Achados oculares e fundoscópicos em pacientes com lupus eritematoso sistêmico

Abstract: purpose: to describe the changes in ocular examination of patients with systemic lupus erythematosus (sle), especially those related to fundoscopy. method: a descriptive study of 41 lupus patients selected at random from may to july 1999 at the collagen disease outpatient clinic of the rheumatology service of the university hospital, federal university of paraná. a protocol of clinical and ophthalmologic evaluation was applied. results: forty-one patients were included in the study; 40 were females (97.56%) and one was a male (2.44%). mean age was 32.22 years (range: 16 to 54 years). thirty-seven patients (87.80%) were white, two (4.89%) were black, and three (7.32%) were mulattoes. time since sle diagnosis ranged from two months to 18 years. nineteen of the 41 patients (46.34%) presented fundoscopic alterations related to sle. the major lesions detected were cotton-wool spots and arteriolar narrowing (68.42%) followed by increased excavation (10.52%) and optic disk pallor (10.52%), perivascular damage (5.26%), and changes in the pigment epithelium of the retina (5.26%). conclusion: there was a high prevalence of fundoscopic changes related to sle, showing the importance of regular fundoscopic examinations even in asymptomatic lupus patients or in lupus patients with no associated diseases.
Influência da peritonite sobre a síntese de colágeno em anastomoses do cólon distal: estudo experimental em ratos

Abstract: Com o objetivo de verificar a rela o entre a concentra o de colágeno e a capacidade de suportar press o em anastomoses de cólon quando existe peritonite, utilizaram-se 40 ratos machos, Wistar-TECPAR, com idade média de 120 dias e peso médio de 302 g, os quais foram divididos em 2 lotes de 20 animais que compuseram os grupos controle e peritonite. Sob anestesia inalatória e submetidos à laparotomia os ratos do grupo peritonite tiveram infec o induzida pelo método de Wichterman e col. e os do grupo controle laparotomia com manipula o dos segmentos intestinais envolvidos no experimento. Após 24 horas eram relaparotomizados, sofriam colotomia transversa total esquerda à 2,0 cm da reflex o peritoneal e anastomose em plano único, extra-mucoso. As aferi es foram realizadas nop tereciro e sétimo dias de pós-operatório. Ressecava-se 4,0 cm do cólon esquerdo contendo a anastomose e media-se a press o de ruptura. Realizou-se estudo histopatológico, empregando-se a colora o do Sirius-red e análise da concentra o de colágeno através de método computadorizado. Verificou-se que a capacidade de suportar press o aumentou com o tempo de evolu o de forma semelhante nos dois grupos. A concentra o de colágeno foi menor no grupo com peritonite no terceiro dia (p = 0,000168) e no sétimo dia (p = 0,0020). Os percentuais de colágeno I e III foram semelhantes no terceiro dia. Já no sétimo dia, no grupo peritonite predominou o colágeno tipo III ( p = 0,000079). Os resultados analisados demonstram que existe diminui o da concentra o e da matura o do colágeno nas anastomoses colo-cólicas realizadas quando existe peritonite porém esta altera o n o interfere na capacidade da anastomose suportar press o.

Abstract: the purpose of the present study was to evaluate the burst strength and the collagen concentration in a left colonic anastomosis when peritonitis was present. forty wistar male rats were used, with a mean age of 120 days and a mean weight of 302 g. the animals were allocated in two groups of 20 animals each: a control group and an experimental group. under ether anesthesia, the rats of the experimental group were submitted to a laparotomy and the abdominal sepsis was induced according to the wichterman method, while those in the control group were submitted to laparotomy and manipulation of the intestinal segments involved in the experiment. twenty four hours later, the animals underwent to a new laparotomy and a left colonic anastomosis, with a single layer of polypropilene interrupted suture, 2,0 cm above the peritoneal reflection. on the 3rd and 7th post-operative day, the colonic segment with the anastomosis was resected in order to evaluate the bursting strength . the red sirius dye was used to evaluate collagen concentration in the histopathological study. results showed in both groups an increase in the bursting strength , relating to post-operative day. collagen concentration was poorer in the peritonitis group on the 3rd and 7th post-operative days (p = 0.000168) (p = 0.0020) respectively. the percentages of collagen i and iii were similar on the 3rd post-operative day. on the 7th day, however, collagen type iii (p =0.000079) predominated in the peritonitis group. the results above showed that the concentration and maturation of collagen are reduced in a colonic anastomosis performed when peritonitis is present. these alterations does not interfere in the bursting strength.
O pulm o na doen a inflamatória do cólon: estudo experimental em ratos

Abstract: Embora a colite ulcerativa seja conhecida desde 1875 e muitas sejam as manifesta es extra-intestinais descritas nesta doen a, só recentemente chamou-se a aten o para o envolvimento do aparelho respiratório. Severas complica es têm sido descritas em pacientes como: estenose traqueal inflamatória, bronquiolite com pneumonia, pneumonite intersticial, granulomatose de Wegener, bronquite cr nica com bronquiectasia, nódulos necrobióticos, vasculites, fibrose e alveolites. O presente estudo visa reconhecer as altera es pulmonares, na fase aguda da doen a inflamatória do cólon, induzida em ratos com ácido acético à 10% e comparar com controles normais. Foi possível constatar que 100% dos animais com colite apresentaram rea o inflamatória pulmonar (p=0,0210) de intensidade moderada à severa (p=0,0340). Vasculite foi vista em 58,33% dos pulm es (p=0,0060) e em 3 animais detectou-se hemorragia focal, necrose e abscesso. Estes achados permitem atribuir uma forte associa o entre a doen a inflamatória do cólon e altera es do aparêlho respiratório, durante a fase aguda da doen a, em ratos.

