Abstract: Das "Zeytregister" des Ulmer Bauern und Landhandwerkers Hans Heberle ist in seiner Art eine einzigartige Hinterlassenschaft eines b uerlichern Schriftstellers des 17. Jahrhunderts. Der Gesichtskreis Heberles geht weit über den engen Horizont der Wirtschaftsführung hinaus, indem er pers nliche Erfahrungen mit der Schilderung der Kriegsereignisse seiner Zeit verbindet. Unter Berücksichtigung von Schreibintention und -praxis relativieren sich indes die M glichkeiten einer pers nlichkeitsnahen Aussch pfung der Quelle im Sinne eines 'Ego-Dokuments'. So sind den im wesentlichen auf Geburt und Tod beschr nkten Notaten über die eigene Familie, über pers nliche Erlebnisse und Beobachtungen aus n herer Anschauung durch die absorbierende Kriegsschilderung Grenzen auferlegt. Die selektive Auswahl der Themenpunkte und das weitestgehende Zurücktreten affektiver Momente erkl rt sich auch durch die technische Verfahrensweise Heberles, der nach Ausweis inhaltlicher und formaler Aspekte der Handschrift in deutlichem zeitlichem Ereignisabstand an seiner Chronik arbeitete.

Abstract: Die Mobilit t nimmt in modernen Gesellschaften einen zentralen Stellenwert ein und wird als wichtig für den Erhalt von Unabh ngigkeit angesehen. Ein Beispiel für die vielf ltigen Alltagsrisiken stellt der Stra enverkehr dar. Die epidemiologische Datenlage in Bezug auf Unfallrisiken von psychiatrischen Patienten unter Psychopharmaka ist jedoch dünn und durch eine Reihe von methodischen Problemen gekennzeichnet. Einige Untersuchungen weisen auf ein erh htes Risiko von psychiatrischen Patienten hin, in einen Verkehrsunfall verwickelt zu werden. Eine pauschale Bewertung von Arzneimitteln, die die Fahrtüchtigkeit beeintr chtigen k nnen ist nicht m glich. Unter den potentiell verkehrsbeeintr chtigenden Psychopharmaka stehen Tranquilizer und Hypnotika an erster Stelle. Im Gegensatz zu vielen trizyklischen Antidepressiva führen neuere selektive Antidepressiva zu keinen signifikanten Beeintr chtigungen psychomotorischer und kognitiver Funktionen. Neuere atypische Antipsychotika scheinen hinsichtlich Vigilanz und Psychomotorik Vorteile gegenüber konventionellen Neuroleptika zu haben. Die Beratung und Aufkl rung von Patienten zu Fragen der Fahrtauglichkeit muss individuell, unter Berücksichtigung des Krankheitsbildes, Pharmaka-differenzieller Aspekte sowie beruflicher und sozialer Wiedereingliederungsbemühungen erfolgen.
Emergence of new oral antithrombotics: a critical appraisal of their clinical potential

Current anticoagulant therapies available for the prevention and treatment of VTE have several drawbacks that make them either difficult to manage effectively, due to a need for careful monitoring to control coagulation, or, in the case of parenterally administered agents, inconvenient for long-term use. To address some of these issues, new anticoagulants are in clinical development that can be orally administered and directly target specific factors in the coagulation cascade. This article reviews the rationale behind development of these novel agents and provides a critical appraisal of their clinical potential. In addition, the impact that the introduction of such agents into clinical practice would have is discussed from the patient perspective.Keywords: antithrombotic agents, venous thromboembolism, Factor Xa, thrombin
Osteoarthritis and functional disability: results of a cross sectional study among primary care patients in Germany

Abstract: 1250 questionnaires were distributed to OA outpatients from 75 general practices; 1021 (81.6%) were returned. Questionnaires included sociodemographic data, the short form of the Arthritis Impact Measurement Scale (AIMS2-SF), and the Patient Health Questionnaire (PHQ-9) to assess concomitant depression. A hierarchical stepwise multiple regression analysis with the AIMS2-SF dimension "lower body" as dependent was performed.Main factors associated with functional disability were depression symptoms, as reflected in a high score of the PHQ-9 (beta = 0.446; p < 0.0009), pain as reflected in the AIMS2-SF symptom scale (beta = 0.412; p = 0.001), and few social contacts (beta = 0.201; p < 0.042). A high body mass index was associated with lower functional ability (beta = 0.332; p = 0.005) whereas a higher educational level (beta = -0.279; p = 0.029) predicted less impairment. Increased age was a weak predictor (beta = 0.178; p = 0.001) of disability. With a p of 0.062 the radiological severity according to the grading of Kellgren and Lawrence slightly surpassed the required significance level for remaining in the final regression model.The results emphasize that psychological as well as physical factors need to be addressed similarly to improve functional ability of patients suffering from OA. More research with multifaceted and tailored interventions is needed to determine how these factors can be targeted appropriately.Disability has a substantial impact on the quality of life of patients and affects daily living [1]. Osteoarthritis: quality of life, comorbidities, medication and health service utilization assessed in a large sample of primary care patients

