Abstract: abstract . introduction: in cases of clinically suspected scaphoid fracture, without radiographic evidence, immobilization and symptomatic treatment is recommended. the use of mri would define how many of this cases actually present scaphoid fracture, modifying treatment. materials and methods: in 15 patients with clinically suspected scaphoid fracture and negative radiograph, mri exams were done 9 days or earlier after trauma. the study included coronal turbo-spin-echo (tse) t1 weighted images and coronal short tau inversion recovery (stir) sequences. results: mri showed occult scaphoid fractures in 5 patients (33%), with the combination of t1 and stir sequences. patients with negative radiograph and mri studies reintegrated to normal activities in a period shorter than 2 weeks after trauma. as a consequence there was a reduction in economic costs of at least 50% considering only radiologic studies, and decreased number of medical consultation. discussion: mri early after trauma show a high sensitivity detecting scaphoid fractures radiographically occult, allowing early diagnosis and treatment, avoiding unnecessary immobilization and reducing costs in those patients without fracture

Abstract: Resumen. Introducción: La sospecha clínica de fractura de escafoides sin confirmación radiográfica, habitualmente lleva a inmovilización y tratamiento sintomático. La resonancia magnética (RM) precoz permite definir cuales casos presentan realmente fractura, modificando la conducta terapéutica. Material y métodos: En 15 pacientes con sospecha clínica de fractura de escafoides y radiografía normal, se realizó RM dentro de un plazo no mayor a 9 días después del trauma. Se utilizaron secuencias T1 turbo-spin-echo (TSE) coronal y short-tau-inversion recovery (STIR) coronal. Resultados: La RM demostró fracturas ocultas de escafoides en 5 pacientes (33%), todas ellas fueron detectadas con una combinación de secuencias STIR y T1. Los pacientes con radiografía y RM negativos para fractura se reintegraron a sus actividades habituales en un plazo menor a dos semanas. Esto se tradujo en reducción de al menos 50% de costos en exámenes radiológicos, además de disminuir el número de consultas de control y ausentismo laboral. Conclusión: La RM precoz tiene alta sensibilidad para la detección de fracturas de escafoides radiográficamente ocultas y permite un diagnóstico definitivo y tratamiento oportuno, evitando inmovilización innecesaria en pacientes sin fractura, llevando a una significativa reducción de costos Abstract . Abstract: Las normas de bioseguridad implican un costo adicional que debe manejar el cirujano dentista. En una economía de libre mercado, en la cual el precio es el principal factor de comparación para elegir al prestador, la incorporación de un nuevo ítem deja en desventaja al prestador responsable, en beneficio del que no considera estas normas. La imposibilidad de ejercer un control riguroso sobre todos los prestadores demuestra que la única posibilidad de lograr una aplicación uniforme de normas de bioseguridad es la existencia de valores individuales en los odontólogos. En este punto la bioética cobra vital importancia para apoyar la atención odontológica As normas de biosseguran a implicam num custo adicional ao cirurg o dentista no seu exercicio profissional. Numa economia de mercado, em que o pre o da presta o de servi o é o principal fator de compara o para a escolha do prestador, a incorpora o de um novo item deixa em desvantagem ao prestador responsável, em benefício daquele que n o leva em conta estas normas em seus procedimentos. A impossibilidade de exercer um controle rigoroso com todos os prestadores faz com que a única possibilidade de lograr uma aplica o uniforme de normas de biosseguran a efetivas por parte de todos eles, seja a existencia de valores individuais nos odontólogos. Output Feedback Nonlinear -Tracking Control of a Nonminimum-Phase 2-DOF Underactuated Mechanical System

Abstract: Nonlinear ∞ synthesis is developed to solve the tracking control problem restricted to a two degrees-of-freedom (DOF) underactuated mechanical manipulator where position measurements are the only available information for feedback. A local ∞ controller is derived by means of a certain perturbation of the differential Riccati equations, appearing in solving the ∞ control problem for the linearized system. Stabilizability and detectability properties of the control system are thus ensured by the existence of the proper solutions of the unperturbed differential Riccati equations, and hence the proposed synthesis procedure obviates an extra verification work of these properties. Due to the nature of the approach, the resulting controller additionally yields the desired robustness properties against unknown but bounded external disturbances. The desired trajectory is centered at the upright position where the manipulator becomes a nonminimum-phase system. Simulation results made for a double pendulum show the effectiveness of the proposed controller.

Abstract: the author reviews the most common drugs used for the treatment of postmenopausal osteoporosis to determine their efficacy in increasing bone mass and decreasing fracture rates. estrogens, selective estrogen receptor modulators (serms), bisphosfonates, calcitonin plus calcium and vitamin d were analyzed, and in conclusion the author suggests a practical algorithm for the treatment of early and late postmenopausal osteoporosis based on bone density and biochemical markers results.
