Insuficiência mitral: compara??o entre o tratamento clínico e cirúrgico a médio prazo de acordo com a classe funcional

Abstract: material and methods: we analyzed retrospectively the medium term mortality index in 113 patients with mitral regurgitation diagnosed clinically and echocardiographic who have undergone clinical or surgical treatment since january 1995 until december 1998. the age ranged from 29 to 67 years (mean 47 ± 4.3 years ) and 81 (71.7%) patients were female. in nyha classification, 51 (45.1%) patients belonging to functional class i, 24 (21.3 %) class ii 21 (18.6%) class iii and 17 (15 %) class iv. results: after 4 years we observed that the mortality rate was 21.2%. all the 75 (66.4%) patients belonging to fc (nyha) i and ii, who received only clinical treatment presented mortality rate of 12%. from the 38 patients belonging to fc (nyha) iii and iv, 18 (47.4 %) were submitted to surgical treatment and showed mortality rate of 22.2%: (5.5% surgical mortality and 16.7% during 4 years). the 20 (52.6 %) patients that refused the surgery and presented similar clinical conditions to those operated, were treated clinically and demonstrated mortality rate of 55%. conclusion: we conclude that the mitral regurgitation exhibit high mortality rate and that surgical treatment to the patients belonging to fc ( nyha) iii and iv reveals low mortality rate medium term when compared with clinical treatment.

Abstract: CASUíSTICA E MéTODOS: Analisamos, retrospectivamente, o índice de mortalidade a médio prazo de 113 doentes portadores de insuficiência mitral, diagnosticados clínica e ecocardiograficamente, e submetidos a tratamento clínico ou cirúrgico. A idade dos pacientes variou de 29 a 67 anos, com média de 47± 4,3 anos, sendo 81 (71,7%) doentes do sexo feminino. De acordo com a classifica o de NYHA, 51 (45,1%) doentes encontravam-se em classe funcional I, 24 (21,2%) em classe II, 21 (18,6%) em classe III e 17 (15,1%) em classe IV. RESULTADOS: Após 4 anos, observamos que o taxa de mortalidade global foi de 21,2%. Todos os 75 (66,4%) doentes que se encontravam em classe funcional I e II receberam apenas tratamento clínico e apresentaram taxa de mortalidade de 12%. Dos 38 pacientes que se encontravam em classe funcional III e IV, 18 (47,4%) foram submetidos a tratamento cirúrgico e apresentaram taxa de mortalidade de 22,2%, sendo 5,5% intra-hospitalar e 16,7% no período de 4 anos. Os 20 (52,6%) doentes que recusaram a opera o apresentavam condi es clínicas semelhantes aos operados e foram submetidos a tratamento clínico, havendo índice de mortalidade de 55%. CONCLUS O: Concluímos que a insuficiência mitral apresenta alto índice de mortalidade e que o tratamento cirúrgico para os doentes que se encontram em classe funcional III e IV da NYHA apresenta menor índice de mortalidade a médio prazo quando comparado ao tratamento clínico, devendo ser indicado sempre quando possível.
Hypoparathyroidism in an Egyptian child with Hutchinson-Gilford progeria syndrome: a case report

Abstract: A 10-year-old Egyptian boy was referred to our institution for an evaluation of recurrent attacks of muscle cramps, paresthesia of his fingertips and perioral numbness of two months duration. On examination, we found dilated veins present over his scalp with alopecia and frontal bossing, a beaked nose, thin lips, protruding ears, a high pitched voice with sparse hair over his eyebrows and eyelashes and micrognathia but normal dentition. His eyes appeared prominent and our patient appeared to have poor sexual development. A provisional diagnosis of progeria was made, which was confirmed by molecular genetics study. Chvostek's and Trousseau's signs were positive. He had low total calcium (5.4 mg/dL), low ionized calcium (2.3 mg/dL), raised serum phosphate (7.2 mg/dL), raised alkaline phosphatase (118 U/L) and low intact parathyroid hormone (1.2 pg/mL) levels. He was started on oral calcium salt and vitamin D; his symptoms improved with the treatment and his serum calcium, urinary calcium and alkaline phosphates level were monitored every three months to ensure adequacy of therapy and to avoid hypercalcemia.Routine checking of serum calcium, phosphorus and parathyroid hormone will help in the early detection of hypoparathyrodism among children with progeria.Progeria is a rare combination of dwarfism and premature aging characterized by thin, atrophic shiny skin with sclerodermoid changes, sparse to absent hair of the scalp, eyebrows and eyelashes, craniofacial disproportion, a sculpted beaked nose, short stature, a pyriform thorax, thin limbs and musculoskeletal and cardiovascular manifestations [1]. The classic form is known as Hutchinson-Gilford syndrome. It occurs sporadically, with a reported incidence of one in eight million births and male predominance with male to female ratio of 1.5:1 [2]. Abstract: this article is a project in hybridity that joins russian philosophical history with a project on russian national identity between 1700 and 1861. based on the works of various russian philosophical historians, and emphasizing, as the background, the preoccupation for developing a national identity within russia's arts, this text makes a novel suggestion: that russian philosophical history should not only be read as an agglomeration of philosophical systems, but also as a science that contributes to the multishaped imaginary that is national identity. like the biological fusion between two cells of different species to form a new one of mixed characteristics, the essay presents a interlaced hybrid that: (1) offers refections on the characteristics of the mother (russian philosophical history); and (2) synthesizes refections on the features of the father (the project of national identity).

Abstract: there is increasing evidence that cutaneous nerve fibers play a modulatory role in a variety of acute and chronic skin processes. local interactions between skin cells, skin immune components and neuronal tissues occur specially through neuropeptides, a large family of chemically-related neurotransmitters exhibiting a heterogeneous nomenclature. neuropeptides are ubiquitous in central and peripheral nervous systems, being directly released in skin by cutaneous nerve fibers. this review is focused on the actions of substance p (sp), calcitonin gene-related peptide (cgrp), vasointestinal peptide (vip), pituitary adenylate cyclase-activating polypeptide (pacap), neuropeptide y (npy) and somatostatin (som). neuropeptide-related functions on skin and immune cells are also discussed, as well as recent findings implicating nerve fibers in cutaneous inflammatory responses, hypersensitivity reactions and dermatoses, namely psoriasis, atopic dermatitis, leprosy and alopecia.

Abstract: Há evidências crescentes de que a inerva o cutanea é capaz de modular uma variedade de fen menos cutaneos agudos e cr nicos, interagindo com as células da pele e seus componentes imunes. Essa forma de sinaliza o local entre tecido nervoso e tecido cutaneo ocorre especialmente por meio dos neuropeptídeos, uma numerosa família de neurotransmissores de natureza química comum e nomenclatura heterogênea presentes em todo o sistema nervoso e secretados pelas fibras nervosas cutaneas. S o alvo desta revis o os neuropeptídeos substancia P (SP), o peptídeo relacionado ao gene da calcitonina (CGRP), o peptídeo vasoativo intestinal (VIP), o peptídeo ativador da adenilato-ciclase pituitária (PACAP), o neuropeptídeo Y (NPY) e a somatostatina (SOM).
