Abstract: Aim: In our study the purpose is to look over clinical characteristics of children with mitral valve prolapse (MVP) with the current literature concepts.Material and Method: One hundred thirteen cases admitted to Kocaeli University Faculty of Medicine Department of Pediatric Cardiology between April 2005 and March 2009 were evaluated retrospectively regarding their complaints on admission, physical examination, electrocardiography (ECG), telecardiography, Holter ECG and echocardiography findings. Mitral valve prolapse was defined as superior displacement of the mitral leaflets more than 2 mm into the left atrium during the systole.Results: The causes in 113 patients with MVP were; Marfan syndrome in six, Noonan syndrome in two, mucopolysaccaridosis in six and romatizmal carditis in nine patients. The remaining were accepted as primary MVP. The age of these subjects (59 girls and 31 boys; M/F: 1.9) ranged between 2-17.5 years with the mean of 10.2±5.6 years. The most frequent causes of referring the children to cardiologist were; cardiac murmur (44%), chest pain (25%) and feeling of cardiac palpitations (21%). Fourty of 90 patients (44.4%) were asymptomatic. In 20% of patients, BMI percentile was<5%. Eleven percent of patients had thoracic skeletal anomalies. A murmur±mid-systolic click was detected in 58%. In 23.3%, routine ECG showed T wave abnormalities. Mitral regurgitation was established on echocardiography in 81.1% of patients. Familial inheritance of MVP was found only in 3.6%.Conclusions: Our results indicate that MVP is not an uncommon disease and it is more common in adolescent girls with asthenic built. Mitral valve prolapse should be kept in mind and echocardiography should be performed in the patients with persistant chest pain, palpitation and fatigue even if there is not any murmur or click on oscultation. Abstract: Objective: The objective of this study is to describe the clinical and epidemiological characteristics of children with Kawasaki disease (KD) in the Kocaeli region of Turkey.Material and Method: The epidemiological, clinical and laboratory findings of twenty-four patients diagnosed and treated in the Kocaeli University Pediatric Cardiology Department between 2003-2009 were evaluated retrospectively. IVIG and acetylsalicylic acid were given to all patients. Results: The male to female ratio was 1.18: 1.The mean age of patients were 2.7±2.6 years (median: 26 months; range: 2 months -11 years). Four patients were under 6 months, 16 were between 6 months-5 years and 4 were over 5 years. The duration of illness at admission was mean 10.5±4.36 days (median:10.1 days; range: 5-22 days).16 (66.6%) patients were diagnosed at the acute stage, and 8 (33.3%) were diagnosed at the sub-acute stage. The most frequent findings-except fever- were oral mucosal changes (95.8%) and conjunctivitis (91.6%). Three patients (12.5%) had minimal coronary artery dilatation. Intravenous gamma globulin and acetylsalicylic acid were administered to all patients. The clinical and laboratory findings of all patients returned to within normal ranges after treatment. Coronary artery dilatation was not detected in the follow up in any of the patients or in 3 patients who had coronary artery dilatation previously.Conclusions: We suggest that, by increased awareness of the epidemiology and spectrum of the clinical presentation, KD could be diagnosed more frequently than before, so it may become the most freqeuent vasculitis causing cardiac disease in future. All pediatric clinicians should certainly be aware of the clinical presentation of KD for early recognition and optimal management in order to prevent fatal complications.
Can More than One Fistula Be Treated by a New Type of Seton Concomitantly?  [PDF]

Abstract:

Objective: The treatment of trans-sphincteric fistulas is a surgical challenge since eradicating the fistula tract while maintaining the anal continence is vitally important. In this study we documented the outcomes of the patients with one or more fistulas treated at the same operation with a new type of seton. Patients and Methods: The study included 78 patients with 107 high anal fistulas who were treated with seton between 2007 and 2011. Fifty seven patients had only one fistula, 13 patients had two isolated fistulas and 8 patients had three isolated fistulas. Group I was the patients with one fistula and Group II was the patients having more than one fistula. Postoperative complications, hospital stay, cutting through the sphincter and healing times and Wexner’s scores were compared between the two groups. Results: In Group I, there were 57 patients and in Group II, 21 patients were operated for 50 fistulas. No postoperative complications were observed except urinary retentions (4 patients in Group I, 3 patients in Group II). No infection on the operation site was documented in both groups, in Group I none of the patients needed extra dose of narcotic analgesic but in Group II two of the patients who had 3 fistulas were discharged on the second postoperative day because of pain on the operation site. The median for the cutting seton to cut through the sphincter was 31 days and complete healing was achieved with a median of 40 days in Group I, whereas it was 56 and 65 days respectively for Group II. There were no recurrences in our patients in both groups. Although the Wexner scores of the both groups were low, the difference between the groups was statistically significant. Conclusion: It seems to be feasible to perform seton by using a cheap, effective, easily inserted material, also in treatment of patients with more than one fistula while preserving anal continence.

