within their own health system, as well as in the government contracted private hospitals.
Alojamento conjunto e parto cesáreo em maternidades do Estado do Rio de Janeiro, Brasil

Abstract: OBJETIVO: Conhecer a existência de alojamento conjunto (AC) nos hospitais que possuem leitos obstétricos da rede pública e conveniada do Estado do Rio de Janeiro e verificar se o AC guarda associa o com outro indicador de qualidade de assistência que tem influência sobre o aleitamento materno: as taxas de cesárea (TC) praticadas por esses hospitais. METODOLOGIA: Procedeu-se a um levantamento sobre a existência de AC através de questionário enviado às Secretarias Municipais de Saúde, informa o validada por inquérito telef nico. As TC foram obtidas junto à Secretaria Estadual de Saúde-RJ. Foi utilizada a raz o de prevalência para a medida da associa o entre as variáveis. RESULTADOS: Foi encontrada uma propor o de 65,2% das maternidades com AC, no Estado. Verificou-se rela o direta entre a presen a de AC e baixas taxas de cesárea no conjunto dos hospitais, porém essa associa o n o se confirmou com o mesmo peso em todas as regi es. A menor propor o de hospitais com AC foi verificada no cintur o metropolitano (44,2%), enquanto a maior propor o de hospitais com elevadas taxas de cesárea está no interior (73,1%).
Evolu??o da mortalidade neonatal no Estado do Rio de Janeiro, Brasil (1979-1993): análise por causa segundo grupo de idade e regi?o de residência

Abstract: this paper reports the results of a study on neonatal mortality in the state of rio de janeiro (1979-1993), according to major causes of death, by age and region of residence. the authors suggest criteria for grouping neonatal deaths according to causes, taking into consideration frequency and preventability. for deaths which occurred in the first day of life, immaturity and fetal growth retardation were the only causes of death that showed significant decline. an increase in deaths due to respiratory affections of the fetus and newborns was observed. causes of death said to be reducible by "adequate control of pregnancy and attention at delivery" and "early diagnosis and treatment" remained unaltered. the authors noted that a decrease in death rates took place starting with the 1-6 day age group. the comparison with other countries called attention to the high rates of neonatal death in rio de janeiro. it also pointed out an increase in the proportion of deaths classified as "respiratory problems of the newborn", demonstrating lack of specificity in defining the basic cause of death.
Hospitaliza??es por diarréia infecciosa no Estado do Rio de Janeiro

Abstract: diarrhea is an important cause of hospitalization among infants. there are many complex factors that influence hospital use: socioeconomic and cultural characteristics, access, medical needs, and supply. the objective was to measure hospitalization rates from diarrhea among infants in rio de janeiro in 1996 and the association with demographic, geographical, and clinical data comparing differentials between public/university and private/philanthropic hospital care under the unified national health system (sus). the authors used data from the hospital information system. private/philanthropic hospitals admitted approximately four times more children than public/university hospitals. analysis shows that variation in age, length of hospital stay, and use of pediatric intensive care may reflect differences in physicians' practice styles. this may in turn influence the respective health care unit's capacity to prevent death associated with diarrhea. the authors conclude that it is necessary to continue the analysis of hospital utilization under the sus due to implications for the cost and quality of pediatric care.
