Multimodality treatment of unresectable hepatic metastases from pancreatic glucagonoma

Rare Tumors , 2009, DOI: 10.4081/rt.2009.e6
Abstract: Glucagonomas are pancreatic islet cell tumors arising from the alpha cells which belong to neuroendocrine tumors. They frequently metastasize to the liver. We report the case of a 52- year old man with a pancreatic glucagonoma with synchronous multiple liver metastases treated by surgery, transarterial chemoembolization, percutaneous radiofrequency thermal ablation and long-acting octreotide. Our report confirms that a multimodal approach is very effective in patients with unresectable liver metastases from pancreatic endocrine tumors providing long-lasting palliation and probably prolonging survival.

Abstract: The currently recommended treatment for patients infected with hepatitis C virus (HCV) is pegilated interferon α (IFN α) plus ribavirin. Despite the numerous benefits of this therapy, there is an increasing concern regarding his tolerance. Among the most common side effects, interferon may trigger the onset or exacerbation of autoimmune diseases. When chronic hepatitis C coexists with an autoimmune disorder, it is not clear whether using interferon is better than avoiding it. We evaluated the disease state of a 55-year old female affected by sistemic sclerosis (SSc), during and after therapy with IFNα pegilated plus ribavirin for chronic HCV infection. We were worried about the potential worsening of the autoimmune disease during the therapy, but we were confident that we would give our patient a short course of peginterferon and ribavirin. A mild, asymptomatic worsening of lung SSc was observed during IFN administration, without life threatening symptoms. After 24 months follow up we observed the maintenance of the virological response and a good control of the rheumatological disease. Thus, in liver disease at high risk of progression and concomitant SSc, the antiviral therapy with IFNα is a feasible approach.

Abstract: Here, a case of Hodgkin's disease (HD), as unusual presentation of PTLD after autologous HSCT for malignant glioma is described.60-years old man affected by cerebral anaplastic astrocytoma underwent subtotal neurosurgical excision and subsequent high-dose chemotherapy followed by autologous HSCT. During the post HSCT course, cranial irradiation and corticosteroids were administered as completion of therapeutic program. At day +105 after HSCT, the patient developed HD, nodular sclerosis type, with polymorphic HD-like skin infiltration. The clinical and pathological findings were consistent with the diagnosis of PTLD.Post-transplant lymphoproliferative disorders (PTLDs) are severe complications that occur in solid organ and allogeneic hematopoietic cell transplantation (HSCT), and their incidence ranges from 1% to 20% [1].Most PTLDs are classified as B-cell (rarely T-cell) PTLD, often related to Epstein-Barr virus (EBV) infection. Sometimes, after allogeneic HSCT, non Hodgkin lymphoma (NHL) or Hodgkin's disease (HD) have been described as PTLDs [2].Herein, we report the first case of HD, as unusual presentation of PTLD, in a patient undergoing high dose sequential chemotherapy and autologous HSCT for malignant glioma.A 60-year-old male was diagnosed with anaplastic astrocytoma involving the left parietal and temporal lobes. The patient underwent surgical resection with subsequent magnetic resonance imaging (MRI)-proven complete excision. In the context of a clinical trial, exploring the role of high-dose chemo-radiotherapy in malignant glioma, the patient underwent high dose sequential chemotherapy consisting of cyclophosphamide 7 g/m2 plus rhG-CSF (5 μg/kbw) and hematopoietic cells harvest, followed by methotrexate 8 g/m2 and thiotepa 900 mg/m2. Autologous unfractionated hematopoietic stem cells were administered at doses of 7.5 × 106/kg CD34+ cells and the patient experienced an uncomplicated hematological recovery within 10 days. One month later, the patient received 7
Filosofia da ciência e ensino da ciência: uma analogia

Abstract: in this work, initially we will make explicit our understanding of the debate among kuhn, popper, lakatos and feyerabend in the international conference on philosophy of science (1965), by outlining the differences in the positions of these philosophers. afterwards, we will present a chart of positions about the teaching of science, the way they seem for us to characterize the last 30 years of research in this field. finally, we will try to put the two fields closer together, while attempting to highlight the implications of the philosophical problematic for a better understanding of the science and mathematics education.
SCYTHAE: un problema linguistico, etnografico e culturale dell età moderna
Francesco Villani
Studi Linguistici e Filologici Online , 2003,
Abstract: In the history of linguistics the theory according to which the European Languages have an eastern origin (the Scythian Theory) has played an important role in developing the Indo-european Theory. I would like to demonstrate that the 15th and 16th century are the crucial point for the linguistic aspect of this theory: some authors, called the polyglotti , like Gessner, Bibliander and Megiser are the starting point from which stems the epistemological paradigm of the modern Indo-european historical linguistics. Other languages and their mythical origin in the East, like Uralic and Semitic languages, are to be seen as the starting point for the Scythian Theory. A special concern of the author is the give a glimpse of the contest in which such theory has been created.