Evaluation of the Efficacy and Safety of Oral buprofen in the Treatment of Patent Ductus Arteriosus

Abstract: Since indomethacin has many side effects, ibuprofen has been started to be used with beneficial results and less side effects for the closure of patent ductus arteriosus (PDA) in recent years. The frequency of PDA, and the effects and side effects of oral ibuprofen were investigated by echocardiographic evaluation, in 164 preterm neonates in Neonatology Unit of Dicle University,between April and December 2004. Oral ibufrofen was given at 10 mg/kg/day dose to infants who had significant left-right shunt on the third day of birth but those who had contraindication for ibuprofen were excluded. By daily echocardiographic evaluations in those without closure after the first dose, a second and third dose of 5 mg/kg/day were given if necessary. Ductus closure has ocured in 24 of 27 (88.8%) patients, at a mean period of 1.7±0.9 (1-4) days. Complications like hyponatremia, hypercreatininemia, thrombocytopenia, and necrotizing enterocolitis were not seen. Only in one patient intracranial hemorrhage was occured two days after the treatment. In conclusion, treatment with oral ibuprofen is an effective and safe treatment method for the closure of the PDA in preterm infants.
CD44 and MMP-2 expression in urothelial carcinoma

Abstract: Aim: CD44, one of the adhesion molecules, is thought to play an important role in cell-cell and cell-matrix interactions. Matrix metalloproteinases are degradative enzymes that remodel extracellular components. In this study the relation of MMP-2 and CD44 expressions with the histologic classification and the pathologic stage of urothelial carcinoma was revealed using immunohistochemistry.Material and Methods: Thirty-nine patients with urothelial carcinoma of the bladder were studied. The histological classification was performed according to WHO criteria. Patients were grouped as infiltrating urothelial carcinoma, low grade non-invasive papillary urothelial carcinoma, and high grade non-invasive papillary urothelial carcinoma. The pathological staging was done according to the TNM classification. Immunohistochemical staining using CD44 and MMP-2 antibodies was performed on tissue blocks.Results: CD44 immunoreactivity was detected in 77% (30/39) of the tumours which was significantly higher in non-invasive papillary urothelial carcinomas, low grade non-invasive papillary urothelial carcinomas, high grade infiltrating urothelial carcinomas (p≥0.05). MMP-2 expression was observed in 69% (27 of 39) of the tumours. There were no significant differences in MMP-2 expression between various histologic subtypes and noninvasive and infiltrative tumours.Conclusion: In conclusion, higher expression of CD44 is inversely correlated with infiltrative potential of urothelial carcinoma. These results should be supported by further studies.

Abstract: Lycopene, an acyclic, open chain, unsaturated carotenoid having 13 double bonds, of which 11 are conjugated, arranged in a linear array, is considered to be a pigment of potential commercial importance in the emerging market for nutraceutical products because of its function as a health-promoting ingredient. Lycopene has received a great deal of attention as an effective antioxidant that can play an important role in reducing the risk of several chronic diseases. In this study, we reviewed extraction parameters of lycopene from tomato and tomato products by supercritical fluids and pre-extraction procedures. For extraction, temperature range as 50-110°C, extraction time range as 0.5-8.0 hours, extraction pressure range as 300-400 bar and using co-solvent, especially ethanol, are common parameters.
Phytoestrogens: Plant-derived Estrogenic Compounds

Abstract: Estrogen is a hormone, which is produced in ovary and testis; however, it has many biological effects besides the reproductive system. Phytoestrogens are the compounds, which have estrogen-like structure and activities, taking place in structure of various edible plants at different levels and in different compositions. These compounds attracted notice after the first quarter of 20th century upon they had been associated with infertility seen in some of animals fed with alfalfa, and these compounds have been identified in human-derived biological samples and its effects on health have been taken under study in the recent 30 years. These materials have especially antioxidant role in plants while they have activities in animals and humans as estrogen agonist and antagonists. Based on their chemical structure, they may be gathered under especially isoflavon and lignan groups while some of members of coumestan and stilbene groups are also identified as phytoestrogenic compound.
LAPAROSCOPIC APPROACH FOR EPIPHRENIC ESOPHAGEAL DIVERTICULA

Abstract: Epiphrenic diverticula of the esophagus are rare disorders frequently associated with esophageal dismotility that is thought to be the cause of the diverticulum and some of the symptoms. Treatment is surgery and only indicated to the symptomatic patients. The operation can be performed successfully by laparoscopy, which offers good access to the distal esophagus and the inferior mediastinum. A 68 year-old woman presented with symptoms of heartburn, regurgitation and vomiting. Two diverticula located in distal esophagus were detected with barium esophagography and endoscopy. The biggest diverticulum was resected by the endoscopy assisted laparoscopic approach.Laparoscopic approach to epiphrenic diverticula seems to be feasible and safe provided when surgeons trained in minimally invasive procedures of the gastroesophageal junction perform it.
