Role of cytologic grading in prognostication of invasive breast carcinoma

Abstract: Background: Evaluation of cytologic features is indispensable in the preoperative diagnosis and grading of infiltrating ductal breast carcinoma (CA) in fine-needle aspiration cytology (FNAC) material and this method can also provide additional information regarding intrinsic features of the tumor as well as its prognosis. Aim: This study has been done to evaluate comparatively the cytologic and histomorphologic grading of infiltrating ductal carcinoma of breast with specific reference to lymph node metastasis and its role in prognostication. Materials and Methods: Forty three patients who underwent FNAC and mastectomy for infiltrating ductal carcinoma were cytologically and histologically graded (employing Robinson′s cytologic grading system and Elston′s modification of Bloom-Richardson system, respectively). Statistical analysis was done employing ′z′ test and c2 test to compare the two grading system and to examine the degree of correlation between the cytologic and histologic grades. Multiple regression analysis was done to assess the significance of every cytologic and histologic parameter. All 43 cases, graded cyto-histologically were also evaluated for presence or absence of metastasis to the regional lymph nodes employing c2 test. Results: With histologic grade taken as the standard, cytology was found to be fairly comparable, for grading breast carcinoma (overall sensitivity 89.1%, specificity 100%). Further comparison of the two grading systems by Z-test showed that difference between the cytologic and histologic grading was insignificant in all the three grade (p > 0.05). Of the six parameters studied, cell dissociation, nucleoli and chromatin pattern were the most influential features (p < 0.001). The statistically significant difference (p < 0.001) was found in incidences of axillary lymph node metastatic rate in three cytologic grades (15.4% in grade I vs. 83.3% in grade III) as well. Conclusions: Apart from being simple and noninvasive, cytologic grading is comparable to histologic grading and might provide relevant information on the aggressiveness of invasive ductal carcinoma of breast and could be a useful parameter to take into consideration when selecting mode of therapy and to predict tumor behavior.
Neoplastic and nonneoplastic ovarian masses: Diagnosis on cytology

Abstract: Objective : To evaluate the role of fine needle aspiration cytology (FNAC) in the distinction between neoplastic and nonneoplastic ovarian masses. Materials and Methods : One hundred and twenty patients with ovarian masses were studied. After detailed history and clinical examination, ultrasound (USG)-guided FNAC was performed in 92 clinical benign cases while FNAC and/or imprints of surgically resected ovarian masses was performed in 28 clinically suspected malignant cases. The smears were stained with Papanicolaou stain and histopathological sections were stained with hematoxylin and eosin stain with inclusion of special stain whenever required. Serum b-human chorionic gonadotrophin and a-fetoprotein estimations were carried out in cytologically diagnosed germ cell tumors. Results : The overall sensitivity, specificity and diagnostic accuracy of FNAC in diagnosing various ovarian masses were 79.2%, 90.6% and 89.9%, respectively. Conclusions : The clinical examination, pelvic ultrasound and FNAC were complementary and none of the methods was, in itself, diagnostic. However, USG-guided FNAC was found to be a fairly specific and accurate technique and should be employed as a routine, especially in young females with clinically benign ovarian lesions. The reasons for false diagnosis and limitations of USG and FNAC have been analyzed.
A Case of Pure Endophytic Squamous Cell Carcinoma of the Gallbladder: A Rare Entity with Aggressive Behaviour

Abstract: Pure squamous cell carcinoma of gallbladder is a rare tumor accounting for only 3% of the malignancies of gallbladder. A 35-yearold female presented with pain and lump in right hypochondrium. Ultrasonography and computed tomography revealed a thickened gallbladder filled with stones without any obvious growth and a large hepatic mass. Histologically, a pure type of well differentiated squamous cell carcinoma of gallbladder was diagnosed after excluding all possible differential diagnoses owing to different histogenesis and aggressive biological behaviour. Early diagnosis is the most important parameter for improving the survival indices among the patients with squamous cell carcinoma of the gallbladder.
Eumycetoma versus actinomycetoma: Diagnosis on cytology

Abstract: Eumycetoma is a chronic cutaneous and subcutaneous infection caused by various genera of fungi producing specific colored granules known as grains. A 45-year-old farmer presented clinically with a left foot mass with multiple discharging sinuses existing for last 3 years. Clinical and radiological findings suggested a diagnosis of chronic osteomyelitis with suspicion of tuberculosis. Imprints plus fine needle aspiration cytology (FNAC) smears exhibited distinct brown-black colonies of a fungus having branching and septate hyphae embedded in matrix like material against a mixed inflammatory background. Periodic acid Schiff (PAS) stain gave positive staining and subsequent fungal culture confirmed the cytological diagnosis and aided in species identification as Madurella mycetomatis. Thus, eumycetoma can precisely be diagnosed and confidently differentiated from similar conditions such as actinomycetoma by simple and inexpensive cytological techniques such as FNAC and imprint smears, employing routine May-Gr nwald-Giemsa, Papanicolaou and simple PAS stains on cytological specimen, thus leading to rapid diagnosis for institution of correct treatment.