Abstract: Cross sectional study with 1250 OA patients attending 75 primary care practices from March to May 2005. QoL was assessed using the GERMAN-AIMS2-SF. Data about comorbidities, prescriptions, health service utilization, and physical activity were obtained by questioning patients or from the patients' medical files. Depression was assessed by means of the Patient Health Questionnaire (PHQ-9).1021 (81.7%) questionnaires were returned. 347 (34%) patients were male. Impact of OA on QoL was different between gender: women achieved significantly higher scores in the AIMS 2-SF dimensions lower body (p < 0.01), symptom (p < 0.01), affect (p < 0.01) and work (p < 0.05). Main predictors of pain and disability were a high score in the "upper body "scale of the AIMS2-SF (beta = 0.280; p < 0.001), a high score in the PHQ-9 (beta = 0.214; p < 0.001), duration of OA (beta = 0.097; p = 0.004), age (beta = 0.090; p = 0.023) and the BMI (beta = 0.069; p = 0.034). Predictors of pain and disability did not differ between gender. 18.8 % of men and 19.7% of women had a concomitant depression. However, no gender differences occurred. Women visited their GP (mean 5.61 contacts in 6 months) more often than men (mean 4.08; p < 0.01); visits to orthopedics did not differ between gender.The extent to which OA impacts men and women differs in primary care patients. This might have resulted in the revealed differences in the pharmacological treatment and the HSU. Further research is needed to confirm our findings and to assess causality.Osteoarthritis is one of the most prevalent chronic diseases worldwide and is associated with substantial impact on patients' individual quality of life as well as on healthcare costs. Abstract: RESUMO Este artigo aborda as principais bases do empreendedorismo, as características preponderantes do empreendedor e a rela o desses últimos com as institui es de ensino superior (IES). Advogamos aqui o empreendedorismo, como uma vantagem competitiva quando bem desenvolvido por meio de políticas e incentivos de amplo espectro e de fontes diversas. Discutimos aqui um rol de políticas e diferentes formas de incentivo ao empreendedorismo em IES. Esta análise nos permite concluir que o desenvolvimento e expans o das IES, hoje, dependem mais de uma atitude empreendedora, ou um movimento empreendedor de dentro para fora, lastreado em parcerias devidamente planejadas, na excelência ou qualidade de seus produtos/servi os e na vis o do negócio educacional como sustenta o da educa o, do que na simples racionaliza o de seus custos operacionais. Auswirkungen eines kontrollierten Ausdauertrainings auf die kognitive Leistungsf higkeit schizophrener Patienten

Abstract: Ziel: Untersucht wurden die Auswirkungen eines aeroben Ausdauertrainings auf die kognitive Leistungsf higkeit bei schizophrenen Patienten. Methodik: 31 Patienten wurden randomisiert einer Ausdauertrainings- bzw. Ergotherapiegruppe zugewiesen. An zwei Testzeitpunkten wurden Parameter der kognitiven Leistungsf higkeit erhoben. Ergebnisse: In beiden Gruppen ergaben sich, mit Ausnahme der Aufmerksamkeit, für die erhobenen kognitiven Bereiche signifikante Verbesserungen über die Zeit. Bezüglich verbaler mnestischer Leistungen zeigten sich spezifische Interventionseffekte bei den Patienten der Ausdauertrainingsgruppe.Schlussfolgerung: Schizophrene Patienten k nnen im Hinblick auf verbale Ged chtnisleistungen von einem kontrollierten Ausdauertraining profitieren.

Inhibition of Protein Tyrosine Kinase Activity and Induction of Apoptosis in Epithelial Cells by Oxindole Alkaloids of Uncaria tomentosa (Willd.) D. C

Abstract: Root bark of Uncaria tomentosa (Willd.) D. C. is used for a variety of therapeutic purposes, including reported efficacy as both an immunostimulant and anticancer therapy. The oxindole alkaloids (OAs) of U. tomentosa were investigated for cytotoxicity toward epithelial cells, specifically focusing on lines derived from breast (MB-MDA-231, MB-MDA-468, BT-20 and SKBR-3) and skin (A431) carcinomas, as well as a non-cancer line (Cos). Alkaloid extracts were shown to be minimally cytotoxic at concentrations as low as 27 M, with an IC50 in the range of 0.9 to 1.7 mM, depending on the cell line tested. Microscopic evaluation of cells treated with marginally cytotoxic levels of OAs, and stained with fluorescent indicators, propidium iodide and Hoechst 33342, specifically demonstrate the induction of apoptosis, rather than necrosis. Evaluation of protein tyrosine kinase (PTK) activity in treated cells further demonstrated that minimally cytotoxic concentrations of the OAs inhibited tyrosine phosphorylation of endogenous proteins in MB-MDA-231 cells, when compared to controls. Given the recognized role of protein tyrosine kinases in the coordination of cell proliferation, in this research, propose that inhibition of PTK activity by the OAs contributes to the apoptotic cell-death, and perhaps other biological activities associated with these compounds.