Abstract: although ulcerative colitis has been known since 1875 and many extraintestinal manifesattions of this disease have been described, it was only that emphasis was placed on the involvement of the respiratory tract. severe complications have been described in patients, such as inflammatory tracheal stenosis, bronchiolitis with pnemonia, interstitial pneumonitis, wegener granulomatosis, chronic bronchitis with bronchiectasis, necrobiotic nodules, vasculitis, fibrosis and alveolitis. the objective of the present study was to determine the pulmonary changes occuring during the acute phase of the disease in rats submitted to induction of the disease with 10% acetic acid and to compared to normal controls. it was observed that 100% of the animals with colitis developed a pulmonary inflammatory reaction (p=0,0210) of moderate to severe intensity (p=0,0340). vasculitis was observed in 58,33% of the lungs (p=0,0060) and focal hemorrhage, necrosis and abscesses were detected in 3 animals. these findings permit us to conclude about the presence of a strong correlation between inflammatory colon disease and changes in the respiratory apparatus during the acute phase of the disease in rats.
Características dos idosos vítimas de violência doméstica no Distrito Federal

Abstract: objective: to describe the profile of the aged victims of domestic violence in the federal district, brazil. method: this is a descriptive exploratory study, with historical series of collected data of the millenium system of the civil police of the df, from 2003 to 2007. the sample comprised 4,896 registers. the analysis contemplated sex, marital status, aggressor and type of aggression. results: we noticed an increase of the number of denunciations from 2003 on, most of them for males. the married ones are the most attacked. in relation to the aggressors, 62.81% do not have kinship with the victim and 13.56% of the aggressions were committed by son. the results showed the vulnerable situation of this group and the importance of the public services directed toward the protection of the aged one. conclusion: stimulating health professionals to make the notification/individual inquiry of domestic violence and/or others becomes essential.

Abstract: objectives: to describe the epidemiological and clinical profile of female children and adolescents establishing victims'biological and socio-demographic characteristics, types of violence and intimidation, inflicted wounds and aggressor's profile. methods: a cross sectional study based on data from records of 170 patients assisted at the women support service of agamenon magalh?es hospital, recife, pernambuco, from june 2001 to december, 2002. results: the median age was 16 years old. the majority of the victims were mulattoes (65.3%) and single (95.9%). one hundred and thirteen subjects did not finish elementary school. for 31,2% family income was under one minimum wage. sexual aggression represented 64.7% with the genital region being the most injured in 54.4% of the cases. bruises and haematomas were present in 23,8% of the cases. seventeen were pregnant at the time of aggression. perpetrators were unknown (31.7%) and used physical force (79.8%) as a mean of intimidation. age, income and schooling were not informed and 34.2% had signs of alcohol consumption at the time of aggression. conclusions: the subjects of this study were poor, mulattoes, illiterate adolescent girls. the victims of sexual violence suffered genital lesions and did not know their aggressors, who used physical force for intimidation.
Uso do infliximab na consulta de gastrenterologia pediátrica

Abstract: introduction: infliximab is a monoclonal antibody used in refractory inflammatory bowel disease (ibd). aim: to evaluate the clinical response and adverse effects of infliximab therapy in paediatric ibd patients. methods: analytic, descriptive and cross sectional study of outpatient clinic of pediatric gastroenterology treated with infliximab. results: of six patients treated with infliximab, five have moderate to severe crohn’s disease and one present ulcerative colitis. there was clinical remission in five patients. resolution in analytical changes was observed after 6 months of therapy. there were two mild allergic reactions and a transient elevation of transaminases. conclusions: in accordance with the literature, in our population, infliximab therapy has been well tolerated and effective in disease control.

Abstract: probiotics are live microorganisms which, when administered in adequate amounts, beneficially affect the general health status of man and animal. the great interest in probiotic microganisms is based on evidences from clinical studies indicating benefits in the prevention or treatment of a broad spectrum of gastrointestinal and systemic disorders. the potential mechanisms by which probiotics beneficially affect health include strengthening of the intestinal barrier, modulation of the immune response, and antagonism of pathogens either by the production of antimicrobial compounds or through competition for mucosal binding sites. in some specific clinical circumstances, there is clear evidence of benefit whereas in others, the results are dubious and important questions remaining unanswered. the aim of this review article is to focus probiotics on their potential as biotherapeutic agents against intestinal parasites.
Dificuldade diagnóstica em dois casos de neoplasia de células linfóides precursoras cutaneas primárias

Abstract: the lymphoblastic lymphoma (lbl) is a malignant neoplasm of precursor lymphocytes (b, t or nk-cells). the primary involvement of the skin is rare. we examined the clinical, anatomopathological and immunophenotypic features of two patients with primary cutaneous involvement. histologically they showed an infiltrate of immature cells. the immunophenotypic analysis was performed with a comprehensive panel of antibodies. t-cell receptor rearrangement (tcr-gamma) was analyzed with polymerase chain reaction (pcr) and it was positive in one case, which was cd56 positive, classified as blastic nk-cell-like lymphoma. this case represents a distinct entity derived from precursor cells at an early stage of a common developmental pathway for t and nk cells. the other case was classified as t-cell lymphoblastic lymphoma with aberrant expression of cd79a, what could be a diagnostic pitfall. the accurate diagnosis depends on the use of a comprehensive panel of antibodies for immunophenotypic characterization and molecular analysis.