Abstract: Solanum nigrum (family: Solanacae) commonly called Black nightshade, grows as a weed, found in the dry parts of India and other parts of the world. It has a medicinal usage and has been used as a traditional folk medicine for treating various ailments such as pain, fever and liver disorders. Generally, black nightshade is very rich in nutritive values, which are capable of supplying minerals , vitamins, hormones and proteins. This herb elaborates a wide variety of medicinal properties such as anticancer, antioxidant, neuroprotective, cytoprotective, antimicrobial, antinociceptive and antipyretic properties. It has been claimed that Solanum nigrum particular are an excellent remedy for liver disorders.Present investigation is therefore designed to determine the effect of crude ethanolic extract of S. nigrum on protein content of liver and kidney after daily administration of dose at the level of 250mg/kg b. w. for three, five and seven days respectively. It was noticed that the chronic administration for longer duration leads to significant increase in protein contents of kidney and liver.
Immunopathology of skin lesions

Abstract: A study was conducted on 130 patients suffering from skin lesions which included psoriasis, lichen planus, DLE, pemphigus, vitiligo and alopecia areata. Forty age-and-sex-matched healthy individuals served as control. Serum IgG, IgM, and circulating immune complexes (CIC) were estimated. Significant increase in serum IgG (1937.2 ± 1030.43 mg%) and IgM (232.12 ± 136.98 mg%) was observed in all the skin lesions when compared with controls except in lichen planus where they were significantly lowered, values being 580.61± 77.35 mg% and 66.88 ± 6.59mg% respectively. CIC levels were significantly raised (P<0.00 1) in various skin lesions (40.49±23.29) when compared with controls (17.68± 3.21), but no significance was observed in lichen planus( 17.72 ± 4.28). Serum IgG, IgM and CIC were statistically significantly altered depending on the extent of the lesion and lowered significantly to almost normal values following treatment, thereby confirming the role of immunity in the pathogenesis of these skin disorders.
Actinomycosis - Left Post Chest Wall

Abstract: A forty year old female of weak body built presented with recurring small hard lumps in let posteriorchest wall for 3 years and discharging ulcers for 3 months duration. Clinically, the provisional diagnosiswas malignancy with secondary infection. FNAC showed features suggestive of dysplasia buthistopathology confirmed the diagnosis as actinomycosis. The present case is reported due to rare incidenceof actinomycosis at post chest wall with muscle involvement.
Pindborg Tumor in an Adolescent-An Atypical Presentation

Abstract: Calcifying epithelial odontogenic tumor (Pindborg tumor), is a rare benign odontogenic neoplasm representingabout 0.4-3% of all odontogenic tumors. This tumor more frequently affects adults in an age range of 20-60 years, with a peak incidence in the 5th decade of life. Calcifying epithelial odontogenic tumour has amuch lower recurrence rate than ameloblastoma and malignant transformation with metastasis is rare.

Abstract: Supported tritonX100 polyaniline nano-porous electrically active film has been fabricated successfully onto indium-tin-oxide conducting probe using electrochemical polymerization process. The doping of TX-100 in the polymeric network of PANI was suggested using cyclic voltammeter, UV-vis spectroscopy, and Fourier Transform Infrared spectroscopy. The change in the surface morphology of PANI thin film due to incorporation of tritonX-100 was investigated using Atomic Forced Microscopy and porosity has been confirmed scanning electron microscopy, respectively. The surface morphology, uniformly disperse hexagonal close packing of TX-100 in PANI matrices due to the incorporation of TX-100 in polymeric network of PANI was confirmed by Atomic Force Microscopy. The electrical conductivity of PANI-TX-100 increases from 1.06 x 10-2 S/cm-1 to 4.95 x 10-2 S/cm-1 as the amount of TX-100 increases during the polymerization. The change in the morphology and electrical conductivity of PANI due to incorporation of TX-100 prove as a promising material for the sensing application.

Abstract: Non-deflated balloon of a transurethral inserted bladder catheter is a problem that could be difficult to manage, especially in males. A variety of methods have been described to puncture the balloon and remove the catheter, though these can be associated with complications. We illustrate a simple and a safe method to overcome this problem using a flexible cystoscope under local anaesthesia.
